A Clinical Report about Primary Trigeminal neuralgia patient (원발성 삼차신경통 환자 치험 1례)

  • The trigeminal neuralgia whose pain is led repeatedly and fitfully is limited inside the trigeminal nervous territory. The cause reveals so far clearly from the western medicine. According to this cause, the treatment is divided into surgical and non-surgical method. But the reduction of pain is not certificate and the case which concurs a sequela is many. So we made up to control typical primary trigeminal neuralgia by oriental medicine treatment like Herb Medication and A-Tx. In this case, patient is diagnosed 'Fire from stagnation of liver and stomach'(肝胃實熱)based on several symptoms. According to this, we used herbal medicine-chungpyesagantang-, and these efforts helped the case of disease.

A Retrospective Study on Dementia Outpatients in Oriental Neuropsychiatry Clinic of One Medical Center (치매 환자에 대한 후향적 연구 - 일 기관 한방신경정신과 내원 환자를 중심으로 -)

  • Objectives : The aim of this study was to describe the clinical aspects in dementia outpatients who visited oriental neuropsychiatry clinic and to analyze the efficacy of herbal medicine. Methods: We analysed medical records of dementia outpatients who visited oriental neuropsychiatry clinic of K medical center from August. 2000 to July, 2010 and matched our criteria. Results: Ninety-two subjects were selected for analysis and consisted of 77.3% dementia of Alzheimer type. 13.0% vascular dementia. and 9.8% mixed type of dementia. Herbal medicine prescribed mainly were Jowjseungchung-tang(Tiaowejshengqing-tang). Kunneo-tang(Jjannao-tang) and Chenwangbosim-dan(Tianwangbuxin-dan) for dementia of Alzheimer type, and Sunghyangjungki-san(Xjngxjangzhengqi-san) and Upung-dan (Yufeng-dan) for vascular dementia. Scores of K-DRS were not changed for some subs cales and decreased but less than natural decline of Alzheimer disease. Score on the Memory subscale in group of Kunneo-tang(Jjannao-tang) increased significantly. Conclusions : These results suggest that the herbal medicine may be effective for dementia. More prospective studies are needed to assess the efficacy and prognosis of herbal medicine for dementia. Clinical guideline is needed for systematic data accumulation.

The Effects of Suggestion on Cerebral blood flow velocity (암시가 정상인의 뇌혈류 속도에 미치는 영향)

  • Objectives : The purpose of this study is to investigate the change of cerebral blood flow velocity by different types of suggestion in normal subject. Methods : Suggestion of ascending qi inducement and descending qi inducement which were recomposed from autogenic training, was operated on 60 normal subjects individually. Then cerebral blood flow velocity was examined by Transcranial doppler ultrasonography(TCD) each 5 minutes before and after suggestion. Results : The result shows that cerebral blood flow(CBF) velocity was increased significantly in suggestion of ascending qi inducement and CBF velocity was decreased significally after 1 minute in suggestion of descending qi inducement. Conclusion : Suggestion could have affected to CBF velocity, and also different types of suggestion could have affected to the change of brain blood flow. Various clinical studies should be completed on patients in the future.

Antianxiety drug and Herb medicine (항불안제와 한약 - 경계(驚悸).정충.공증(恐證)의 처방을 중심으로)

  • Objective : This study was performed to investigate the prescriptions and herbs about Keongke(驚悸), Cheongchung and fear affection(恐證) related to the anxiety disorder Method : 9 main texts in oriental medicine were selected and investigated for this study. And then the results of research were analyzed. Result : 87 prescriptions and 123 herbs were described in 9 books. Especially Chusaanshinhwan(朱砂安神丸), Ginseng Radix(人蔘), Poria(茯神), and Angelicae Gigantis Radix(當歸) were mainly used. Conclusion : The results suggest that the prescriptions and herbs frequently described about Keongke(驚悸), Cheongchung and fear affection(恐證) might have antianxiety effect.

Personality Profiles of Patients Who Visit Oriental Neuropsychiatric Clinic : Analysis of Temperament and Character Inventory(TCI) (기질 및 성격검사(TCI)에 나타난 한방신경정신과 환자의 전반적 특성 및 화병 환자의 특성)

  • Objectives : The purpose of this study is to examine the general personality profiles of patients who visited an oriental neuropsychiatric clinic and to investigate the specific profiles of patients diagnosed with Hwa-byung. Methods : We analyzed the profiles of 122 patients who completed TCI-RS questionnaire for the purpose of counseling. Patients were divided by sex, age and diagnosis related groups. Scales and subscales of each group were compared by an independent t-test and ANCOVA with SPSS windows 16.0. Types of temperament and character were classified by percentile ranks of the total patients and diagnosis related groups. Results : 1. Total patients were classified as the high harm avoidance type of temperament and of low self-directedness type of character. 2. Male patients showed significantly higher scores on NS, all subscales of NS, P2 and P3 than female patients. HA4 and C5 scores were significantly higher in the female group. Younger patients showed significantly higher scores on NS, all subscales of NS, HA1 and P3. However, older patients had significantly higher scores on SD4, SD5, ST2 and ST3. 3. Between diagnosis related groups, Hwa-byung group had significantly higher scores on RD, ST, SD3 and ST2. However P3 score of non-hwa-byung patients was higher than hwa-byung patients. In typical classification, both hwa-byung and non-hwa-byung groups were classified as the same types as total patients. Conclusions : This study showed that the patients visited an oriental neuropsychiatric clinic and had similar personality with that of general psychiatric disorders. Hwa-byung patients exhibited higher RD and ST scores than other disorders, but they had no typical differences.

