To establish the fundament for EBM of Traditional Korean Medicine, the papers on Pyeongwi-san (Pingwei-san) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. The papers were classified by the registration of domestic or international journals, the year of publishment, experimental fields and the kinds of studies on biological activities. Of total 518 papers on Pyeongwi-san (Pingwei-san), 32 volumes were selected according to selection creteria. 20 volumes were published in domestic journals, 11 in Chinese journal and 1 in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Pyeongwi-san (Pingwei-san) using HPLC, GC-MS. The papers on biological activities of Pyeongwi-san (Pingwei-san) showed improvement of gastrointestinal activity and water-electrolyte metabolism, immune regulation, anti-oxidant, anti-inflammatory, analgesic, anti-convulsant activities, hypnotic duration, blood pressure regulation, hepatic protection, congestive heart failure, anti-cancer activities. Further studies including gastrointestinal activity need to be preceeded to establish the fundament for EBM of Pyeongwi-san (Pingwei-san).
This is a preliminary study for examining the identity issue of the current Traditional Korean Medicine(TKM). In order to examine the identity issue, it is necessary to understand "what" current TKM is. The current TKM has been formed in the complicated historical settings and the newly formed academic geography of modern times, completely different from those of the traditional era. This paper took diachronic and synchronic approaches in order to understand the current TKM. In the process of modernization and scientization of the TKM-which had begun in the early twentieth century-the western medical knowledge merged into the TKM. Also, as the College education of the TKM began after the Liberation from the Japanese colonial rule, the TKM scholars accepted the western medical department system to develop the TKM into professional knowledge. Meanwhile, since the late 1970s, the TKM has been influenced by the modern Traditional Chinese Medicine (TCM), and the TCM knowledge also merged into the TKM. And recently, the research methodology of the complementary and alternative medicine (CAM), and the clinical research technique of the western medicine-like Evidence based Medicine (EBM)-are affecting the scientization and clinical study of the TKM. The current TKM has been formed as a result of the hybridization of these different knowledges. These changes in the TKM were inevitable for the survival of it in the fast changing world. But these changes brought with them problems. Among those, the identity issue is the most important and crucial.
Objective : To establish the fundament for EBM of Traditional Korean Medicine, the papers on Bojungikgitang(Buzhongyiqi-tang) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. Methods : The papers were classified by the registration of domestic or international journals, the year of publishment, experimental fields and the kinds of studies on biological activities. Results : Among 2005 volumes of papers on Bojungikgi-tang(Buzhongyiqi-tang), 219 volumes were selected according to selective criteria. 71 volumes were published in domestic journals, 52 volumes were in Chinese journal, 96 volumes were in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Bojungikgi-tang(Buzhongyiqi-tang) using HPLC method. The papers on biological activities of Bojungikgi-tang(Buzhongyiqi-tang) showed mainly immune regulation, anti-infection, anti-cancer, gastrointestinal activity, neuropsychiatric regulation, anti-inflammation, hepatoprotection, musculoskeletal regulation. Among biological activities, papers on immume regulation were reported mostly. Conclusions : The efficacy of Bojungikgi-tang(Buzhongyiqi-tang) could be related to immune regulation, gastrointestinal activity, anti-fatigue and further study need to be preceeded to establish the fundament for EBM of Bojungikgitang(Buzhongyiqi-tang).
"Kenjurok" is a set of medical records of Todo Yoshimasu, a Japanese docotr in the eighteenth century, who suggested that all diseases have originated from a poison, which is his own pathological term describing abnormal states of the body, The 54 records in "Kenjurok" were analyzed in statistical respects, including gender ratio, demographic distribution of patients, types of diseases, and herbal prescriptions used. Among 54 cases, male patients outnumbered female, as much as four times. The patients were quite evenly distributed according to ages. In 23 cases out of 54, abdomen palpation data were mentioned, Majority of the prescriptions used were originated from Sanghanron(傷寒論:Treaties on Febrile Diseases)/Geumgeyoryak(金匱要略: Synopsis of Golden Chamber). In frequency of use of prescriptions, however, showed somewhat different result, that is although Sanghan/Geumge prescriptions were used most often, esoteric prescriptions handed down in his family also composed significant part. The speculations derived from these statistical results are: Although Todo favored abdommen palpation to locate the poison and to decide a prescription, the proportion of abdomen palpation was not as high as expectation, He did use prescriptions not only in Sanghan/Geumge, but also other diverse prescriptions, rather often than not, which are regarded unique Japanese traditional prescriptions including poisonous minerals such as mercury and arsenic.

Objective: Leejung-tang (Rechu-to in Japanese) is a traditional Korean herbal formula used for treatment of gastrointestinal disorders such as vomiting, stomach pain, chronic gastritis and gastrointestinal ulceration. The present study was carried out to investigate the potential acute toxicity of Leejung-tang water extract (LJT) by a single oral dose in Crl:CD (SD) rats in compliance with current guidelines. Methods: In the preliminary study, there were no adverse effects such as death, clinical signs, and body weight changes at dose levels of 500, 1000, and 2000 mg/kg/day body weight. Based on the results, a dose of 2000 mg/kg was selected as the toxicological limited dose. LJT was administered once by gavage to male and female rats at dose levels of 0 and 2000 mg/kg bodyweight. During the study period, mortalities, clinical findings, and body weight changes were observed for 14 days following the administration. On day 14 after the treatment, the animals were sacrificed by carbon dioxide overdose and complete gross postmortem examinations were performed. Results: In present study, no treatment-related deaths were observed. There were no adverse effects on clinical signs and body weight changes. In addition, there were no observed gross findings in all groups except for a kidney cyst in the 2000 mg/kg/day female group. Conclusion: The results indicated that LJT did not induce toxic effects at a dose level up to 2000 mg/kg in rats and its median lethal dose ($LD_{50}$) was considered to be over 2000 mg/kg/day body weight for both genders.
