Patients' Perceptions of Korean Medicine Treatment for Osteoporosis in a Clinical Trial of an Osteoporosis Herbal Medicine Drug: a Qualitative Study (골다공증 한약제제 임상시험 참여자의 골다공증 한의치료에 대한 인식 : 질적 연구)

  • Objective: This research aimed to understand patients' perceptions of Korean medicine treatment for osteoporosis in a clinical trial setting. Methods: Ten patients participating in a clinical trial of a herbal medicine drug for osteoporosis were invited to an interview. Data were collected by in-depth interviews and patient observations. This qualitative study adopted the case study research method, and within-case and cross-case analyses were conducted. Results: A model of the patients' osteoporosis management planning was derived from the study. The results showed that the patients' perceptions of osteoporosis were derived from their knowledge and experience during diagnosis and treatment of the condition. Two groups of patients were recognized: those who overlooked the importance of osteoporosis and those who recognized the importance of osteoporosis. Before making treatment decisions about osteoporosis, the patients evaluated the treatment options and weighed the advantages and disadvantages of each option. When evaluating their treatment, the patients combined their knowledge and experience of Korean and western medicine treatments. Their experience of participating in the clinical trial influenced the management planning of osteoporosis. Two major reasons for low compliance with osteoporosis treatment were ignorance of the disease and insufficient information provided by doctors. Conclusion: The results of this qualitative study pointed to four strategies that could be employed to improve accessibility to Korean medicine treatment for osteoporosis: the dissemination of comprehensive information on osteoporosis in the clinic and mass media, promotion of Korean medicine therapies for osteoporosis, management of clinical trial participants, and insurance coverage for Korean medicine.

Case Report of 6 Patients Not Improved after Conservative Treatment of Herniated Lumbar Intervertebral Disc (요추간판탈출증으로 보존적 치료후 호전되지 않는 환자 6 례 보고)

  • Objectives : The purpose of this study is to observe the clinical characteristics of 6 patients who were not improved after admitted conservative treatment in this 311 patients. Methods : We retrospectivety reviewed 311 patients of Herniated Lumbar Intervertebral Disc which undenvent admitted conservative treatment from November 1997 to June 2000. 311 patients were diagnosed by computed tomography or magnetic resonance imaging in addition to physical examination. Other complications were not detected in 311 patients with Hemiated Lumbar Intervertebral Disc. In case that the scoliosis appeared to caused by severe pain, we included this scoliosis patient with the object of survey. Results : 1. In 6 patients who were not improved, 4 patients were large herniation and 2 patients were medium herniation. 2. In all of 6 patients, the angle of straight leg raising test was low and was not imporved unril discharge. 3. X-ray findings showed straightening in all of 6 patients, disc space narrowing in 3 of 6 patients. 4. X-ray findings showed scoliosis in 3 of 4 patients who were severe pain. 5. The onset was relarively long from 30 days to 7 years. 6. The age was relatively young from 10's to 30's. 7. The adimission date was ralatively long from 32 days to 105 days. Conclusions : In 6 patients, there were observed much volume of disc herniation, low angle of straight leg raising test, young people in an age group, straightening of x-ray finding.

A Case Report of facial wrinkles and folds improved by using the Thread Embedding Acupuncture (TEA) (볼륨매선침을 병행한 매선침법의 안면주름 개선 효과 3례: 증례 연구)

  • Objectives : Recently, Thread Embedding Acupucnture (TEA) targeting under dermis became known to be effective for facial wrinkles and folds; its practice has begun in clinics. The introduction of a new form of TEA is continuing, and 'Volume TEA' is introduced recently. Therefore, we would like to report on the improvement of facial wrinkles by using the recently introduced 'volume TEA'. Methods : We reviewed the medical records of three patients who received 'Volume TEA' treatment at a Korean medical clinic, the photographs taken before and after the procedure were corrected using a Photoshop program and the length of the each wrinkles and folds were measured. Results : The length of wrinkles measured in all three cases decreased. Especially, the decrease of the length of the nasolabial folds was obvious. In case 1, the length of the nasolabial folds of 177mm and 97mm were reduced to 94mm and 63mm, respectively. In the case 2, the length of the nasolabial folds of 155mm and 155mm were reduced to 0mm and 70mm, respectively, after the procedure. In case 3, the length of the nasolabial folds of 170mm and 50mm decreased to 38mm and 37mm after the procedure, respectively. Conclusions : At present, it seems that the ongoing accumulation of relevant studies on the efficacy and safety of wrinkle and folds of TEA seems necessary. This study also has some significance in the level of case report.

Effects of Administration of Pyritum on Activation of Osteoblast Cells in Human Body & on Tibia Bone Fracture in Mice (자연동(自然銅)의 투여가 인체의 뼈모세포 활성과 생쥐 정강이뼈 골절에 미치는 영향)

