A Case of Syringomyelia with Back and Shoulder Pain

  • The purpose of this study was to describe the treatment of the clinical symptoms of syringomyelia using Korean medicine. A patient with syringomyelia complained of back and shoulder pain, numbness of the upper right limb, headaches and dyspnea. He was treated with herbal medicine, acupuncture, moxibustion, and physical therapy. The Visual analog scale (VAS) was used to score sensory symptoms (pain, numbness) and Modified Borg scale (mBorg) was used to score dyspnea. After the treatment, his symptoms were relieved. The patient's chief complaint was back and shoulder pain which was reduced greatly from a VAS score of 6 between 1-2. The results in this study may contribute to the development of the Korean medicine field of syringomyelia in the future.

The Domestic Trends of Korean Medicine Treatments of Spondylolisthesis (척추전방전위증의 한의학적 치료에 대한 국내 연구 동향)

  • Objectives The purpose of this study is to investigate the trends of Korean medicine treatments of spondylolisthesis. Methods We investigated the studies on Korean medicine treatments for spondylolisthesis via searching 4 Korean web databases (OASIS, KTKP, RISS, DBpia). We used "Spondylolisthesis", "Lumbar spondylolisthesis" as keyword. Limitations were as follows; Domestic studies, mentioning the treatments of spondylolisthesis in Korean medicine. This study researched and classified the papers according to type of study, published year, titles of journals, published institution, the number of cases, and types of treatments. Results 14 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, and the types of treatments. 14 papers were published since 2000. The studies on Korean medicine treatments about spondylolisthesis were mainly published in the journal of Korean acupuncture & moxibustion medicine. 7 case reports, 6 case series, 1 RCT had been under research. In 14 cases of the studies, various Korean medicine treatments were used to treat the symptoms. Numeric rating scale (NRS) was used as primary assessments. Conclusions In this study, we analyzed the trends of Korean medicine treatments of spondylolisthesis. Reviewing the domestic trends of studies on Korean medicine treatments of spondylolisthesis and examining the strong and weak points of those treatments are essential for the further studies.

A Rehabilitation for Total Hip Replacement in Korean Medicine: A Report of Three Cases (고관절 전치환술 후 한방 재활치료에 대한 증례 보고)

  • The objective of this study was to propose a rehabilitation protocol for total hip replacement in Korean medicine and to report its effectiveness. Three patients who had undergone total hip replacement were treated using the rehabilitation protocol in Korean medicine. We evaluated the outcome evaluating physical examination findings of the hip joint, numeric rating scale, pain disability index, Harris hip score, and walking state. After the treatment, we observed that hip movement and muscular strength improved. Moreover numeric rating scale, pain disability index, Harris hip score, and walking state of patients improved. A rehabilitation in Korean medicine can be effectively used for total hip replacement patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean medicine.

Effect of Complex Traditional Korean Medicine Treatment and Rehabilitation on Acetabular Fracture: A Case Report

  • This report presents the clinical progress of a 50-year-old male diagnosed with a left acetabular fracture and left rib fracture after falling from a height of 3 m. The patient was treated nonsurgically and underwent complex traditional Korean medicine (TKM) treatment and rehabilitation, including acupuncture, electroacupuncture, pharmacopuncture, bee venom, herbal medicine, physiotherapy, and moxibustion. Patient progress was assessed using the numerical rating scale (NRS), manual muscle test (MMT), and modified Harris Hip Score (mHHS). During 113 days of hospitalization, the NRS score decreased from 7 to 3, and the MMT grade increased to normal in the hip, knee, and ankle joints. The mHHS score increased from 26 to 99 points. Radiographs and computed tomography scans of the left acetabulum were obtained periodically, and fracture union was successfully achieved. To our knowledge, this is the first case report of complex TKM treatment and rehabilitation of acetabular fractures.

  • In this study, we propose a rehabilitation protocol involving Korean medicine for patients after high tibial osteotomy (HTO) and report its effectiveness. Three patients underwent HTO using the rehabilitation protocol involving Korean medicine. We estimated the outcome evaluating physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After the treatment, we observed that knee movement and muscular strength were improved. In addition, the numeric rating scale, pain disability index, Lysholm knee score, and walking state of patients were improved. Therefore, a rehabilitation protocol involving Korean medicine can be effectively used for patients after HTO. The limitation of this study was the insufficient number of cases and the difficulty of follow-up because of the characteristics of this operation and the long periods of time that are needed for each phase of the protocol. Further studies are needed to design a rehabilitation protocol involving Korean medicine.

