Cervical Radiculopathy Caused by Vertebral Artery Loop Formation : A Case Report and Review of the Literature

  • Vertebral artery loop formation causing encroachment on cervical neural foramen and canal is a rare cause of cervical radiculopathy. We report a case of 61-year-old woman with vertebral artery loop formation who presented with right shoulder pain radiating to her arm for 2 years. Plain radiograph and computed tomography scan revealed widening of the right intervertebral foramen at the C5-6 level. Magnetic resonance imaging and angiogram confirmed the vertebral artery loop formation compressing the right C6 nerve root. We had considered microdecompressive surgery, but the patient's symptoms resolved after conservative management. Clinician should keep in mind that vertebral artery loop formation is one of important causes of cervical radiculopathy. Vertebral artery should be visualized using magnetic resonance angiography in suspected case.

Medial Loop of V2 Segment of Vertebral Artery Causing Compression of Proximal Cervical Root

  • Objective : It is rare that the medial loop in the V2 segment of the vertebral artery (VA) causes compression of the proximal cervical root of the spinal cord without leading to bony erosion and an enlarged foramen. We evaluated the clinical significance and incidence of the medial loop in the V2 segment of the VA. Methods : We reviewed the records from 1000 consecutive patients who had undergone magnetic resonance imaging evaluation of the cervical spine between January 2005 and January 2008. The inclusion criteria were that over a third of the axial aspect of the VA located in the intervertebral foramen was inside the line between the most ventral points of the bilateral lateral mass, and that the ipsilateral proximal root deviated dorsally because of the medial loop of the VA. We excluded cases of bone erosion, a widened foramen at the medial loop of the VA, any bony abnormalities, tumors displacing VA, or vertebral fractures. The medical records were reviewed retrospectively to search for factors of clinical significance. Results : In six patients (0.6%), the VA formed a medial loop that caused compression of the proximal cervical root. One of these patients had the cervical radiculopathy that developed after minor trauma but the others did not present with cervical radiculopathy related to the medial loop of the VA. Conclusion : The medial loop of the VA might have a direct effect on cervical radiculopathy. Therefore, this feature should be of critical consideration in preoperative planning and during surgery.

Study of Computed Tomography Reconstruction Angle of Bony Stenosis of the Cervical Foramen (경추 신경공 골성 협착에서 CT 재구성 각도 연구)

  • This study is an image reconstruction method after cervical CT scan. According to the oblique coronal reconstruction angle, i would like to suggest the reconstruction angle that can best express neural foraminal bony stricture. The angle created by drawing a line passing through the center of the spinal cord in the midsagittal plane of the cervical spine and the uncovertebral joint was measured. A line passing through the center of the spinal cord was drawn from the neural foramen, and the angle formed with the midsagittal plane was measured and compared. At the C4-5 level, the average was 9.2°, at the C5-6 level, the average was 9.9°, and at the C6-7 level, the average was 8.4°, the neural foraminal angle was measured to be larger than the uncovertebral joint angle. There was a statistically significant difference in mean (p<0.01). Also, it was found that the angle increased toward the lower cervical vertebrae. The angle between the neural foramen of the lower cervical vertebrae (C5-6, C6-7) and the center of the spinal cord is between 55 and 60°. Here, if the oblique coronal image is reconstructed to be 90°, the degree of neural foramen stenosis can be observed well. Because it is an image reconstruction using a conventional CT scan image, it does not receive additional radiation exposure. It is of great significance in diagnosing cervical neural foramen bony stenosis.

The Comparison of $45^{\circ}$ and $55^{\circ}$ Anteroposterior Oblique View for Observating the Intervertebral Foramen (경추 추간공 관찰을 위한 촬영법의 고찰)

  • The cervical spine of anteroposterior oblique view is essential to observe the intervertebral foramen(IVF). The morphologic changes of IVFs were proved to be abnormal with nerve roots and peripheral structures. The purpose of this study is to evaluate the effective projection angle for observing the IVFs in the Korean adults. In a prospective clinical study of 100 normal persons, $45^{\circ}$, $50^{\circ}$ and $55^{\circ}$ oblique views were compared by measuring the maximal transverse diameter of all the cervical IVFs. $45^{\circ}$ oblique views provided slightly better visualization of upper cervical level(C2-C3, C3-C4, C4-C5), but the lower cervical level(C5-C6, C6-C7, C7-T1) of IVF transverse diameters were substantially increased on the $55^{\circ}$ AP oblique projection. In the comprasion of mean differences between 8 obese person(BMI > 25) and 58 normal person(18.5 < BMI < 22.9) proved to be statistically not significant. Consequently this study shows that $55^{\circ}$AP oblique(tube angle $15^{\circ}$cephalad) view is optimal for evaluating the lower cervical IVFs.

Morphometric Relationship between the Cervicothoracic Cord Segments and Vertebral Bodies

  • Objective : The objective of this study was to investigate the morphologic characteristics between the vertebral body and the regions of the cervical and thoracic spinal cords where each rootlets branch out. Methods : Sixteen adult cadavers (12 males and 4 females) with a mean age of 57.9 (range of 33 to 70 years old) were used in this study. The anatomical relationship between the exit points of the nerve roots from the posterior root entry zone at each spinal cord segment and their corresponding relevant vertebral bodies were also analyzed. Results : Vertical span of the posterior root entry zone between the upper and lower rootlet originating from each spinal segment ranged from 10-12 mm. The lengths of the rootlets from their point of origin at the spinal cord to their entrance into the intervertebral foramen were 5.9 mm at the third cervical nerve root and increased to 14.5 mm at the eighth cervical nerve root. At the lower segments of the nerve roots (T3 to T12), the posterior root entry zone of the relevant nerve roots had a corresponding anatomical relationship with the vertebral body that is two segments above. The posterior root entry zones of the sixth (94%) and seventh (81%) cervical nerve roots were located at a vertebral body a segment above from relevant segment. Conclusion : Through these investigations, a more accurate diagnosis, the establishment of a better therapeutic plan, and a decrease in surgical complications can be expected when pathologic lesions occur in the spinal cord or vertebral body.

In a comparison of the measured angles in foraminal image was not significantly different (P>0.01).
