우리나라 골관절염 환자의 관절통증: 2012년 국민건강영양조사 자료를 이용하여 (Joint Pain in Patients with Osteoarthritis: Based on the 5th KNHNES)

  • Purpose:The purpose of this study was to examine the degree of joint pain, limitation of daily activities, and frequency of outpatient visits. Methods: This secondary analysis study used the data from the fifth Korean National Health and Nutrition Examination Survey (KNHNES). The data about general characteristics, prevalence and treatment of osteoarthritis, limitation of activities of daily living (ADL) and frequency of outpatient visits and hospitalization in 424 osteoarthritis patients over 50 years old were derived from the database. Data were analyzed with complex samples in SPSS ver. 20.0. Results: Among people with knee joint pain, 79.3% reported they had more than 4 out of 10 points of the degree of pain. Meanwhile, 97.3% of people with hip joint pain reported that they had more than 4 out of 10 points of the degree of pain. People reporting 10 out of 10 point of pain were 21.4% of those with knee joint pain and 25% of those with hip joint pain. The mean of the degree of knee joint pain was 6.35, and the mean of the degree of hip joint pain was 6.89 out of 10 points. About 50% of people with osteoarthritis visited an outpatient clinic within the last 2 weeks, and 27% had limitation of ADL. Conclusion: It is necessary to develop an integrated intervention program to improve quality of life in patients with osteoarthritis.

혈우병 환자의 중증도에 따른 건강문제와 관절출혈 및 장애 (Health Problem and Joint Bleeding, Limitation according to Severity in Patients with Hemophilia)

  • Purpose: The purpose of this study was to identify health problem and joint bleeding, disability according to severity in patients with hemophilia. Method: The research design was survey study for conformed severity in patients with hemophilia, health problem and bleeding of joint and degree of limitation. The subjects were 186 men in patients with hemophilia by convenience sampling. These results were analyzed statistically by frequency, percentage, t-test, chi-square. Result: 1. Health problem of severe hemophilia patients were significantly higher than moderate group(t=3.17, p=.002). A severe hemophilia patients were significantly higher than moderate group in health problem of socio-psychological, emotional, physical, interpersonal. 2. There was relation between severity and the frequency of bleeding($X^{2}=7.642$, p=.054). 3. A severe hemophilia patients were more than moderate group in disability of ankle joint(t=2.52, p=.013). Conclusion: A severe hemophilia patients has more severe problems than moderate patient in case of health problem, joint bleeding and degree of disability.

Correlation between clinical symptoms and magnetic resonance imaging findings in patients with temporomandibular joint internal derangement

  • Objectives: The purpose of this study was to clarify which findings in magnetic resonance imaging (MRI) are good predicators of pain and mouth opening limitation in patients with temporomandibular joint (TMJ) internal derangement (ID). Materials and Methods: Clinical examinations for pain and mouth opening limitation were conducted for suspected TMJ ID. MRI scans were taken within a week of clinical examinations. On the oblique-sagittal plane image, readings were obtained in terms of the functional aspect of disc position, degree of displacement, disc deformity, joint effusion, and osteoarthrosis. Multiple logistic regression analyses were conducted to identify the predictors of pain and mouth opening limitation. Results: A total of 48 patients (96 TMJs) were studied, including 39 female patients and 9 male patients whose ages ranged from 10 to 65 years. The resultant data showed significant correlations between pain and the MR imaging of the degree of disc displacement (P<0.05). The probability of there being pain in moderate to significant cases was 9.69 times higher than in normal cases. No significant correlation was found between mouth opening limitation and MRI findings. Conclusion: We identified a significant correlation between clinical symptoms and MRI findings of ID. The degree of anterior disc displacement may be useful for predicting pain in patients with TMJ ID.

  • The purpose of this study is to know load for each human body's knee joint to endure load efficiently, when weight lifter pulls suitable bar in the each weight lifting level. To use the difference of load and correlation between men and women, the conclusion of this stduy is to find parameters to conduct maximum limitation of maximum load to search maximum limitation. The moment of each joint in the lower area was to have a much difference between group of the turunk in the first step. It decreased rapidly until decent and greatly pulled in the second step. Neuromusclar moment increased suddenly in the decrease phase. This phenomenon resulted from shock-absorbing function to sbsorb a load from bar.

측두하악장애 환자의 임상적 양태에 대한 연구 (A prevalence of clinical sign and symptom in temporomandibular disorders patients)

  • A prevalence of temporomandibular disorders(TMD) based on the clinical sign and symptom in 155 patients were investigated. History taking with interview chart and clinical examination were performed. Age and gender of the patients, duration of TMD, location of pain, joint sound, limitation of mouth opening and more detailed diagnostic name were identified. The results of this study were as follows: 1. TMD was more prevalent in female than in male. TMD was the most prevalent at the age of 20s and decreased with age. 2. Acute TMD was more prevalent than chronic one. 3. About ninety percent of TMD patients had pain. Pain had mainly a unilateral origin. Muscle pain was mainly related with the masseter muscle. 4. Joint sound was identified in about fifty-six percent of the TMD patients and unilateral joint sound was more prevalent than bilateral one. 5. Limitation of mouth opening was observed in about forty percent of the TMD patients. 6. In TMD patients, muscle dysfunction was the most prevalent one. In muscle disorders local muscle soreness was the most prevalent one. In joint disorders, capsulitis was the most prevalent one. In muscle-joint disorders, trigger point pain with retrodiscitis was the most prevalent one.