A Study on Overseas Traditional Chinese Medicine Treatment of Pre-eclampsia (자간전증의 국외 중의학 치료 고찰)

  • Objectives: The purpose of this study is to review the overseas clinical study trends on Chinese Medicine treatment for Pre-eclampsia. Methods: We searched articles published from Pubmed, Embase and CNKI. The period was set from 2004 to 2023. Searched keywords were "Pre-eclampsia", "Preeclampsia", "Chinese medicine", "Herbal medicine", "子癎前期", "子癎前症", "中藥", "中醫". Results: 21 articles were finally selected. There were 16 RCTs, 3 case control studies, 2 case series. 2 articles used both acupuncture and western medicine, 19 articles used both herbal medicine and western medicine. 風池 (GB20), 太衝 (LR3), 足三里 (ST36) were the most frequently used acupoints, Paeoniae Radix alba (白芍藥) is the most frequently used in herbal medicine treatment. Conclusions: Our review found that chinese medicine combined with western medicine is more effective for alleviating symptoms of pre-eclampsia.

The Effect of Combined Acupuncture Treatment on Acute Ankle Sprain: Case Series (급성 족관절 염좌에 대한 복합 침치료의 효과: 사례군 연구)

  • The purpose of this study is to report the effect of combined acupuncture treatment on acute ankle sprain patients. The subjects included in this study were 15 acute ankle sprain patients who visited Jaseng Hospital of Korean Medicine from Oct. 15th, 2012 to Feb. 8th, 2013. We treated 15 patients with combined acupuncture treatment. The treatment consisted of Hwangrunhaedok-tang (Huanglianjiedutang) pharmacupuncture, electroacupuncture (applied to GB39, ST36 on the affected side), acupuncture (applied to TE17, SI6 on the unaffected side with Dong-Qi therapy). To assess the effect of treatment, the numeric rating scale (NRS) was applied before and after treatment. After first combined acupuncture treatment, the NRS scores significantly decreased from $8.33{\pm}0.94$ to $2.26{\pm}0.44$ (p<0.01). We suggested that Combined acupuncture treatment are effective and useful on acute ankle sprain. And, further studies will be needed.

A Narrative Review of Carpal Tunnel Syndrome in Traditional Chinese Medicine: Using China National Knowledge Infrastructure Database (수근관 증후군의 최근 6년간의 연구 동향 분석: China National Knowledge Ingrastructure (CNKI) 보고 연구를 중심으로)

  • Objective The purpose of this study was to investigate clinical studies between 2015 and 2020 on the traditional Chinese medicine treatment for carpal tunnel syndrome (CTS) using the China National Knowledge Infrastructure (CNKI) database. Methods We searched clinical studies on the Chinese medicine treatment published in traditional Chinese medical journals on CTS through the CNKI search. Fifteen published studies between 2015 and 2020 were chosen and examined for the year of publication, sample size, study design type, diagnosis and evaluation of diverse criteria, treatment period, purpose of the study, and classification based on the treatment technique. Results Out of 73 studies, 15 had practiced various Chinese medical treatments, such as acupuncture, moxibustion, manipulation, herbal medicine application therapy, and external herbal treatment. Conclusions Various studies have investigated the Chinese medicine treatment for CTS in China. To envelope the treatment of CTS in Korean medicine, steady and valuable therapies for high-level clinical research are required. In the future, this study could be helpful in the development of the treatment of CTS with Korean medicine.

A Web-based Survey on Clinical Practice Patterns of Korean Medicine Treatment for Traffic Injuries (교통사고 상해증후군의 한의 임상진료 현황조사를 위한 웹기반 설문조사)

  • Objectives The purpose of this study is to understand current status of Korean Medicine treatment practice patterns for traffic injuries by web-based survey. Methods The structured questionnaire was distributed by a web based survey to 1,630 potential respondents by email from December 19th in 2016 to February 13th in 2017. All data were statistically analysed. Results The response rate was 79.4%. According to the analysis of the outcome of the survey, 88.0% of the participants felt necessity of the development of Korean medicine clinical practice guidelines (CPGs) for traffic injuries. Survey results showed cognitive degree about Korean Medicine CPGs' necessity, individual Korean medicine treatments and requirement for extending insurance coverage for patients with traffic injuries. Conclusions This survey study helps to determine current clinical practice patterns of Korea medicine treatments for traffic injuries. These results further anticipate to provide basic data for CPGs for traffic injuries.

The Clinical Effect of Rehabilitation Protocol for Distal Radius Fracture in Korean Medicine: A Report of 3 Cases (원위 요골 골절에 대한 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고)

We estimated the outcome evaluating physical examination findings of the wrist joint, numeric rating scale and quick the Disabilities of the Arm, Shoulder and Hand (DASH) score. After the treatment, we observed that wrist movement, numeric rating scale and quick DASH score were improved. A rehabilitation in Korean Medicine can be effectively used for distal radius fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine.
