This study aims to identify the dietary patterns relevant to obesity of Korean women among low income classes. Adults 20-64 years were used as study subjects from the data of 2005 Korea National Health and Nutrition Examination Survey. We compared obese and normal-weight women in terms of their nutrients intake, diet quality and food patterns. Diet quality was assessed by using the Nutritional Adequacy Ratio (NAR) and Index of Nutritional Quality (INQ). Our results showed higher prevalence of obesity among lower socioeconomic status women. In men, there were no significant associations with socioeconomic status and prevalence of obesity. Higher risk of nutritional inadequacy was observed among obese women compared to normal weight women. Obese women showed significantly lower INQ for nutrients such as Ca, Fe, Vitamin A, Thiamin, Riboflavin and Vitamin C compared to other women. They consumed significantly higher amount of rice (p < 0.05) and lower amount of vegetables (p < 0.01). By contrast, obese men from low income classes showed higher intake of those nutrients. Obese men also consumed significantly higher amount of meats than normal weight men. Therefore, this study suggests that gender-specific approaches based on economic situation should be considered in developing the intervention program for managing obesity for low income classes.

The safety and security of food supply should be one of the primary responsibilities of any government. Estimation of nation's food commodity intakes is important to control the potential risks in food systems since food hazards are often associated with quality and safety of food commodities. The food intake databases provided by Korea National Health and Nutrition Examination Survey (KNHANES) are good resources to estimate the demographic data of intakes of various food commodities. A limitation of the KNHANES databases, however, is that the food intakes surveyed are not based on commodities but ingredients and their mixtures. In this study, reasonable calculation strategies were applied to convert the food intakes of the ingredients mixtures from the KNHANES into food commodity intakes. For example, Perilla leaf consumed with meat, raw fish, and etc. in Korean diets was used to estimate its Korean intakes and develop algorithms for demographic analysis. Koreans have consumed raw, blanched, steamed, and canned perilla leaf products. The average daily intakes of the perilla leaf were analyzed demographically, for examples, the intakes by gender, age, and etc. The average daily intakes of total perilla leaf were 2.03${\pm}$0.27 g in 1998, 2.11${\pm}$0.26 g in 2001, 2.29${\pm}$0.27 g in 2005, 2.75${\pm}$0.35 g in 2007, and 2.27${\pm}$0.20 g in 2008. Generally, people equal to or over 20 years of age have shown higher perilla leaf intakes than people below 20. This study would be contributed to the estimation of intakes of possible chemical contaminants such as residual pesticides and subsequent analysis for their potential risk.
The aim of this analysis was to investigate the relationship between the level of safety behavior and the level of oral health behavior among Korean children. Data used in this analysis were from the 2005 Korean National Health and Nutrition Examination Survey (KNHANES). A representative sample of 4,292 children aged from three to eleven years who completed both health interview and health behavior survey. Simple and multiple logistic regression analyses were conducted. All analyses were weighted and SAS 9.1 survey data analysis procedures were used to estimate standard errors accounting for the complex sampling design of the KNHANES. More than seventy percent of children did not keep general safety behaviors except seat on back seat in a car. About seventy five percent of children had experience of preventive oral care during last one year, but about fifty six percent of children brush their teeth twice a day. Socioeconomic status and health behaviors were significantly related in three to six old age group only (p<.05). Generally significant relationship was not found between general safety and oral health behaviors. Among Korean children, general safety and oral health behaviors might be not related each other. Education for general safety behaviors should be underlined for all children. Especially preschool-children with low socioeconomic status need to be educated for healthy behaviors.
This study aimed to identify a nutritionally vulnerable group and to examine their nutritional problems based on a relationship between socioeconomic position and nutritional status through life-course. A cross-sectional nationwide survey of 2005 Korean National Health and Nutrition Examination Survey (KNHANES) was used. A total of 8,930 participants aged $\geq$ 1 year were included. The socioeconomic position indicator was education level. Nutritional status was assessed by the percentage attainment of a dietary reference intake (DRI) and dietary quality based on nutrient intakes estimated by a 24 hour-recall data. Food insufficiency was examined by one-item food insufficiency questionnaire. The difference in nutritional status and food insufficiency according to educational level was tested by General Linear Model and Chi-square test, respectively. The nutritional status and food insufficiency was the worst during adolescence and older age than other period. Both quantity and quality of nutrient intakes was poorer in low education group than high education group. The prevalence of food insufficiency also was higher in low education group. The results were consistent across the life-course and sex. Based on these findings, we suggest that the development of various policy and strategies targeted to nutritionally vulnerable group is necessary to reduce nutritional inequality by socioeconomic position.
