개에서 외골격 고정을 이용한 성장 기형의 교정 (Application of External Skeletal Fixations to Correct the Growth Deformities in a Dog)

  • A male, 11-month-old, Shih-tzu dog weighing 5.2kg showing knuckling secondary to severe cranial bowing of left antebrachium with valgus deformity of the carpus was presented to the Veterinary Medicine Teaching Hospital of Seoul National University. Radiographic findings were a premature closure of the distal ulnar physis, a cranial bowing of the radius, subluxation of the elbow joint, and degenerative changes in the carpal joint. The patient was treated by performing an oblique osteotomy of the radius and ulna, followed by application of External Skeletal Fixations. At 18 weeks postoperatively, the radius was healed. An incongruency of the elbow joint and radial angular deformity was corrected.

  • Specific diagnosis of orthopedic disease can be diffcult in canine practice. Failure to detect the clinical signs of a disorder during physical examination of dogs with acute or chronic lameness is the most common reason for failure to make specific diagnosis. A 6-month-old, female doberman with history of swelling and non-weight-bearing lameness in the left forelimb was referred to Beterinary Teaching Hospital of Chungbuk National University. Physical examination, plain radiography, and conventional three-phase radionuclide bone scan were performed in the patient. Based on the physical exam and radiography, this case was diagnosed as elbow strain and subluxation. Conventional three-phase bone scan detected soft tissue inflammation and osteochondral lesions of elbow joint, and revealed good agreement with clinical findings. Therefore, conventional three-phase bone scan was able to provide the precise information about inflammation of soft tissue and osteochondral lesions of joint.

주관절과 전완부의 초음파 이상 소견 (Abnormal Findings of the Ultrasonography for Elbow and Forearm)

  • Ultrasonography (US) of the elbow is an increasingly utilized modality for a variety of diagnoses. In this brief review, US findings for the pathologic conditions of forearm and elbow are described. The most common pathologies discussed here include distal biceps tendon and triceps tendon lesions, medial and lateral epicondylopathies, ulnar collateral ligament tears, ulnar nerve subluxation, joint effusions, and intra-articular bodies.

환축추 회전 아탈구 환자에 대한 도수치료 효과 (Effect of Manual Therapy on a Patient With Atlantoaxial Rotatory Subluxation)

  • Background: An 8-year-old girl had severe neck pain and stiffness after trauma. CT scan showed atlantoaxial rotatory subluxation (AARS). She had conservative treatment because she did not have neurological symptoms and spinal basilar artery dysfunction. Conservative therapy was halter traction twice for 4 weeks. However, pain and stiffness persisted. She had been recommended to have surgery from her physician, but she received manual therapy for non-surgical procedures. Methods: The joint mobilization, muscle energy technique, motor control exercise, and deep neck flexor (DNF) endurance exercise were applied as manual therapy and 10 session for 2weeks. Results: Clinical outcomes were measured at initial baseline, after 2 weeks, and after 6weeks. Active range of motion was completely restored after 6weeks and numeric pain rating scale was completely reduced after 2 weeks. The strength of neck flexor muscle recovered to normal after 2 weeks, and the DNF endurance was improved to 25 seconds after 2 weeks and to 42 seconds after 6weeks. Motor control capacity recovered to 30 ㎜Hg after 2 weeks. Conclusions: This case report describes the immediate and short-term clinical outcomes for a patient presenting with symptoms of neck pain following AARS. Clinical rationale and patient preference aided the decision to incorporate manual therapy as a treatment for this patient. Manual therapy has shown a successful recovery in AARS patients, more research is needed to validate the inference of this case report.

성장판 조기페쇄에 따른 요척골 외전과 완관절 아탈구 교정들 위한 요척골 절단술 및 광범위 완관절 고술의 일례 (A Case of Osteotomy of the Distal Radius and Ulna with Panarthrodesis of Carpus for Reform of)

  • A 23.5 kg179, 8 months olds non-spaycd female Rottweiler dog was submitted to the veterinary teaching animal hospitals Chonbuk: national university, for chronic bitten trauma on right foreleg with pain, lameness, and pronation. The patient fought with a neighboring dog about five months ago and had mild anorexia, depression, bolt normal walking at that time. CBC and blood chemical examination were in normal range. Physical examination resulted in the right foreleg with edema and interval rotation of carpal joint. Radiograph of the right carpus showed varus and subluxation. We finally diagnosed the patient as the growth deformity of distal radius and ulna caused by medial premature closure of distal radial physis. Osteotomy was performed fur reforming of the varus of the distal radius and ulna with 6-hole straight plate and six 3.5 mm screws. Panarthrodcsis of carpus was preformed for correcting subluxation using 6-hole T-shaped plate and four 3.5 mm screws with cortical onlay autograft and strengthened by two 3.5 mm screws and tension wire band in lateral aspect of the carpus. Follow-up radiographs after 16 weeds of the surgery showed complete coaptation of osteotomy sire of the radius and uIna. After 6 months of the procedurc, talc plate, screws and the wire were removed except 7-shaped plate and four screws fur arthrodesis. The patient was seen in normal forelimb and could walk and run without lameness after 7 months of the procedure.