악관절장애 환자 치험 2례 (Two Cases of Temporomandibular Joint Disorder)

  • Temporomandibular joint disorder is quite common dental problem. Research has shown that 5 to 15$\%$ of the population have a TMJ disorder requiring some type of treatment. TMJ disorder's symptoms are pains in the jaw and muscles of your face, limitation of opening or closing jaw and noises in jaw joint such as clicking or popping sounds. Two patients with TMJ symptoms visited our clinic. They complained pain, limitation of jaw opening and clicking sound. We treated them by stimulating trigger points on the TMJ muscle with acupuncture and had a good effects. The symptoms is reduced or disappeared.

  • Background: The purpose of this study is to evaluate the outcomes of four patients receiving stock Biomet TMJ prosthesis for reconstruction of the TMJs. Methods: TMJ reconstruction with stock Biomet TMJ prosthesis was performed in four patients who had joint damages by trauma, tumor, resorption, and ankylosis, which represent the indications of alloplastic prosthesis. Results: Loss of condyle from trauma and resorption of joint are good indications for prosthesis, but the patients should be informed about limitation of jaw movement. In case of structural damage of TMJ by tumor, tumor recurrence should be considered before planning TMJ reconstruction. Considering heterotopic bone formation in case of ankylosis, periodic follow-up and special surgical technique are required. Conclusions: Given careful treatment planning and understanding the functional limitation of TMJ prosthesis, alloplastic prosthesis is a safe and effective management option for the reconstruction of TMJs.

Updates on the treatment of adhesive capsulitis with hydraulic distension

  • Adhesive capsulitis of the shoulder joint is a common disease characterized by pain at the insertional area of the deltoid muscle and decreased range of motion. The pathophysiological process involves fibrous inflammation of the capsule and intraarticular adhesion of synovial folds leading to capsular thickening and contracture. Regarding the multidirectional limitation of motion, a limitation in external rotation is especially prominent, which is related to not only global fibrosis but also to a localized tightness of the anterior capsule. Ultrasound and magnetic resonance imaging studies can be applied to rule out other structural lesions in the diagnosis of adhesive capsulitis. Hydraulic distension of the shoulder joint capsule provides pain relief and an immediate improvement in range of motion by directly expanding the capsule along with the infusion of steroids. However, the optimal technique for hydraulic distension is still a matter of controversy, with regards to the infusion volume and rupture of the capsule. By monitoring the real-time pressure-volume profile during hydraulic distension, the largest possible fluid volume can be infused without rupturing the capsule. The improvement in clinical outcomes is shown to be greater in capsule-preserved hydraulic distension than in capsule-ruptured distension. Moreover, repeated distension is possible, which provides additional clinical improvement. Capsule-preserved hydraulic distension with maximal volume is suggested to be an efficacious treatment option for persistent adhesive capsulitis.

무릎 인대 손상환자의 퇴원 전.후 관심사 (A study the concerns of patient with the knee ligament injury)

  • This study has purpose to perform leaving the hospital nursing plan during hospital treatment of patient with the knee ligament Injury on the basis of the concerns between the time of leaving the hospital after 2 weeks in hospital and the concerns through practical life after leaving the hospital systematically. This study is also tried to make exercise plan to prevent atrophy of Quardriceps muscle of the thigh caused by the limitation of activities. The subjects of study are 12 people who were in the hospital in affiliation to a university in Seoul, were operated on the knee ligament, and were discharged from the hospital within 2 weeks. Data collection time was for about 4 months from December, 94 to March, 95. Research method was performed 2 times by unorganized open questions. The collection of first data is through direct interview about the concerns before leaving the hospital, and secondary data is through direct interview about the concerns by practical life and experiences after leaving the hospital. The data was categorized by classifying several common elements among similar contents according to data by the situational analysis of Giorgi. The result of this study is as follows : 1) The concerns before leaving the hospital are like that the concerns of recovery possibility, the limitation of activities, the grim realities of life, the lack of confidence about self-nursing, the eyes of the surroundings, the burden to the others, and so on. 2) The concerns after leaving the hospital are like that the demands of the inconvenience by the limitation of self-nursing, the support about self-nursing by the difficulties of performing treatment order, the support of physical activities limitation by physical tiredness and the limitation of social activities, the support of information by the necessities of education in using crutches before leaving the hospital and continuing informational needs, psychological and mental support of normal physical recovery by the worries of physical recovery and physical change, and social support by thankfulness for his family and relatives. I suggest to make more systematic leaving the hospital nursing plan and exercise education plan for nursing plan after leaving the hospital of the patients with the knee ligament injury on the basis of this content.

  • Due to less turbulence and no land limitation, offshore wind energy gets more attention than onshore. Jacket structure is regarded as a suitable solution for the water depth ranging from 30 to 80 meters. In general, joint stress concentration of jacket support structures affects their fatigue life. Nowadays, most jacket structures for offshore wind turbines have tubular X-joint between legs. In this paper, a study on X-joint stress concentration of offshore wind turbine jacket structure is performed by using 50m water depth model. Stress of X-joint on offshore environmental conditions are discussed.
