A Study of Statistical Analysis of Rock Joint Size and Intensity by Stereological Approach (입체해석학적 접근법에 의한 암반 절리 크기 및 밀집도의 통계적 분석에 관한 연구)

  • Rock joint system makes a heavy effect on the behavior of rock structures. The definition of a 3D rock joint system is very important in 2D or 3D numerical analysis for the prediction of the behavior of a discontinuous rock mass. To enhance the reality of a 3D definition of rock joint system, it is essential to estimate the unbiased statistics of basic geometric attributes of rock joints. In this study, we have proposed the statistical analysis and derived the related equations for an estimation of statistics of joint size and intensity. Geometry of rock joints in 3 dimensional space can be defined by the aggregate of location, size, orientation and intensity. The dimensional limit of survey method and its data makes 3D geometric attributes probabilistic. In the estimation of statistics of joint size, we have discussed the technique to correct the bias from a dimensional limit and derived the equation of 3D joint intensity by stereological approaches.

  • This paper presents the result of an investigation on the application of the acoustic emission method to the monitoring of fatigue crack initiation, growth and track location in welded joints. Fatigue test was carried out for a typical fillet welded joint of ship structure. AE parameter such as ring down count was analyzed in time domain and crack locations were examined by source location and cluster option which is one of the functions of AE signal processor The usability of AE mettled was confirmed for the detection of the initiation and location of through crack.

  • A prevalence of temporomandibular disorders(TMD) based on the clinical sign and symptom in 155 patients were investigated. History taking with interview chart and clinical examination were performed. Age and gender of the patients, duration of TMD, location of pain, joint sound, limitation of mouth opening and more detailed diagnostic name were identified. The results of this study were as follows: 1. TMD was more prevalent in female than in male. TMD was the most prevalent at the age of 20s and decreased with age. 2. Acute TMD was more prevalent than chronic one. 3. About ninety percent of TMD patients had pain. Pain had mainly a unilateral origin. Muscle pain was mainly related with the masseter muscle. 4. Joint sound was identified in about fifty-six percent of the TMD patients and unilateral joint sound was more prevalent than bilateral one. 5. Limitation of mouth opening was observed in about forty percent of the TMD patients. 6. In TMD patients, muscle dysfunction was the most prevalent one. In muscle disorders local muscle soreness was the most prevalent one. In joint disorders, capsulitis was the most prevalent one. In muscle-joint disorders, trigger point pain with retrodiscitis was the most prevalent one.

  • This paper studies the validity about a joint operation of neutral wire and overhead grounding wire in combined distribution systems. The overhead grounding wire and neutral wire are currently installed separately and grounded by common. However there is no any ineffectiveness or electrical problem in case of the proposed system, such system can be operated at real distribution system. Therefore this paper describes the suitability of a joint operation through lightning surge analysis on combined distribution systems and analyzes the optimal position of lightning arrester on joint operation of neutral wire and overhead grounding wire. Lightning surge analysis is carried out by EMTP/ATPDraw to obtain the overvoltage of overhead line and underground cable in various conditions such as location and current types of lightning stroke. Over voltage gained by the analysis show that the insulation strength of the joint operation case is not stable compare with the current operation case.

Pedicled Perforator Flaps for Reconstruction of Bilateral Knee Defects: A Case Report

  • Reconstruction of soft tissue defects of the knee has always been a challenging task for plastic surgeons. Various reconstructive choices are available depending on the location, size, and depth of the defect relative to the knee joint. Defects on the knee joint have several characteristic features. The use of a free flap is preferred for reconstructions involving obliteration of large-cavity defects, but recipient pedicle isolation can be difficult because of the extent of the injury zone. Furthermore, the true defect during knee joint flexion is larger than during knee joint extension, and a durable flap is necessary for joint movement. We report for the first time on the use of pedicled perforator flaps for reconstruction of bilateral knee defects in a 76-year-old woman. The operative procedure required skeletonizing the perforators of an antero-lateral thigh flap and antero-medial thigh flap and rotating the flap in the defect. The patient returned to normal daily activity and had a full range of motion two months after the accident. The shorter operating time with decreased donor site morbidity and its durability make this flap a valuable alternative for soft tissue reconstruction of the knee.

