Second, according to the intention of participation, there was a statistically significant difference in user satisfaction at Military Hospital Information System. Third, according to a level of dexterity at Military Hospital Information System, there was statistically significant difference only in job satisfaction. Fourth, factors to influence system satisfaction were one's duty, using period of Military Hospital Information System, and a level of dexterity at Military Hospital Information System. Factors to influence job satisfaction were a level of dexterity at Military Hospital Information System, one's duty and intention of participation. Factors to influence information satisfaction were one's duty, a level of dexterity at Military Hospital Information System, and intention of participation. As the first study on user satisfaction at Military Hospital Information System, this study suggests the followings. • Background: Over the last few decades, because hospitals in South Korea also have undergone dramatic changes, Korean hospitals traditionally have provided specialized health care services in the health care market. Inner Herfindahl-Hirschman Index (IHI) measures hospital caseloads based on patient proportions, independent of patient volumes. However, IHI that rely solely on patient proportions might be problematic for larger hospitals that provide a high number of diagnosis categories, as the patient proportions in each category are naturally relatively smaller in such hospitals. Therefore, recently developed novel measure, category medical specialization (CMS) is based on patient volumes as well as patient proportions. Methods: We examine the distribution of hospital specialization score by hospital size and investigate association between each hospital specialization and length of stay per case and hospital cost per case using Korean National Health Insurance Service-cohort sample data from 2002 to 2013. Results: Our results show that IHI show a decreasing trend according to the number of beds and hospital type but CMS show an increasing trend according to the number of beds and hospital type. Further, inpatients admitted at hospitals with higher IHI and CMS had a shorter length of stay per case (IHI: B=-0.104, p<0.0001; CMS: B=-0.044, p=0.001) and inpatients admitted at hospitals with higher IHI and CMS had a shorter hospital cost per case (IHI: B=-0.110, p=0.002; CMS: B=-0.118, p=<0.0001). Conclusion: This study may help hospital policymakers and hospital administrators to understand the effects of hospital specialization strategy on hospital performance under recent changes in the Korean health care environment.

  • Purpose: The purpose of this study was to investigate the extent of empowerment, task performance and job satisfaction perceived by hospital nurses and the relationship among those variables. Methods: The sample consisted of 261 nurses from a university affiliated hospital. Data were collected with self-administrated questionnaires including empowerment, performance and job satisfaction and analyzed using mean, standard deviation, ANOVA and $Scheff{\acute{e}}$ test. Results: The mean scores of variables were as follows. Empowerment was 4.6472, task performance was 3.7814, and job satisfaction was 3.1240. The older, more educated and more experienced nurses showed more empowered and higher job performed than the younger, less educated and less experienced nurses. Empowerment had a positive correlation with task performance and job satisfaction. Conclusions: The results imply that hospital should develop the empowerment program for nurses to be more job performed and satisfied.

  • Background: Patients were subjected to post-discharge follow-up (by telephone) in order to investigate the potential complications of outpatient general anesthesia or deep sedation that could develop in disabled dental patients discharged from the hospital. The ultimate aim of this study was to establish an appropriate response measure for such complications. Methods: The caregivers of 79 disabled patients who underwent dental procedures under general anesthesia at our outpatient clinic were interviewed over telephone. Necessary care instructions were provided during the phone calls when required. The patient satisfaction level regarding the telephonic follow-up care was surveyed by additional telephone calls. Results: Most of the patients did not suffer any serious complications; however, some reported fever and bleeding. The data obtained in this study can be utilized towards the development of caregiver education pertaining to the ambulatory general anesthesia of dental patients with disabilities. Conclusions: Additionally, we hope that the findings of this study will help minimize the effects of complications experienced by disabled dental patients undergoing ambulatory general anesthesia, as well as increase the overall patient satisfaction level.

  • The purpose of this study is to report a clinical progress of sudden hearing loss through korean medicine. Between 26th Mar. 2014 and 19th Apr. 2014, she was treated with Chuna manual therapy every day and observed by Pure Tone Audiometry (before and after) and visual analogue pain scale (VAS). A patient was diagnosed with sudden hearing loss on 23th Dec. 2013 in Severance Hospital, she had sudden hearing loss on left side. She had 19 times treatments. At the first of treatment, her audibility was 20 dB on right side, 74 dB on left side and she had tinitus and ear fullness (VAS 8 / VAS 9). At the middle of treatment (2nd Apr. 2014), symptoms of tinitus and ear fullness started improving. State was tinitus VAS 6, ear fullness VAS 6. At the end of treatment (21th Apr. 2014), state was tinitus VAS 4, ear fullness VAS 4, and recorded 35 dB on right side, 80 dB on left side in pure tone audiometry. Chuna manual therapy is estimated to be good for sudden hearing loss especially on tinitus and ear fullness. More cases are required to develop treatment of sudden hearing loss.

  • Objectives : The perpose of this study was to observe the correlation between lumbar lordotic angle and radiological result. Methods : We randomly selected among the 150 patients with lumbar X-ray and MRI films who have visited Jaseng Hospital of Oriental Medicine with low back pain. Radiographic lumbar lordotic angle and lumbar HIVD were collected and stastically analyzed. Results : In this study, if the finding of a X-ray showed straightening of lumbar lordotic curve, based on MRI finding, the number of HIVD increased. Conclusions : There was a significant correlation between lumbar hypolordosis and HIVD, hypolordosis complained mare HIDVs.

  • A 67-year-old male patient came to the hospital due to lung cancer and mediastinal cystic mass which was suspected to be esophageal duplication cyst. Video-assisted thoracoscopic surgery (VATS) was performed and intra-operative finding suggested it as a cystic mass along the thoracic duct. Thoracic duct was ligated and the cyst was completely resected. A 48-year-old female patient visited the hospital for dysphagia. Mediastinal cystic mass was suspected to be an esophageal duplication cyst. Intraoperative finding suggest a thoracic duct lymphangioma. After thoracic duct ligation, the mass was completely resected with VATS. Postoperative chylothorax did not develop in both cases.
