A Pilot Study to Evaluate the Reliability of a Pattern Identification Tool for Benign Prostatic Hyperplasia and to Analyze Correlations between Pattern Identification Tools and International Prostate Symptom Score (IPSS) and Uroflowmetry (전립선비대증 변증도구의 신뢰도 평가 및 IPSS, 요속과의 상관관계에 대한 탐색적 연구)

  • Objectives: To evaluate the reliability of a pattern identification tool for benign prostatic hyperplasia and to examine the relationship between pattern identification tool readings and IPSS and uroflowmetry. Methods: We analyzed 56 patients diagnosed with benign prostatic hyperplasia from December 27th, 2017 to December 26th, 2018 by two different Korean medical doctors and followed with a pattern identification tool and by IPSS and uroflowmetry. One week later, the patients were retested to analyze the reliability of the pattern identification tool, determined with the intraclass correlation coefficient (ICC) using the test-retest method. The correlation between IPSS and uroflowmetry was analyzed with the Pearson coefficient. Result: The reliability of the pattern identification tool for benign prostatic hyperplasia was evaluated as "poor agreement beyond chance" (ICC=0.349). The reliability of each pattern identification score was evaluated as "good" for Yang Deficiency of Kidney, Yin Deficiency of Kidney, Deficiency of Middle Qi, and Dampness-heat of Lower Energizer. The internal consistency was evaluated as "good" for Yang Deficiency of Kidney, Yin Deficiency of Kidney, and Dampness-heat of Lower Energizer, and as "excellent" for Deficiency of Middle Qi. The correlation between pattern identification and IPSS was evaluated as a "moderate positive correlation" for all pattern identifications. The average flow rate and maximum flow rate using uroflowmetry was evaluated with "moderate negative correlation" for Yang Deficiency of Kidney and Dampness-heat of Lower Energizer. Conclusion: The reliability of a pattern identification tool for benign prostatic hyperplasia was evaluated as "poor agreement beyond chance." Further research is needed.

Analysis of Pattern Identification and Related Symptoms on Idiopathic Short Stature -Focusing on Traditional Chinese Medicine Literature- (특발성 저신장의 변증 유형 및 변증별 증상 분석 -중의학 논문을 중심으로-)

  • Objectives We aimed to analyze traditional Chinese medicine (TCM) literatures in regards to the pattern identification and related symptoms of idiopathic short stature (ISS). Methods We searched relevant literatures published up to September 29, 2020 through three Chinese electronic databases. We performed frequency analysis of the selected studies by extracting information on pattern identification, clinical symptoms, and TCM treatments presenting pattern identification of ISS. Results Sixteen studies were included. Spleen deficiency, kidney deficiency, dual deficiency of spleen-kidney, and liver-kidney yin deficiency were frequently reported. Clinical symptoms of the spleen deficiency include sallow complexion, body constituent weakness, anorexia, lack of qi and no desire to speak, and loose stools. Herbal medicines (HMs) such as Sijunzi-tang were frequently reported. Clinical symptoms of the kidney deficiency include cold limb and fear of cold, soreness and weakness of waist and knees, and clear and long urine. HMs such as Bishendihuang-wan were frequently reported. Clinical symptoms of the dual deficiency of spleen-kidney include body constituent weakness, spirit lassitude and lack of strength, anorexia, soreness and weakness of waist and knees, and cold limb and fear of cold. HMs such as Sijunzi-tang plus Bishendihuang-wan were frequently reported. Clinical symptoms of the liver-kidney yin deficiency include tidal fever and night sweating, heat in the palms and soles, dizziness, and dry throat. HMs such as Liuweidihuang-wan were frequently reported. Conclusions This was the first study to analyze the frequency of pattern identification and related symptoms on ISS. In the future, a standardized Korean medicine pattern identification system should be established.

