Intraventricular Hemorrhage Long after Successful Encephaloduroarterio Synangiosis in Moyamoya Patient

  • Intraventricular hemorrhage long after successful encephaloduroarterio synangiosis (EDAS) is very rare. The effect of revascularization surgery for preventing hemorrhagic event of moyamoya disease remains controversial. We report a 17-year-old female with intracerebral hemorrhage and intraventricular hemorrahge 10 years after successful EDAS. Even though cerebral vessels angiography showed good collateral circulations without specific weak points, a cerebral hemorrhage could occur in patient with ischemic type of moyamoya disease long after successful indirect bypass operations. Good collateralization of cerebral angiography or magnetic resonance perfusion image after indirect bypass surgery would ensure against ischemic symptoms, not a hemorrhage. And, thus a life-time follow-up strategy might be necessary even if a good collateral circulation has been established.

무심폐기하 관상동맥우회술에서의 중등도의 허혈성 승모판막부전증의 중요성 (Long-term Influence of Mild to Moderate Ischemic Mitral Regurgitation after Off-pump Coronary Artery Bypass Surgery)

  • 배경: 술전 경도 및 중등도의 허혈성 승모판막 폐쇄부전증이 동반된 환자들의 무심폐순환 관상동맥 우회로 조성술을 시행한 후의 장기 예후를 추적 조사하였다. 대상 및 방법: 1996년 9월부터 2004년 3월까지 1,000명의 연속된 무심폐순환 관상동맥 우회로 조성술을 시행 받은 환자들의 데이터를 전향적으로 조사하였으며, 97%의 환자에서 추적 조사가 가능하였다. 육십칠명(6.7%)의 환자들이 수술 당시 경도 및 중등도의 허혈성 승모판막 폐쇄부전증이 동반되었다. 허혈성 승모판막 폐쇄부전증이 끼치는 영향에 대하여 수술사망율, 실제생존율 및 주요 심장 부작용이 없는 생존율 등을 비교하였다. 결과: 평균 추적 조사 기간은 $66{\pm}22$개월이었다. 허혈성 승모판막 폐쇄부전증이 동반된 환자들의 연령이 더 높았으며(p<0.001), 더 낮은 심박출량을 보였고(p<0.001) 더 많은 위험 인자들을 갖고 있었다. 여성에게 의미있게 더 많았으며(p=0.002), 수술사망율이나 술전후 심근 경색의 빈도에는 의미있는 차이는 없었다(p=0.25). 팔 년 생존율은 허혈성 승모판막 폐쇄부전증이 동반된 환자들이 더 낮았다($39.6{\pm}11.8%$ vs $76.7{\pm}2.2$, p<0.001). 하지만 동반된 위험 인자들을 교정했을 때 술전 경도 및 중등도의 허혈성 승모판막 폐쇄부전증 자체가 장기 사망률의 의미있는 위험 인자는 아니었다(p=0.42). 8년 간의 주요 심장 부작용이 없는 생존율은 허혈성 승모판막 폐쇄부전증이 동반된 환자들에서 의미있게 낮았다($53.12{\pm}12%$ vs $77{\pm}2%$, p<0.001). 위험 인자들을 교정했을 때 허혈성 승모판막 폐쇄부전증은 주요 심장 부작용이 없는 생존율(HR: 2.31), 울혈성 심기능 저하와 재발성 심근 경색에 의미있는 위험 요소이었다. 결론: 무심폐순환 관상동맥 우회로 조성술을 시행 받은 환자들 중 술전 경도 및 중등도의 허혈성 승모판막 폐쇄부전증이 동반된 경우 더 많은 술전 위험요소들을 갖고 있었다. 술전후의 사망률이나 유병율은 비슷하였으나, 장기적으로 심장 합병증의 유발율은 더 높았다

Simvastatin Reduces Lipopolysaccharides-Accelerated Cerebral Ischemic Injury via Inhibition of Nuclear Factor-kappa B Activity

