  • Objectives: The purpose of this study was to investigate the oral health and hygiene in the neurosurgical patients in intensive care unit(ICU). Methods: The subjects were 92 neurosurgical patients in intensive care unit(ICU) from March, 2011 to December, 2012. The oral examination consisted of number of residual teeth, DMFT index, clinical attachment loss, gingival index, plague index, and Candida species colony of tongue and saliva. Plaque was inoculated from tongue and saliva and incubated in 36.5C incubator for 48 hours using $Dentocult^{(R)}$ CA(Orion Diagnostica, Espoo, Finland). Glasgow coma scale(GCS) was measured to evaluate the consciousness of the patients on the basis of medical record. Results: Oral health was poor in clinical attachment loss and gingival index. Oral hygiene in neurosurgical patients in ICU was very poor due to high plaque index and Candida colonization of tongue and saliva. Plague index was closely related to Candida colonization of tongue and saliva(p<0.05). Conclusions: Oral health and hygiene of patients in neurosurgical ICU were very poor. More careful oral hygiene care is very important and necessary to enhance the oral health improvement of the neurosurgical patients in ICU.

  • Perioperative lumbar drainage of cerebrospinal fluid is commonly used in neurosurgical practice. However, the relationship between lumbar drainage and acquired Chiari malformation is not well established. The authors present an unusual case of paraplegia as a result of acquired Chiari malformation after lumbar drainage. Acquired Chiari malformation can induce compression of cervicomedullary junction and syrinx formation. Foramen magnum decompression is recommended for the solution of such problems.

  • Aortic abdominal aneurysm rarely has been reported as causing osteolytic lesions of the spine. It may produce back and radiating pain patterns similar to those of several commonly encountered neurosurgical processes. We report a uncommon complication of huge pulsating aortic aneurysm causing severe vertebral erosion with incapacitating back and radiating pain.

  • While there are reports regarding burns occurring to patients during the surgery, there are little reports concerning the incidents of the burns related to neurosurgical operations. Moreover, in Korea, even surveys and statistics on the incidents of burns in operating rooms are not known. This report explores burns occurring to a patient in an electrocautery scenario after disinfecting the surgical site with alcohol during the preparation of a neurosurgical operation in an operating room where there is much exposure to oxygen. The authors show a case of a 33-year-old male patient who undergoing evacuation of hematoma on occipital lesion, suffered second degree burns as a result of surgical fires.
