Diagnostic Application of Temporomandibular Joint Disorder and Ultra Sound Guided Oral &Maxillofacial application (초음파를 이용한 턱관절질환의 진단과 초음파 가이드의 활용)

  • Ultrasound images are noninvasive, can be observed in real time, have no radiation exposure, do not cause pain, and are not restricted in use depending on the patient's prosthetic implant or medical condition. Since the use of ultrasound in the dental field was first applied for tooth preparation in 1957, the use of diagnostic ultrasound for the first time in 1963 has been reported. Currently, it is used in the diagnosis of soft tissue lesions such as malignant tumor or salivary gland disease, fine needle aspiration test, temporomandibular joint disease, lymph node metastasis, measurement of muscle thickness and inflammatory diseases, differentiation of periapical cyst and granulation tissue, measurement of periodontal tissue thickness. The ultrasound image can be visualized in real time. The clinician can explain the structure to the patient while consulting the patient and consult the patient. When injecting the drug into a specific site or aspirating a specific site or substance, So that it can be confirmed and practiced. Recently, ultrasonic equipment specialized in the dental field has been developed and marketed, and it is expected that the use of ultrasonic waves will become active in the dentistry. In the future, development of popular equipment with size and frequency suitable for dental diagnosis and various researches on maxillofacial ultrasonic anatomy. If clinical studies are continuously carried out to demonstrate efficacy, ultrasound is expected to aid in accurate diagnosis and treatment throughout the dentistry.

Knee Articular Cartilage Segmentation with Priors Based On Gaussian Kernel Level Set Algorithm (사전정보를 이용한 가우시안 커널 레벨 셋 알고리즘 기반 무릎 관절 연골 자기공명영상 분할기법)

  • The thickness of knee joint cartilage causes most diseases of knee. Therefore, an articular cartilage segmentation of knee magnetic resonance imaging (MRI) is required to diagnose a knee diagnosis correctly. In particular, fully automatic segmentation method of knee joint cartilage enables an effective diagnosis of knee disease. In this paper, we analyze a well-known level-set based segmentation method in brain MRI, and apply that method to knee MRI with solving some problems from different image characteristics. The proposed method, a fully automatic segmentation in whole process, enables to process faster than previous semi-automatic segmentation methods. Also it can make a three-dimension visualization which provides a specialist with an assistance for the diagnosis of knee disease. In addition, the proposed method provides more accurate results than the existing methods of articular cartilage segmentation in knee MRI through experiments.

Model Construction for Treatment-Seeking Behaviors in Patients with Arthritis (관절염 환자의 치료추구행위 분석에 의한 이론구축 - 병원치료와 민속요법 이용행태 -)

  • This study was undertaken to explore the antecedent factors and process of the treatment-seeking behaviors of medical and alternative treatments in patients with arthritis using methodological triangulation. The data were collected from 995 arthritic patients who were registered either in a center of rheumatology for medical treatment or residents of community having no treatment to classify different treatment patterns. Sixteen patients with various types of treatment only, alternative treatment only, and no treatment were selected among the total samples to identify the antecedent factors through in-depth interview. The quantitative data were analyzed by percentile, t-test, chi-square test and discrimant analysis using SAS PC program, while the qualitative data were analyzed by means of grounded theory methodology. Treatment-seeking behaviors of patients change from the early stage to the sick-role stage. At the early stage, initial characteristics of pain and acculturation of medical professionalism affect the choice of treatment patterns. The acculturation of medical professionalism is affected by health care accessibility, level of education, duration of sickness and lay referral system. At the sick-role stage, lay referral system and acculturation of medical professionalism affect the choice of treatment patterns. The acculturation of medical professionalism is affected by characteristics of symtoms, perceived treatment effects, perceived causes of diseases and socio-economic status as well as health care accessibility, level of education and lay referral system. In conclusion, different factors as well as common factors are influencing the treatment-seeking behaviors depending on the disease and treatment stages. More detailed further studies are required to explore the value system or medical acculturation of patients which is one of the most important factors in decision-making about treatment modalities.

