Beam and Diffuse to Global Solar Irradiation Correlation Coefficients for Daejeon (대전지역 직달 및 산란과 전일사 상관계수)

  • The total solar irradiation on horizontal surfaces is separated into the beam and diffuses components. Although horizontal global irradiance is a commonly measured parameter for many sites, horizontal diffuse irradiance is not so readily obtainable. For such sites that measure global irradiation alone, a simple but reasonably accurate method is required to estimate diffuse irradiance from its global counterpart. This study investigates the applicability of correlation coefficients models correlating hourly diffuse and beam fraction and hourly clearness index in Daejeon. The three diffuse to global correlation coefficients models (Orgill and Holland model, CIBSE Guide J model, and Erbs et al. model) are selected and the three modified beam to global correlation coefficients models are generated. MBE, RMSE, r-squared of Daejeon and Daejeon boundary site-fitted models are compared with the case of original coefficients. The comparison result shows that the beam and diffuse to global solar irradiation correlation coefficients models with boundary site-fitted coefficients are best suitable for Daejeon. Further researches will be conducted to find the boundary site-fitting method using measured data of other cities and correlation coefficients models using solar altitude, cloud cover, and sunshine duration.

  • The purpose of this study was to examine the effect of LLLT on current perception threshold in trigeminal neuralgia patients. Affected site and non-affected site was irradiated by GaAlAs diode laser. Current perception threshold was evaluated with $Neurometer^{(R)}$ CPT/C before and after LLLT. As an outcomes,CPT were obtained. We made a comparision of these values and the results were as follows. 1. There were no significant difference in CPT between affected site and non-affected site before and after LLLT irradiation. 2. There were no significant difference in CPT between affected site and non-affected site after LLLT irradiation. 3. But, CPT values of $A{\beta}$ fiber incresed slightly on both site after LLLT irradiation

  • In order to investigate the healing effect on inflammation and bone regeneration of low power density laser radiation in dogs, experimental periodontitis was made in dog mandibular 3rd, 4th premolars. All teeth were classified with four groups of two experimental group and control groups. The second group were irradiated on periodontitis site and the first group were control. The fourth group were irradiated on periodontitis site flap operation and the third group were flap control. Experimental groups were irradiated with GaAs low power density laser of pulse wave and continuous wave of 904nm every day by five days respectively and then control group and experimental groups were evaluated by histo-pathological study. The results were as follows : 1. Experimental periodontits site of dog were irradiated with GaAs low power laser results in reducing of pseudoepitheliomatous proliferation and inflammation at light microscope. 2. After irradiation with low power density laser, experimental groups were revealed that PDL forming activity were increased and newly formed collagen deposition were observed. 3. Low power density lsaer irradiation on experimental periodontits site after flap operation showed that decreasing of inflammation, reducing of osteoclast activity. Capillary proliferation, reduction of pseudoepitheliomatous proliferation. 4. After irradiation with low power density laser on flap experimental site, experimental groups were revealed that newly formed collagen in periodontal ligament and alveolar bone were detected on MT staining.

Biological behavior and Treatment of Adenoid Cystic Carcinoma in The Bead and Neck (두경부 선낭암의 생물학적 특성과 치료)

  • Biological behavior and treatment results of 33 patients with Adenoid Cystic Carcinoma (ACC) in the Head and Neck at Yonsei Cancer Confer for 10 years between 1971 and 1980 were retrospectively analysed. Most common, primary site was minor salivary glands such as maxillary sinus, nasal cavity and base of tongue. The typical biological behavior of these tumors was very slowly in growth with long time of duration(mean 19 months) from 1 month to 10 years and more frequent of nerve invasion but rare invasion of neck nodes. Local control and failure pattern in the results of treatment, 16 of 17 patients with irradiation alone were seen complete or partial response but 5 cases of locoregional recurrence, 2 cases of failure of neck node and 4 cases of distant metastasis as lung and brain. On the other hand, among 10 cases of surgery and postoperative irradiation, 2 cases of locoregional failure and 3 cases of distant metastasis as lung and bone. 2 of 4 cases with surgery alone were recurred within primary site. Actuarial overall NED survival at 3 ana 10 years were $52.6\%$ and $42.8\%$, respectively. Survival rate of 10 Patients with surgery and Postoperative irradiation was more high than 17 Patients of radiation alone. Therefore, we have known that surgery with postoperative adjunctive irradiation is most effective treatment modality of adenoid cystic carcinoma in the head and neck. Primary site, treatment modality and with or without nerve ana bone invasion have influenced on prognosis.

