Incidence and Procedure-Related Risk Factors of Delirium in Patients Admitted to an Intensive Care Unit (중환자실 입원 환자의 섬망 발생과 처치 관련 위험인자)

  • Objectives : Although delirium is a common complication among patients hospitalized in intensive care units(ICUs), little is known about the roles that diagnostic and therapeutic procedures play in its development. This study investigates the procedure-related risk factors of delirium in ICU patients. Methods : All the consecutive patients admitted to the ICU between June 2016 and May 2017 were routinely evaluated for delirium by psychiatrists. In total, 1156 patients met the inclusion criteria and were retrospectively analyzed. A multiple logistic regression analysis was conducted to investigate independent risk factors of delirium development while adjusting for other characteristics. Results : The age, Acute Physiology and Chronic Health Evaluation (APACHE II) score, proportion of patients who had undergone an operation, and proportion of patients who were foley catheterized, mechanically ventilated, and physically restrained were higher in the delirium group. The multiple logistic regression analysis confirmed that the use of restraint was an independent risk factor of delirium (odds ratio : 10.006 ; 95% confidence interval : 6.120-16.360 ; p<0.001). The patient factors independently associated with delirium were an advanced age and a higher APACHE II score. The incidence of delirium was 15.3%. Conclusions : There is a high prevalence of delirium influenced by potentially harmful procedures in patients in ICU settings. The use of physical restraint had the strongest association with the development of delirium. These findings advocate the need to target procedure-related risk factors such as the use of restraints as preventive intervention measures for ICU delirium.

The Characteristics and Types of Psychiatric Consultation for Insomnia Symptom in Hospitalized Patients (불면증으로 의뢰된 입원환자의 임상적 특징 및 협진 유형 분석)

  • Objectives: The purpose of this study was to explore insomniac demographic characteristics and the type of consultation provided to hospitalized patients asked to the Department of Psychiatry for insomnia and to compare patient insomnia characteristics by consultation type. Methods: We performed a retrospective chart review of 4,966 patients who were hospitalized from August 1, 2005 to December 31, 2011 that received consultation in the Department of Psychiatry. Among them, 236 patients were referred for insomnia. We compared the differences in demographic characteristics and types of consultation between the insomnia patient group and other patient group. We also compared the difference between demographic characteristics and type of consultation by dividing total subjects into 'with reconsultation' and 'without reconsultation' groups. Results: Our results came from the analysis of 9,689 consecutive consultation requests. There were 4,966 patients that participated in the study over 6 years and 6 months. The overall consultation rate was 3.3% of all admissions and insomnia patients comprised 4.8% of those. The ratio of re-consultation for insomnia was 27.5%. There was no significant difference in mean age between the insomnia 'with reconsultation group' and the insomnia 'without reconsultation group', but the 'with reconsultation' group had significantly more male patients and medical patients than the 'without re-consultation' group. For insomnia patients, consultation types were in the order of Mending request (51.3%), Paralle request (36.6%), Complementary request (9.0%) and this composition differed from that of total admission patients. Conclusion: Hospitalized patients referred for insomnia showed a higher proportion of male patients, lower rates of re-consultation compared with other patients, and most of these were for secondary insomnia. Each doctor should be aware of the possibility of inpatient insomnia, conduct positive assessments and referrals as necessary, and psychiatrists who might be asked for consultation need to prepare an active intervention with initial diagnosis and treatment, as well as recommendations for the timing of reconsultation.

Calibration of Pyranometer with Solar Radiation Intercomparison Observation at Research Institute for Radiation-Satellite, Gangneung-Wonju National University (강릉원주대학교 복사-위성연구소에서 실외 비교관측을 통한 전천일사계 교정)

  • Although the technology for the observation of solar radiation is rapidly developing worldwide, in Korea the guidelines for comparing observations of solar radiation are only now under preparation. In this study, a procedure for intercomparison observations of solar radiation was established which accounts for meteorological and geographical conditions. The intercomparisons among observations by national reference pyranometers were carried out at the Asia Regional Radiation Center, Japan, in 2017. Recently, the result of the calibration of the reference pyranometer of the Korean Meteorological Administration (KMA) has been reported. Using the KMA pyranometer as a reference, comparisons between observations and calibrations were carried out for the standard (B to J) pyranometers of the KMA, and for the reference (A) and the standard pyranometers of the Gangneung-Wonju National University. The intercomparisons were carried out between October 24 and October 25, 2018. The sensitivity constants were adjusted according to the results of the data analysis performed on October 24. On October 25, a post-comparison observation was also performed, and the data of the participating pyranometers were verified. The sensitivity constants were calculated using only data corresponding to a solar radiation of $450.0W\;m^{-2}$ or higher. The B and I pyranometers exhibited a small error (${\pm}0.50W\;m^{-2}$), and the applied sensitivity constants were in the range $0.08-0.16{\mu}V(W\;m^{-2})^{-1}$. For the C pyranometer, the adjustment of the sensitivity constant was the largest, i.e., $-0.16{\mu}V(W\;m^{-2})^{-1}$. As a result, the nine candidate pyranometers could be calibrated with an average error of $0.06W\;m^{-2}$ (0.08%) with respect to the KMA reference, which falls within the allowed tolerance of ${\pm}1.00%$ (or ${\pm}4.50W\;m^{-2}$).

