식도-공장 문합루에 대한 Stent 삽관치료 (Treatment of Esophagojejunostomy Leakage with Expanding Stent)

  • 장관의 문합부전에 대하여 장관내 삽관이 치료에 도움이 될 수 있을 것으라고 생각되고 있으나, 이에 대한 보고는 찾아보기 드물다. 저자들은 식도와 위의 중복암 환자에서 식도-공장 문합술후에 발생한 문합부전에 대하여 식도내 삽관으로 매우 효과적이고 빠른 호전을 경험하였다. 식도내 삽관은 술후 제28병일에 시행하였으며, 삽입후 약 3주후부터 문합부 유출 및 염증소견들이 소실되었다. 수술후 약 11개월이 경과한 현재, 삽관은 약 1 cm 가량 하방으로 이동되어 있으나 더 이상의 변화는 없으며, 환자는 고형식사을 지속하고 있다. 따라서 저자들은 장문합부전의 제한된 경우에 있어서는 장관내 삽관이 도움을 줄 수 있을 것이라고 생각한다.

Coil Embolization of Ruptured Proximal Posterior Inferior Cerebellar Artery Aneurysm with Contralateral Retrograde Approach for LVIS Jr. Intraluminal Support Deployment

  • The safety and feasibility of simple coil embolization and stent deployment for the treatment of posterior inferior cerebellar artery (PICA) aneurysms, as well as their radiologic and clinical results, have not been adequately understood. Especially, if dissecting aneurysm of proximal PICA is associated with small caliber PICA and stenosis of ipsilateral vertebral artery orifice (VAO), endovascular coiling with saving of PICA is not always easy. This 64-year-old man presented with subarachnoid hemorrhage due to a ruptured dissecting aneurysm of left proximal PICA. The aneurysm was irregularly fusiform in nature with a shallow PICA orifice (1.4 mm) and narrow caliber (0.9-1.5 mm). Moreover, the ipsilateral VAO showed severe stenosis (1.8 mm). We performed bifemoral puncture and chose additional route from right vertebral artery to left vertebrobasilar junction for retrograde approach and deployment of LVIS Jr. intraluminal support at proximal PICA. And then, the antegrade approach and coiling of aneurysm was done. Despite of transient thrombus of PICA, the aneurysm was successfully secured with preservation of whole PICA course. For preservation of narrow PICA with ipsilateral VAO stenosis, the contralateral approach and deployment of LVIS Jr. intraluminal support may be considered.

Application of Two Different Tracheal Stents in Small Toy Dogs with Tracheal Collapse

  • Tracheal collapse is a common respiratory disease in dogs. There are many ways to treat tracheal collapse, one of which is the use of an intraluminal stent. In this study, we divided 21 dogs into two groups and implant conventional stents and new nitinol stents. Comparison of two groups was based on following, feature of stent fracture, form of stent migration, clinical sign improvement, complication and prognosis. Approaching was established via C-arm under spontaneous breathing and placing a stent at the site of collapse. Using radiographic images, determine stent size accurately. For a comparison of identical condition, all intraluminal stents were placed 10 mm caudal from larynx to 10 mm cranial from carina. In this study, new nitinol stents improve the problems of conventional stents and may be effective in the treatment of tracheal collapse in small dogs.

가감보심탕(加減補心湯)의 구강투여가 Intraluminal Filament 삽입술(揷入術)에 의(依)해 유발(誘發)된 백서(白鼠)의 허혈성(虛血性) 뇌손상(腦損傷)에 미치는 영향(影響) (The effects of Gagambosim-Tang of focal brain ischemic injury induced by Intraluminal Filament insertion in the rats)

  • Objective : The aim of this study was to investigate effects of Gagambosim-Tang(GBT) of focal brain ischemic injury induced by intraluminal filament insertion in the rats. Method : The ischemia was induced by intraluminal filament insertion into middle cerebral artery Eight-arm radial maze was designed for the behavioral task. Gagambosim-Tang was orally administrated in SD rats for 21 days. The task was started on the 4th week after focal brain injury, and performed two trials per day for 6days. The effects of Gagambosim-Tang on neuroprotective agents in cresyl violet, choline acetyltranferase(ChAT), nerve growth factor (NGF)-stain and c-Fos with ischemic injury were investigated. Results : The errors in the eight-arm radial maze task were significantly decreased in normal group compared with control group on 2-6days, GBT lX(42.2 mg/ml)orally administrated group on 1days, GBT 6X(253.2 mg/ml) on 3, 5, 6days. The rate of correct choice was increased in normal and GBT 6X groups. The neuroprotective effect in the hippocampal CA1 was increased in normal and GBT 1X, GBT 6X groups compared with control group. The density of ChAT in the hippocampal CA1 was increased in normal and GBT6X groups compared with control group. The density of NGF in the hippocampal CA1 was increased normal and GBT6X groups compared with control group. The number of c-Fos-positive neurons in the hippocampal CA1 was increased in normal and GBT 6X groups compared with control group. Conclusion : These results suggest that Gagambosim-Tang may have protective effect on dementia.

