Spinal segmental myoclonus improved with epidural blockade in a patient with herpes zoster radiculitis

  • The spinal segmental myoclonus by viral radiculitis has been rarely reported and the pathophysiology remains to be elucidated. However, the hyperactivity of contiguous anterior horn neurons induced by viral irritation has been suggested to be a possible patho-mechanism. In general, spinal segmental myoclonus is not well-controlled by medication and the patient suffers from continuous involuntary movement. We recently experienced a case of spinal segmental myoclonus induced by herpes zoster radiculitis, and which was successfully relieved by epidural injections.

  • Hemifacial spasm is a distressing condition characterized clinically by paroxysmal and an involuntary movement in muscles innervated by the facial nerve on one side of the face. Blockade of the facial nerve can be performed percutaneously, without any serious complications. There are certain clinical problems associated with the conventional procedure, such as severe pain and technical difficulties to find facial nerve. This report describes a fluoroscope guided facial nerve block. This new technique reduced the difficulties in identifying the facial nerve and decreased the suffering associated with the conventional way of facial nerve block.

  • Hemifacial spasm is a distressing condition characterized by involuntary, intermittent, unilateral twitching of, all or parts, of the muscles innervated by facial nerve. This ailment is most common among middle-aged women. Neither causative agent nor reliable treatment has been established because etiology of idiopathic hemifacial spasm has remained undefined to date. This report describes two cases of hemifacial spasm treatment by injection of Botulinum toxin. An injection of Botulinum toxin provided relief of hemi facial spasm. Botulinum toxin therapy is an effective and convenient treatment of hemifacialspasm.

  • A hiccup is an involuntary, spasmodic contraction of the diaphragm accompanied by a sudden closure of the glottis, which is reported commonly in patients with brain stem disease such as ischemic stroke, dolichoectatic basilar artery, tumor, encephalitis, and multiple sclerosis. 1) Intractable hiccup is an uncommon, chronic and incapacitating disturbance defined as a hiccup bout lasting more than 48hours or recurring despite various treatments and affecting male subjects more than female. 2) Constipation and hiccup are common symptoms in stroke patients and purgation therapy has been often used. We discovered two patients who had a hiccup symptom after purgation therapy(diarrhea) and so reported course and result of treatment.

Effect of clomipramine in a dog with cataplexy

  • A 5-year-old, castrated male, Maltese was presented with history of acute flaccid paralysis. The dog was presented with sudden loss of muscle tone and involuntary movements of hind limbs. Neurologic examination revealed reduced postural reaction in the bilateral hind limbs. MRI of brain showed moderate hydrocephalus, but other examination results were normal. Based on the characteristic episodes and examination results, canine cataplexy was suspected. Treatment was initiated with clomipramine as cataplexy control. Clinical signs resolved with 3-month medication. This case demonstrates therapeutic diagnosis of cataplexy. To the author's knowledge, this is the first report of cataplexy treating with clomipramine.

  • Objectives : Hemifascial spasm is painless uncommon disorder characterised by involuntary paroxysmal movement of one side of face. In this study, the effect of meridian tendino-musculature acupuncture and pharmacopuncture therapy for two patient with hemifascial spasm were evaluated. Methods : We treated two patients with meridian tendino-musculature acupuncture and placenta pharmacopuncture therapy in sternocleidomastoid muscle. We evaluated the effect by Scott's scale and treatment satisfaction. Results : Before therapy, the grade of spasm intensity classified by Scott's description in two cases were 3. After therapy, the Scott's grade of one case was 0, and another case were 1. Conclusions : Meridian tendino-musculature acupuncture and placenta pharmacopuncture therapy in sternocleidomastoid muscle for hemifascial spasm was effective and will be attemped to the patients with it.

  • The career development of information security workforce affiliated in administrative department is very different from workforce affiliated in private companies. Their career development attempts are made not by voluntary motivation but by involuntary job movement by the principle of internal relocation. So they are not directly linked to monetary compensation or advancement. Due to the nature of the organization, their work attitude is very passive and there is little intention to turnover. They do not need professionalism, but they must be retrained according to the law. In this paper, we investigate and analyze the roles and responsibilities of information security workforce of each administrative department. And we do questionnaire survey to find out current roles and responsibilities of them will not affect the demand for retraining. Through these research, we would like to discuss how to manage information security workforce affiliated in administrative departments.

  • Tremor is a rhythmic and involuntary muscular contraction charaterized by oscillations of body parts. tremor often accompanies neurological disorders associated with aging. Although the disorder is not life-threatening, it can be responsible for functional disability and interpersonal relationship problems. We have recently examined a number of tremor-patients. One of them was improved significantly through the herbal medicine (Ssanghaptang-gamibang) and accupuncture, so we report it for the better treatment.

  • Dental treatment under general anesthesia is considered for behavioral control of disabled patients who have severe anxiety or involuntary movement. However, in case of simple treatment, inhalation or intravenous sedation, which has earlier onset and recovery, is preferred. Conventionally, nitric oxide is used for inhalation sedation, nowadays sevoflurane can also be used due to easily titratable for controllable effect and less complications. In this case report, two patients with mental retardation required simple dental treatment. Deep sedation with inhaled sevoflurane were successfully employed and patients were discharged without any complications.

  • Dystonia is a neurological disorder characterized by involuntary and uncontrollable muscle tonus abnormalities. It is a huge burden not only to the patients and their families, but also to the field of medicine, in that there has hardly been any substantial change in the concept of and approach to this intractable disorder and therefore no breakthrough to its diagnosis, evaluation and treatment. As an effort to solve the current impasse, this review briefly summarizes the current concept, etiology, diagnosis, treatment and management, and then suggests a rather new therapeutic approach to this disorder, based on the concept of neurological balance and TMJ integrative approach. These new approaches will provide a platform for the clinicians and researchers to have a leap in the concept, diagnosis and therapeutics.
