Current Status and Future Prospect of Endovascular Neurosurgery

  • Recently, due to the evolution of technology, the field of neurosurgery is receiving spotlight. In particular endovascular neurosurgery has gained a great interest along with the advancement of the modern neurosurgery. The most remarkable advances were made in embolization of the cerebral aneurysms, arteriovenous malformations and intracranial stenosis during the past 10 years. These advances will further change the role of neurosurgeons in treating cerebrovascular disease. Because interventional neuroradiologists have performed most of procedures in the past, neurosurgeons have been deprived of chances to learn endovascular procedure. This article discusses the development of technological aspect of endovascular neurosurgery in chronological order. By understanding the history and current status of the endovascular surgery, the future of neurosurgery will be promising.

Hybrid Approach for Treatment of Multiple Traumatic Injuries of the Heart, Aorta, and Abdominal Organs

  • A 55-year-old man was admitted to the trauma center after a car accident. Cardiac tamponade, traumatic aortic injury, and hemoperitoneum were diagnosed by ultrasonography. The trauma surgeon, cardiac surgeon, and interventional radiologist discussed the prioritization of interventions. Multi-detector computed tomography was carried out first to determine the severity and extent of the injuries, followed by exploratory sternotomy to repair a left auricle rupture. A damage control laparotomy was then performed to control mesenteric bleeding. Lastly, a descending thoracic aorta injury was treated by endovascular stenting. These procedures were performed in the hybrid-angio room. The patient was discharged on postoperative day 135, without complications.

  • Musculoskeletal ultrasound has evolved as the essential tool to diagnose and guide intervention procedures in people with neuromusculoskeletal conditions. Image optimization and understanding device operations are core components for ultrasound guided intervention procedure training. All ultrasound machines share the common operative features and there are various buttons for the features in the device control panel. Ultrasound "knobology" refers to the thorough understanding of imaging optimization. This review addressed basic information for the transducers, depth setting, gain and focus control, different modes focusing on brightness and doppler modes.

  • Therapeutic endoscopic ultrasonography (EUS) procedures using the forward-viewing convex EUS (FV-EUS) have been reviewed based on the articles reported to date. The earliest reported procedure is the drainage of pancreatic pseudocysts using FV-EUS. However, the study on drainage of pancreatic pseudocysts focused on showing that drainage is possible with FV-EUS rather than leveraging its features. Subsequently, studies describing the characteristics of FV-EUS have been reported. By using FV-EUS in EUS-guided choledochoduodenostomy, double punctures in the gastrointestinal tract can be avoided. In postoperative modified anatomical cases, using the endoscopic function of FV-EUS, procedures such as bile duct drainage from anastomosis, pancreatic duct drainage from the afferent limb, and abscess drainage from the digestive tract have been reported. When a perpendicular puncture to the gastrointestinal tract is required or when there is a need to insert the endoscope deep into the gastrointestinal tract, FV-EUS is considered among the options.

Comparison of ultrasound guided pulsed radiofrequency of genicular nerve with local anesthetic and steroid block for management of osteoarthritis knee pain

  • Background: To compare ultrasound-guided pulsed radiofrequency (PRF) of the genicular nerve with the genicular nerve block using local anesthetic and steroid for management of osteoarthritis (OA) knee pain. Methods: Thirty patients with OA knee were randomly allocated to receive either ultrasound-guided PRF of the genicular nerve (PRF group) or nerve block with bupivacaine and methylprednisolone acetate (local anesthetic steroid [LAS] group). Verbal numeric rating scale (VNRS) and Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) scores were measured at pre-procedure and 1-, 4-, and 12-weeks post-procedure. Results: VNRS scores decreased significantly (P < 0.001) in both the groups at 12 weeks and other follow up times compared to baseline. Seventy-three percent of patients in the PRF group and 66% in the LAS group achieved effective pain relief (≥ 50% pain reduction) at 12 weeks (P > 0.999). There was also a statistically significant (P < 0.001) improvement in WOMAC scores in both groups at all follow up times. However, there was no intergroup difference in VNRS (P = 0.893) and WOMAC scores (P = 0.983). No complications were reported. Conclusions: Both ultrasound-guided PRF of the genicular nerve and blocks of genicular nerve with local anesthetic and a steroid provided comparable pain relief without any complications. However, PRF of the genicular nerve is a procedure that takes much more time and equipment than the genicular nerve block.

초음파 유도하 요추 4번 선택적 신경근 차단술 시 자기공명영상 계측의 유용성 (Efficacy of Preliminary Magnetic Resonance Imaging Measurement in Ultrasonography-Guided L4 Selective Nerve Root Block)

