A Case of Cauda Equina Syndrome Treated with Additional Carthami Semen Herbal Acupuncture Therapy (홍화자(紅花子) 약침요법(藥鍼療法)을 가미한 한방치료로 호전된 마미증후군(馬尾症候群) 환자(患者) 1례(例))

  • Objectives : This study was to investigate the effect of the Carthami Semen Herbal acupuncture therapy on a Cauda equina syndrome patient who has a complex of low back pain, bilateral sciatica, saddle anesthesia and motor weakness in the lower extremity and paraplegia with bladder and bowel incontinence. Methods : Oriental Medical Therapy was performed on the Cauda equina syndrome patient from July 15th 2004 to July 29th 2004. The patient was treated with Carthami Semen Herbal acupuncture at BL22, BL23, BL25, BL28 and GV3 in combination with herbal medicine and conventional body acupuncture. We evaluated The Visual Analog Scale(VAS), Improvement index, The Oswestry Diability Index(ODI), gaiting, dyschezia, bladder incontinence, duration of urination and area of anesthesia, Digital Infrared Thermographic Imaging(DITI) before and after treatment. Results : 1. After treatment, VAS, Improvement index, ODI were improved each from 10 to 2, from 21 to 73, from 333 to 166. 2. After treatment, gaiting, bladder incontinence, duration of urination and area of anesthesia and DITI were improved well, but dyschezia was remained. Conclusions : From this case it is thought Carthami Semen herbal acupuncture therapy is very effective to Cauda Equina Syndrome and further study is needed for the confirmation of the effect of Carthami Semen Herbal acupuncture.

  • The aim of this study was to examine the effects of aqua-acupuncture a mixture of bos taurus domesticus and selenarctos thiberanus, and bos taurus domesticus, selenarctos thiberanus and Moschus moschiferus on an animal model of arthritic pain. Under halothane anesthesia, arthritic pain was induced by the injection of $2\%$ carrageenan into the left knee joint cavity of male Sprague-Dawley rats. The responses of afferents to a movement cycle were recorded before and after aqua-acupuncture. The aqua-acupuncture at acupoints reduced neural responses to noxious movement stimulation. Aqua-acupuncture at Zusanli inhibited neural responses of articular afferents to noxious stimulation more than at Hegu. These results indicate that aqua-acupuncture of a mixture of bos taurus domesticus and selenarctos thiberanus, and bos taurus domesticus, selenarctos thiberanus and Moschus moschiferusmay provide a potent strategy in relieving arthritic pain.

Telephone Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (퇴행성 슬관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전화조사)

  • Objectives : This study was accomplished in order to find out how Korean medical doctors take acupuncture prescription for knee osteoarthritis in clinical practice Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. 206 persons having more than 10 years experience were randomly selected from a list of Korean medical doctors. Telephone interviews with them were conducted by the well-trained interviewers between May 21 and May 27 in 2005. Results : 139 of 206 Korean medical doctors made an acupuncture prescription prior to treatment of knee osteoarthritis. 103 of 139 Korean medical doctors used both local and remote points, and 26 of them used remote points only, and 10 of them used local points only. In case of doctors who use remote points, the five element (45 of 129) or Saam (28 of 129) acupoints were used dominantly, and its was based on the pattern identification of the Jangbu organs related with the pain (56 of 129) or of the pain location (35 of 129). Conclusion : This study showed that Koran medical doctors prefer to use remote points such as five element or Saam acupoints based on the pattern identification of the Jangbu organs related with the pain or the pain location. These result may be useful for developing protocols for clinical trial on acupuncture on knee osteoarthritis.

The Effect of Korean Medical Treatment on Systemic Sclerosis : A Case Report (경피증에 대한 한방치료 효과 : 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of Korean Medical Treatment and acupuncture based on M-test theory & herbal medicine for treating systemic sclerosis. Methods : We treated a patient diagnosed with systemic sclerosis. We used general acupuncture & acupuncture based on M-test theory, herbal medicine, moxibustion, and physical therapy. The outcomes were evaluated by using peripheral body temperature measurement, numeric rating scale(NRS), visual analog scale(VAS), and an activities of daily living(ADL) questionnaire for systemic sclerosis. Results : The average peripheral body temperature increased from $36.16^{\circ}C$ to $36.38^{\circ}C$. The NRS showed no difference, but VAS decreased from 7.43 to 5.43. The ADL questionnaire score decreased from 17 to 12. Conclusion : This case study suggests that Korean Medical Treatment and acupuncture based on M-test theory & herbal medicine could be effective for treating systemic sclerosis. However, further studies are needed to confirm this result.

Survey on Ear Acupuncture Usage of Clinical Traditional Korean Medicine Doctor (임상 한의사의 이침 사용 실태 조사 보고)

  • Objectives : The survey was conducted to find out ear acupuncture usage of clinical traditional Korean medicine doctor and lay a foundation for the standards for its clinical application. Methods : An e-mail was sent to Korean medicine doctors whose email addresses are registered at the Association of Korea Oriental Medicine twice over two weeks. 280 completed questionnaires were used for the analysis. Results : 66.07% of respondents answered in use, whereas the answer that "less than 10 per 100 patient applied" was more than majority of 54.29%. The major field of treatment is addictive disorders such as smoking and the 80.11% patients got less than 3 days of treatment. Ear region was the most commonly treated area with 66.67%. Since the self removal frequency was high as 65.05% in compared to the case of visiting clinic. The rate of side effects was less than 10% (96.70%), and these side effects showed light symptoms such as "pain" (46.95%), itchiness (29.27%), flare (19.51%). The Acquired complements for improving ear acupuncture treatment was "the development of effective treatment manual" (28.57%) and "increasing insurance costs" (27.86%). Conclusions : Based on the survey, the vast majority of the TKM doctor used ear acupuncture but the frequency of use was low. We hope this study can be the basis for development of treatment manuals for various indications and safe treatment guidelines.