The objective of this study was to provide information regarding trends of rice consumption of Korean adults based on different meal types. Respondent reports 24-hour recall data from the Korean National Health and Nutrition Examination Survey were used to assess daily rice consumption and intake ratios of rice for different meal types and places of preparation. Rice intake had gradually decreased from 224.6 g in 1998 to 190 g in 2001 and to 179.4 g in 2005. The rice consumption of Korean adults decreased every year in all ages for all places of meal preparation in 2001 and 2005 compare to 1998. Analysis for each meal type showed that rice intake at breakfast had not considerably changed, but rice intake had decreased at lunch and dinner. While the rice consumption ratio at lunch and dinner decreased, it also decreased or did not change at snack times except for the 19-29 age groups. All the age groups revealed comparable change in the analysis for meal types. There was some diversity between all age divisions in daily rice intake depending on place of meal preparation. The rice consumption by place of meal preparation was generally highest at home, lowest at other places, but it decreased in all places. The rice consumption at home was highest in the over 50 age group, lowest in the 20-30 age group. These changes seem to be related to food intake patterns of rice and substitutional foods in the diets and development regarding socio-economic status. So the need for further study on differences in rice intake based on socioeconomic levels and age group are indicated.
Moderate alcohol consumption has been known to be associated with reduced risk for coronary heart disease (CHD). We assessed the association between alcohol consumption and CHD-related risk factors [hypertension, diabetes mellitus (DM) , high total cholesterol, high triglyceride (TG), low HDL-cholesterol (HDL-C), and high LDL-cholesterol (LDL-C)] in Korean. After excluding those with extreme intake values, the number of final subjects included in the analysis was 4,662 Korean adults aged over 20 years (1,961 men, 2,701 women) who participate in the 2005 Korean National Health and Nutrition Examination Survey. The subjects were divided into four or five groups; none-alcohol consumption group, moderate alcohol consumption group (<15 or 15.0-29.9 g/d), heavy alcohol consumption group (30-69.9 g/d or ${\geq}$ 70 g/d in men, ${\geq}$ 30 g/d in women). Odds ratios (ORs) were estimated from logistic regression adjusting for potential covariates. Alcohol consumption was inversely associated with low HDL-C in both men and women. However, heavy alcohol intake (${\geq}$ 70 g/d) significantly increased risk for hypertension, DM, and hypertriglyceridemia in men. The frequency of alcohol intake was also associated with CHD risk. The risk for low HDLC was decreased with alcohol consumption (${\geq}$ 1 times/wk), but frequent alcohol intake (${\geq}$ 4 times/wk) increased the risk for hypertension. This study revealed that moderate alcohol consumption has protective effect on CHD-related risk factors in Korean population.
This study investigated the health and nutritional characteristics according to household income level and obesity in Koreans aged over 50 years based on the 2005 National Health and Nutrition Examination Survey. Subjects were classified into 3 groups by average household income with reference to the minimum monthly living expenses (MLE): low (n = 319, < 100% MLE), middle (n = 222, < 200% MLE), high (n = 411, ${\geq}$ 200% MLE) and each group was compared by BMI index. With increasing income level, the prevalence of systolic hypertension and hyperlipidemia was increased. In the low income group, serum total cholesterol, triglycerides, and fasting glucose were higher in the obese compared with the normal. In the middle and high income groups, the prevalence of hyperlipidemia and diastolic hypertension were higher in the obese. Subjects had nutritional imbalance, such as inadequate intake of calcium and potassium. With increasing income level, the percentages of protein and fat to total calorie were increased in addition to the intakes and density of nutrients. The obese in the low income group had higher intakes of energy, protein, phosphorus and higher consumption frequency of cereals and potatoes compared with the normal. It was shown that the obese of the middle and high income groups tended to have lower consumption frequency of Korean cabbage and higher frequency of fruits. The obese of high income group also had binge drinking habit. Therefore, this study suggests that specific approaches based on economic status should be considered in developing nutrition education program for the elderly.