Clinical Effectiveness of Ultrasound-guided Costotransverse Joint Injection in Thoracic Back Pain Patients

  • Because of its anatomical location and function, the costotransverse (CTRV) joint can be a source of thoracic back pain. In this retrospective observational study, we evaluated the clinical effectiveness of the CTRV joint injection in thoracic back pain patients with suspected CTRV joint problems. We enrolled 20 thoracic back pain patients with localized tenderness that was provoked by the application of pressure on the affected CTRV joints. We injected it with 0.5 ml of a ropivacaine and triamcinolone mixture at each level. The mean pre-injection pain score decreased by 37.9% ($7.2{\pm}1.5$ to $4.5{\pm}1.7$, P < 0.001) two weeks after CTRV joint injection. In addition, 70% of patients reported an excellent or good level of satisfaction. We demonstrated that an ultrasound-guided injection of the CTRV joint reduced patients' pain scores and led to a high level of satisfaction at short-term follow-ups in patients with suspected CTRV joint problems.

Shear strength of match-cast-free dry joint in precast girders

  • Shear keys in precast concrete segmental bridges (PCSBs) are usually match-casting which is very labour intensive. In this research, an innovative match-casting-free construction was proposed by leaving small gap between the convex and the concave castellated shear keys in the joints of PCSBs. Specimen experiment, shear strength analysis and numerical simulation were conducted, investigating the loading performance of this new type of dry joints, the gap dry joints. Compared with match-casting joint specimens, it has been found from experiment that shear capacity of gap joint specimens significantly decreased ranging from 17.75% to 42.43% due to only partially constrained and contacted in case of gap dry joints. Through numerical simulation, the effects of bottom contacting location, the heights of the gap and the shear key base were analyzed to investigate strength reduction and methods to enhance shear capacity of gap joint specimens. Numerical results proved that shear capacity of gap dry joints under full contact condition was higher than that under partial contact. In addition, left contact destroyed the integrity of shear keys, resulting in significant strength reduction. Larger shear key base remarkably increased shear capacity of the gap joint. Experimental tests indicated that AASHTO provision underestimated shear capacity of the match-casting dry joint specimens, while the numerical results for the gap dry joint showed that AASHTO provision underestimated shear capacity of full contact specimens, but overestimated that of left contact specimens.

Effects of Joint Position on the Distraction Distance in Patients with Adhesive Capsulitis of Glenohumeral Joint

  • The purpose of this study was to investigate the effects of joint position on the distraction distance in patients with adhesive capsulitis of glenohumeral joint. The study was conducted upon 20 adults in their 40's with the findings of adhesive capsulitis of glenohumeral joint. These subjects were subdivided into 3 groups, which were a group with neutral position(n=7), second group with resting position(n=7) and third group with end-range position(n=6). After having the subject wearing sleeveless shirts exposing armpit and lying straight on the plinth, a physical therapist with OMT qualification pulled glenohumeral joint at the Grade III of Kaltenborn-Evjenth traction; and the distance between glenoid fossa and humeral head was measured with ultrasound. Following the application of traction, the group with resting position($.67{\pm}0.29$) exhibited the longest distance between humeral head and glenoid fossa, and it was followed by neutral position($.50{\pm}0.25$) and end-range position($.35{\pm}.21$) in this order. From the comparison of these groups, there was no significant difference in distraction distance between resting position and neutral position; and there was again no significant difference in distraction distance between end-range position and neutral position. However, there was a significant difference in distraction distance between end-range position and resting position(p<.05). Upon application of the Grade III of Kaltenborn-Evjenth traction, it was evident that the distance between humeral head and glenoid fossa can be varied depending on the location of the joint.

  • For an effective acupuncture treatment, the location of muscles around Sutaeumkyongkun and Suyangmyongkyongkun were researched and they were made contracted. The conclusion is as follows; 1. The contraction of muscles around Sutaeumkyongkun gives appearance of the postures ; free movement of thumb, abduction of extension of wrist, flexion and pronation of elbow, depression and abduction of girdle of superior limb, flexion, internal rotation and horizontal flexion of shoulder joint These postures all together consequently produces the action 'holding something in arms'. 2. The contraction of muscles around Suyangmyongkyongkun gives appearance of the postures; extension of metacarpophalangeal and interphalangeal joint of index finger extension and abduction of thumb, extension of wrist, extension and supination of elbow, adduction, elevation and upward rotation of girdle of superior limb, extension, abduction, adduction, internal rotation, external rotation, horizontal extension of shoulder joint, flexion and opposite rotation of neck. These postures all together consequently produces the action 'raising arms'.