A Patient with Guillain-Barre Syndrome Treated with Combined Korean Medicine Treatments

  • A patient diagnosed with Guillain-Barre syndrome (GBS) suffering from numbness in the toes and fingers, paralysis in the face, and back pain received intravenous immunoglobulin on December 28, 2019. Without an improvement in symptoms the patient was admitted to a Korean medicine hospital on January 1, 2020 to receive combined Korean medicine treatment including herbal medicine (Sipjeondaebo-tanggami and Hyangbujasunkipalmul-tang), acupuncture treatment, moxibustion, cupping treatment, and physical therapy. Treatment was administered over 51 days, of which 17 days were during hospitalization. Paralysis of facial muscle, and numbness in fingers and toes were evaluated using the Yanagihara score and numeric rating scale (NRS) score, respectively. Facial paralysis improved (21 to 40 based on Yanagihara score) and paresthesia of the hands and feet disappeared (NRS score 6-7 to NRS score 0). There were no side effects or sequela. This study suggested combined Korean medicine treatment may be effective for the treatment of GBS.

  • Objective : Facial embedding therapy is a korean medical treatment technique for face lifting. The authors aimed to evaluate the effects of embedding therapy on facial skin elasticity and moisture content. This paper was written to provide objective datas about Facial embedding therapy. Results : Mean elasticity of both $ST_3$ increased after facial embedding therapy. skin elasticity of both $ST_3$ showed significant improvement. Mean moisture content of both $ST_3$ increased after facial embedding therapy. skin moisture content of both $ST_3$ showed significant improvement. Conclusion : Mean elasticity and moisture content of both $ST_3$ increased after facial embedding therapy. Facial embedding therapy may be an effictive treatment for improving elasticity and moisture content of skin in clinic.

A Clinical Case Study of Residual Symptoms after Decompression of Traumatic Compartment Syndrome (외상성 구획증후군의 감압술 후 잔여 증상에 대한 치료 1례)

  • Objectives : The purpose of this study is to report on the residual symptoms after decompression of traumatic compartment syndrome experienced by a patient whose condition was much improved by electroacupuncture and bee venom therapy. Methods : The patient was treated with electroacupuncture and bee venom therapy. The progress of symptoms was checked by visual analog scale, disabilities of the arm, shoulder, and hand(DASH) and range of motion. Results : All results were improved. Scores from the visual analog scale and DASH decreased, while the range of motion increased. Conclusions : Electroacupuncture and bee venom therapy have an effect on residual symptoms after decompression of traumatic compartment syndrome.

  • Objective: For designing an optimal countermeasure against ageing and ageing related disease, it is necessary to understand the ageing mechanism and other reactive species. Free radicals are associated with the mechanisam of biological ageing and age-related diseases. This study was performed to identifiy the scavenging effect of Radix Cynanchi Wilfordii Herbal-Acupunture Solution(RCWHA) on Nitric Oxide(NO), 1, 1-diphenyl-2-picryl-hydrazyl(DPPH) and Interleukin-4(IL -4). Methods: The activity to scavenge DPPH radicals and NO was observed in vitro, We evaluated the antioxidant activity of RCWHA by using DPPH or NO free radical method. Also the inhibitory effect on IL-4 in RCWHA was observed in vitro, Results: The followings are the summary of the results; (1) There is a significant scavenging effect of RCWHA on NO in 0.1mg/ml and 10mg/ml group after 24hrs. (2) There is no significant scavenging effect of RCWHA on DPPH. (3) There is a significant scavenging effect of RCWHA on IL-4 in $10{\mu}g/m{\ell}$ and $100{\mu}g/m{\ell}$ group. Conclusions: These findings indicate that RCWHA can be used as antioxidant or antiimflamatory drug. But further study is needed about the effect of RCWHA.

  • Objectives : This study was designed to find out the differences of the acupuncture sensation by type of pharmacopuncture. And furthermore we try to find out whether normal saline(NS) is able to be constituted as an appropriate control group for the Calculus Bovis Fel Ursi Moschus(BUM) pharmacopuncture, mountain ginseng pharmacopuncture and sciatica no. 5 pharmacopuncture. Methods : NS and three type of pharmacopunctures were inserted into $ST_{36}$, and $ST_{37}$ of the subjects. Before and after the treatment, subjects completed a questionnaire rating the intensity of 13 kinds of acupuncture sensation(acupuncture sensation scale, ASS). We compared the subjective acupuncture sensation between the NS and three type of pharmacopunctures. Results : BUM pharmacopuncture showed significantly intense acupuncture sensation comparing other two pharmacopunctures and NS. There was no statistically significant difference among mountain ginseng pharmacopuncture, sciatica no. 5 pharmacopuncture and NS. Conclusions : We found that NS may be able to be an placebo pharmacopuncture for mountain ginseng pharmacopuncture and sciatica no. 5 pharmacopuncture. Additional study is needed for placebo pharmacopuncture of BUM pharmacopuncture.