Radiotherapy of Stage I and B localized Bead and Heck Non-Hodgkin's Lymphoma (두경부에 국한된 Stage I, II, Non-Hodgkin's Lymphoma의 방사선치료)

  • 53 patients of previously untreated Stage I&II Non-Hodgkin's Lymphoma in head and neck treated with irradiation at Yonsei Cancer Center from January, 1970 to December, 1978 were retrospectively analysed. 5 year survival rate and 5 year disease free survival rate were $51.5\%\;and\;42\%$. Local control rate by irradiation was $92.4\%$ with mainly $4,000\~6,000$ rads. 21 patients suffered relapses after radiotherapy, 4 cases recurred within irradiated area, 4 cases at contiguous site of irradiation field, and 13 cases recurred at distant area, more commonly below diaphragm. Most cases relapsed within 1 year 6 months after treatment. Optimum irradiation field for head and neck localized lymphoma, prognostic factors ana usefulness of chemotherapy are also discussed.

Effects of Irradiation on the Muscle Activity Around an Amputation Site During Proprioceptive Neuromuscular Facilitation Pattern Exercise for Upper Extremity and Scapular Exercise on the Non-Amputated Part -A Case Study- (비 절단부에 적용한 PNF 상지 패턴과 견갑골 움직임에 의한 방산이 절단부 주변 근육 활성도에 미치는 영향 -단일 사례연구-)

  • Purpose: This study verifies the muscle activity around the amputation site during proprioceptive neuromuscular facilitation (PNF) pattern exercise for the upper extremities on the non-amputated part in upper extremity amputees and provides basic data on effective exercise around an amputation site. Methods: Manual resistance was applied to the PNF upper extremity pattern of the non-amputated part to generate muscle activity around the amputation site. The resistance was adjusted to an intensity that could cause maximal isometric contraction. The muscle activity of the amputation site and the non-amputated part was measured using a surface electromyogram for the upper trapezius, middle trapezius, infraspinatus, serratus anterior, and pectoralis major. Results: During the scapular exercise in the painless range, the amputated side showed significantly lower muscle activity and a lower muscle contraction ratio compared with the non-amputated side. During the PNF pattern exercise in the painless range, the amputated side showed lower muscle activity and a lower muscle contraction ratio compared with the non-amputated side. When the direct scapular exercise of the amputated side was compared with the PNF pattern exercise of the non-amputated side, their muscle contraction ratios were similar. Conclusion: This study confirmed the effectiveness of the PNF pattern exercise of the non-amputated part as a way to indirectly train the injured site with no pain for rehabilitation of patients with serious body injuries, such as amputation. It is necessary to develop effective exercise programs for the rehabilitation of the amputation site based on the results of this study.

Photochemistry of Conjugated Poliynes : Photochemical Generation of Silacyclopropenes from 1-Aryl-4-(pentamethyldisilanyl)buta-1,3-diynes

  • No fluorescence was observed from 1-aryl-4-(pentamethyldisilanyl)buta-1,3-diynes except 1-(1-naphthyl)-4-(pentamethyldisilanyl)buta-1,3-diyne. Irradiation of 1-aryl-4-(pentamethyldisilanyl)buta-1,3-diynes (1) with methanol gives photoaddition products in relatively low yields compared to arylethynyldisilanes which show dual fluorescence. Irradiation of 1 with acetone yields site specific and regioselective 1:1 photoadducts through silacyclopropene intermediates. The triplet excited state of the silacyclopropene reacts with acetone to give addition photoproducts and the triplet energy of the silacyclopropenes lies around 260-286 kJ/mol.

