Study about the Increase of Welfare, Medical Care and Education Effect in the River

  • The river technology of Japan was highly developed and achieved much result in flood protection, water use and the environment. Nowadays, the function which is requested to the river is changes because the society changes from growth to maturity. In this study, we suggest the functions of the river in the maturity society about future welfare, medical care and education based on field research of case study. In this research, we pay attention to the functions related with culture, one) function of healing, mental stress improvement and life, two) function of landscape, education and recreation. And this paper proposed the usage of the river where adopt to the times, a function and the value of "the river as the space". We made a questionnaire to 187 river managers, and 90% of answers wear positive for use the river as the medical aspect, health and the welfare. Furthermore, based on research result of hospitals and welfare facilities which used a river for their activities, we suggest to develop and use the river walk, to design a city and rivers as one, and to use many facilities which are located on river front more active, those are improve the new river functions.

A Case-Control Study on the Relationship between Obesity and Female Colorectal Cancer (여성 대장암 발생과 비만의 관련성에 관한 환자-대조군 연구)

  • Objectives : A hospital-based case-control study was conducted to evaluate the role of obesity in the development of colorectal cancer. Methods : Three hundred and twenty four histologically confirmed female colorectal cancer cases and 26,998 non-cancer controls were selected from patients invited to the Aichi Cancer Center, Nagoya, Japan between 1989 and 1995. Information concerning demographic factors, medical history, family medical history, reproductive factors and dietary factors were obtained from self-administered questionnaires and medical records. The effects of weight and body mass index to colorectal cancer were examined using multiple logistic regression to control for other risk factors. Results : There was no significant association between female colorectal cancer and obesity. Heavier weight adjusted for height or body mass index did not increase the risk of colorectal cancer. Conclusions : These results suggest that there is no association between colorectal cancer risk and obesity in women.

Rare Helicobacter pylori Infection May Explain Low Stomach Cancer Incidence: Ecological Observations in Bali, Indonesia

  • The incidence rate of stomach cancer in Bali, Indonesia, is estimated to be strikingly lower than that in Japan. We conducted an on-site ecological study to investigate the association between the stomach cancer incidence and Helicobacter pylori (H. pylori) infection. Recruiting 291 healthy persons (136 men and 155 women) from the general population in Bali, Indonesia, we conducted a urea breath test (UBT) to examine H. pylori infection, along with a pepsinogen test to detect chronic atrophic gastritis and urine analysis to estimate sodium and potassium excretion. UBT positivities were 9% (2-15, 95% confidence interval) for men and 7% (1-12) for women, and positive cases for H. pylori IgG antibodies were 1% (0-3) for men and 3% (0-5) for women, significantly lower than the respective values in Japan. Positive pepsinogen tests in Bali were 0% (0-0) for men and 1% (0-4) for women, also significantly lower than the Japanese figures. Computed values for daily salt excretion were $13.3{\pm}4.1g$ (mean${\pm}$SD) for men and $11.1{\pm}3.1g$ for women, as high as corresponding Japanese consumption values. Moreover, the estimated potassium excretion was $3.2{\pm}0.7g$ for men and $2.8{\pm}0.6g$ for women in Bali, significantly higher than the figures in Japan. There were no associations across genetic polymorphisms of IL-beta, TNF-alpha, and PTPN11 with UBT positivity. The low incidence of stomach cancer in Bali may thus mainly be due to the rare H. pylori infection. Namely, the bacterium infection seems to be a critical factor for gastric cancer rather than host or other environmental factors.

A Case Study on the Local-based Welfare Facilities of Urban Renaissance Agency in Japan by Business Entry System for Stable Living (일본 UR도시기구의 안심주공간사업자참여제도를 통한 지역복지거점시설 정비사례 연구)

  • The purpose of this study is to analyze local-based social welfare facilities to respond to secular change of the UR rental housing estates in Japan. The selected case studies for the analysis are Hibarigaoka, Hamakousien, Turumai UR rental housing estates constructed around 1960's. The analysis was done through UR website, MLIT (Ministry of Land, Infrastructure, Transport and Tourism) and MHLW (Ministry of Health, Labour and Welfare) policy report, and related literature. These case studies are connected with housing policy as well as welfare policy and performed to Stable Living Creation Project by Business Entry System for Stable Living. The types of local-based social welfare facilities are classified by welfare corporation and medical corporation. The results are as follows: 1) The implementation of cooperative and participative system not only the private sector (welfare and medical Corporation, etc.) but also the public (MLIT, MHLW, UR Renaissance Agency, Local government, etc.). 2) Repurposed of land through rent and transfer of land for local-based social welfare facilities in the process of housing stock renewal utilization. 3) The pursue of regional revitalization through attached to multi-generation facility or local community space for elderly in local-based social welfare facilities.

