Recent Interventional Procedures for Spinal Discogenic Pain for Life Care : Technical and Ethical Issues (라이프케어를 위한 추간판성 통증 치료의 최신중재기법 기술적 윤리적 문제들에 관하여)

  • Discogenic pain which develops during the degenerative process of intervertebral disc is an intractable disease of musculoskeletal system. Unlike other diseases in modern medical science, it is not clinically confirmed through objective imaging and clinicopathologic index. Moreover, current technology has been facing difficulties finding fundamental treatment. This study examined through reviewing literature whether the therapeutic rationales of interventional procedures for the discogenic pain, such as nerve block, intradiscal electrothermal therapy and nucleoplasty, are valid and whether safety reviews such as preclinical tests are carried out faithfully during the technology development process. As a result, it is presumed that there is not yet an interventional treatment for discogenic pain, whose therapeutic rationale has been objectively verified. And there are some cases of interventional treatment that have not been adequately equipped with the preclinical test steps necessary to review the safety of the procedure during the technology development process. In conclusion, since there is still no interventional treatment for discogenic pain which has clear therapeutic rationale, it is necessary to recognize that a more careful approach is needed to judge whether the procedure is performed in clinical settings. And medical ethical issues may arise for some interventional treatment that are deemed to have been carried out without a thorough review of safety.

A Study on the Evaluation of Patient Dose in Interventional Radiology (중재적방사선검사에서 환자 피폭선량에 관한 연구)

  • To perform patient dose surveys in major interventional radiography procedures as a mean of inter-institutional comparison and of establishing reference dose levels with the ultimate goal of optimizing patient doses in the field of interventional radiography. We reviewed international patient dose survey data in the literature and measured patient dose in major interventional radiography procedures (TACE, AVF, PTBD, TFCA, GDC embolization). ESD(Entrance Skin Dose) was measured using TLD chips attached to the patient skin and ED(Effective Dose) was calculated using angiography unit-derived DAP. A survey of patient dose in interventional radiography procedures were also performed with a questionnaire for interventional radiologists and we proposed a guideline for optimizing patient doses in the field of interventional radiology. The patient dose survey data in interventional radiography procedures were very rare in literature compared with those in diagnostic radiography procedures. In TACE, the mean ED was 25.43 mSv and the mean ESD was 511.75 mGy. The mean ED of TACE was not high, but the cumulative dose should be checked, due to longer procedure TACE. In TFCA, the mean ED was 22.6 mSv and it was relatively high compared with data of other countries. In GDC embolization, the mean ED was not available, because GDC embolization was performed with old Image-Intensifier-type unit and there has no unit-installed ionization chamber. Also, the mean ESD of GDC embolization was up to 2,264 mGy and further studies are needed to calculate the net ED of GDC embolization. Patient dose occurred during interventional radiography procedures are high related with the difficulty of the procedure, fluoroscopy time, the number of angiographies and the treatment protocol. Therefore, continuous education and efforts should be made to optimize the patient dose in the field of interventional radiology.

Measurement of Spatial Dose Distribution for evaluation operator dose during Neuro-interventional Procedures (두경부 질환의 인터벤션 시술 시 시술자의 피폭선량평가를 위한 공간선량측정에 관한 연구)

  • The spatial dose distribution was measured with ionization chamber as preliminary study to evaluate operator dose and to study dose reduction during neuro-interventional procedures. The zone of operators was divided into four area (45, 135, 225, and 315 degree).We supposed that operator exist on the four area and indicated location of critical organs(eyes, breast, gonad). The spatial doses were measured depending on distance( 80, 100, 120, and 140 cm) and location of critical organs. The spatial doses of area of 225 degree were 114.5 mR/h (eyes location), 143.1 mR/h (breast location) and 147 mR/h (gonad location) in 80 cm. When changed location of x-ray generator, spatial dose increased in $18.1{\pm}10.5%$, averagely. We certified spatial dose in the operator locations, Using the results of this study, It is feasible to protect operator from radiation in neuro-interventional procedures.