  • Preceding infection or inflammation such as bacterial meningitis has been associated with poor outcomes after stroke. Previously, we reported that intracorpus callosum microinjection of lipopolysaccharides (LPS) strongly accelerated the ischemia/reperfusionevoked brain tissue damage via recruiting inflammatory cells into the ischemic lesion. Simvastatin, 3-hydroxy-3-methylgultaryl (HMG)-CoA reductase inhibitor, has been shown to reduce inflammatory responses in vascular diseases. Thus, we investigated whether simvastatin could reduce the LPS-accelerated ischemic injury. Simvastatin (20 mg/kg) was orally administered to rats prior to cerebral ischemic insults (4 times at 72, 48, 25, and 1-h pre-ischemia). LPS was microinjected into rat corpus callosum 1 day before the ischemic injury. Treatment of simvastatin reduced the LPS-accelerated infarct size by 73%, and decreased the ischemia/reperfusion-induced expressions of pro-inflammatory mediators such as iNOS, COX-2 and IL-$1{\beta}$ in LPS-injected rat brains. However, simvastatin did not reduce the infiltration of microglial/macrophageal cells into the LPS-pretreated brain lesion. In vitro migration assay also showed that simvastatin did not inhibit the monocyte chemoattractant protein-1-evoked migration of microglial/macrophageal cells. Instead, simvastatin inhibited the nuclear translocation of NF-${\kappa}B$, a key signaling event in expressions of various proinflammatory mediators, by decreasing the degradation of $I{\kappa}B$. The present results indicate that simvastatin may be beneficial particularly to the accelerated cerebral ischemic injury under inflammatory or infectious conditions.

Carotid Intraplaque Hemorrhage is Associated with Acute Cerebral Ischemic Events and Progression of Stenosis on Magnetic Resonance Imaging

  • Purpose: To investigate the association of carotid intraplaque hemorrhage (IPH) with acute cerebral ischemic events and progression of stenosis using magnetic resonance (MR) imaging. Materials and Methods: From April 2014 to December 2016, 53 patients underwent carotid plaque MR imaging, including magnetization-prepared rapid acquisition with gradient-echo (MPRAGE) sequence. A total of 66 carotid arteries in 53 patients had carotid stenosis, and they were included in this study. Carotid arteries were classified according to the presence of IPH, the age of hemorrhage, and degree of stenosis. We assessed ipsilateral cerebrovascular event rates and progression of stenosis between the IPH and no-IPH groups. Results: Of the 61 carotid arteries assessed, 34 (56%) had IPH, and 27 (44%) had no IPH. Acute cerebral ischemic events were more frequent in the IPH group (47% vs. 22%, P = 0.045), especially in the < 30%-stenosis group (100% vs. 0%, P = 0.028). However, there was no significant difference in the incidence of ischemic events according to the age of hemorrhage (50% vs. 44%, P = 0.492). Among the 61 carotid arteries, 20 carotid arteries had previously undergone carotid artery imaging and were evaluated for plaque progression. The trend for progression of stenosis favored the IPH group versus the no-IPH group, with a marginal P-value ($20%{\pm}12.7$ vs. $9.6%{\pm}5.7$, P = 0.063). Conclusion: IPH was associated with an increased incidence of acute ischemic events, especially in the mild-stenosis group and it was also associated with progression of stenosis. Evaluation of the carotid IPH by carotid plaque MR could improve discrimination of carotid plaques that cause ischemic events and progression of stenosis.

Urgent Recanalization with Stenting for Severe Intracranial Atherosclerosis after Transient Ischemic Attack or Minor Stroke

  • Objective : Stenting of symptomatic intracranial stenosis has recently become an alternative treatment modality. However, urgent intracranial stenting in patients with intracranial stenosis following a transient ischemic attack (TIA) or minor stroke is open to dispute. We sought to assess the feasibility, safety, and effectiveness of urgent intracranial stenting for severe stenosis (>70%) in TIA or minor stroke patients. Methods : Between June 2009 and October 2010, stent-assisted angioplasty by using a balloon-expandable coronary stent for intracranial severe stenosis (>70%) was performed in 7 patients after TIA and 5 patients after minor stroke (14 stenotic lesions). Technical success rates, complications, angiographic findings, and clinical outcomes were retrospectively analyzed. Results : Stenting was successful in all 12 patients. The mean time from symptom onset to stenting was 2.1 days (1-8 days). Post-procedural angiography showed restoration to a normal luminal diameter in all patients. In-stent thrombosis occurred in one patient (n=1, 8.3%), and was lysed with abciximab. No device-related complications, such as perforations or dissections at the target arteries or intracranial hemorrhaging, occurred in any patient. The mortality rate was 0%. No patient had an ischemic event over the mean follow-up period of 12.5 months (range, 7-21 months), and follow-up angiography (n=7) revealed no significant in-stent restenosis (>50%). Conclusion : Urgent recanalization with stenting is feasible, safe, and effective in patients with TIA or acute minor stroke with intracranial stenosis of ${\geq}$ 70%.