  • The cause and mechanism of eye diseases are follow in biblography. The etiological factor of eye diseases are nearly caused by heat. It is important to distinguish deficiency and substance. In external etiologic factors wind-fever is most of all in six exogenous factors. In internal etiologic factors xu(deficiency) of kidney, blood and fire of seven emotions are lots. In factors other than the internal and external, eating greasy food and hyper-caloric food are a lot of case. In relation with the yin-yang andxu-shi, ther are a lot of eye diseases symptom as dark-dizziness due to the yin-xu of hepatic-kidney. According to eight principles of differentiation of syndromes, syndromes of exterior, heat, shi, almost fall into the category of yang and syndromes of interior, cold, xu, nearly fall into the category of yin. In interior treatment, inducing wind and clearing heat, using Qi and tonifying blood, tonifying and suing of hepatic-kid ney are useful and often used treatment.

  • The hysterocele means the descent state of uterus than a normal position. We had medical treatments 3 patients with hysterocele (grade 1, 2 or 3) by invigorating qi and tonifying the kidney. And we had significant effects on patients with hysterocele grade 1, 2 by Soeuminbojungikgitang and Gamibojungikgitang. But we didn't cure a patient with hysterocele grade 3.

Evaluation of Women with Myofascial Abdominal Syndrome Based on Traditional Chinese Medicine

  • Objectives: This study used semiology based on traditional Chinese medicine (TCM) to investigate vital energy (Qi) behavior in women with abdominal myofascial pain syndrome (AMPS). Methods: Fifty women diagnosed with chronic pelvic pain (CPP) secondary to AMPS were evaluated by using a questionnaire based on the theories of "yin-yang," "zang-fu", and "five elements". We assessed the following aspects of the illness: symptomatology; specific location of myofascial trigger points (MTrPs); onset, cause, duration and frequency of symptoms; and patient and family history. The patients tongues, lips, skin colors, and tones of speech were examined. Patients were questioned on various aspects related to breathing, sweating, sleep quality, emotions, and preferences related to color, food, flavors, and weather or seasons. Thirst, gastrointestinal dysfunction, excreta (feces and urine), menstrual cycle, the five senses, and characteristic pain symptoms related to headache, musculoskeletal pain, abdomen, and chest were also investigated. Results: Patients were between 22 and 56 years old, and most were married (78%), possessed a elementary school (66%), and had one or two children (76%). The mean body mass index and body fat were 26.86 kg/cm2 (range: 17.7 - 39.0) and 32.4% (range: 10.7 - 45.7), respectively. A large majority of women (96%) exhibited alterations in the kidney meridian, and 98% had an altered gallbladder meridian. We observed major changes in the kidney and the gallbladder Qi meridians in 76% and 62% of patients, respectively. Five of the twelve meridians analyzed exhibited Qi patterns similar to pelvic innervation Qi and meridians, indicating that the paths of some of these meridians were directly related to innervation of the pelvic floor and abdominal region. Conclusion: The women in this study showed changes in the behavior of the energy meridians, and the paths of some of the meridians were directly related to innervation of the pelvic floor and abdominal region.

Literature Review on Parkinson's Disease in Oriental Medicine (파킨슨병의 한의학적 고찰 -병인병리(病因病理)와 침구요법(鍼灸療法)을 중심으로-)

  • Objective: In order to find oriental medical therapies on Parkinson's disease and to make a fundamental basis for clinical application, this study was performed. Methods: We reviewed 35 kinds of the ancient and modern text, and related articles. Results: Parkinson's disease is an extrapyramidal disease characterized by akinesia, tremor at rest, rigidity, and slowness of movement. In old oriental medical text, Parkinson's disease is described as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風). According to the text, major pathological causes were Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). And Parkinson's disease can be classified into four clinical types as liver & kidney yin-deficiency, qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis. Standardized acupuncture points are GV20, GB20, GV14 on head, CV12, ST25 on abdomen, GV26, ST7, GB1, S14, LI20 on face, LI4, LI11, TE5, SI3, HT3, LI15, SI6 on upper extremity, and ST36, GB34, SP6, LR3, KI1, GB30, BL40 on lower extremity. Other methods, such as scalp acupuncture, electro-acupuncture, and herb-acupuncture, can be applied to treat Parkinson's disease. Conclusions: We find out that there are oriental medical concepts related with Parkinson's disease such as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風), of which major causes are Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). We can also apply many effective acupuncture points and acupuncture therapies according to differential diagnosis, for example, liver & kidney yin-deficiency. qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis.