A Study of Drug Content and Cell Cytotoxicity of Paclitaxel-eluting Stents Coated with Various Biopolymer (다양한 생체고분자로 코팅된 Paclitaxel Eluting Stent의 약물함량과 세포독성 연구)

  • In this study, the paclitaxel eluting stent (PES) was prepared by coating a biliary stent with paclitaxel using various biopolymer such as poly (vinyl acetate) (PVAc), poly (lactic-co-glycolic acid) (PLGA), Silicone rubber for restenosis prevention in gastrointestinal disease by a dip-coating method. Drug contents of PES were increased as surface area of stent, concentration and molecular weight of coating polymer increase. In $^1H-NMR$ specta, we know that drug did not change by confirming specific peaks of paclitaxel in PES. As shown in SEM image, PES prepared using various biopolymer is coated clearly and regularly except Silicone rubber coating polymer. In in vitro cell cytotoxicity test, bare stent showed low cytotoxic effect against CT-26 colon carcinoma cell line on 3 day. However, PES coated with PLGA 502H showed the highest cytotoxicity because PLGA 502H is biodegradable polymer and has less molecular weight than other coating polymer. These results suggest that PES coated various biopolymer can be prevented restenosis in gastrointestinal disease.

A systematic review of domestic oral health care programs for elementary school children (초등학생 대상의 국내 구강건강관리 프로그램에 대한 체계적 문헌고찰)

  • Objectives: This purpose of this study was to examine the trends in interventional research and analyze the contents and results of intervention through a systematic literature review of the domestic research literature, suggesting the effect of an oral care program on elementary school students. Methods: A total of 262 articles were retrieved initially, and the duplicate articles were then removed, excluded after screening titles, abstracts, and full-text articles. Finally, a total of 16 papers were used in the review. Results: The study included 9 articles with the nonequivalent control group pretest-posttest design and 7 with the one-group pretest-posttest design. All articles were of non-randomized studies. The most common index of the interventional effect was dental plaque. Further, oral health behaviors and knowledge, perception, self-efficacy, and DMFT index were common indices. In the quality assessment, in the "blinding of outcome assessment" among six items of ROBANS, 12 studies showed a high risk of bias. Conclusions: In order to develop an interventional oral health care program for elementary school students and evaluate the effectiveness, a strict research design and qualitative improvement of research reports are required, and continuous research should be conducted to develop a systematic protocol.

A Study on Fatigue Characteristic of Stent Using Finite Element Analysis (나이티놀 와이어 스텐트의 피로도 특성에 대한 유한요소 해석)

  • Stents are frequently used throughout the human body. They keep pathways open in vascular or nonvascular duct for a long time. Therefore its stability is very important factor. In recent years, aconsiderable amount of research has been carried out in order to estimate mechanical properties of the stent such as expansion pressure behavior, radial recoil and longitudinal recoil using FEM (Finite element analyses). However, published works on simulation of stent fatigue behavior using FEM are relatively rare. In this paper, a nonlinear finite-element methodwas employed to analyses the compression of a stent using external pressure and fatigue behavior. Finite element analyses for the stent system were performed using NASTRAN FX. In conclusion this paper shows how the stent is behaved in the body, and its fatigue behavior.

Differentiation of Benign from Malignant Adnexal Masses by Functional 3 Tesla MRI Techniques: Diffusion-Weighted Imaging and Time-Intensity Curves of Dynamic Contrast-Enhanced MRI