  • Backgrounds and Objectives: A fracture means a loss of continuity in the substance of bone. Bone differs from other musculoskeletal tissue due to its ability to repair and heal itself without leaving a scar. The cutter head has multinucleated osteoclast cells to resorb the dead bone. The tail, with its conical surface, is lined with osteoblast cells laying down new bone. The conjugation of fracture is a unique biological process regulated by a complex array of signaling molecules and proinflammatory cytokines. Pyritum, one of the important prescriptions in the oriental medicine, has been used for conjugation fracture. The purpose of this study is to evaluate the effects of administration of Pyritum on activation of osteoblast cells in human body & on tibia bone fracture in mice. Materials and Methods : Four weeks aged 30 female DBA mice were used for this study. They were divided three groups, normal group, control group(fracture elicitate mice: FE group) and experimental group(Pyritum administered mice group after fracture elicitation : PA group). Left tibia bones of mice in FE and PA groups were fractured by bone cutters. MG-63 cells in human body th Pyritum in the ratio of 1 mg/m${\ell}$, and the cells were further incubated for 24 hours. Activation of osteoblast was identified using osteopontin, FGF in vitro test. In vivo test, regeneration of fractured tibia through the morphological changes was observed, and also activation of inflammation through NF-${\kappa}$B p65, iNOS, COX-2, osteoblast through osteopontin, FGF and osteoblast's proliferation in each group was measured. Results and Conclusions : 1. In vitro test for activation of osteoblast cells in human body by Pyritum, osteopontin and FGF production were remarkably increased in Pyritum treated MG-63 cells. 2. In regeneration of fractured tibia by Pyritum, fractured area in external tibia morphology was decreased more in the PA group than that of the FE group. Osteogenesis in fractured area was increased more in the PA group than that of the FE group. Also, endochodrial ossification in central area of fracture and osteoid in lateral area of fracture were increased more in the PA group than those of the FE group. 3. In activation of inflammation by Pyritum administered, activation of NF-${\kappa}$B p65, increase of iNOS and COX-2 production were higher in the PA and the FE groups than those of the control group. Especially, the PA group showed higher activation and increase than those of the FE group. 4. In activation of osteoblast by Pyritum, increase of osteopontin, FGF and osteoblast's proliferation were higher in the PA and the FE groups than those of the control group. Especially, the PA group showed higher increase and proliferation than those of the FE group.

A Case Report of Diabetic Neuropathy Treated with Korean Medicine (한의학적 치료로 호전된 당뇨병성 말초신경병증 및 혈당 개선 보고 1례)

  • A diabetic neuropathy patient was treated with Korean medicine for 2 weeks. In this case report, we report the efficacy of a complex treatment comprising various Korean medicine methods by evaluating the differences in pain scores. The patient reported improvement in both pain score and glucose index. Korean medicine could therefore be effective for the treatment and prevention of diabetic neuropathy.

  • Gak-gi-byung in Korean Medicine have many things in common with Beriberi disease, Guillain-Barre syndrome, Transverse myelitis and Cauda equina syndrome. Dong-Ui-Bo-Gam define Gak-gi-byung as syndrome that includes sudden lower limbs weakness, pain and edema. Gak-gi-byung start from the foregoing symptoms and could progress to general digestive, neurological or respiratory symptoms is found on many medical practitioners. In this case, we described a 60-years old man diagnosed as Gak-gi-byung in Korean Medicine. He complained Rt lower limb weakness, Rt Knee Clumsiness and limitation of his Rt knee & ankle motor. And his condition was improved through Korean Medical treatment such as acupuncture and herbal medicine like 'Chung-yul-sa-seup-tang' about 15 days.

  • Objectives : The purpose of this study is to report the effect of traditional Oriental hair care products on inhibition of extensive alopecia areata. Methods : We have experienced a 29-year-old woman with extensive alopecia areata to get worse quickly. The patient was treated by acupuncture and traditional oriental hair care products in 9 months. We used HRV(Heart Rate Variability), APG(Accelerated Photoplethysmograpy) and SALT(Severity of alopecia tool) scale for the assessment of alopecia areata and her physical condition. Results & Conclusions : Her hair loss and physical condition was completely improved.

A Case Study of Pain Disorder Treated by Hypnosis (척추질환으로 인한 통증환자의 최면치료 1례 보고)

  • This study is designed to assess the effect of hypnosis on the pain disorder patient. We treated a patient by using hypnosis as well as traditional oriental therapy-acupuncture, herbal medicine, Chu-Na therapy. We divide the pain into four parts and then we checked the intensities of them comparing with those of the first treatment. The pain worsened by psychic factor was decreased after treatment. Especially, after hypnosis, the intensity of the pain is highly changed. Psychotherapy in oriental medicine can be helpful to treat patients who have psychic and physical disorders.

Methods : The relationship between the pelvis and the brain was searched in PubMed, and these searching studies were reviewed. Conclusions : 1. Urinary disorder is influenced by brain. 2. Brain is influenced by luteinizing hormone. 3. Pelvic floor muscles are influenced by brain. 4. Urological Chronic Pelvic Pain Syndrome(UCPPS) is influenced by brain. 5. Brain is influenced by the low intensity laser acupuncture stimulating thirteen ghost acupoints(includes CV1).

A Case Report on Post Spinal Surgery Syndrome in Restless Leg Syndrome (하지불안 증후군을 동반한 척추 수술 후 증후군 환자 치험 1례)

  • Objective: To report the effect of oriental medical treatment on post spinal surgery syndrome (PSSS) in restless leg syndrome (RLS). Methods: We treated this patient with oriental medicine and measured his progress using a Visual Analogue Scale (VAS), the Oswestry Disability Index (ODI), and the International Restless Leg Scale (IRLS). Results: After treatment, most symptoms had decreased: the VAS score dropped from 6 to 3, ODI from 22 to 14, and IRLS from 34 to 20. Conclusions: Oriental medicine may be effective in alleviating PSSS and RLS. However, more rigorous studies are required to identify exactly what treatment is more efficient for PSSS and RLS.