  • Gastric cancer shows the most high friquency in cancers that occurs in Korea. The western medicine treatment for gastric cancer has radiation and surgery, chemical treatment. While, oriental medicine cures the gastric cancer by herb-drugs, acupunture , moxa and srigma. With just one way of treating gastric cancer can't be effective remedy. Because each medicine has a strength and weakness. Thus, it is effective treatment when two medicine combins and supplement each other. We got the following result about a trend of oriental and westernal combination treatment for gastric cancer through studing records. 1. The western medicine treats gastric cancer patient with surgery first and right after surgery. They need on assembly treatent such as chemical and immune treatment. In oriental medicine, they treats gastric cancer patients with differentiation of symptone and signs and treatment(辨證施治)[for example:incoordination between liver and stomach(肝胃不和), insufficiency of spleen and stomach(脾胃虛弱), stagnation of blood stasis and toxic agent(瘀毒內阻), deficiency of yin by stomach heat(胃熱傷陰), reinforcing both qi and blood(氣血雙虧), stagnation of damp-phlegm(痰濕凝結)] and cure for them by acupuncture and stigma, too. 2. In combination with oriental and western medical treatment principle of gastric cancer by each stage is as follows. First stage and second stage gastric cancer is cured with radical surgery mainly. After operation, the herb of invigoration of the spleen(健脾), coordination of the stomach(和胃), and smoothing the liver and regurating the circulation of qi(疏肝理氣), is used for good gastroenteric condition. The second stage patients can be concidered using in combination with chimical treatment. The third stage gastric cancer is treated with radical surgery or with temporizing surgery. After those surgery, herb-drugs treatment is used jointly. The fourth stage patients who have no extensively metastasis or local contraindication can undergo temporizing and curcuit surgical operation. Herb-drugs and chemical treatments are used together for patients after operating. If he has operating contraindication, he would be treated with herb-drugs and chemical treatment. 3. In case of using in combination with oriental and western medical treatment as follows. As for herb-drugs with chemical treatment, reinforcing both qi and blood(補益氣血), invigorate the spleen and the stomach(健脾和胃), reinforcing liver and kidney(滋補脾腎), clear out the heat and relieve the toxic agent(淸熱解毒), can be used and with radiation treatment, clear out the heat and relieve the toxic agent(淸熱解毒), promoting the production body fluid and moisturizing the vicera(生津潤燥), reinforcing both qi and blood(補益氣血), invigorate the spleen and the stomach (健脾和胃), reinforcing liver and kidney(滋補肝腎) etc, can be used. 4. According to the research of oriental and western medical combination treatment are the 5-year-survival degree with oriental and western medicine combination treatment was for better than that just with oriental or western medical treatment. Especially, it has good effect on the third, fourth stage gastric cancer. That is, the middle and the end of stage gastric cancer. 5. The merits of oriental and western medicine combination treatment are lengthers one's life and diminish the bad effect of chemical treatment and radiation treatment be near completion, prevent from relapsing, maintain the balance in their eveirenment of body and improve immunity.

  • 1. According to causes of attack and symptoms, tinnitus is divided into two categories; deficiency and excess. Causes of excess syndrome of tinnitus were wind fire in the liver and gallbladder, phlegm fire, blood stagnation, and heat in meridian system and the causes of deficiency syndrome of tinnitus were qi deficiency or blood deficiency after an illness or delivery, yin deficiency of liver and kidney, and deficiency of sea of the marrow. 2. Tinnitus was related to the vicera and bowels, especially to liver, gallbladder and urinary bladder. 3. In regard of method of treatment, tonify kidney, nourish heart, clear the liver and discharge heat are used according to visera and bowel theory. Clear phlegm and downbear fire are used for phlegm fire. Tonify spleen and kidney is used for ancestral vessel deficiency. Dispel wind and dissipate fire can be used according to theory of five elements' motion and six kinds of natural factors. 4. The basal meridian of acupuncture and moxibustion treatment were the channels of Shaoyang.. 5. Regarding neuropsychiatric aspect of tinnitus, sudden anger and depression of mind were the main mechanism of disease and liver fire was the main cause. The prescriptions for neuropsychiatric tinnitus were Dangguiyonghuehwan, and Yongdamsagantang.

