Effect of the Hip Joint Adduction on Walking During the Plank Exercise (엉덩관절모음과 플랭크운동이 보행에 미치는 영향)

  • The purpose of this study is to see how the stability of posture due to elastic band during plank movement affects walking. Thirty healthy men in their 20s were given a total of 30 minutes of 10 general plank exercises, 10 plank exercises with two hip joints adduction, and 10 plank exercises with one hip joint adduction. The plank movement was conducted three times a week for a total of six weeks, with a five-minute warm-up. The results of this study are as follows: There was a significant difference between period and group interactions on cadence (p<0.05), with significant difference between the period and group between the left foot on stride length of the left side foot (p<0.05), with significant difference between the period and the period of the interaction between the right foot and the period on stride length of the right side foot (p<0.05), and a one-step change between the right foot and the military interaction on Step time of the right side foot(p<0.05). The conclusion of this study is that the resistance movement on the adduction of both hips increases the instability of the trunk over the normal plank movement and adduction of one hip, thereby enhancing walking ability along with the stability of the trunk.

Debridement Arthroplasty using Posteromedial Approach in Elbow Joint with Pain and Bony Limitation of Motion (동통과 골성 운동 제한이 있는 주관절에서 후내측 도달법을 이용한 변연 절제 관절 성형술)

  • Purpose: This study evaluated the clinical outcomes of debridement arthroplasty using the posteromedial approach in elbow joints with pain and bony limitation of motion. Materials and Methods: This study involved 16 elbows in 16 patients with pain and bony limitation of motion, which were treated by debridement arthroplasty using the posteromedial approach from March 2005 to March 2008. The mean follow up period was 27.6 (13~52) months. The clinical outcomes were analyzed using the Visual Analogue Scale(VAS) for pain scale, the preoperative and postoperative range of motion and the Mayo Elbow Performance Scores(MEPS). Results: The VAS was decreased significantly from a preoperative mean of 4.5 to a postoperative mean 1.1 (p<0.001). The average arc of motion improved significantly from $61.6 (0~90)^{\circ}$ preoperatively to $109.4 (80-120)^{\circ}$ postoperatively (p<0.001). The MEPS also improved significantly from 59.4 to 85.6 postoperatively (p<0.001). There were no complications, such as hematoma and elbow instability. Conclusion: Debridement arthroplasty using the posteromedial approach is a useful surgical procedure in the elbow joint with pain and bony limitation of motion, where all compartments can be debrided, the ulnar nerve can be manipulated easily and damage to the medial collateral ligament can be minimized.

Anterior Cruciate Ligament Reconstruction Using Bone-Patellar Tendon-Bone Autograft - The comparison of mid term & long term follow-up results - (골-슬개 건-골을 이용한 전방 십자 인대 재건술 - 중기 및 장기 추시 결과의 비교 -)

  • Purpose : To compare the mid term with long term follow up results of the anterior cruciate ligament(ACL)> reconstruction using patellar tendon. Materials and Methods : Seventy-six cases, which could be followed up more than 2 years were included in this study. They were classified into two groups, mid-term group(41 cases) with mean follow-up periods of 2 years and 10 months and long-term group(35 cases) off years and 6 months. Clinical and radiological results and peripatellar complications were evaluated & compared. Results . There was no statistically significant difference in radiological finding and anterior instability between two groups. However, the incidence of anterior knee pain, paresthesia at incision site, kneeling pain, crepitus and quadriceps weakness was lower in long-term group than in mid-term group. Conclusion : ACL reconstruction using patellar tendon graft could successfully restone the stability of knee joint, but there are some complications in donor site and patellofemoral joint. However this complications were gradually decreased with long term follow-up.

