A Case of Hepatic and Pulmonary Metastatic Colorectal Cancer Patient Treated by Traditional Korean Therapy and XELOX Chemotherapy (한방치료와 XELOX 요법을 병행하여 호전된 간, 폐 전이 대장암 환자 1례)

  • Objective : This report is aimed to investigate the effectiveness of traditional korean therapy including intravenous Cultivated Wild Ginseng Pharmacopuncture (CWGP) and Soram immunopharmacopuncture with XELOX chemotherapy in treating metastatic colorectal cancer patient. Methods : A 47-year-old woman who was diagnosed as metastatic colorectal cancer on Oct 2011 was concurrently treated with traditional Korean therapy (TKT) and XELOX (capecitabine plus oxaliplatin) for 7 months. TKT includes intravenous CWGP, Soram immuno-pharmacopuncture, acupuncture, moxibustion, and herbal medicine. The effectiveness of therapies was evaluated with computed tomography and tumor marker levels such as carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9). And pain on the lateral abdomen was recorded with Visual Analogue Scale (VAS). Results : The tumor mass size of metastatic liver was decreased from 10 cm to 4.3 cm. The tumor marker levels such as CEA and CA19-9 are also decreased. From these results, this case report suggests that the TKT with palliative chemotherapy may be a useful method to treat unresectable metastatic colorectal cancer.

A Case of Intractable Cervical Dysplasia Patient Treated with Korean Medicine for Three Months (3개월간 한방치료를 받은 난치성 자궁경부 이형증 환자 1례)

  • Objectives: This report is aimed to investigate the efficacy of Korean medicine including intravenous Cultivated Wild Ginseng(Panax ginseng C.A. Meyer) Pharmacopuncture(CWGP) with intractable cervical dysplasia patient. Methods: A 49-year-old woman who was diagnosed as cervical dysplasia on Feb 2012 was treated with Korean medicine for 3 months. Korean medicine includes intravenous CWGP, acupuncture, moxibustion and herbal medicine. The effect of therapies was evaluated with human papillomavirus deoxyribonucleic acid(HPV DNA) testing. And lower limb cold sense and genital pruritus were recorded with Visual Analogue Scale(VAS). Results: Lower limb cold sense and genital pruritus were decreased from VAS 8-9(2012.04.11.) to VAS 1-2(2012.07.10.). HPV DNA test result was changed from positive to negative. From these results, this report suggests that the Korean medicine, especially CWGP may be a useful method to treat intractable cervical dysplasia. Conclusions: This report shows that Korean medical treatments are effective on intractable cervical dysplasia.

A Clinical Study on the Effects of Intravenous Wild ginseng Herbal Acupuncture on the Human Body (정맥 주입용 산삼약침이 인체에 미치는 영향에 관한 임상적 연구)

  • Background : This study was conducted to evaluate the effects of wild ginseng herbal acupuncture developed for the intravenous use. Healthy male and female volunteers(n=57) went through Randomized Control Trials(RCT). Methods : For those who are under a medication due to common cold or other illnesses were excluded in the primary stage and the subjects with possible abnormalities in the pre-screening process were also excluded in the secondary stage. Then the examination groups were determined by random sampling. Experiment groups were divided into Normal saline injection group(control group), cultivated wild ginseng herbal acupuncture group(experiment group 1) and natural wild ginseng herbal acupuncture group(experiment group 2) Blood tension, body temperature, pulse, and other criteria were measured and analyzed. Results : 1. Intravenous injection of cultivated wild ginseng herbal acupuncture and natural wild ginseng herbal acupuncture didn't cause significant changes in the blood tension, pulse, body temperature, and etc. 2. No significant differences were witnessed in CBC, ESR, biochemistry of blood test and UA between the experiment groups. 3. No significant changes were noted in the thermography before and after the test in the experiment groups. 4. Some of the common physical changes occurring during and after the administration were fatigue, chest distension, and headache in all of the experiment groups. 5. Comparing general condition after one week from the termination of administration, the control group showed worst condition while as the natural wild ginseng herbal acupuncture group displayed best condition. Conclusion : From the above results, we can carefully deduce that the intravenous injection of the wild ginseng herbal acupuncture didn't show significant differences compared to injection of the normal saline. We can infer it is safe on the human body and further studies and reports must be followed.