  • 1. Objectives This paper was written in order to understand the formative process of Soeumin pharmacology. 2. Methods Souemin pharmacology was analysed with pathology and new prescription in Gabobon(甲午本) and Sinchukbon(辛丑本) of ${\ulcorner}$Dongyi Suse Bowon${\Ircorner}$. 3. Results and Conclusions Soeumin is charactrized to much output of kidney and a little input of speen in sight of ingestive food(水穀). So deficiency of YangQi is a peculiarity of pathology and ascending-Qi is a basic pharmacology. The pharmacology in the exterior disease of Soeumin is built up to base on the old prescription of previous text in Gabobon. Ascending-Yang is the basic pharmacology of Sinchukbon and prescription is summarized to base on the Kyuji-tang. So new prescriptin of Chungoongkyuji-tang and Hwangkikyuji-tang is made by combine Kyuji-tang with Koongkihyangso-san and Bojoongikki-tang. The pharmacology in the interior disease of Soeumin is built up to devide to the weakness of Stomach-Qi, dyspepsia and invasion of cold-Qi. Descending Yin is the basic pharmacology of Sinchukbon and prescription is summarized to base on the Yijoong-tang. Sokunjoong-tang's pharmacology of abdominal pain is newly added and applided to Baekhaoyijoong-tang and Kwankyuboojayijoong-tang. The discourse of the symptoms and diseases at the Sasang Constitutional Medine is built up to base on the previous text in Gabobon and base on clinical experience in Sinchukbon. So clinical experience is the power of summarizing the pharmacology and escaping the previous pharmacology.

The Study about the Comparison of Korean-Western Medicine on Hair (모발에 대한 동서의학적 고찰)

  • Objectives : The purpose of this study is to compare hair and alopecia of Korean Medicine with those of western medicine. Methods : We studied relationships between hair and essense(精), qi(氣), blood(血), five vicera(五臟) and meridians(經絡) through literature review about hair and alopecia. We compared Korean medicine with western medicine on physiology of hair and treatment of alopecia based on the study. Results & Conclusion : 1. Congenital essense(先天之精) is related with genetic factor and acquired essence(後天之精) is connected with nutritional factor. Defending function of Defense qi(衛氣) is related with immune reaction and qi stagnation(氣鬱) is associated with stress reaction. Atrophy of vascular tissues observed in alopecia scalp means deep relationship between blood(血) and alopecia, further deficiency of blood can cause telogen effluvium. 2. Kidney qi(腎氣) is related with inhibiting combination of Androgen receptor and Dihydrotestosterone(DHT) or activating hair growth factior. Pi(脾) is connected with alopecia seborrheica caused by damp-heat(濕熱) and alopecia areata caused by excessive prudence(思慮過度). Heart(心) is associated with atrophy in vascular tissue of scalp and liver(肝) is connected with metabolism. 3. Armpit hair and pubic hair as secondary sex characteristics are realated with Yangming Meridian(陽明經) and beard and hair at crown part where Type II $5{\alpha}$-reductase is activated much are associated with Taiyang Meridian(太陽經). 4. Juglandis Semen pharmacopuncture and Ganoderma lucidum pharmacopucture have better effects on inhibit $5{\alpha}$-reductase than Finasteride. Minoxidil and PRP are similar with promoting blood flow and removing stasis(活血祛瘀). Seven-star needling(七星針) is similar with microneedling. 5. Alopecia can be caused by due to lack of circulation Views we need solution to improve circulation coincide in opinion with Korean Medicine and Western medicine.