  • Background: Though clinical practice guidelines (CPGs) for cardiac rehabilitation (CR) are an effective and widely used treatment method worldwide, they are as yet not widely accepted in Korea. Given that cardiovascular disease is the second leading cause of death in Korea, it is urgent that CR programs be developed. In 2008, the Government of Korea implemented CR programs at 11 university hospitals as part of its Regional Cardio-Cerebrovascular Center Project, and 3 additional medical facilities will be added in 2019. In addition, owing to the promotion of CR nationwide and the introduction of CR insurance benefits, 40 medical institutions nationwide have begun CR programs even as a growing number of medical institutions are preparing to offer CR. The purpose of this research was to develop evidence-based CPGs to support CR implementation in Korea. Methods: This study is based on an analysis of CPGs elsewhere in the world, an extensive literature search, a systematic analysis of multiple randomized control trials, and a CPG management, development, and assessment committee comprised of 33 authors-primarily rehabilitation specialists, cardiologists, and thoracic surgeons in 21 university hospitals and 2 general hospitals. Twelve consultants, primarily rehabilitation, sports medicine, and preventive medicine specialists, CPG experts, nurses, physical therapists, clinical nutritionists, and library and information experts participated in the research and development of these CPGs. After the draft guidelines were developed, 3 rounds of public hearings were held with staff members from relevant academic societies and stakeholders, after which the guidelines were further reviewed and modified. Results: CR involves a more cost-effective use of healthcare resources relative to that of general treatments, and the exercise component of CR lowers cardiovascular mortality and readmission rates, regardless of the type of coronary heart disease and type and setting of CR. Conclusion: Individualized CR programs should be considered together with various factors, including differences in heart function and lifestyle, and doing so will boost participation and adherence with the CR program, ultimately meeting the final goals of the program, namely reducing the recurrence of myocardial infarction and mortality rates.

  • If a new utility tunnel is planned for high density existing urban areas in Korea, a rational decision-making process such as the determination of optimum design capacity by using the feasibility evaluation system based on quantitative evaluation indexes and the economic evaluation is needed. Thus, the previous study presented the important weight of individual higher-level indexes (3 items) and sub-indexes (16 items) through a hierarchy analysis (AHP) for quantitative evaluation index items, considering the characteristics of each urban type. In addition, an economic evaluation method was proposed considering 10 benefit items and 8 cost items by adding 3 new items, including the effects of traffic accidents, noise reduction and socio-economic losses, to the existing items for the benefit cost analysis suitable for urban utility tunnels. This study presented a quantitative feasibility evaluation method using the important weight of 16 sub-index items such as the road management sector, public facilities sector and urban environment sector. Afterwards, the results of quantitative feasibility and economic evaluation were compared and analyzed in 123 main road sections of the Seoul. In addition, a comprehensive evaluation method was proposed by the combination of the two evaluation results. The design capacity optimization program, which will be developed by programming the logic of the quantitative feasibility and economic evaluation system presented in this study, will be utilized in the planning and design phases of urban community zones and will ultimately contribute to the vitalization of urban utility tunnels.

Status of serving labeling of home meal replacement-soups and stews, and evaluation of their energy and nutrient content per serving (가정간편식-국·탕·찌개류의 인분표시 및 영양표시 실태와 1인분 제공량 당 열량 및 영양성분 함량 평가)