Unexpected postoperative atlantoaxial rotatory subluxation after excision of melanocytic nevi of the head and neck in older children: two case reports and literature review

  • Postoperative atlantoaxial rotatory subluxation (AARS) is a rare complication that develops almost exclusively in children following oropharyngeal and otologic surgeries, proposing that oropharyngeal inflammatory responses and excessive head rotation are responsible factors. However, there have been no reports of AARS after excision of a nevus on the head and neck. Here, we present two cases of AARS following limited head rotation during simple nevus excision. Patient 1, a 9-year-old girl, complained of neck pain and limited range of motion after excision of the nevus on the neck. After 2 months, computed tomography and magnetic resonance imaging finally revealed AARS with a ruptured transverse atlantal ligament. A month of halo traction was required for the treatment. Patient 2, an 11-year-old girl, presented with immediate pain and limited neck extension after tissue expander insertion under the upper chest and excision of the nevus on her left cheek. The diagnosis was promptly made using cervical spine radiography. A cervical collar was applied for 1 month. Both patients recovered without any complications after treatment. This report highlights the importance of suspicion for AARS after surgery regardless of surgical duration or amount of head rotation.

소아 고관절 질환(I): 발달성 고관절 이형성증의 진단 및 초기 치료 (Pediatric Hip Disease (I): Diagnosis and Treatment of Developmental Dysplasia of the Hip)

  • 소아 고관절 이형성증은 비구 또는 근위 대퇴골, 혹은 양쪽 모두를 포함하는 고관절의 부적절한 발달을 지칭하는 광범위한 개념이다. 초음파적 검사로 영아기의 고관절 탈구 진단이 간과되거나 수술적인 치료가 필요한 경우는 감소하고 있으나 모든 고관절 이형성증을 찾아내지는 못하고 있다. 아탈구가 없는 고관절 이형성증은 우연히 진단되며, 40대 이후 여자에서 퇴행성 관절염과 강한 연관성을 가진다. 아탈구를 동반한 고관절 이형성증은 아탈구의 정도에 따라 증상이 다양한 시기에 걸쳐 나타나며, 특히 여성에서 임신 중 발현되는 경우가 많다. 신생아에 대한 정확한 이학적 검사와 조기 치료는 이 질환의 양호한 결과를 위해 매우 중요하다. 진단의 간과를 막고 조기 치료의 기회를 제공하기 위해 저자는 고관절 이형성증이 의심스러운 2세 이하 영아 모두에게 고관절 검사를 추천한다. 본 연구에서는 고관절 이형성증의 진단 및 초기 치료에 대해 고찰해 보고자 한다.

Ehlers-Danlos Syndrome with Classical Subtype in a Cat

  • Ehlers-Danlos syndrome (EDS) is a rare genetic disorder in dogs and cats and has been mostly reported in purebred cats. In this study, we report a case of a 1-year-old castrated male Korean shorthair cat, who presented with multiple small skin tears and bruises distributed over the entire trunk area. The cat's skin was hyperextensible and easily torn with gentle touch. The skin extensibility index of the cat was 25%, indicating the possibility of EDS. The cat exhibited no signs of pruritus or inflammation, and no underlying disease was found. However, radiography revealed hip joint subluxation and arthritis. Following this, biopsy of the lacerated skin was performed. Histopathological examination of the skin revealed that in the dermis adjacent to the lesions, the collagen fibers were irregular in size and width, with a slightly thinner epidermis, and increased interfibrillar spaces containing low numbers of scattered well-differentiated fibroblasts and mast cells. Histopathological examination of the skin confirmed EDS. The symptoms observed in the cat, including skin hyperextensibility, multiple bruising, hip joint subluxation, and arthritis, corresponded to the classical subtype of EDS in humans. Thus, this study is a rare report of a classical EDS case in a Korean shorthair cat. This study suggests that skin extensibility index and biopsy are useful diagnostic procedures for confirming EDS in animals until a more definitive genetic test is established.