The Effects of Coordinative Locomotor Training Program for Life-Care Promotion on Balance of Obese Elderly Women (라이프케어 증진을 위한 협응적 이동훈련 프로그램이 비만 여성노인의 균형에 미치는 영향)

  • The purpose of this study was to examine the effects of coordinative locomotor training (CLT) program on the balance for the promotion of life care of elderly obese women. Ten participants of elderly obese women who were able to walk independently without surgery experience of lumbar, hip or knee joint within the past year were recruited and under went the pretest, CLT (20 sessions), followed by the post-test. The test included BMI test using In-body, joint kinematics using myoVIDEO, muscle activation using surface EMG test (erector spinae (ER), external oblique abdominalis (EO), quadriceps femoris (Quad), hamstring muscle (Ham)) and balance tests including dynamic balance test using forced treadmill, Berg balance scale (BBS) and timed up go (TUG). The CLT program was conducted 60 minutes a day, 5 days a week, over 4 weeks period. As a result of this study, The the trunk and hip joints kinematics during the stance and swing phases of gait were a statistical significance levels were set at p <0.05. The ER and EO muscle activation were significantly improved after intervention (p <0.05). The length of gait line and single support line of change of center of pressure (COP) were significantly increased after intervention (p <0.05). The BBS and TUG were also significantly enhanced after intervention (p <0.05). The results of this study showed that CLT program for the improvement of life care had significant effects on improving postural instability, muscle weakness, reduced balance ability and falling risk of obese elderly women. Therefore, it is recommended to apply CLT program to improve life-care through improving balance ability and preventing fall of obese elderly women.

Analysis of Kinetic Differences According to Ankle Taping Types in Drop Landing (드롭랜딩 시 발목테이핑 유형에 따른 운동역학적 차이 분석)

  • The purpose of this study was to compare and analyze kinetic variables of lower limbs according to types of ankle taping in drop landing. For this, targeting seven male basketball players (average age: $20.8{\pm}0.74yrs$, average height: $187.4{\pm}3.92cm$, average weight: $79.8{\pm}7.62kg$) with no instability of ankle joints, the drop landing motion was conducted according to three types of inelastic taping (C-type), elastic taping (K-type), and no treatment (N-taping). Based on the result, the next conclusion was reached. First, the effect of taping for the players with stable ankles was minimal and the high load on ankle joints offset the fixing effect of inelastic taping. Thus the inelastic taping for the players with stable ankles did not have an effect on the control of dorsal flexion during one-foot landing. Second, increasing angular velocity by increasing the movable range of knee joints disperses impact forces, yet inelastic taping restricted the range of knee joint motion and at the same time increased angular velocity, adding to a negative effect on knee joints. Third, inelastic taping induced inefficient motion of Lower limbs and unstable impact force control of ankle joints at the moment of landing and produced maximum vertical ground reaction force, which led to an increase of load. Therefore, inelastic ankle taping of players whose jump actions occur very often should be reconsidered. Also, it is thought that this study has a great meaning in proving the problem of inelastic taping related to knee pain with unknown causes.

Treatment of Surgical Site Infection and Delayed Union in Fetlock Arthrodesis of a Mare

  • A 6-year-old Thoroughbred mare presented to the Korea Racing Authority Equine Hospital with dropping of the left front fetlock due to an injury sustained while racing. Radiographic examination revealed a comminuted fracture of both proximal sesamoid bones of the affected fetlock. Arthrodesis of the fetlock joint using a broad dynamic compression plate with a tension band wire was performed as a salvage procedure for the future use as a broodmare. After surgery, however, a delayed union of the bones and surgical site infection was present for a prolonged period. Staphylococcus aureus was persistently identified from the surgical site, and antimicrobial therapies were based on antibiotic sensitivity tests, including regional perfusions. The removal and replacement of surgical implants associated with seropurulent discharge was based on coordinating the development of fetlock ankylosis and infection control over 13 months. Firstly, seven screws associated with surgical drainage were replaced and bone morphogenetic protein-2 (BMP-2) and local antibiotics were placed into the surgical site to accelerate bone fusion at postoperative month 7. Further six screws, along with drainage, were removed at postoperative month 10. The plate and screws were removed from the limb due to the progression of bone fusion at postoperative month 13; BMP-2 and local antibiotics were also used. Delayed healing of arthrodesis due to surgical site infection and implant instability were treated by implant removals and antibiotic therapies, and the horse eventually showed improved weight-bearing ability of the affected limb.