  • Purpose: In this study, the serving size of home meal replacement (HMR)-soups (Guk, Tang) and stews (Jjigae) available in the Korean market was investigated, and an evaluation of the nutrition per serving was conducted based on the nutrition labeling. Methods: The market research was conducted from March to August 2021 on products sold on the internet, convenience stores, supermarkets, and hypermarkets. A total of 370 products were investigated and classified into 3 types: Guk (n = 129), Tang (n = 132), and Jjigae (n = 109). Results: An analysis of the survey revealed that 72.9% of Guk, 71.2% of Tang, and 79.8% of Jjigae had labels with servings per container, and 89.2% of Guk, 91.7% of Tang, and 99.1% of Jjigae had labels with nutrition facts. The nutritional evaluation per serving of Guk, Tang, and Jjigae was conducted for 259 products (87 Guk, 86 Tang, and 86 Jjigae) having labels containing both the servings per container and nutrition facts. The average serving size of Tang was 367.6 g, which was significantly higher than Guk (325.3 g) and Jjigae (305.1 g) (p < 0.001). The calorie content of Jjigae (171.4 kcal) and Tang (162.3 kcal) was significantly higher than Guk (90.8 kcal) (p < 0.001), and the protein content was the highest in Tang (16.3 g) (p < 0.001). The sodium content per serving of Jjigae (1,479.0 mg) was significantly higher than Guk (1,073.3 mg) and Tang (959.8 mg) (p < 0.001). The percent daily value per serving of all three types was less than 10% on average for calories and 15-30% for protein, whereas for sodium showed an average of around 50% (48-74%). Conclusion: The serving size and nutritional value per serving of the HMR-soups and stews found in this study can be used as basic data to establish the reference serving size.

  • 상추(Lactuca sativa L.)는 대표적인 쌈 및 샐러드 채소로 우리나라 기준(2016년) 3,387 ha의 면적에서 86,128톤을 생산하여 엽채류 중 배추, 양배추 다음으로 많이 생산되는 작물이다. 안토시아닌(Anthocyanins)은 열매, 꽃, 줄기, 잎 등 식물계에 널리 분포되어 있는 페놀 화합물 중 하나로 적색, 자색 등의 색을 나타내는 수용성 flavonoid계 색소이다. BSLs (Bitter sesquiterpene lactones)는 항암, 항균, 해열과 염증완화에 효과가 있는 것으로 알려져 있다. 본 연구는 농촌진흥청 농업유전자원센터에서 보유 중인 상추 66자원의 형태학적 특성 및 액체크로마토그래피(HPLC, UPLC)를 이용한 안토시아닌과 BSLs성분을 분석하여 함량이 높은 자원을 선발하고자 한다. 상추시료 0.05 g을 $MeOH/H_2O/AcAc$로 추출 한 후, UPLC를 사용하여 안토시아닌 함량을 분석하였으며, 상추시료 0.25 g을 100% MeOH로 추출 한 후 HPLC를 사용하여 BSLs 함량을 분석하였다. 연구 결과, 상추 유전자원의 안토시아닌 함량 범위는 0 mg/100 g에서 371.94 mg/100 g이고, BSLs성분 함량 범위는 $60.28{\mu}g/g\;DW$에서 $2821.92{\mu}g/g\;DW$ 이었다. 상추 66자원 중 안토시아닌함량이 200 mg/100 g이상인 자원은 IT217012, IT218395, IT231524, IT231525, IT260852이며, BSLs 함량이 $1700{\mu}g/g\;DW$이상인 자원은 IT231524, IT231525, IT231527, IT264971, IT271118이다. 두 성분의 함량이 모두 높은 자원 IT231524와 IT231525 이었다. 이 두자원의 형태적 특성은 초형이 잎상추로 잎이 넓은 타원형에 가장자리 결각이 강한 자주색이다. 따라서 본 연구는 다양한 상추 유전자원의 형태학적 특성 및 BSLs, 안토시아닌 성분이 높은 자원을 선발하여 육종소재로 활용하고자 한다.

Key Foods selection using data from the 7th Korea National Health and Nutrition Examination Survey (2016-2018) (제7기 국민건강영양조사 (2016-2018) 자료를 활용한 한국인의 주요 식품 (Key Foods) 선정에 관한 연구)

