외상성 상완신경총 손상에 대해 온침요법과 봉독약침요법을 중심으로 한방복합치료를 적용한 치험 1례 증례보고 (Treatment of Traumatic Brachial Plexus Injury with Traditional Korean Medicine Focusing on Warm Needling and Bee Venom Injection: A Case Report)

  • Objectives : The purpose of this report is to show the effect of complex traditional Korean medical care focused on warm needling(WN) and bee venom(BV) injection for a patient suffering from traumatic brachial plexus injury(BPI). Methods : A 51-year-old female patient with BPI was suffering from paresis and hypoesthesia with upper-extremity impairment. The patient had been treated once a day from 4 Feb to 11 Apr of 2015 mainly with WN and BV injection on the acupoints near the distribution of the brachial plexus. Improvement of the symptoms was measured by range of motion(ROM), manual muscle test(MMT) grip strength, and with the numeric rating scale(NRS) of tactile and pain sensitivity. Results : After 67 days of treatment, all obtained results showed remarkable improvement. The patient's satisfaction declaration was recorded through an interview at the end of the treatment. Conclusions : The results suggest that anatomically concerned WN and BV treatment can be a valuable option in reducing or normalizing pains from traumatic BPI.

알레르기性 鼻炎에 對한 文獻的 考察 (The Bibliogrphical Study on the Allergic Rhinitis)

  • The study has been carried out to investigate of the Allergic Rhinitis by referring to 87 literatures. The results were as follows; 1. In oriental medical science, Allergic Rhinitis is belong to the category of the 'BiGu'(鼻구) 'GuChe'(구체). The 'Gu'(구) of the BiGu means watery rhinorrhea, the 'Che'(체) of the GuChe means sneezing. 2. The cause of a disease summarize the weak of Lung, Spleen and Kidney, and invasion into the nasal cavity of PoongHan etc a wrong air. Sometimes the pathologial change appear PoongHan changeHwa(風寒化火), HwaYeol is hidden in the inside(火熱內伏). The contributing factors are found a season(spring, winter), an abnormal weather(運氣 : 少陰 少陽 陽明 司天, 歲金不及), an emotional stress, an external wound of the harmful air, a food allergens and fatigue, a contact of substances, a sunlight etc. 3. Predominant symptoms are watery rhinorrhea, sneezing and nasal obstruction. Sometimes accompanic symptoms are nasal bleeding, mucopurulent rhinorrhea, olfactory disturbance, nasal polyp, rhinolalia clausa, respiratory disfunction etc. 4. The treatment-methodes is as follows, OnBoPaeJang GeoPoongSanHan(溫補肺臟 祛風散寒), GeonBilkGi(健脾益氣), BoSinNabGi (補腎納氣). The treatmentherbs is as follows, OnBoJiLuDan GaGam(溫補止流丹 加減), OkByeongPoongSan plus ChangIJaSan GaGam(玉屛風散合 蒼耳子散 加減), BoJungIkGiTang GaGam plus SoCheongLyongTang(補中益氣湯加減 配合 小靑龍湯), SinGiHwan GaGam(腎氣丸加減), GaeJiTang(桂枝湯) etc. 5. The external treatment is as follows, JeokBi(滴鼻), ChuiBi(吹鼻), SaekBi(塞鼻), stick and herbs-injection on the acupuncture-point, pressure ear acupuncture-point, herbs-pillow etc. 6. The acupuncture-moxa treatment is as follows, the methodes of cure apply TongJoGyeongGi(通調經氣), SanTongBiGui(宣通鼻竅) etc. Predominent acupuncture-points are YoungHyang(迎香), InDang(印堂), BiTong(鼻通), SangSeong(上星), HabGok(合谷) and so on. As mentioned above, from now on, it's need to the oriental medical scientific study of the Immunity and Allergy and to the external treatment's application for the ascent of the treatment-effect of the allergic disease.

국내 한방병원의 IRB 및 임상시험 실태조사 (Current Status of Institutional Review Boards and Approvals of Clinical Research in Oriental Medical Hospitals in Korea: A Survey)

  • Objective: To investigate institutional review boards and approved clinical trials of Oriental medical hospitals in Korea Methods: We e-mailed 16 Oriental medical hospitals a survey consisting of questions that addressed two topics, institutional review boards and the clinical trials they approved. The first part included questions about whether each hospital had an institutional review board, that board's staffing and education, reviews, and expedited reviews. The clinical trials portion covered the number and types of approved clinical trials, treatment methods, whether an investigator or a sponsor initiated the trials, diseases, time frame, and clinical trial fee. Results: We received a response to our e-mail from 14 hospitals, all of which had an institutional review board. The average number of institutional review board members was 13.57. Of these Oriental medical hospitals, 70% and 49% said that they regularly educated their institutional review board staff and clinical trial, investigators, respectively. 79% of the hospitals claimed to conduct regular reviews, and 50% said that they did so monthly. The number of approved clinical trials increased sharply from 11 in 2005 to 102 in 2008. 68% of these clinical trials were randomized, and the most-used treatment methods were herbal medicine (35%) and acupuncture (29%). The most common target diseases were circulatory (19%), urinogenital (14%), and musculoskeletal (13%) disease. Conclusions: Despite the rapid increase in clinical research in oriental medical hospitals, many more efforts including raising IRB quality, varying research diseases and increasing clinical trials in the hospitals located in non-metropolitan area, should be made.