Biceps Load Test: A Test of SLAP lesion in the Recurrent Anterior Dislocation of the Shoulder (이두건 부하 검사(Biceps Load Test): 견관절 재발성 전방 탈구시 SLAP 병변 진단의 새로운 검사방법)

  • The following will describe a method of evaluating the SLAP lesion in the recurrent anterior dislocation of the shoulder. We have named it the biceps load test. The biceps load test is performed with the patient in the supine position and the arm to be examined is abducted 90/sup°/, and the forearm is in the supinated position. First, the anterior apprehension test is performed. When the patient become apprehensive, the patient is allowed active flexion of the elbow, while the examiner resists elbow flexion. If the apprehension is relieved or diminished, the test is negative. If aggravated or unchanged, the test is positive. A prospective study was performed, in which 75 patients who were diagnosed as having recurrent unilateral anterior instability of the shoulder underwent the biceps load test and arthroscopic examination. The biceps load test showed negative results in 64 of these patients, of which the superior labral-biceps complex was intact'in 63 cases and only I shoulder revealed a type n SLAP lesion. E]even patients with a positive test were confirmed to have type n SLAP lesions. A positive biceps load test represents an unstable SLAP lesion in a patient with recurrent anterior dislocation of the shoulder. The biceps load test is a reliable test for evaluating the SLAP lesion in the recurrent anterior dislocation of the shoulder(sensitivity: ,9] .7%, specificity: 100%, positive predictive value: 1.00 and negative predictive value: 0.98). Biceps contraction increases the torsional rigidity ?of the glenohumeral joint and long head of biceps tendan act as internal rotator of the shoulder in the abducted and externally rotated position. These stabilize the shoulder in abduction and external rotation position in the biceps load test.

  • The muscular button holing of vastus medialis is a very rare case of irreducible knee dislocations, and rapid reduction of this can diminish the complications which delayed reduction accompanies. We diagnosed a patient who appeared posterolateral dislocation of the knee and protrusion of the medial femoral condyle with MRI grossly. That was reduced by open arthrotomy, followed the reconstruction of both cruciate ligaments and repair of medial collateral ligament. Patient didn't show joint instability except minor posterior sagging and had full range of motion postoperatively after 10 months.

The Value and Limitation of the Modified Mau Osteotomy (30 Cases Follow Up Report) (변형 마우 절골 술기의 유용성과 한계점 (30예 추시 보고))

  • Purpose: To evaluate the value and limitation of modified Mau osteotomy through the review of 30 feet treated by this procedure. Materials and Methods: We retrospectively analyzed 30 cases treated with modified Mau osteotomy since 2002. The mean duration of follow-up was 10 months. We reviewed medical records to describe each case and select several clinical factors which related with surgical procedure and could influence on final results. We measured radiographic parameters such as hallux valgus angle (HVA), intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA), sesamoid position and also assessed clinical outcomes by AOFAS score and satisfaction degrees. Results: The mean preoperative HVA and IMA were $40.4^{\circ}$, $17.4^{\circ}$ and the mean amounts of correction were $31.2^{\circ}$ and $11.5^{\circ}$. Amounts of delayed loss of correction were 16.8% in HVA and 19.2% in IMA. Initial HVA, rotational angle and translation distance of the distal fragment, stability of fixation, first ray instability were revealed as significant factors for the final result from this procedure. DMAA was increased by rotation of the distal fragment and decreased by adding translation on the rotation. Conclusion: Modified Mau osteotomy is an effective procedure to get enough correction. But, it is important to try to avoid excessive rotation of the distal fragment because it may worse joint congruity. It may be worthwhile to pay close attention to the direction of saw and stability of fixation.

  • Rock property of the Seosan Maaesamjonbulsang is composed of biotite granite with medium grained texture. The triad Buddha is highly deteriorated by the joint, fracture, break-out, exfoliation, dew condensation of the surface and discoloration of the secondary pollutant. Host rock of the triad Buddha is divided dozens of rock blocks with various shape, and developed irregular discontinuity planes. Besides, the host rock is promoted biological pollution due to the surrounding crowded vegetation and high humidity environment. As the results of structural stability, it is confirmed that developed discontinuity system in the host rock is exposed instability sloping environment. Therefore, the host rock and surroundings are required maintenance, and required preservation by continuance monitoring for understand behavior of discontinuity system.