Effects of Cultivated Wild Ginseng Pharmacopuncture on Surgically Induced Endometriosis in Rats (산삼약침이 자궁내막증 유발 흰쥐에 미치는 영향)

  • Purpose : This study was carried out to identified the effects of Cultivated Wild Ginseng Pharmacopuncture(CWG Pharmacopuncture) to the rats with surgically induced endometriosis. Methods : Endometrial tissue was implanted in the serosal wall of the small intestine in rats. The rats were divided randomly into an experimental and control group. The experimental group was treated with CWG Pharmacopuncture intravenous injection to the vein of tail and the control group was given same dose of normal saline for 4 weeks. And the size of the ectopic uterine tissue was estimated, and the serum progesterone, estradiol and cytokine($TNF-{\alpha}$, IL-2, IL-4, IL-6, IL-10) concentrations were analyzed. Results : 1. The size of the ectopic uterine implants in the experimental group was much smaller than that in the control group. 2. In analysis of serum hormone, the concentration of progesterone showed higher value than that of control group and estradiol showed lower value. 3. In analysis of cytokine, $TNF-{\alpha}$, IL-2 and IL-6 concentrations showed lower value than that of control group. 4. In analysis of cytokine, IL-4 concentration showed no significant difference and IL-10 concentrations showed higher value than that of control group. Conclusion : These results indicated that CWG Pharmacopuncture is useful treatment for endometriosis.

Effect of Intravenous Lipid Emulsion on Clozapine Acute Toxicity in Rats

  • Objectives: Many studies have been reported the efficacy of intravenous lipid emulsion (ILE) as an antidote on acute lipophilic drug toxicity. Clozapine, highly lipophilic dibenzodiazepine neuroleptics, is an important medication in the schizophrenia therapy regimen. Acute intoxication with antipsychotics is one of the main reasons for the referral of poisoned patients to the hospital. We expected that ILE could be used for the therapy of acute clozapine intoxicated patients. Methods: We used two groups of consisting of six male rats. Both groups received a toxic dose of clozapine (40 mg/kg) intravenously, via the tail vein. After 15 minutes, they were treated with intravenous infusion of 18.6 mg/kg normal saline (NS group), or 18.6 mg/kg ILE 20% (ILE group). We evaluated blood pressure (BP) and heart rate by power lab apparatus through the tail artery, ataxia by a rat rotary circle, seizure scores and death in multiple times after starting clozapine administration. For biochemical and pathological evaluations the samples of tissue and blood were taken. Results: Our results demonstrated that ILE 20% could return hypotension-induced clozapine better than normal saline. Furthermore, ataxia and seizure have rectified more rapidly and deaths reduced. Clozapine administration causes pancreatitis and lung injury but fat emulsion did not show an optimal effect on tissue damages caused by clozapine toxicity. Conclusion: In conclusion, ILE can remove toxic signs of clozapine same as other lipophilic medicines, however, clinical uses of ILE for this intention requires more appraisement to determine the precise implication and safety.

A Case Report for Stage ⅢB Squamous Cell Lung Carcinoma Patient Treated with Cultured Wild Ginseng Pharmacopuncture Therapy (6개월간 산삼약침요법을 시행 받은 ⅢB기 편평세포폐암 환자에 대한 증례보고)

  • Objective To derive further studies evaluating the effectiveness of Cultured Wild Ginseng Pharmacopuncture (CWGP) Therapy on squamous cell carcinoma as a first line. Methods Three cycles (4 weeks/cycle) of CWGP were administered as a dosage of 10 ml per day. Patient was diagnosed with stage IIIB squamous cell carcinoma and refused all therapy of conventional medicine because of old age and cardiac invasion of tumor. Intensive treatment of CWGP for 3 cycles was done on the patient. Computed Topography (CT) was performed to evaluate the therapeutic efficacy. Results After the intravenous infusion of 2 cycles of CWGP, chest CT revealed the mass size and pleural invasion sustained stable disease. After the point injection of 1 cycle of CWGP, chest CT revealed progressive disease. The disease free survival rate was 1 month. Conclusion This case may provide us the possibility that CWGP offers potential benefits for patients with squamous cell lung carcinoma. But this is a single case study and further case-series research should be compensated.

Improved Case of Recurred and Metastatic Ascending Colon Cancer by Combination of Oriental Medical Therapy and FOLFIRI Chemotherapy (산삼약침과 FOLFIRI 항암화학요법의 병행으로 호전된 전이성 대장암 환자 1례)

  • This study is aimed to investigate the effects of intravenous Cultivated Wild Ginseng Pharmacopuncture(CWGP) and the FOLFIRI chemotherapy combination on recurred and metastatic ascending colon cancer patient. A 42-years-old man was diagnosed as ascending colon adenocarcinoma on 9th Mar. 2011. After performing right hemicolectomy and 12 cycles of FOLFOX chemotherapy recurrence at hemicolectomy site and metastases in liver, spleen and lungs were found on 7th Feb. 2012. Intravenous CWGP were performed during total 12 cycles of FOLFIRI chemotherapy from 3rd Mar. 2012 to 27th Sep. 2012. The effects and toxicities of CWGP and FOLFIRI chemotherapy combination were evaluated with PET torso(AA) and National Cancer Institute-Common Toxicity Criteria. The tumor mass in the splenic hilum and liver S4 was disappeared and multiple pulmonic lymph nodules were decreased in size. The recurred lesion on the site of right hemicolectomy showed no changes. During the treatment the patient had no toxicity over grade 1.

