Correlation of ketone bodies in blood and spleen

  • Starvation, diabetes, alcoholism and hypothermia cause ketoacidosis in the human body; therefore, the cause of death can be determined by analyzing ketone bodies in the blood of the deceased. In the case of decomposition of the cadaver, however, since collecting intact blood is impossible, ketone body analysis is performed using the spleen. However, the index for diagnosing ketoacidosis is based on blood concentration, and its correlation with ketone bodies present in the spleen remains unknown. In particular, since decomposition proceeds rapidly during summer, when temperature and humidity are high, understanding the correlation between ketone bodies in the blood and spleen is important to estimate the state at the time of death from a decaying body. Therefore, in the present study, the correlation between ketone bodies in the blood and spleen of the deceased was explored. Ketone bodies (beta-hydroxybutyric acid [BHB] and acetone) in the blood and spleen were analyzed and compared from autopsies (>100 mg·L-1 BHB, blood basis) conducted at the Daejeon Forensic Research Institute from June to December 2021. Moreover, the concentration of ketone bodies in the spleen juice and tissues was compared assuming the scenario of extreme decomposition. Ketone retention concentration in the blood and spleen was positively correlated, and the ratio of BHB concentration in the spleen to BHB concentration in the blood ranged from 0.52 to 1.08 (mean = 0.85 ± 0.12), although the ratio may vary depending on the degree of decomposition of the corpse.

Simultaneous Determination of Plasma Lactate, Pyruvate, and Ketone Bodies following tert-Butyldimethylsilyl Derivatization using GC-MS-SIM

  • Lactate and ketone bodies are considered biological markers for ketosis and several inherited metabolic disorders. In the current study, the specific ratios of lactate and ketone bodies as analytical tools for differential diagnosis of various lactic acidosis were devised. The study included a protein precipitation step following tert-butyldimethylsilyl derivatisation. Total run time was approximately 30 min including sample preparation and GS/MS analysis. The limits of detection were below 0.1 pg/mL over the targeted 4 analytes. The calibration curve was linear over the concentration range of $0.001{\sim}250{\mu}g/mL$ for pyruvate, beta-hydroxybutyrate, and acetoacetate ($R^2$ > 0.99). Inter-day accuracy and precision were 87.7~94.8% with RSD of 2.5~5.7% at 2 levels. Absolute recoveries (%) of target analytes were 87.0~98.4%. The method was validated for the quantification of lactate and ketone bodies for differentiation of lactic acidosis.

Ketone ester supplementation of Atkins-type diet prolongs survival in an orthotopic xenograft model of glioblastoma

  • Heavy reliance on glucose metabolism and a reduced capacity to use ketone bodies makes glioblastoma (GBM) a promising candidate for ketone-based therapies. Ketogenic diet (KD) is well-known for its promising effects in controlling tumor growth in GBM. Moreover, synthetic ketone ester (KE) has demonstrated to increase blood ketone levels and enhance animal survival in a metastatic VM-M3 murine tumor model. Here, we compared the efficacy of a KE-supplemented Atkins-type diet (ATD-KE) to a classic KD in controlling tumor progression and enhancing survival in a clinically relevant orthotopic patient-derived xenograft GBM model. Our findings demonstrate that ATD-KE preserves body weight (percent change from the baseline; 112±2.99 vs. 116.9±2.52 and 104.8±3.67), decreases blood glucose (80.55±0.86 vs. 118.6±9.51 and 52.35±3.89 mg/dl), and increases ketone bodies in blood (1.15±0.03 mM vs. 0.55±0.04 and 2.66±0.21 mM) and brain tumor tissue (3.35±0.30 mM vs. 2.04±0.3 and 4.25±0.25 mM) comparable to the KD (results presented for ATD-KE vs. standard diet [STD] and KD, respectively). Importantly, the ATD-KE treatment significantly enhanced survival compared to the STD and was indistinguishable from the KD (47 days in STD vs. 56 days in KD and ATD-KE), suggesting that a nutritionally balanced low carbohydrate ATD combined with KE may be as effective as the KD alone in reducing brain tumor progression. Overall, these data support the rationale for clinical testing of KE-supplemented low-carb diet as an adjunct treatment for brain tumor patients.