  • Background: The aim of this study was to evaluate and compare the accuracy of diffusion-weighted imaging (DWI), apparent diffusion coefficient (ADC) value, and time-intensity curve (TIC) type analysis derived from dynamic contrast-enhanced MR imaging (DCE-MRI) in differentiating benign from malignant adnexal masses. Materials and Methods: 47 patients with 56 adnexal masses (27 malignant and 29 benign) underwent DWI and DCE-MRI examinations, prior to surgery. DWI signal intensity, mean ADC value, and TIC type were determined for all the masses. Results: High signal intensity on DWI and type 3 TIC were helpful in differentiating benign from malignant adnexal masses (p<0.001). The mean ADC value was significantly lower in malignant adnexal masses (p<0.001). An ADC value< $1.20{\times}10^{-3}mm^2/s$ may be the optimal cutoff for differentiating between benign and malignant tumors. The negative predictive value for low signal intensity on DWI, and type 1 TIC were 100%. The pairwise comparison among the receiver operating characteristic (ROC) curves showed that the area under the curve (AUC) of TIC was significantly larger than the AUCs of DWI and ADC (p<0.001 for comparison of TIC and DWI, p<0.02 for comparison of TIC and ADC value). Conclusions: DWI, ADC value and TIC type derived from DCE-MRI are all sensitive and relatively specific methods for differentiating benign from malignant adnexal masses. By comparing these functional MR techniques, TIC was found to be more accurate than DWI and ADC.

Review of National Diagnostic Reference Levels for Interventional Procedures

  • Diagnostic reference level (DRL) is employed to optimize the radiation doses of patients. The objective of this study is to review the DRLs for interventional procedures in Korea and abroad. Literature review was performed to investigate radiation dose index and measurement methodology commonly used in DRL determination. Dose area product (DAP) and fluoroscopy time within each major procedure category were systematically abstracted and analyzed. A wide variation was found in the radiation dose. The DAP values and fluoroscopy times ranged 0.01-3,081 Gy·㎠ and 2-16,878 seconds for all the interventional procedures, 8.5-1,679 Gy·㎠ and 32-5,775 seconds for the transcatheter arterial chemoembolization (TACE), and 0.1-686 Gy·㎠ and 16-6,636 seconds for the transfemoral cerebral angiography (TFCA), respectively. The DRL values of the DAP and fluoroscopy time were 238 Gy·㎠ and 1,224 seconds for the TACE and 189 Gy·㎠ and 686 seconds for the TFCA, respectively. Generally, the DRLs of Korea were lower than those of other developed countries, except for the percutaneous transluminal angioplasty with stent in arteries of the lower extremity (LE PTA and stent), aneurysm coil embolization, and Hickman insertion procedures. The wide variation in the radiation doses of the different procedures suggests that more attention must be paid to reduce unnecessary radiation exposure from medical imaging. Furthermore, periodic nationwide survey of medical radiation exposures is necessary to optimize the patient dose for radiation protection, which will ultimately contribute to patient dose reduction and radiological safety.

Analysis on Content and Trend of Nursing Intervention Studies Published in Adult Nursing Area (성인간호학 영역에서의 간호중재논문 내용 분석)

  • Purpose : This descriptive study was performed to analyze the trends of nursing interventional studies in adult nursing area. Method : All interventional studies of 'Journal of Korean Academy of Adult Nursing Society', fifty three articles, from 1989 to 2000 were collected and reviewed Result : The research subject were patients in 38(66.7%) and 81.8% of researches was performed in hospital environment. There was only one true experimental design and most of articles(79.2%) were designed by quasi-experimental method Only 17% of articles have theoretical framework and 73.6% of articles described research hypotheses. The data collection methods for variables were as follows: Psychosocial measurement 60.8%, physiological measurement 25.7%, and observation 13.5%, respectively. Lecture(16.7%), audio-visual(16.7%), and book, pamphlet, or leaflets(13.6%) were the most frequently used instruments of the interventions. Behavioral domain(60.3%) and Physiological: Basic domain(29.3%) was the most common, when the interventions were classified by Nursing Intervention Classification. Physiological(50.8%) and psychosocial variables(49.2%) were the two major dependent variables. Conclusion : Considering the above results obtained, the nursing intervention studies in adult nursing area have been tested with quasi-experimental studies through clinical trials. For the development of clinical nursing practice and advancement of nursing theories, the amount of interventional studies should be increased and the quality of research methods refined more to develop nursing science and practice in adult nursing area.