고선량율 관내 방사선치료를 위한 종양선량분포의 최적화에 대한 연구 (Optimization of Dose Distribution for High Dose Rate Intraluminal Therapy)

  • 원격조종 아프터로딩에 의한 고선량율 관내삽입조사는 체내 발생된 종양에 방사선원을 근접시켜 치료하는 방사선요법으로서 신속한 선량계산과 선량의 정확성 및 다양한 모양의 최적선량분포가 요구된다. 저자들은 크기가 작고 선량율이 높은 고선량율의 방사성동위원소에 대한 정확한 조사선량과 최적선량분포를 얻기 위하여 수학적인 콤퓨터 계산프로그램과 실측으로서 비교하였다. 고선량율 선원에 의한 방사선 조사선량과 조직내 흡수선량분포는 각각 Sievert적분식과 Meisberger의 다항식을 이용하여 작성하였다. 종양크기와 모양에 가장 알맞는 선량분포의 최적화를 실현하기 위하여 저자들은 치료기준점의 선량을 일정한 값으로 고정시키고 선원의 조사시간을 조정하는 선형반복 계산방정식을 이용하였다. 모형선원이 장착된 아프터로딩관을 삽입하고 조준엑스선으로 촬영하여 종양부위를 결정한 후 콤퓨터의 도움으로 아프터로딩관의 축과 평행한 등량곡선 또는 과일모양의 선량분포 및 기관지 모양의 등선량분포가 성취되도록 선량최적화를 시행하였고 선량계에 의한 실측치와 오차가 $3\%$이하로 잘 일치하였다.

  • A 14-year old boy was admitted with stuporous mentality. CT scan, MRI and cerebral angiogram revealed SAH and a giant aneurysm of right middle cerebral artery($4{\times}5{\times}5.3cm$). To minimize surgical risk, endovascular treatment was done with MDS(mechanical detachable system)-spiral coil. Follow up MRI showed intraluminal thrombus formation of the aneurysm.

Unicystic ameloblastoma with diverse mural proliferation - a hybrid lesion

  • A 46-year-old man was referred to our hospital for treatment, complaining of swelling on the right mandibular molar region. Radiographic examination revealed a well defined multilocular radiolucent lesion with root resorption of right lower anteriors and molars. Following biopsy, a diagnosis of unicystic ameloblastoma of mural type was made and hemimandibulectomy was performed under general anesthesia. Histopathological examination of the surgical specimen exhibited a unicystic ameloblastoma of luminal, intraluminal, and mural type. Intraluminal proliferation was of plexiform pattern and mural proliferation showed unusual histopathological findings, which revealed follicular, acanthomatous areas coexisted with desmoplastic areas. This mural picture was similar to the so-called 'hybrid lesion of ameloblastoma', whose biological profile is not elicited due to the lack of adequate published reports. Two years follow up till date has not revealed any signs of recurrence.

  • Leiomyoma of the trachea is extremely rare but important to recognize early because they are curable. A case of leiomyoma of the trachea is described in 340-year-Old woman. She was admitted for dyspnea, coughing and sputum for 3 years. Under the impression of asthma she was treated but not improved. Chest CT showed an intraluminal tracheal mass just above the carina. Fiberoptic bronchoscopy revealed a round intraluminal mass on the membranous portion of trachea 4cm above the carina. The tumor was excised by wedge resection and end-to-end anastomosis of the trachea was performed. The pathologic examination revealed leiomyoma of the trachea. We report a case of leiomyoma of the trachea which was successfully resected.

  • Phlegmonous esophagitis must be treated aggressively; therefore, appropriate antibiotic therapy and drainage are critical. Although a conventional surgical approach has been used previously, internal drainage could be another treatment option in light of advances in endoscopic techniques. We report 2 cases in which patients suffering from phlegmonous esophagitis were successfully treated with endoscopic intraluminal drainage and antibiotics.

생산공정 차이에 따른 죽력(竹瀝)이 apoptosis 및 신경세포 보호 효과에 미치는 영향 (Anti-apoptotic and Neuroprotective Effects of Bambusae Caulis in Liquamen Manufactured by Different Production Process)

  • Bambusae Caulis in Liquamen(BCL) has been commonly prescribed for stroke patients in the traditional Oriental medicine. So this study is aims to investigate the anti-apoptotic and neuroprotective effects of Bambusae Caulis in Liquamen(BCL) manufactured by different production process on the focal ischemia induced by intraluminal filament insertion in rats. The focal ischemia was induced by intraluminal filament insertion into middle cerebral artery. The animals were divided into four groups (n=15 in each group). The ischemia induced and not treated group : Control group, the ischemia induced and oral medication of the three kinds of BCL : BCL-A group, BCL-B group, BCL-C group. BCL-A was produced by heating at a low temperature$(250^{\circ} C)$ in electric kiln and filtering. BCL-B was produced by heating at a high temperature$(900^{\circ} C{\sim}1,000^{\circ}C)$ in yellow earth kiln and refining and filtering. BCL-C was produced by heating at a low temperature$(400^{\circ} C)$ yellow earth kiln and no refining and filtering. The anti-apoptotic and neuroprotective effects of the oral medication of BCL were observed by Bax, BCL-2, cytochrome c, mGluR5, cresyl violet and ChAT-stain. Our study suggests that BCl-A(was produced by heating at a low temperature in electric kiln and filtering) and BCL-C(was produced by heating at a low temperature in yellow earth kiln and no refining and filtering) show anti-apoptotic and neuroprotective effects on the focal ischemia induced by intraluminal filament insertion in rats and BCL-C is more effective than BCL-A.
