Treatment of a Twelfth Rib Syndrome -A case report- (열두 번째 갈비뼈 증후군 환자의 치료경험 -증례보고-)

  • Twelfth rib syndrome is thought to be due to intercostal nerve irritation by a mobile twelfth rib, and presents with upper abdominal pain, or low thoracic pain. This syndrome appears to be a fairly common entity and diagnosis is based on clinical findings. Patients with twelfth rib syndrome can be misdiagnosed when it has been overlooked. We report a case of a 34-year-old male along with a presentation of twelfth rib syndrome. One patient was transferred from urologic clinic to pain clinic due to right flank pain and admitted. The patient had direct tenderness on twelfth rib area and direct tenderness was reproducible. Pain increased when the patient flexed laterally, rotated trunk. There were no specific abnormal findings in laboratory test, electrocardiogram, and radiologic examination. After intercostal nerve block and epidural block, pain decreased and the patient was discharged.

Effect of Super Heated Steam Treatment on Physical Property and Smoke Component of Burley Cut Tobacco (과열증기처리에 의한 Burley종 각초의 물리성 및 연기성분 변화에 관한 연구)

  • This study was carried out to investigate the effects of super heated steam on the physical and chemical changes of burley cut tobacco. Total sugar, total alkaloid, ether extracts, crude ash, total nitrogen and pH for leaf chemical constituents were analyzed. Filling power and fineness index for physical properties, and carbonyl compounds, phenol compounds, amonia, pH, hydrogen cyanide nicotine, carbon monoxide, total volatile base and tar for smoke components were also analyzed. The cut tobacco treated with super heated steam showed significant decrease in total sugar and total alkaloid. The filling power of the sample treated with the super heated steam system was increased abruptly when heated at over 250℃. The fineness index showed similar tendency to that of common toast method. Super heated steam treatment slightly decreased carbonyl compounds, phenol compounds, hydrogen cyanide, nicotine, carbon monoxide and tar in the tobacco. Especially the decrease of ammonia was the most remarkable. The pH of smoke was a little different compared with that in the common toast. The sensory test results showed that, compared to the common conveyer moving system, the tobacco treated with super heated steam system brought out more roasted flavor, lowered impact, irritation, and sting, further improved aftertaste, and lowered bitterness. The super heated steam treatment method used in the studies is expected to give better filling power, mild taste and toasted odor of tobacco than that of the common method for toast treatment. Effect of Bulk Fermentation on Chemical, Chromatic, and Organoleptic Characteristics of Burley Leaf Tobacco (버어리종 잎담배의 퇴적발효가 화학성분, 색상 및 끽미에 미치는 영향)

  • This study was carried out to evaluate the effect of bulk fermentation on chemical, chromatic, and organoleptic characteristics of burley leaf tobacco. The pile of ferment processing was taken up 32 days under the conolitions of leaf moisture contents of $28\pm1%$, with a pressure of some 200kg/$m^2$ within a closed room (mean air temperature and relative humidity ; 20.5$^{\circ}C$ and 58.7%). The pile was opened up and reconstructed two times when the maximum inside temperature reached at $45~46^{\circ}C$. The nicotine content was decreased, but amomnia contents and pH were significantly increased by bulk fermentation. Otherwise, the contents of total nitrogen, total volatile base, organic acids, and fatty acids were not affected by same treatment. The value of L(black to white), a(red to green) and b(yellow to blue) in chromatic characteristics were significantly decreased by bulk fermentation. In sensory test of the cigarettes made by addition of the tormented tobacco leaves after toasting in proportion of 19-25%, no negative characteristics in irritation, taste, and preference were detected in comparison with normally processed cigarettes(19%, 2 years fermentation, toasting). The results suggest that bulk fermentation may be useful to increase the proportion of burley leaf tobacco in the cigarettes and to shorten the period of storage for aging.

The Synthesis and Safety of 3-Aminopropyl dihydrogen phosphate, a New Anti-aging Agent

  • The novel synthesis of 3-aminopropyl dihydrogen phosphate(3-APPA; 3-Aminopropane phosphoric acid), and its applicability to the skin as a cosmetic raw material in terms of its efficacy and toxicology were presented. The phosphorylation of 3-amino-1-propanol was carried out via cyclization into 6-membered 2, 6-oxaza-phosphoryl ring in the presence of phosphorous oxychloried and an organic base. The subsequent ring-opening hydrolysis and crystallization afforded the highly purified product in 90% isoloated yield. The method is much superior to the previous literature phosphorylation methodsm, as the procedure is simple and high-yielding. To confirm the efficacy of 3-APPA, several activities related to anti-aging capacity were measured. In-vitro human fibroblast, linear and 3-dimensional collagen matrix culture revealed that 3-APPA stimulated the proliferation of fibroblasts, and enhanced the synthesis of collagen, which showed 3-APPA's potency for skin wrinkle reduction. The toxicolgical aspect of 3-APPA was also extensively examined. In vivo toxicity tests such as acute oral toxicity, eye irritation, human patch, and the repeat insult human patch test proved 3-APPA to be a safe material. Thus 3-APPA can be used as an effective anti-aging agent for various cosmetic formulations.