Displacement of scan body during screw tightening: A comparative in vitro study

  • PURPOSE. The purpose of this study was to evaluate the occurrence of displacement while tightening the screw of scan bodies, which were compared according to the material type. MATERIALS AND METHODS. Three types of scan bodies whose base regions were made up of polyether ether ketone (PEEK) material [Straumann Group, Dentium Group, and Myfit (PEEK) Group] and another scan body whose base region was made up of titanium material [Myfit (Metal) Group] were used (15 per group). The reference model was fabricated by aligning the scan body library on the central axis of the implant, and moving this position by the resin model. The screws of the scan bodies were tightened to the implant fixture with torques of 5 Ncm, 10 Ncm, and a hand tightening torque. After the application of the torque, the scan bodies were scanned using a laboratory scanner. To evaluate the vertical, horizontal, and 3-dimensional (3D) displacements, a 3D inspection software program was used. To examine the difference among groups, one-way analysis of variance and Tukey's HSD post hoc test were used (α=.05). RESULTS. There were significant differences in 3D, vertical, and horizontal displacements among the different types of scan bodies (P<.001). There was a significantly lower displacement in the Straumann group than in the Myfit (PEEK) and Dentium groups (P<.05). CONCLUSION. The horizontal displacement in all groups was less than 10 ㎛. With the hand tightening torque, a high vertical displacement of over 100 ㎛ occurred in PEEK scan bodies (Myfit and Dentium). Therefore, it is recommended to apply a tightening torque of 5 Ncm instead of a hand tightening torque.

케톤음료를 보충한 저탄수화물·고지방식이 섭취가 치매고위험 노인의 인지기능 및 신체활동 능력 변화에 미치는 영향 (Effects of Low-carbohydrate and High-fat Diet Supplemented with Ketogenic Drink on Cognitive Function and Physical Performance in the Elderly at High Risk for Dementia)

  • Objectives: Reduced glucose utilization in the main parts of the brain involved in memory is a major cause of Alzheimer's disease, in which ketone bodies are used as the only and effective alternative energy source of glucose. This study examined the effects of a low-carbohydrate and high-fat (LCHF) diet supplemented with a ketogenic nutrition drink on cognitive function and physical activity in the elderly at high risk for dementia. Methods: The participants of this study were 28 healthy elderly aged 60-91 years showing a high risk factor of dementia or whose Korean Mini-Mental State Examination (K-MMSE) score was less than 24 points. Over 3 weeks, the case group was given an LCHF diet with nutrition drinks consisting of a ketone/non-ketone ratio of 1.73:1, whereas the control group consumed well-balanced nutrition drinks while maintaining a normal diet. After 3 weeks, K-MMSE, body composition, urine ketone bodies, and physical ability were all evaluated. Results: Urine ketone bodies of all case group subjects were positive, and K-MMSE score was significantly elevated in the case group only (p=0.021). Weight and BMI were elevated in the control group only (p<0.05). Grip strength was elevated in all subjects (p<0.01), and measurements of gait speed and one leg balance were improved only in the case group (p<0.05). Conclusions: We suggest that adherence to the LCHF diet supplemented with a ketogenic drink could possibly influence cognitive and physical function in the elderly with a high risk factor for dementia. Further, we confirmed the applicability of this dietary intervention in the elderly based on its lack of any side effects or changes in nutritional status.

Effects of a Very Low Carbohydrate (Pork Rind-Based) Diet on Weight Gain, Serum Levels of Cholesterol, Triacylglycerol, Glucose, Ketone Bodies and Insulin and Body Composition in Adult Rats

  • A study was carried out to determine the effect of a very low-carbohydrate diet on weight gain, body composition, and serum levels of cholesterol, triacylglycerol, glucose, ketone bodies and insulin. Twenty rats (mean initial weight, 212 g) were divided into two groups and each group was assigned a conventional high-carbohydrate diet (control) or a very low-carbohydrate diet containing 59.8% ground pork rind snack and fed the diet for four weeks. Average daily body weight gain was not different between the two groups during the first two weeks, but was significantly lower in rats fed the very low-carbohydrate diet than in those in the control group during third (p<0.05) and fourth weeks (p<0.01). Feedintake as well as energy intake was lower in rats fed the very low-carbohydrate diet than in those in the control group. The very low-carbohydrate diet reduced (p<0.01) serum triacylglycerol (34$\pm$83 vs 82$\pm$8 mg/l00 mL) and insulin (3.90$\pm$0.53 vs 7.60$\pm$0.61 $\mu$IU/mL) levels, while increasing (p<0.01) ketone body level (368$\pm$25 vs 236$\pm$24 $\mu$mol/L), compared with the control. Serum glucose and total cholesterol levels were not different (p>0.05) between the two dietary treatments. Proximate analysis of carcasses showed that the very low-carbohydrate diet decreased (p<0.01) body fat (26.1$\pm$1.04 vs 30.5$\pm$0.86%), while increasing (p<0.01) body protein (63.1$\pm$0.94 vs 59.4$\pm$0.70%) contents. Results indicate that short-term feeding of a very low-carbohydrate diet is beneficial for alleviating risk factors known to involve cardiovascular diseases or artherosclerosis. However, more studies with model animals as well as humans are recommended to examine the long-term health benefits of low-carbohydrate diets.