Myoclonic status epilepticus in hypoxic ischemic encephalopathy which recurred after somatosensory evoked potential testing

  • A 77-year-old male with amyotrophic lateral sclerosis had a hypoxic event. After resuscitation, generalized myoclonus appeared and resolved after two days. Five days after the hypoxic event, myoclonic seizures re-emerged right after performing a somatosensory evoked potential and persisted for ten days. Electroencephalogram revealed frequent bi-hemispheric synchronous spike and waves in the central areas. We suggest that somatosensory evoked potential testing may trigger myoclonic status epilepticus. Underlying cortical degeneration associated with amyotrophic lateral sclerosis could attribute to this phenomenon.

Sequential Vestibular Neuritis: Report of Four Cases and Literature Review

  • Background and Objectives: Bilateral sequential vestibular neuritis (BSVN) is a rare condition in which an inflammation or an ischemic damage of the vestibular nerve occurs bilaterally in a sequential pattern. We described four cases of BSVN. Subjects and Methods: Every patient underwent video-head impulse test during the first and the second episode of vestibular neuritis (VN), furthermore they have been studied with radiological imaging. Results: Contralateral VN occurred after a variable period from prior event. Vestibular function recovered from the first episode in one case. The other three patients developed contralateral VN. One case was due to a bilateral VN in association with a Ramsay-Hunt syndrome, in another patient clinical records strongly suggested an ischemic etiology, whereas in two cases aetiology remained uncertain. Two patients subsequently developed a benign paroxysmal positional vertigo involving the posterior canal on the side of the latest VN (Lindsay-Hemenway syndrome). Conclusions: Instrumental vestibular assessment represents a pivotal tool to confirm the diagnosis of VN and BSVN.

The Extent of Late Gadolinium Enhancement Can Predict Adverse Cardiac Outcomes in Patients with Non-Ischemic Cardiomyopathy with Reduced Left Ventricular Ejection Fraction: A Prospective Observational Study

  • Objective: The clinical course of an individual patient with heart failure is unpredictable with left ventricle ejection fraction (LVEF) only. We aimed to evaluate the prognostic value of cardiac magnetic resonance (CMR)-derived myocardial fibrosis extent and to determine the cutoff value for event-free survival in patients with non-ischemic cardiomyopathy (NICM) who had severely reduced LVEF. Materials and Methods: Our prospective cohort study included 78 NICM patients with significantly reduced LV systolic function (LVEF < 35%). CMR images were analyzed for the presence and extent of late gadolinium enhancement (LGE). The primary outcome was major adverse cardiac events (MACEs), defined as a composite of cardiac death, heart transplantation, implantable cardioverter-defibrillator discharge for major arrhythmia, and hospitalization for congestive heart failure within 5 years after enrollment. Results: A total of 80.8% (n = 63) of enrolled patients had LGE, with the median LVEF of 25.4% (19.8-32.4%). The extent of myocardial scarring was significantly higher in patients who experienced MACE than in those without any cardiac events (22.0 [5.5-46.1] %LV vs. 6.7 [0-17.1] %LV, respectively, p = 0.008). During follow-up, 51.4% of patients with LGE ≥ 12.0 %LV experienced MACE, along with 20.9% of those with LGE ≤ 12.0 %LV (log-rank p = 0.001). According to multivariate analysis, LGE extent more than 12.0 %LV was independently associated with MACE (adjusted hazard ratio, 6.71; 95% confidence interval, 2.54-17.74; p < 0.001). Conclusion: In NICM patients with significantly reduced LV systolic function, the extent of LGE is a strong predictor for long-term adverse cardiac outcomes. Event-free survival was well discriminated with an LGE cutoff value of 12.0 %LV in these patients.

실험적 뇌허혈로 인한 편마비 흰쥐에서 운동 적응 훈련이 기능 회복에 미치는 영향 (The Effect of Exercise Adaptive Training on Motor Function after Experimental brain ischemia in Rats)

  • The purpose of this study was to test that the exercise adaptive training enhance behavioral outcome significantly after focal brain ischemia in rats. After occlusion of middle cerebral artery in rats, they were housed in individual standard cages fur 24 hours. The control group was sacrificed 24 hours after ischemic event. The experimental group I was housed in standard cages for 7days. The experimental group ll was housed in enriched environment and had got exercise adaptive training fur 7days. The rats were examined five motor behavioral tests. In motor behavioral tests :postural reflex test, limb placement test, beam-walking test, rotarod test, horizontal wire test. The outcomes of control group and group I were significantly lower than the group II. The conclusion was that exercise adaptive training induced functional repair.
