Acne Treatment Cases with Hwangryeonhaedok-tang (황련해독탕가미방(黃連解毒湯加味方) 발효한약으로 치료한 여드름 환자 증례)

  • Objectives : Acne, one of the most common disorders in dermatology clinic, is a chronic inflammatory disease which has the symptoms of comedones, papules, pustules, cysts, nodules and scars mainly on the face. Although some of pathologic findings are suggested, but the exact causes and mechanisms are not yet known in Western Medicine. Hwangryeonhaedok-Tang(HRHDT) is an antiinflammatory, antipyretic and detoxifying herb decoction. In this report, we would like to share our experience of acne treatment with HRHDT. Methods : We had treated several patients with acne, facial flushing and uprising febrile sensation on face using oral administration and external application of HRHDT. HRHDT is basically made up with Scutellariae Radix, Coptidis Rhizoma, Phellodendri Cortex and Gardeniae Fructus. In this trial, several other herbs were added according to the individual patient's accompanying symptoms. After the decoction of herbs in amount for 10 days, $Yogourmet^{(R)}$ Kefir Starter 10 g was added to the herbal solution that was then fermented in an incubator at $25^{\circ}C$ for 72 hours, and divided into 30 doses. Results : Photographies were taken before and after the treatment. The severities of acne were evaluated with these photos according to the Korean Acne Grading System(KAGS). As results, we observed dramatic clinical improvements and the decreases of KAGS grades after the average treatment period of 8 weeks. The medication was orally administered 3 times a day and the external treatment was applied average twice a week during the whole treatment period. Conclusions : From these results, HRHDT may be considered as a good prescription for the febrile and toxic type of acne patients.

Acne Vulgaris Improved in Female Patients Diagnosed as 'Stagnation Pattern' Treated with Yukul-tang Gamibang and External Treatments: 2-Case Report (육울탕가미방(六鬱湯加味方)과 외치법(外治法)을 병용한 울증(鬱症)으로 변증된 여성 여드름 환자 치험 2례)

  • Objectives : Acne vulgaris is a common skin disorder prevalent among adolescence into adulthood, and its consequences can be detrimental especially for women. The purpose of this study was to investigate the effect of Yukul-tang Gamibang (YG) on female acne vulgaris. Methods : We treated 2 cases of female acne patients diagnosed as ‘Stagnation Pattern’ with herbal medication and external treatments. Herbal medication was orally administered 2 times a day and external treatments were applied once a week on average during the whole treatment period. Results : Photographs were taken at the start of each session, and the pictures of before and after the treatment period were compared. The severity of acne vulgaris was evaluated according to the Korean Acne Grading System (KAGS). We observed clinical improvement and decrease in KAGS grades after treatment. Conclusions : After taking YG, acne vulgaris was significantly improved in both patients. The results suggest that YG may be effective in treating acne vulgaris in female patients diagnosed as ‘Stagnation Pattern’.

Twenty-two Case of Acne Treated by Hwangryunhaedok-tang Pharmacopuncture Therapy Combined with Herbal Medicine and Extrusion (한약과 압출 치료에 병행한 황련해독탕 약침의 여드름 치료 22례 임상 보고)

  • Objectives : The objective of this study was to evaluate the effect of Herbal medicine, extrusion and Hwangryunhaedok-tang Pharmacopuncture therapy for treating Sil-Yol(Excess heat) type acne.Methods : Total of 22 patients with Sil-Yol(Excess heat) type acne were treated with the Herbal medicine(Chungsangbangpoong-tang, Yangkyuksanwha-tang, Hwalhyeolsano-tang, Galgeunhaegi-tang, Daeshihotang) and weekly Hwangryunhaedok-tang Pharmacopuncture & extrusion therpay. By the standardized photography before and after treatment, KASG and survey for patient`s satisfaction, we checked effect of Herbal medicine and Hwangryunhaedok-tang Pharmacopuncture therapy for treating acne.Results : 1. In 22 patients who underwent the Herbal medicine, extrusion and Hwangryunhaedok-tang Pharmacopuncture therapy, there was an significant improvement in KASG. Mean value of KAGS before treatment is 3.59±0.80, mean value of KAGS after treatment is 1.95±65.2. The responses to the survey for satisfaction revealed that 5 patients were very satisfied with their outcomes, 11 patients were satisfied, 5 patients were somewhat satisfied, 1 patient was dissatisfied.Conclusion : In Korean medicine, Herbal medicine and Hwangryunhaedok-tang Pharmacopuncture therpay might be effective method for treating Sil-Yol(Excess heat) type acne.

A Clinical Study about the effect of Bojoongikki-tang on Acne (보중익기탕(補中益氣湯) 가미방(加味方)으로 치료한 여드름 환자의 임상 보고)

  • Objective : This study is to investigate the effect of Bojoongikki-tang on Acne patients Methods : We treated 3 acne patients diagnosed as the deficiency of Qi with Bojoongikki-tang(Buzhongyiqitang), comedon extraction by needle and stamp acupunctures. After treatments we compared the before and after photos, and estimated by Korean Acne Grading System(KAGS1-4). Results & Conclusions : After taking Bojoongikki-tang(Buzhongyiqi-tang) and extruding comedones by needle, the acne was improved. After stamp acupuncture, the acne scar was improved. This study shows that the acne can possibly be diagnosed as the deficiency of Qi. So further studies are needed continuously.