Improvement in availability and stability of to 106w by inclusion with $\beta-cyclodextrin$ and its derivatives ($\beta-cyclodextrin$ 및 유도체의 포접체 형성에 의한 LG 106W의 유용성 및 안정성 개선에 관한 연구)

  • A newly synthesized polyhydroxy aromatic compound(LG 106w) has good skin lightening effect. Inclusion complexation of LG 106w with $\beta$-cyclodextrin and its hydroxypropyl and dimethyl derivatives was studied by the solubility method, scanning electron microscopy and differential thermal analysis. A relationship between host and guest was clearly reflected in the magnitude of the stability constant(DM-$\beta$> HP-$\beta$>$\beta$ -cyclodextrin). Formulation problems, which resulted from its very low solubility in aqueous solution, were resolved by the inclusion formation. LG 106W from inclusions is much more water-soluble than pure one. The improvement of pH and temperature stability correlated with the increased solubility was also observed. Inclusion complex of LG 106w had similar activity to pure LG 106w on the inhibition of melanin synthesis in B-16 melanoma cell but showed lower irritation on cultured cell test in vitro. According to the results, cyclodextrins might be one of the reliable candidates for improving the availability of LG 106w.

  • To develop a new natural cosmetic hair care product for scalp treatment, we investigated the effect of enzyme modified (Viscozyme, Pectinex) oriental medical prescriptions on hair growth and melanogenesis. Enzyme modified oriental medical prescriptions showed the antioxidative effect, inhibitory effects on lipoxygenase activity and $5{\alpha}$-reductase activity and melanogenic activity. In human irritation test, they did not show any adverse effect. Based on these results, we suggest that enzyme modified oriental medical prescriptions may potentially be used in a cosmeceutical hair care product for scalp treatment.

  • The cutaneous tolerability of detergent formulations can be improved by means of suitable additives. They complex the surfactant molecules lowering the concentration of their free monomeric species. Proteins derivatives used as additives for detergency are usually prepared by partial hydrolysis of plant reserve proteins. The main purpose of the hydrolytic cleavage is to make them water soluble and suitable for liquid products. Water solubility and stability are obtained by means of complexation with surfactants which also increase their actual hydrophobicity, an important parameter affecting cosmetic properties of proteins. Transepidermal water loss (TEWL) and electric capacitance (EC) have been adopted as investigation technigues to evaluate the skin integrity/damage in vitro tests, The performance of native wheat protein / surfactant complexes has been compared with traditional protein hydrolysates as detergent additives. The results show a noticeable reduction of skin irritation in surfactant formulations with addition of native wheat protein.

Serum Periostin Is Negatively Correlated With Exposure to Formaldehyde and Volatile Organic Compounds in Children

  • Epidemiological studies have shown that exposure to tobacco smoke causing irritation and inflammation in the airways tends to reduce serum periostin concentrations in adults. We now investigate prospective cross-sectional study on 135 Korean students aged 7 years in the first grade who were participating in the Seongnam Atopy Project for Children's Happiness 2016 ($SAP_{2016}$) cohort. To the best of our knowledge, this is the first study to show significant inverse correlations between serum periostin concentration and exposure to xylene and formaldehyde in children. Our findings suggested the need for caution in using the serum periostin level as a marker for allergic diseases, since exposure to volatile organic compounds and formaldehyde may confound the interpretation of these results.

  • Cytotoxicity assays using artificial skins have been proposed as in vitro alternatives to minimize animal ocular and dermal irritation testing. Accordingly, the responses of artificial skins to the well-characterized chemical irritants toluene, glutaraldehyde, and sodium lauryl sulfate (SLS), and the nonirritant polyethylene glycol were studied. The evaluation of the irritating and non-irritating test chemicals was also compared with the responses observed in human dermal fibroblasts and human epidermal keratinocytes grown in a monolayer culture. The responses monitored included an MTT mitochondrial functionality assay. In order to better understand the local mechanisms involved in skin damage and repair, the production of several mitogenic proinflammatory mediators, interleukin-l$\alpha$, 12-HETE, and 15-HETE, was also investigated. Dose-dependent increases in the levels of かIn and the HETEs were observed in the underlying medium of the skin systems exposed to the two skin irritants, glutaraldehyde and SLS. The results of the present study show that both human artificial skins can be used as efficient in vitro testing models for the evaluation of skin toxicity and for screening contact skin irritancy.