벨마비 환자의 한의학적 치료 시작시기에 따른 신경손상률 비교 : 후향적 관찰 연구 (Facial Nerve Damage in Bell's Palsy according to When Korean Medical Treatment was Started : A Retrospective Study)

  • Objectives : Although integrative Korean medical treatments have been suggested to be effective for treating Bell's palsy, the effect of Korean medical treatment according to when treatment was started is unknown in the clinical field. Therefore, this study was conducted to determine the results of treating Bell's palsy according to different starting points of integrative Korean medical treatments. Methods : We screened patients who visited the Facial Palsy Center at Kyung Hee University Hospital for Korean and Western combined medical treatment from March 2011 to February 2016. A total of 821 out of 2086 patients were studied, including their basic characteristics and results of an electromyography. Patients who started Korean medical treatment within 3 days of onset were placed in group A, within 9 days of onset in group B, and patients who started treatment after 10 days of onset were placed in group C. We tried to compare the level of facial nerve damage by electromyography between groups. Results : The patients in group C had the highest axonal loss rates in all branches(frontal, oculi, nasal, oris). The post hoc analyses revealed the difference of axonal loss rates between group A and B was not statistically significant. Only group C showed statistically higher axonal loss rates in all branches. Conclusion : The study results showed that the patients who had delayed Korean medical treatments had a higher level of facial nerve damage by electromyography. According to the results of this study, early application of integrative Korean medical treatment is suggested.

피부 열전도 온도에 근거를 둔 거짓 뜸 개발 및 평가 연구 (Credibility of a Newly Developed Sham Moxibustion)

  • Background : To demonstrate the efficacy of moxibustion therapy, randomized controled trials are required. But, clinical trials of moxibustion had limitations due to the absence of a sham moxibustion model for an appropriate placebo. Objectives : To develop a new sham moxibustion model based on the thermal characteristics of commercial indirect moxibustion, especially temperature, and to evaluate whether it could be applied in clinical trials. Methods : By applying heat insulation, we created a sham moxibustion device that was indistinguishable from a real one with the naked eye. It also stimulated heat but had inert remedial value. A clinical trial was performed on subjects to test double blinding. The subjects were randomly assigned into two groups, a treatment group and a sham group. Acupoint Zusanli($ST_{36}$) was used in each group for 3 times. A sham acupuncture credibility questionnaire was modified into a moxibustion credibility questionnaire and was filled out after treatment. Results : No major difference was detected in the subjects' baseline data. Most subjects and practitioners could not distinguish the sham moxibustion device from the real one. But, subjects who had experience of moxibustion therapy more likely to distinguish the sham moxibustion device from the real one than subjects who didn't have experience of moxibustion therapy. The treatment group showed a significant difference in the VAS(Visual Analog Scale) for intensity of sensation during treatment than that of the sham group. Conclusions : The sham moxibustion device in this study is proved sufficient and credible to be applied in investigations of the effect of moxibustion. But it is more appropriate for the people who don't have experience of moxibustion therapy.

Alternation of Topical Heat and Cold for Chronic Low Back Pain : A Randomized Controlled Pilot Trial

  • Objectives : This is a pilot study for a large randomized controlled trial to investigate the efficacy and safety of a newly developed contrast therapy device-- alternating topical heat and cold -- for patients with chronic low back pain. The main objective of this study is to confirm the feasibility of the study design. Methods : The design was a randomized, 2-arm, parallel-group, single-blind, placebo controlled trial. Patients in each group received real or sham contrast therapy in an acupuncture point 10 times over four weeks. The primary outcome measure was pain intensity on a 100-mm visual analogue scale (VAS). The secondary outcomes were back-related dysfunction based on the Oswestry Disability Index (ODI), the Roland-Morris disability questionnaire (RMDQ), and range of motion of lumbar spine based on the modified Schober test (mSchober test), Finger-to-Floor distance (FTF distance), and Finger-to-Thigh distraction (FTT distraction). Results : A total of 30 subjects with chronic low back pain were randomly assigned to a contrast therapy group (n=15) or a sham group (n=15). A repeated-measures analysis of variance showed statistically significant group time interaction for VAS, RMDQ, mSchober test and FTF distance (p<0.05). The treatment group showed significant improvement in pain intensity and functional disability as compared to the sham group. Conclusion : Contrast therapy may be an effective and safe treatment for chronic low back pain.