Effect of Acupuncture at Oegwan(TE 5) on Auditory P300 (외관(外關) 자침이 Auditory P300에 미치는 영향)

  • Objective : Audio-evoked potential is technique used for measuring brain functions such as memory, attention, and concentration. Although a number of studies on acupuncture for pain, there are few reports on the effect of acupuncture cognitive function. Methods : Fifteen healthy volunteers with no neurological condition took part of the study. $(23.27{\pm}2.09)$. In this study, we examined the changes in auditory P300 by acupuncture stimulation to 'Oegwan' on the left hand. Auditory P300 was recorded before, during and after acupuncture. The amplitude and latency of P300 at Fz, Cz, Pz point were calculated. Results : Auditory P300 amplitude decreased during and after acupuncture, but there was no statistical significance.(P< .05) Auditory P300 latency significantly decreased during and after acupuncture.(P< .05) Conclusions : The decrease of Auditory P300 latency time suggests that acupuncture at Oegwan has some effects on the cognitive function.

Preliminary Study to Develop the Instrument of Pattern Identification for Jing Ji and Zheng Chong (경계정충(驚悸怔忡) 변증도구 개발을 위한 기초 연구)

  • Objectives : This study was performed to develop a standard instrument of Pattern Identification for jing ji and zheng chong. Methods : The advisor committee on this study was organized by 15 neuropsychiatry professors of oriental medical colleges. The items and structure of the instrument were based on review of published literature. We took consultation 2 times from the advisor committee and we also took additional advices by e-mail. Results : 1. We divided the symptoms and signs of jing ji and zheng chong into 9 pattern identification. - heart deficiency with timidity(心膽虛怯), heart qi deficiency(心氣虛), heart blood deficiency(心血虛), heart yang inactivity(心陽不振), heart blood stasis(心血瘀阻), phlegm turbidity obstructing(痰濁阻滯), yin deficiency with effulgent fire(陰虛火旺), water qi intimidating the heart(水氣凌心), dual deficiency of the heart and spleen(心脾兩虛). 2. We got the mean weights that reflect standard deviation to each symptom of 9 pattern identification which had been scored on a 100-point scale. 3. We made out the Korean instrument of the pattern identification for jing ji and zheng chong. It was composed of 17 questions in question-and-answer form. Conclusions : Instrument of Pattern Identification for jing ji and zheng chong was developed through experts' disscussion. If the validity and reliability of this instrument is confirmed through additional clinical trial, the instrument of pattern identification for jing ji and zheng chong is expected to be applied to the subsequent research.

The Effects of Benson's Relaxation Technique on Heart Rate Variability (벤슨의 긴장이완법이 정상 성인의 심박변이도에 미치는 영향)

  • Objectives: The purpose of this research is to investigate the effects of Benson's Relaxation Technique on autonomic nervous system for normal subjects with Heart Rate Variability(HRV) and find out relationship with anti - stress effects. Methods: First, Benson's Relaxation Technique was applied on 5 normal subjects for three weeks. Then HRV was examined for 15 minutes Benson's Relaxation Technique application. Next, Benson's Relaxation Technique each 8 times was applied on 5 normal subjects for 1 weeks. Then :HRV was examined each 5 minutes before and after Benson's Relaxation Technique application. Results: 1. SDNN, SDSD increased continuously in HRV for three weeks Benson's Relaxation Technique application. 2. Mean HRV, SDNN, SDSD were significant different in HRV between before and after Benson's Relaxation Technique application at fourth week. Conclusion: The results suggest that Benson's Relaxation Technique application have shown anti-stress effects and the more Benson's Relaxation Technique is repeated, the more effective.

A Preliminary Study on the Construction of Clinical Data for Korean Herbal Prescription Recommendations for Anxiety, Depression, Anger, and Insomnia (불안, 우울, 분노 및 불면 증상에 대한 한의학파 처방 추천 임상 데이터 구축을 위한 기초 연구)

  • Objectives: To build basic clinical data for developing an artificial intelligence algorithm for Korean herbal prescriptions for anxiety, depression, anger, and insomnia. Methods: Subjects were recruited among those who reported mild or more severe symptoms of anxiety, depression, anger, and insomnia (Anxiety: State-Trait Anxiety Inventory≥40, Depression: Beck Depression Inventory≥14, Anger: State-Trait Anxiety Inventory≥16, Insomnia: Insomnia Severity Index≥8). Clinical observation items including basic medical information and symptoms were collected from them. These data were then analyzed by experts in Hyungsang medicine, Sasang constitutional medicine, and Sanghan-Geumgwe medicine. Results and Conclusions: Experts of the three societies presented key clinical data and recommended prescriptions. Results of this study can be used as basic data for developing an artificial intelligence algorithm for Korean herbal prescriptions in the future.

A Comparative Study on the Psychotherapies between Western and Oriental Medicine (한방정신요법과 서구 정신요법의 비교연구)

  • Objectives: Oriental psychotherapy and western psychotherapy in Korea were investigated in order to develop a new oriental psychotherapeutic model for modern Korean. Methods: Oriental psychotherapy and Western psychotherapy were compared and investigated in some viewpoints such as cognition, behaviour, emotion and ultimate purposes for the therapies. Cultural differences are examined, too. Results: There are clear differences between two psychotherapies. It seems that cognition is more important than behaviour or emotion in western psychotherapy. On the other hand, emotion is more important than cognition or behaviour in oriental psychotherapy. Ultimate purposes for the therapies are also different because of cultural differences. Conclusions: New oriental psychotherapeutic model considering cultural differences and merits of two therapies is needed.