  • Frozen shoulder is known to be a self-limited disease, and it is associated with chronic pain and limitation of joint movement. Although its etiology is still unknown, frozen shoulder is associated with several diseases. The diagnosis is made based on the medical history, the clinical and radiological examinations and exclusion of other shoulder pathologies. The skeleton is one of the most common sites of metastasis in patients with lung cancer. It has been reported that the incidence of bone metastases in lung cancer patients is approximately 30-40%, and the median survival time of patients with such metastases is 6-7 months. We experienced a case of a 77-year-old female patient who complained of right shoulder pain and limited joint mobility, and these symptoms were due to metastatic lung cancer in the shoulder.

  • Human beings have made remarkable advances in medical technology as well as technological advances. However, as was the case in the past, incurable diseases still exist: temporo-mandibular joint (TMJ). The diagnosis of the Korean medical staff, currently called a "medical advance," is adhering to the outdated patient's comments, diagnosis using a doctor's auditory diagnosis and a ruler, and diagnosis of X-ray imaging. Therefore, it is important to have accurate patient symptoms, to have a doctor's own diagnosis and experience, to increase the number of diagnoses due to the severity of the symptoms, and to cover the costs of medical care. To solve this problem, the core conductive signal generated from the bridge was quantified through %MVC. Quantified EMG will be assessed and compared with Cortex to establish a jaw joint condition evaluation criterion.

The Utilization of Dental Hygienists in Oral Healthcare Exchanges between the South and North Korea

  • Background: This study aimed to identify the role of dental hygienists in exchanges between North and South Korea to lower gaps in the level of dental healthcare between the two countries by conducting a Delphi survey with specialists and identifying alternative policies regarding the utilization of dental hygienists in such exchanges. Methods: Two Delphi surveys were conducted with the participation of nine specialists, and descriptive statistical analyses including mean and standard deviation were performed on the collected data. Results: Among methods of exchange and cooperation regarding oral healthcare under the current North Korean medical system, the issue considered most urgent was the "establishment of oral healthcare infrastructure." The most important short-term strategy was identified as the "selection and formation of partnerships in the field of inter-Korean oral health exchange and cooperation." The mid-term strategy was identified as the "establishment of cooperation in the dental industry, centered on educational cooperation projects." The long-term strategy included "joint R&D projects, oral health surveys, and business development." In order to determine how best to use dental hygienists during inter-Korean exchanges and cooperation, the respondents placed urgency on the "establishment of joint cooperation projects for oral health promotion and early examination and the treatment of dental diseases and planning of community research projects" and "the role of oral health education and media development for residents." Conclusion: Cooperation is necessary regarding the preparation of oral healthcare exchanges that aim to encourage unity between North and South Korea and reduce the gaps between the North and South regarding oral health conditions. Therefore, continuous and reasonable discussions and research are needed regarding the utilization of dental hygienists in such exchanges.

  • Purpose: Metabolic syndrome is known as a factor that increases the incidence of chronic diseases, such as diabetes and cardiovascular disease. In particular, the metabolic syndrome among a middle-aged population is rapidly increasing from 15.6% to 31.9%. The purpose of this study was to investigate the effect of muscle strength exercise on the metabolic syndrome in middle aged. Methods: This study was a secondary data analysis using National Health and Nutrition Survey 8th, including 2,739 middle aged people (40~64 years old). We used multivariate logistic regression to identify risk factors associated with metabolic syndrome. Statistical analysis was performed using the SPSS 23.0 program. Results: There were 772 patients in the group with metabolic syndrome and 1,967 patients in the non-metabolic syndrome group. The risk of metabolic syndrome was 1.29 times higher in those who did not do muscle strength exercise than those who did exercise (OR=1.29, 95% CI=1.01~1.66). Conclusion: We have found that muscle strength exercise was effective in lowering the risk of developing metabolic syndrome in middle aged. Thus, it is necessary to develop practical muscle strength exercise and education programs.