Case Study of Treating Acne Scar Using Scar Regeneration Acupuncture Therapy and Micro-needle Therapy (흉터재생침과 미세피부침을 이용한 여드름 흉터의 치험례)

  • Objectives : There are numerous methods in treating acne scar. On the contrary there are hardly any cases on treating acne scars through Korean medicine. By observing we compared how much acne scar was treated by scar regeneration acupuncture therapy and micro-needle therapy treatment through observing pre and post case photos, KAGS, KCADI and patient satisfaction. Results : By comparing pre and post procedure through acne scar grade criteria, total of 5 patients showed improvement and could see distinct change in skin comparing case photos. After comparing pre and post procedures, total of 5 patients showed improvements in figures of Korean acne index. 4 patients replied very satisfied and 1 replies satisfied on patient satisfaction survey. Conclusions : After observing patients satisfaction, and comparing case photos, acne scar grade criteria, korean acne index, it can be considered that scar regeneration acupuncture therapy and micro-needle therapy done in our clinic to be valid treatment.

A Case Report of Soeumbyeong Acne Vulgaris Patient Treated with Osuyu-tang (Wuzhuyu-tang, 吳茱萸湯) (오수유탕(吳茱萸湯)으로 호전된 소음병(少陰病) 여드름 1례)

  • Objective : This study aims to explain the effect of Osuyu-tang on an acne vulgaris which a Soeumbyeong patient has. Methods : This is a case report on a female patient who has been suffering from acne vulgaris which is not controlled by normal dermatological treatments. According to Diagnostic system based on Shanghanlun provisions, Osuyu-tang was provided for fifty days. Results : After a series of Osuyu-tang treatments, the patient's symptoms and result of KAGS improved. Conclusions : This case study showed an effectiveness of using Osuyu-tang according to Diagnostic system based on Shanghanlun provisions on acne.

  • Objective : This study was carried out to analyze the trends of Acne related articles that have been published in the Journal of Korean Medicine Ophthalmology, Otolaryngology & Dermatology(JKOOD). Method : We studied 29 research papers relevant to Acne that published in JKOOD from 1998 to 2013. All the paper were classified into three categories; original article, review article and case report. We sub-classified the original articles into three categories; descriptive study, analytic study and experimental study. We analyzed the case report according to various types of treatment. Results : 1. The number of searched journals is 29 papers; 17 original ordicles, 4 review articles, 8 case reports. 2. Classification of 17 original articles into three categories; 10 experimental studies, 7 descriptive studies, 0 analytic study. 3. Method of Evaluation; Korean Acne Grading System(KAGS) is used frequently in descriptive studies and experimental studies. 4. Treatment in Case reports; 7 Case reports used External medical treatments. Conclusions : It is needed to develop the methods of grading system in acne related to Oriental medicine to obtain objectivity of studies. The number of analytic study, clinical trial and experimental study is need to increase.

A Review on Acne Treatment in Korean Medicine by Analyzing Case Studies with Herbal Medicine Treatment (국내 한약 치험례 분석을 통한 한의학적 여드름 치료법에 대한 고찰)

  • Objectives : The purpose of this study is 1) to explore the general tendency of the herbs or herbal formula for acne treatment in clinical practice 2) and to provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism by analyzing the herbs or herbal formula using for acne treatment in clinical practice. Methods : In the domestic databases (Oriental medicine Advanced Searching Integrated System-OASIS, Korean Traditional Knowledge Portal-KTKP, National Discovery for Science Leader-NDSL, Research Information Sharing Service-RISS), we selected among the papers published using search terms related to "acne". Reports related to sasang constitutional medicine were excluded and a total of 23 papers were finally selected. Results : 29 prescriptions were retrieved from 23 papers. Chungan-tang (淸顔湯) notified formulas ranked first by Cheongsangbangpung- tang (淸上防風湯), Hwangnyeonhaedok-tang (黃連解毒湯), Dangguijakyaksan (當歸芍藥散) and Bipachungpe-eum (枇杷淸肺飮). Commonly used herbs are Forsythiae Fructus (連翹), Scutellariae Radix (黃芩), Gardeniae Fructus (梔子), Ligustici Rhizoma (川芎), and Angelicae Gigantis Radix (當歸). In most patients with inflammatory lesions or excessive flushing, exterior-releasing medicinal (解表藥) and heat-clearing medicinal (淸熱藥) were mainly prescribed for acne treatment, while in patients with acne accompanied hyperkeratinization, Excess sebum secretion, Dysmenorrhea or indigestion, qi-tonifying medicinal (補氣藥) or blood-tonifying medicinal (補血藥) were mainly used. KAGS (Korean Acne Grading System) was the most commonly used method for acne severity assessment. Conclusions : These findings suggest that the herbs or herbal formula for acne treatment in Korean Medicine is prescribed differently according to the cause and symptoms of acne in a unique way for control the whole body balance. Also, our findings could provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism in clinical practice.

Effect of Acupuncture on Inflammatory Lesions and Body Image Disturbance in Patients with Acne Vulgaris (염증성 여드름 환자에 대한 침 치료 효과와 신체이미지 인식에 대한 연구)

  • Objectives : The purpose of this study is to evaluate the efficacy of acupuncture treatment on the inflammatory lesions and body image disturbances in patients with vulgaris acnes. Methods : Fifteen inflammatory acne patients were enrolled and treated with facial acupuncture during 4 treatment sessions. We evaluated the number of papules and nodules using the Korean Acne Grading System (KAGS). We also evaluated patients' general subjective satisfaction and objective changes of inflammatory acnes symptoms using DermaVision-pro. Moreover, we observed the skin quality-of-life scale with skindex-29 and body image disturbance questionnaire (BIDQ) as secondary outcome. Adverse events were recorded as well at every visit. Results : After 4 treatment sessions, there were significant reductions in the number of papules and nodules of the patients. We found that subjective satisfactions of patients were enhanced as well as the objective inflammatory symptoms. We also found significant improvements in the quality of life and BIDQ. No serious treatment-related adverse events were reported. Conclusions : This study demonstrated that standardized facial acupuncture were effective and safe in the treatment of facial inflammatory acnes.