A Comparison of the Certification System for Nurses in HIV/AIDS Care among the U.S., the UK, and Japan (외국의 에이즈 전문간호사제도 비교 연구;미국, 영국, 일본을 중심으로)

  • Purpose: The purpose of this study was to compare the role and the certification system for nurses working in HIV/AIDS care among the U.S., the UK, and Japan. Method: The searching for the internet website and literature was used and visiting the institutes and interviewing the key informants were done from August, 2006 to February, 2007. Results: In the U.S., there are two types of nurse specialist in HIV/AIDS care. One is the HIV/AIDS Certified Registered Nurse which is without any academic degree requirement, and the other is the Advanced HIV/AIDS Certified Registered Nurse which requires master level degree and 5 years career. There are three level of nurses' role in HIV/AIDS care in the UK. Also, the National HIV Nurses Association of the UK developed generic competencies and specialist competencies according to this three level of nurse's role. In the case of Japan, the certification system is not formalized yet, but the AIDS coordinator nurses are working as a specialist in the Japan International Medical Center. Conclusion: Based on this result, it is suggested that the comprehensive role of nurses in HIV/AIDS should be developed under the governmental support.

Consumer Impairment and Its Improvement on Longterm Care Insurance Service : Focused on the Current Conditions and the Analysis of Counseling Cases in Korea and Japan (노인장기요양보험 서비스에 관한 소비자피해와 개선방안 : 한일 서비스이용현황 및 상담사례 분석을 중심으로)

  • This study looked into the current conditions in Korean and Japanese care management for the elderly, cases of consumer counseling in these countries, and the supporting institutions for Japanese users regarding the use of care management for the elderly. The number of recipients of care management for the elderly in both Korea and Japan is growing every year, and more Japanese users receive various services compared to Korean users. The results of an analysis on consumer counseling regarding the use of Korean and Japanese care management revealed, two types of complaints: counseling for the improvement of the institution and complaints related to the procurement of service. Regarding the insurance system, the complaints were mostly related to affirmation of a rating and the burden incurred by cost-sharing. Regarding the use of service, such key impairment cases were related to in-service medical accidents, illegal acts including caregiver contract violations and forced retirement, careless service by workers, and human rights violations of the elderly. Japan has developed governmental and, related-administrative and business services as well as a local governmental system to redress customer impairment issues as this pertains to service for better quality care management. In addition, they have enlarged the locally closed service, provided exact information about the services offered, and improved service appraisal techniques. The Japanese cases will serve as a good reference to improve the Korean system. In particular, the construction of a system that reduces customer losses and the availability of more counseling information are urgently required to improve the system in terms of customer experiences when they seek care management.

Endoscopic Resection for Early Gastric Cancer beyond Absolute Indication with Emphasis on Controversial Issues

  • Endoscopic resection is the established treatment for early gastric cancer in selected patients with negligible risk of lymph node metastasis ('absolute indication'). Based on clinical observations and large pathological databases, expanding indications for endoscopic resection beyond absolute indication has been tried in Japan and Korea. However, controversies exist regarding the safety of treating early gastric cancer beyond absolute indication in terms of pathological evaluation of the resected specimen, definition of expanded indication, discrepancy between pre-endoscopic resection and post-endoscopic resection diagnoses of gastric neoplasm, and the best strategy for cases with non-curative resection. In this brief review, current evidence and clinical experience regarding issues of endoscopic resection beyond absolute indication will be summarized.

  • We studied about the webdirectory of Korean traditional medicine which receives footlights from medical field in the research and the world traditional medicine. Korean traditional medicine is an integral part of Korean ancient folk medicine. It includes Korean medication, hebalogy, acupuncture, massage and Qigong. Hospitals in Korea are classified as specialized in western medical system. Korean traditional medicine or both. Scientific research and clinic experiments in Korean traditional medicine are continuously progressing. Successes have been made in many of the areas such as in circulating paths of meridians in healing emergency patients with shock, acute myocardial infarction and acute renal failure. Acupuncture anesthesia and acupuncture analgesia has promoted the acupuncture therapy to many patients. Expansion of Indication for Endoscopic SD in Early Gastric Cancer

  • Endoscopic resection is now accepted as curative treatment modalities for early gastric cancer without lymph node metastasis. However, based on large-scaled data about the risk of lymph node metastasis in early gastric cancer and as a result of the technical development of endoscopic devices, it was suggested that the criteria for endoscopic resection should be extended. According to the treatment guidelines for gastric cancer in Japan, the extended indications include the following: differentiated-type mucosal cancer without ulceration and greater than 2 cm in diameter, differentiated-type mucosal cancer with ulceration and up to 3 cm in diameter, undifferentiated-type mucosal cancer without ulceration and up to 2 cm in diameter, and, in the absence of lymphovascular invasion, a tumor not deeper than submucosal level 1 (less than $500\;{\mu}m$). In this review, we discuss the evidence of the application of expanded endoscopic indication based on analysis of biologic behavior and data of endoscopic resection.

  • In Vol. 31, No. 2, our journal prepared a review article, two original papers, and three case reports. First, autonomous systems are increasingly being introduced in aircraft systems. Therefore, it is crucial that flight crews are trained to adapt and handle these autonomous systems better. Consequently, we discussed crew resource management for handling autonomous systems. The fatigue of aviation workers is one of the biggest threats to aviation safety. Therefore, we analyzed the degree of fatigue in commercial aircraft pilots and evaluated the relationship between fatigue and their workload and the effect of fatigue on sleep. Since obesity has a significant impact on the health of aviation workers, we also comparatively analyzed the prevalence of obesity in Korea and Japan based on the data from the National Health and Nutrition Examination Survey. We defined cases that are difficult for aviation medical examiners to evaluate. This issue included cases of peripheral dizziness and tinnitus, cases of atopic dermatitis treated with biological agents, and cases of malignant tumors.