Patellofemoral contact mechanics after transposition of tibial tuberosity in dogs

  • Background: Tibial tuberosity transposition (TTT) causes caudalization of the patellar ligament insertion in canine medial patellar luxation, which can lead to increases in patellofemoral contact pressure. Objectives: The purpose of this study is to confirm the effect of patellofemoral contact mechanics after craniolateral and caudolateral transposition of tibial tuberosity in normal canine hindlimbs. Methods: Craniolateral and caudolateral transposition of tibial tuberosity was performed in 5 specimens, respectively. The pressure was measured in the specimen before TTT, and then in the same specimen after TTT. In this process, data was obtained in 10 specimens. The measurement results were output as visualization data through the manufacturer's software and numerical data through spreadsheet. Based on these 2 data and the anatomical structure of the patellofemoral joint (PFJ) surface, whole measurement area was analysed by dividing into medial, lateral and central area. Results: In craniolateralization of tibial tuberosity, total, medial, central contact pressure was decreased and lateral contact pressure was not statistically changed lateral contact pressure than normal PFJ. In caudolateralization of tibial tuberosity, total, lateral contact pressure was increased and medial contact pressure was not statistically changed than normal PFJ. Although not statistically significant changed, central contact pressure in caudolateralization of tibial tuberosity was increased in all 5 specimens. Conclusions: These results imply that traditional TTT, prone to caudal shift of patellar tendon, can increase retropatellar pressure may lead to various complications and diseases of the stifle joint.

Effects of Kinesio Taping on Edema Control in Patients With Musculoskeletal Injuries: A Literature Review

  • Background: The application of Kinesio tape (KT) has become an alternative treatment for the reduction of edema owing to its distinct characteristics that mimic skin behavior. Although many studies have found that KT application has a positive effect on edema related to breast cancer and rehabilitation following mandibular third molar surgery, there is little evidence to support the use of KT for musculoskeletal injuries. Objects: The purpose of this study was to review the literature related to KT application for reducing edema caused by musculoskeletal disorders. Methods: A literature search (July 2022) was performed on PubMed for articles published between January 2012 and June 2022. The following keywords were used: "Kinesio taping," "Kinesio tape," "swelling," and "edema," with different combinations and derivations. Only articles available in English were included in this study. Results: Among 68 identified studies, seven met our search strategy and criteria and were included in the literature review. Five of these studies investigated musculoskeletal disorders of the knee joint; two of them reported that KT application had a positive effect on edema measured using perimetry following total knee replacement and anterior cruciate ligament reconstruction. However, the KT application did not improve swelling in patients with acute lateral ankle sprains. Pediatric patients with acute proximal phalangeal joint sprain experienced a more significant improvement in the reduction of swelling than the group using a splint. Conclusion: This literature review found discrepant evidence to support using KT for edema control in musculoskeletal disorders. Further research is needed to determine the effectiveness of KT for controlling edema following musculoskeletal injuries.

Study of the Effect and Underlying Mechanism of Clove Extract on Monosodium Iodoacetate-Induced Osteoarthritis in Rats (정향(丁香) 추출물이 골관절염 흰쥐에 미치는 효과 및 기전 연구)

  • Objective: The aim of this study was to identify the efficacy and underlying mechanism of cloves as an osteoarthritis (OA) treatment in a monosodium iodoacetate (MIA)-induced rat OA model. Osteoarthritis (OA) is nowadays one of the most prevalent degenerative joint diseases. Methods: Sprague-Dawley rats treated with MIA (50 μL; 80 mg/mL) were used as in vivo OA models. Cloves (100 and 200 mg/kg b.w.) were administered orally once daily for 2 weeks from 7 days after MIA injection. Changes in hindpaw weight distribution (HWD) were measured as a joint discomfort index. Activation markers related to inflammatory responses and cartilage degeneration in the right knee joints were evaluated by serum analysis and western blotting. Results: HWD decreased in the MIA control group but showed a dose-dependent elevation after clove treatment. Clove treatment inhibited inflammatory factors by PI3K/Akt/NF-κB signaling pathways, while also activating antioxidant factors through Sirt1/AMPK signaling pathways. Clove treatment also suppressed matrix metalloproteinase (MMP) overexpression and significantly increased the levels of tissue inhibitors of metalloproteinases (TIMPs). Conclusions: Treatment with cloves effectively reversed MIA-induced effects. Therefore, clove treatment could have the potential to protect against or treat OA.