Objective This study analyzed randomized clinical trials (RCTs) on pediatric asthma using traditional Chinese medicine (TCM) to determine its efficacy and safety. Methods We searched electronic databases in English, Chinese, Japanese, and Korean and evaluated 35 articles published up to December 28, 2022. Based on data from the literature, we analyzed treatments, results, composition of herbal medicine, frequency of medicinal herbs, and decoction. Results TCM for pediatric asthma was compared to supportive therapy, inhaled corticosteroids, and standardized treatments. Of the 35 studies, 32 showed that TCM treatments were effective in alleviating asthma symptoms. Furthermore, TCM treatment was considered safe compared to common asthma treatments. Among the TCM treatments for pediatric asthma, the most frequent decoctions were Gamisaganmahwangtang (加味射干麻黃湯) and Saganmahwangtang (射干麻黃湯). Ephedra Herba (麻黄), Armeniacae Semen (杏仁), Asiasari Radix et Rhizoma (細辛), Belamcandae Rhizoma (射干) were herbs that were frequently used for asthma in TCM decoctions. Conclusion This review showed that TCM is effective in treating pediatric asthma. More clinical RCTs are needed to confirm the efficacy and safety of TCM treatment.

Results : Among 324 volumes of papers on Bangpungtongseong-san (Fangfengtongsheng-san), 38 volumes were selected according to selective criteria. 18 volumes were published in domestic journals, 20 volumes were in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Bangpungtongseong-san (Fangfengtongsheng-san) using HPLC method. The papers on biological activities of Bangpungtongseong-san (Fangfengtongsheng-san) showed inhibitory effect to obesity, arteriosclerosis, allergic response, inflammation, pain, hyperlipidemia, hypertension, atopy, diabete mallatus and improvement of gastrointestinal activity and cerebral blood circulation. Among biological activities, papers on anti-obesity effect were reported mostly. Conclusions : Further studies including anti-obesity activity need to be preceeded to establish the fundament for EBM of Bangpungtongseong-san (Fangfengtongsheng-san).
Yoshimasu Todo(吉益東洞) said in his book Yakucho, that Ramulus Cinamoni(桂枝) cures ascending impulsively & adversely of poison(衝逆). Here, the meaning of ascending impulsively & adversely of poison(衝逆) is not obvious. The objective of this thesis was to discover the exact meaning of ascending impulsively & adversely of poison(衝逆), through the analysis of Todo's books, and specifically the publication Yakucho(藥徵). Todo hadn't used the term ascending impulsively & adversely of poison(衝逆) from the beginning. He had used the term together with ascending adversely of poison(上逆) and ascending adversely of qi(上氣) in the early stage and had united the term into ascending impulsively & adversely of poison(衝逆) in the middle stage. From this study, it was determined that ascending impulsively & adversely of poison(衝逆) is the combined terms of ascending impulsively of poison(上衝) and ascending adversely of poison(上逆). Considering his view of illness, Todo had explained that every disease is caused by poison in the body. It was appropriate for him to use the term ascending impulsively & adversely of poison(衝逆) when he described the state of attacking and resisting in the body.
Objectives : The purpose is to discuss the clinical applications of Orungsan(Goreisan: 五苓散) as an alternative management for increased intracranial pressure in the field of neurosurgery in Japan. Methods and Results : Attention has focused on Kampo medicine(traditional Japanese medicine) for some cerebral disease including chronic subdural hematoma(CSDH) and cerebral infarction in Japan. Orungsan and one of its classes, Sirungtang(Saireto: 柴苓湯) are well known their effects on brain edema. After some studies of Orungsan has the anti-edemic effects by the inhibition of aquaporin, this herbal medicine has been used widely in the neurosurgery field in Japan. It is high time to think about where we are and we go ahead for the progress and the integration in medicine. We have reviewed the studies using Orungsan or Sirungtang, that was reported at the 20th annual meeting of 'the Japan society for Kampo medicine and neurological surgery' was held on November 5, 2011 in Tokyo. Fifteen studies related with Orungsan or Sirungtang were reported among all 32 studies at the meeting. Orungsan in ten, and Sirungtang in five among 14 studies contained specific clinical case. In the aspects of disease, thirteen papers were related with SDH, including CSDH(11), SSDH(1), aneurism clipping for SDH prevention(1), and one was acute cerebral infarction and one was multiple metastatic brain tumor. In the report style, case control study was 7(mostly retrospective), and the case report was 8. Conclusions : Orungsan may be plausible to be an alternative method to reduce brain edema after SDH and other brain injury in the field of neurosurgery.