  • Purpose: Key Foods refers to foods that have a high contribution in the nutrient intake of individuals, and exert important effects on their health. This study was undertaken to identify Korean Key Foods, using data from the 7th Korea National Health and Nutrition Examination Survey (KNHNES). Methods: The data source for the extraction of Key Foods was the 24-hour dietary survey data obtained from the 7th KNHNES (2016-2018), and 21,271 subjects were evaluated. A total of 17 nutrients were selected as the key nutrients for identifying the Key Foods, including energy, carbohydrates, protein, lipid, dietary fiber, calcium, phosphorus, iron, sodium, potassium, vitamin A, thiamin, riboflavin, niacin, vitamin C, cholesterol, and sugars. The nutrient consumption approach was applied to generate a list of potential Key Foods. Foods included in 85% of the cumulative intake contribution from one or more key nutrients, were subsequently selected as Key Foods. Results: Of the 1,728 foods consumed by survey respondents, we extracted 728 Key Foods. These Key Foods explained 94% key nutrient intakes of the subjects. Based on the contribution rate to key nutrient intake, the top 10 Key Foods identified were multigrain rice (5.32%), plain white rice (4.23%), milk (3.3%), cabbage kimchi (2.82%), grilled pork belly (1.56%), apples (1.52%), fried eggs (1.49%), cereal (1.36%), instant coffee mix (1.21%), and sweet potatoes (1.12%). These 10 foods accounted for 23.93% total key nutrient intake of the survey respondents. Conclusion: Seven hundred and twenty-eight foods were extracted and identified as the 2020 Korean Key Foods. These Key Foods can be considered the priority foods to be analyzed for establishing a national nutrient database.

A Study on the Characteristics and Consultation Request Type of Inpatients Referred for Depressive Symptoms (우울 증상으로 의뢰된 입원환자의 임상적 특징 및 자문 의뢰 형태에 관한 연구)

  • Objectives : The purpose of this study is to investigate the characteristics of depressive patients who admitted to general hospital. We examined the clinical characteristics of patients who were referred to the Department of Psychiatry as depressive symptoms, according to the type of consultation request, and comparing 'with re-consultation' and 'without re-consultation' groups. Methods : We performed a retrospective chart review of 4,966 inpatients who were referred to the Department of Psychiatry from August 2005 to December 2011. Results : For about 6 years, among the inpatients referred for psychiatric consultation, a total of 647 patients were referred for depressive symptoms, accounting for 13.82% of the total consultations. The average age of depressive patients was 58.6 years, which was higher than the average of 56.4 years of overall patients. Among the depressive patients, 275 patients were included in 'with re-consultation' group and there was no statistically significant difference when comparing 'with re-consultation' group and 'without re-consultation' group. However, there was a difference in the tendency of the two groups in the type of consultation request. 'With re-consultation' group was in the order of frequency of consultation type 3-2-1, whereas the 'without re-consultation' group was in the order of frequency of consultation type 2-3-1. Conclusions : The group of inpatients who were referred for depressive symptoms in general hospital showed the largest proportion of the group of patients referred to the Department of Psychiatry. 'With re-consultation' group had a higher rate of re-consultation due to the occurrence of new symptoms after hospitalization compared to 'without re-consultation' group. Therefore, doctors in each department and psychiatrists should pay attention to the depressive symptoms of inpatients and actively discuss treatment plans to improve the quality of medical services, identify risk factors, and make efforts to intervene early if necessary.

Change of dietary habits and the use of home meal replacement and delivered foods due to COVID-19 among college students in Chungcheong province, Korea (충청지역 일부 대학생의 코로나-19 이후 식생활 변화, 가정간편식과 배달음식 이용 실태)

  • Purpose: The purpose of this study was to evaluate the change in the use of home meal replacement (HMR) and delivered foods and food habits of college students due to coronavirus disease 2019 (COVID-19). Methods: A survey was conducted on 460 male and female college students in Chungcheong province in December 2020. Results: The methods of participation in classes in the 2nd semester of 2020 were 40.2% for '100% non-face-to-face' and 40.4% for 'more than 70.0% of non-face-to-face classes'. 52.8% of the subjects responded that their physical activity had decreased, while 36.1% of the subjects responded that their body weight had increased over the past 6 months. Regarding the use of HMR, 62.7% of the male students and 69.6% of the female students responded with '1-2 times a week or less' before the outbreak of COVID-19. After the outbreak, 57.4% of males and 46.7% of females responded with '3-4 times a week or more' (p < 0.05, p < 0.001). As for the use of delivered food, 58.3% of the females responded with '2-3 times a month or less' before the outbreak, whereas the rate of responding with '1 or more times a week' after the outbreak was 64.6% (p < 0.001). Negative changes in food habits caused by COVID-19 were in the order of irregular meals (56.7%), increased intake of delivered food (42.2%), increased intake of snacks (33.3%), and increased intake of processed foods such as HMR (30.4%). Conclusion: As mentioned above, many college students had non-face-to-face classes due to COVID-19. During this period, they experienced decreased physical activity, weight gain, and undesirable eating habits such as irregular meal time, and increased intake of processed foods, delivery foods, and snacks. Therefore, there is a high need for nutrition education and policy support for the prevention and management of health and nutrition problems of college students, which can be caused due to the COVID-19 pandemic.