Component Analysis and Toxicity Study of Combined Cultivated Wild Ginseng Pharmacopuncture (산양산삼복합약침의 표준화 및 급성독성시험)

  • Objectives: The marker substances of cultivated wild ginseng pharmacopuncture that may not be detected during the process of steaming remain controversial. We developed a combined cultivated wild ginseng pharmacopuncture that contains all the marker substances. The aim of this experiment was to investigate the marker substances and test the toxicity of the combined cultivated wild ginseng pharmacopuncture. Methods: The marker substances were detected using HPLC. Intravenous injection toxicity studies were conducted at Medvill, an authorized institution for non-clinical studies, under the regulations of Good Laboratory Practice. We observed survival rates, abnormal behaviors, weight changes, gross findings in autopsy, blood biochemical properties, and histological abnormalities of organs such as the liver and kidney. Results: HPLC data showed that ginsenosides Rg1, Rb1, and Rg3 were detected at concentrations of 19.29, 47.64, and 3.02 μ g /ml, respectively. Administration of combined cultivated wild ginseng pharmacopuncture resulted in no dead animals or significant toxicological changes. Conclusions: The combined cultivated wild ginseng pharmacopuncture contains all the marker substances and is a relatively safe treatment medium. Further studies should be conducted to confirm the present findings.

The Effect of Intravenous Injection of the Water Extract of Angelica gigas Nakai on Gliosis in the Middle Cerebral Artery Occlusion Rats (당귀 추출물 정맥 주사가 Middle Cerebral Artery Occlusion 모델 흰쥐에서 Gliosis 억제에 미치는 영향)

  • Objectives : Gliosis becomes physical and mechanical barrier to axonal regeneration. Reactive gliosis induced by middle cerebral artery occlusion is involved with up-regulation of CD81 and GFAP (Glial fibrillary acidic protein). The current study is to examine the effect of the Angelica gigas Nakai(intravenous injection. 100 mg/kg twice in a day) on CD81 and GFAP of the rat in the brain after middle cerebral artery occlusion. Methods : Cerebral infarction was induced by middle cerebral artery occlusion. And after intravenous injection of water extract of Angelica gigas Nakai, the size of cerebral infarction was measured. Examination of optical microscope were also used to detect the expression of CD81 and GFAP in the brain of the rat. Results : The following results were obtained : We found that size of cerebral infarcion induced by MCAO (Middle Cerebral Artery Occlusion) in rats were decreased after intravenous injection of Angelica gigas Nakai. We injected the extract of Angelica gigas Nakai to the MCAO in rats, and the optical microscope study showed that Angelica gigas Nakai had effect on protecting the cells of hippocampus. We found that GFAP, CD81 and ERK of the brain in rats with cerebral infarction after MCAO were meaningfully decreased after intravenous injecting Angelica gigas Nakai. We found that c-Fos expression of the brain in rats with cerebral infarction after MCAO were significantly increased after intravenous injecting Angelica gigas Nakai. Conclusions : These results indicate that Angelica gigas Nakai could suppress the reactive gliosis, which disturbs the astrocyte regeneration in the brain of the rat with cerebral infarction after MCAO by controlling the expression of CD81 and GFAP. And the effect may be modulated by the up-regulation of c-Fos and ERK.

A Study on recent tendency of anti-tumor herbal acupuncture (항암약침의 최신경향에 대한 고찰)

  • Objective: The purpose of this study is to develop and activate anti-tumor herbal acupuncture for cancer patients in South Korea. Methods: We investigated some literatures on anti-tumor herbal acupuncture which is used in South Korea and China, and made diagrams. Results: The results are summarized as follows. Anti-tumor herbal acupuncture is one of the traditional oriental medical method which is effective for cancer patients. In domestic studies, most of herb materials are belong to action of cooling&detoxification(25.0%) and strengthening body resistance(46.4%) which are proved to have effects of anti-tumor, immune activation and preventing tumor. In China, point injection therapy are used for improving symptoms of cancer patients and hea1ing tumor. Also herbal intravenous injection is used for combination of chinese traditional and western cancer therapy and treating cancer patients variously. Conclusions: From the above results. it is expected that anti-tumor herbal acupuncture is useful to improve clinical symptoms and quality of life(QOL) of cancer patients. Also we must develop new progressive methods of point injection and herbal intravenous injection for treating cancer patients, and advance clinical studies and trials.