전남지방의 우유에 발생하는 준임상형 케토시스 조사 (Prevalence of Subclinical Ketosis in the Dairy Cows of Chonnam Area})

  • Ketosis can exist in both clinicla and subclinical forms. Detection of the subclinical form of ketosis by the use of a nitroprusside based test powder gas been shown to be a relatively simple and reliable procedure. Howere, very liffle is known about the indcidence or prevalence of the subclinical ketosis in Korea. In the present study, samples of urine taken from 288 dairy cows in Chonnam area were examined to demonstrate the presence of ketone bodies, using ross test. All the cows were within 4 weeks either before or after parturition. The gerd size was from 10 to more than 100 cows, and the cows did not show any clinical signs of ketosis. Of the 288 cows, 85 (29.5%) were positive to Ross test; of the positive cases 42.4% were +, 44.7% were ++, and the remaining 12.9% were +++. The prevalence increased from 3rd parity, peaked at 6th parity and thereafter decreased. The prevalence was higher after parturition than before parturition, with the highest occurrence during the 2nd week after calving. The prevalence was higher during winter. Also cows kept indoor throughout the year showed higher occurrence of the subclinical ketosis. The subclinical ketosis did not affect the specific gravity of the urine; however, increasing amount of ketone bodies in the urine decreased the pH of urine.

Effects of Starvation on Lipid Metabolism and Gluconeogenesis in Yak

  • This research was conducted to investigate the physiological consequences of undernourished yak. Twelve Maiwa yak ($110.3{\pm}5.85kg$) were randomly divided into two groups (baseline and starvation group). The yak of baseline group were slaughtered at day 0, while the other group of yak were kept in shed without feed but allowed free access to water, salt and free movement for 9 days. Blood samples of the starvation group were collected on day 0, 1, 2, 3, 5, 7, 9 and the starved yak were slaughtered after the final blood sample collection. The liver and muscle glycogen of the starvation group decreased (p<0.01), and the lipid content also decreased while the content of moisture and ash increased (p<0.05) both in Longissimus dorsi and liver compared with the baseline group. The plasma insulin and glucose of the starved yak decreased at first and then kept stable but at a relatively lower level during the following days (p<0.01). On the contrary, the non-esterified fatty acids was increased (p<0.01). Beyond our expectation, the ketone bodies of ${\beta}$-hydroxybutyric acid and acetoacetic acid decreased with prolonged starvation (p<0.01). Furthermore, the mRNA expression of lipogenetic enzyme fatty acid synthase and lipoprotein lipase in subcutaneous adipose tissue of starved yak were down-regulated (p<0.01), whereas the mRNA expression of lipolytic enzyme carnitine palmitoyltransferase-1 and hormone sensitive lipase were up-regulated (p<0.01) after 9 days of starvation. The phosphoenolpyruvate carboxykinase and pyruvate carboxylase, responsible for hepatic gluconeogenesis were up-regulated (p<0.01). It was concluded that yak derive energy by gluconeogenesis promotion and fat storage mobilization during starvation but without ketone body accumulation in the plasma.

혈장 중 케톤체의 옥심-TMS 유도체화 후 GC-MS/SIM을 이용한 분석 (Determination of plasma ketone body following oximation-trimethylsily| derivatization using gas chromatography-mass spectrometry selected ion monitoring)

  • 케톤체는 생체 에너지 생산과정이 탄수화물보다는 지방산의 의존도가 높을 때 생성되며, 과도한 분비는 당뇨병성 케토시스나 선천성 유전성 대사이상질환을 의심할 수 있는 근거가 된다. 따라서 이의 신속 정확한 분석법의 개발이 필요하다. 본 연구에서는 혈장을 제단백한 후 hydroxylamine을 가하여 60 ℃에서 30 분간 반응시켜 oximation 후 BSTFA를 가한 후 trimethylsilylation 유도체화하여 혈장 중에서 케톤체를 신속하게 정량할 수 있는 GC-MS/SIM 분석법을 개발하였다. 케톤체의 직선성의 범위는 0.001-250 μg/mL 이었고, 혈장에서의 검출한계는 0.1 pg 이었다. 직선성을 가지는 범위에서의 상관계수(R2)는 0.998-0.999이었고, 회수율은 1 μg/mL의 표준품을 첨가하였을 때 88.2-92.3 %, 10 μg/mL의 농도를 첨가하였을 때 89.5-94.8 % 였으며 RSD는 6.3-9.4 %였다. 이 분석법을 정상인과 케토시스 환자의 검체에 적용하여 벨리데이션 하였으며, 본 분석방법은 어린이나 성인의 당뇨성 케토시스나 여러 유전성대사질환 환자 중 케토시스를 보이는 환자의 혈장 중 β-hydroxybutyric acid/acetoacetic acid의 비를 계산하여 케톤체를 신속하고 효율적으로 임상검체 분석에 응용할 수 있음을 보여주었다.