Background: The objective of this study was to evaluate the clinical applicability of the repeatability criteria recommended by the American Thoracic Society/European Respiratory Society (ATS/ERS) spirometry guidelines and to determine which factors affect the repeatability of spirometry in Korean adults. Methods: We reviewed the spirometry data of 4,663 Korean adults from the Korean National Health and Nutritional Examination Survey (KNHANES) Chronic Obstructive Pulmonary Disease Cohort (COPD cohort) and the Community-based Cohort Study VI-Fishing village/Islands (community cohort). We measured the anthropometric factors and differences between the highest and second-highest FVC (dFVC) and $FEV_1$ ($dFEV_1$) from prebronchodilator spirometry. Analyses included the distribution of dFVC and $dFEV_1$, comparison of the values meeting the 1994 ATS repeatability criteria with the values meeting the 2005 ATS/ERS repeatability criteria, and the performance of linear regression for evaluating the influence of subject characteristics and the change of criteria on the spiro-metric variability. Results: About 95% of subjects were able to reproduce FVC and $FEV_1$ within 150 ml. The KNHANES based on the 1994 ATS guidelines showed poorer repeatability than the COPD cohort and community cohort based on the 2005 ATS/ERS guidelines. Demographic and anthropometric factors had little effect on repeatability, explaining only 0.5 to 3%. Conclusion: We conclude that the new spirometry repeatability criteria recommended by the 2005 ATS/ERS guidelines is also applicable to Korean adults. The repeatability of spirometry depends little on individual characteristics when an experienced technician performs testing. Therefore, we suggest that sustained efforts for public awareness of new repeatability criteria, quality control of spirograms, and education of personnel are needed for reliable spirometric results.
Objectives: This study investigated the gender and age differential effect of major chronic diseases on activity of daily living (ADL) disability. Methods: Surveyfreq and Surveylogistic regression analyses were employed on the 2005 Korean National Health and Nutrition Examination Survey (KNHANES) with a sample of 3,609 persons aged 65 - 89. Results: After adjusting for potential covariates, stroke, among elderly men more so than women, had a 2-3 times greater odds of engendering ADL disability in the 65-69 (p < 0.05) and 70-79 age groups (p < 0.01). In comparison to elderly women, cancer, diabetes, and incontinence in elderly men was associated with a higher risk of ADL disability in the 70 - 79 age group (p < 0.05), and this association was also observed for pulmonary disease in the 80-89 age group. Among elderly women, however, a significant association between incontinence and ADL disability was identified in all three age groups. In addition, this association was found in pulmonary disease and diabetes in elderly women aged 70 - 79 years. Significant gender differences were observed in the association between stroke in the 60 - 79 age group and cancer in the 70 - 79 age group. Conclusions: Age and gender differences were observed in the effect of chronic diseases on ADL disability.
Objectives : The aim of this study is to summarize the current conditions and implications of health inequalities in South Korea. Methods : Through a literature review of empirical studies and supplementary analysis of the data presented in the 1998, 2001, and 2005 KNHANEs, we evaluated the extent and trends of socioeconomic inequalities in both health risk factors, such as smoking, physical activity, and obesity, and outcomes, such as total mortality, subjective poor health status by self-reports and metabolic syndrome. Relative risks and odds ratios were used to measure differences across socioeconomic groups, and the relative index of inequality was used to evaluate the changes in inequalities over time. Results : We found clear inequalities to various degrees?in most health indicators. While little change was observed in mortality differences over time, the socioeconomic gaps in risk factors and morbidity have been widening, with much larger differences among the younger population. Conclusions : Socioeconomic inequalities are pervasive across various health indicators, and some of them are increasing. The trends in socioeconomic inequalities in health should be carefully monitored, and comprehensive measures to alleviate health inequalities are needed, especially for young populations.