  • Irradiation of 1-(o-hydroxyphenyl)-2-pentamethyldisilanylethyne 1 in a concentrated benzene solution has provided photodimer product 7 along with other reported photoproducts. Irradiation of 1 in methanol yields 1-(o-hydroxyphenyl)-2-trimethylsilylethyne 2 and its reduction product 13, via silacyclopropene intermediate 10, and the reduction product 8 and two methanol addition products, 11 and 12, via o-hydroxyphenylethyne. Photolysis of 1 with acetone in deaerated methylene chloride affords site specific and regioselective 1 : 1 adduct 16 via silacyclopropene intermediate 10.

Patterns of failure after the reduced volume approach for elective nodal irradiation in nasopharyngeal carcinoma

  • Purpose: To evaluate the patterns of nodal failure after radiotherapy (RT) with the reduced volume approach for elective neck nodal irradiation (ENI) in nasopharyngeal carcinoma (NPC). Materials and Methods: Fifty-six NPC patients who underwent definitive chemoradiotherapy with the reduced volume approach for ENI were reviewed. The ENI included retropharyngeal and level II lymph nodes, and only encompassed the echelon inferior to the involved level to eliminate the entire neck irradiation. Patients received either moderate hypofractionated intensity-modulated RT for a total of 72.6 Gy (49.5 Gy to elective nodal areas) or a conventional fractionated three-dimensional conformal RT for a total of 68.4-72 Gy (39.6-45 Gy to elective nodal areas). Patterns of failure, locoregional control, and survival were analyzed. Results: The median follow-up was 38 months (range, 3 to 80 months). The out-of-field nodal failure when omitting ENI was none. Three patients developed neck recurrences (one in-field recurrence in the 72.6 Gy irradiated nodal area and two in the elective irradiated region of 39.6 Gy). Overall disease failure at any site developed in 11 patients (19.6%). Among these, there were six local failures (10.7%), three regional failures (5.4%), and five distant metastases (8.9%). The 3-year locoregional control rate was 87.1%, and the distant failure-free rate was 90.4%; disease-free survival and overall survival at 3 years was 80% and 86.8%, respectively. Conclusion: No patient developed nodal failure in the omitted ENI site. Our investigation has demonstrated that the reduced volume approach for ENI appears to be a safe treatment approach in NPC.

Desensitizing Effects of a Nd:YAG Laser Irradiation on Hypersensitive Dentine (지각과민치아에 대한 Nd:YAG 레이저 조사의 효과)

  • The purpose of this study was to evaluate the desensitizing effects of a Nd:YAG laser (Sunlase, SUNRISE Technologies, Inc., USA) irradiation on cervically exposed hypersensitive dentine. 45 patients was irradiated with pulsed Nd:YAG laser (1.5 W, 20 Hz, 75 mJ/pulse, 4 minutes) as the experimental group, 27 patients was mock irradiated as the control group. The degree of sensitivity to the thermal and tactile stimuli were determined qualitatively with an evaporative stimulus defined as two times air blast at a distance of 3 mm from each site to be tested and with a mechanical stimulus as a slightly scratching the cervical site with a dental explorer. A qualitative registration of the degree of discomfort was determined according to a numerical pain scale(NPS) in an 11-point scale in which 0= "no pain" and 10="most excruciating pain imaginable". Recordings were assessed before treatment, immediately after, 1 and 2 weeks after treatment. Pain tolerance threshold and pulp vitality were evaluated with electric pulp tester before and immediately after treatment. 1. Desensitizing of hypersensitive dentine with Nd:YAG laser irradiation was more effective than that with mock irradiation. 2. The placebo effect of mock irradiation was recognized for severe sensitive teeth($NPS\;{\geqq}\;6$), but not for moderate sensitive teeth(NPS < 6). 3. Laser irradiation did not affect the pain tolerance threshold and pulp vitality of the hypersensitive teeth. 4. Desensitizing effect of laser irradiation for the hypersensitive teeth had been continuing at least 2 weeks. It was concluded that desensitizing of hypersensitive dentine with a Nd:YAG laser is effective and the maintenance of the positive result was more prolonged than the placebo effect.