Puerariae Radix Induces Angiogenesis in vitro and in vivo

  • Background & Objective : Angiogenesis consists of the proliferation, migration, and differentiation of endothelial cells, and angiogenic factors and matrix protein interactions modulate this process. The aim of this study was to determine whether Puerariae radix could induce angiogenic activity in human umbilical vein endothelial cells (HUVECs). Methods: The angiogenic activity of Puerariae radix were evaluated by using BrdU assay, chemotactic migration assay, tube formation assay, measurement of bFGF in HUVECs, and Matrigel plug assay in mice. Results : Puerariae radix significantly increased HUVECs proliferation in a dose-dependent manner. In addition, Puerariae radix increased migration and tube-like formation in HUVECs. Interestingly,the expression of basic fibroblast growth factor (bFGF), an angiogenesis-stimulating growth factor, was dose-dependently increased by Puerariae radix. The angiogenic activity of Puerariae radix was confirmed using an in vivo Matrigel angiogenesis model, showing promotion of blood vessel formation. Conclusion : Puerariae radix significantly induces angiogenesis in vitro and in vivo. These results suggest that Puerariae radix is a potent angiogenic agent, and a promising drug, for the induction of neovascularization.

침을 이용한 국소 피부주름 치료에 대한 연구 (Study on the Local Wrinkle Cure Using Acupuncture Needle)

  • The wrinkle is a typical sign showing aging. These days lots of people want to have a young and healthy appearance and many medical doctors are studying about wrinkle cure. In fact a young face without wrinkle looks more healthy and energetic. Therefore the wrinkle cure is important at the point of medical view. This research is an investigation of several studies about wrinkle cure using acupuncture needle. We have to consider the problem of the whole body which is related 12 meridians and meridian muscles as well as topical skin and tissues. There are three parts affecting wrinkle formation and cure. The first is a meridian and meridian muscle, the second is a local muscle and fascia on the head and neck, the third is a local skin structure. There are three kinds of method to cure the wrinkle locally. The first one is the perpendicular treatment of acupuncture, called the microneedle therapy, the process of this method is that lots of microneedles stab in the skin to dermis. The second one is the transverse treatment of acupuncture. The process of this method is that an acupuncture needle inserts just under the wrinkle skin. The third one is the incision treatment of acupuncture, called subcision, the process of this method is to cut the fibrous band which connect from the facial muscle to SMS(superficial musculoaponeurotic system) using cuttable needle. The hematoma after treatment let a collagen increase.

Effect of Traditional Korean Medicine Treatment Including Acupotomy on the Level of Pain and Quality of Life of Patients with Cervical Herniated Intervertebral Disc: A Retrospective Observational Study

  • Background: The purpose of this study was to statistically analyze the treatment effect and patient satisfaction of traditional Korean medicine, including acupotomy treatment of cervical herniated intervertebral disc. Methods: This was a retrospective study of 22 patients who received traditional Korean medicine including acupotomy treatment amongst all patients diagnosed with cervical herniated intervertebral discs at the Korean Medicine hospital in Daejeon, Korean, from January 01, 2020 to April 30, 2021. The clinical data from patient medical records were statistically analyzed. Results: The Numeric Rating Scale, the European Quality of Life 5 Dimensions, and the European Quality of Life Visual Analogue Scale questionnaire scores were each compared before and after traditional Korean medicine treatment, including acupotomy, showed significantly improved scores after treatment (p < 0.001). Out of a total of 22 patients, 11 rated the treatment "very satisfactory" (50%), 5 "satisfactory" (22.7%), 5 "indifferent" (22.7%), 1 "unsatisfactory" (4.5%), and 0 "very unsatisfactory" (0%). Of the total 22 patients, 17 patients (77.3%) were willing to have further treatment, and 5 patients (22.7%) were not willing. Conclusion: Traditional Korean medicine treatment including acupotomy was an effective treatment for cervical herniated intervertebral discs. To determine the effect of an individual application (e.g., acupotomy) of traditional Korean medicine treatment for cervical herniated intervertebral disc, a prospective, controlled study is needed.

침에 대한 정서와 인지요소 분석 (An Analysis of Emotional and Cognitive Factors on Acupuncture)

  • Objectives : Placebo phenomena have been considered as a confounding factor of clinical trial. Expectancy and belief of acupuncture have not been evaluated quantitatively. The present study was performed to analyze the emotional and cognitive factor .of acupuncture and investigate whether the expectancy of acupuncture treatment is associated with the cognition of acupuncture. Methods : The expectancy and the perception of bodily sensation (PBS) of 22 participants were assessed using self-reported questionnaire. The subjects used the self assessment manikin (SAM) to rate each of the standard affective image of the international affective picture system (lAPS) and other acupuncture-related image. Based on the degree of expectancy, the high expectant (HE) and the low expectant (LE) group were classified. The thermal and pressure pain threshold was objectively evaluated using radiant-heat device and algometer. The degree of expected pain of acupuncture and the actual pain of painful stimulation was subjectively evaluated using facial pain scales (FPS). Results : Using SAlVI analysis, we identified the negative correlation between hedonic valence and arousal dimension on acupuncture-related visual cue. The degree of the PBS and general pain threshold did not show any significant difference between the HE and the LE group. The HE group rated the acupuncture images as more pleasant, more arousing, than the LE group. In addition, we also found that the higher expectancy marked the lower FPS of the expected pain of acupuncture, but not of the actual pain of painful stimulation. Conclusions : Our preliminary study identified the psychological dimensions of acupuncture-related visual cue. These findings indicate that the expectancy of acupuncture could affect the cognition of acupuncture.
