Gleason's Grading of Prostatic Adenocarcinoma: Inter-Observer Variation Among Seven Pathologists at a Tertiary Care Center in Oman

  • Prostatic adenocarcinoma is the commonest solid malignancy seen in Omani elderly males 60-80 years of age. The Gleason grade is the most widely used grading system for prostatic carcinoma and is recommended by the World Health Organization. A peer review was carried out at the Pathology Department of Sultan Qaboos University Hospital (SQUH), Oman, to assess the quality of reporting at the center. The aim of this study was to determine inter-observer variation among 7 pathologists working at a tertiary care center in Oman. A total of 47 consecutive prostatic biopsies were interdependently reviewed by seven pathologists and the results obtained were compared with each other and the original diagnosis. This peer review indicated a fair inter-observer agreement (0.482) among 7 pathologists in the department, with fair to moderate agreement when the results were compared to the reported diagnosis, comparable to the published literature. Dual and sub-specialty reporting are being instituted to improve the performance in this vital aspect of pathology.

The Clinical Study on 88 Patients of Cervical Disc Herniation (경추 추간판 탈출증 환자 88례에 대한 임상적 고찰)

  • Objectives : This study is to survey the effectiveness of the oriental medicine treatment on cervical disc herniation. Methods : The clinical study was done on 88 cases of patients with cervical disc herniation diagnosed by M.R.I, symptoms and physical test who admitted in Ja-seng Oriental Medicine Hospital from January 2006 to March 2008. After treatment, the efficacy was evaluated according to the criteria of Robinson et al. Results : The clinical results were satisfactory as excellent in cases, good in cases, fair in cases, 92.05% of all were improved above good state Conclusions : Oriental medicine treatment is effective on the recovery of cervical disc heniation.

  • Objectives : The propose of this study was to observe the correlation between Cervical hypolordosis and radiological result. Methods : We randomly selected among the 110 patients with X-ray and C-spine MRI films who have visited Jaseng Hospital of Oriental Medicine with neck pain. Radiographic measures of cervical lordosis and herniated disc were collected, and statistically analyzed. Results : In this study, if the finding of a X-ray showed straightening of cervical lordotic curve, based on MRI finging, the amount of herniation was more severe. Conclusions : There was a significant correlation between Cervical hypolordosis and herniated disc. Hypolordosis group complained a severe herniated disc.

  • Patients with traumatic aortic rupture rarely reach the hospital alive. Even among those who arrive at the hospital alive, traumatic aortic rupture after high-speed motor vehicle accidents leads to a high in-hospital mortality rate and is associated with other major injuries. Here, we report a rare case of descending midthoracic aortic rupture with blunt diaphragmatic rupture. Successful management with emergency laparotomy after an immediate endovascular procedure resulted in a favorable prognosis in this case.

  • In the present study, we intended to report a clinical pediatric case of thelaziasis in Korea. In addition, we briefly reviewed the literature on pediatric cases of thelaziasis in Korea. In the present case, 3 whitish, thread-like eye-worms were detected in a 6-year-old-boy living in an urban area and contracted an ocular infection known as thelaziasis incidentally during ecological agritainment. This is the first report of pediatric thelaziasis in Seoul after 1995.

  • Neurolymphomatosis (NL) is a rare disease characterized by lymphomatous invasion of the cranial or peripheral nerves by lymphoma. A high suspicion is important due to the various presenting symptoms mandating consideration of many differential diagnoses. We report a case of NL of the cranial nerves and plexus presenting as diplopia, facial palsy, and weakness of the upper and lower limbs in sequence.