  • 목적: 초음파 유도하 요추 4번 신경근 차단술을 시행함에 있어 시술 전 자기공명영상(magnetic resonance imaging, MRI) 계측의 유용성을 알아보고자 하였다. 대상 및 방법: 이 연구는 후향적 연구로 2016년 3월부터 2017년 12월까지 본원 외래에 방문한 환자 중 요추 4번 신경근의 병변이 확인되며 선정 기준에 합당한 71명의 환자를 대상으로 하였다. 2016년 3월부터 2017년 2월까지 MRI 계측 없이 신경근 차단술을 시행한 31명의 환자를 A군, 2017년 3월부터 2017년 12월까지 시술 전 MRI 계측을 통해 신경근 차단술을 시행한 40명의 환자를 B군으로 분류하였다. A군은 MRI 계측 없이 pararadicular 접근법을 통하여 주사를 시행하였고 B군은 시술 전 MRI로 측정한 계측치를 바탕으로 하여 초음파 유도하 주사를 시행하였다. 시술 전, 시술 3시간 후, 2주 후, 6주 후, 12주 후에 수치평가척도(numeric rating scale, NRS) 점수를 이용하여 통증 호전 정도를 판정하였다. 결과: 시술 3시간 후 양호 이상의 결과를 보인 경우는 A군에서 51.6%, B군에서 67.5%였으며 시술 2주 후 양호 이상의 결과를 보인 경우는 A군에서 48.4%, B군에서 70.0%였다. 시술 6주 후 양호 이상의 결과를 보인 경우는 A군에서 58.1%, B군에서 62.5%, 시술 12주 후 양호 이상의 결과를 보인 경우는 A군에서 67.7%, B군에서 62.5%였다. 시술 3시간 후, 2주차에 유의하게 B군이 A군보다 증상 호전에 좋은 결과를 보였다(p<0.05). 만족스런 통증 완화를 위해 2주차에서 6주차 사이에 A군의 경우 2.8회, B군의 경우 1.7회의 반복적인 시술이 시행되었다(p<0.05). 결론: 시술 전 MRI를 통한 계측을 먼저 시행하고 이를 참고하여 초음파 유도하 요추 4번 선택적 신경근 차단술을 시행하는 것은 환자의 시술의 성공률을 높여 시술 후 2주 이내의 초기 통증을 완화하는데 도움이 될 것으로 생각된다.

대량객혈로 인한 기관지 동맥 색전의 중재적 방사선시술 (Interventional radiology of Bronchial artery embolization due to massive hemoptysis)

  • 호흡기 질환환자에서 비교적 흔한 증상인 객혈은 여러 치료법에도 불구하고 사망의 위험성이 높은 임상증상이다. 기관지색전술은 내외과적 치료에도 객혈이 멈추지 않을 경우, 즉각적인 지혈목적 외에도 수술이 불가능한 환자나 수술전 환자상태를 호전시키기 위한 시간을 얻기 위한 목적으로 매우 유용한 시술이다. 이와 관련하여 본 연구는 기관지동맥 색전술의 유용성에 대해서 알아보고자 한다. 대상은 J병원의 2007년 03월부터 2009년 12월까지 발생한 대량 객혈로 인한 기관지동맥 색전술을 시행한 60명으로 하였다. 이들은 하루 400ml이상, 한번에 200ml이상의 대량객혈이 있었거나 객혈양이 하루 400ml이하였지만 10일 이상의 내과적 치료에 반응하지 않았던 경우였다. 평균연령은 60.5세였으며, 원인질환별로는 결핵, 기관지확장증, 폐렴, 폐암 등이었다. 색전부위는 Rt bronchial artery 19예, Lt broncial artery, both bronchial artery, Rt Intercostobronchial artery, 여러 혈관을 동시에 시행한 경우였다. 색전물질은 PVA(conteour)와 microcoil을 이용하였다. 환자 60명중 76.6%에서는 시술 후 시간이 지남에 따라 출혈이 멈추었으며, 4예(6.6)에서는 시술 후 재출혈로 인한 재색전술을 시행하였다. 기관지 동맥 색전술은 객혈 치료에 있어서, 높은 조기 성공률과 객혈을 효과적으로 조절함으로, 대량 객혈로 인한 출혈에 효과적인 응급치료법으로 사망률을 줄이는 좋은 시술 방법이 될 것이다. 또한 일차적으로 내과적 치료와 같이 병행하여 시도한다면 대량 객혈로 인한 출혈에 효과적인 치료법으로 사망률을 줄이는 좋은 중재적 방사선 시술 방법이 될 것이다.

종합병원 혈관조영촬영유니트의 건축계획에 관한 연구 (A Study on the Architectural Planning of the Angiography Unit in General Hospital)

  • Angiography means that a check up to know an abnormal condition in all the blood vessels include from the heart, aortae, cerobrovascular and abdonominal artery to hands and feet. Main examples of this are cerebral angiography, abdominal, liver for urinary anomaly, renovascular angiography, and artery and vein in arms and legs. Angiography uses radial rays or angiography equipment for an image output during interventional procedure and compositive diagnosis. The acts which performed in a projection room have changed drastically. In general, it is performed by using equipment which is attached one or two C-arms and the method of inserting catheter in vein after anesthesia. For this reason, some rooms that consist of angiography room units should be planned not only for expensiveness equipment and facilities also to be germ-free. Nowadays, in the angiography unit case, it is placed independently as the central part of many hospitals. It does not belong to the imaging medical department any more as considering raising filming times and the relation between C.C.U.(coronary care unit) and operation unit. This means the acts performed are diversified and well-organized rooms in support of diagnosis are required. However, it is difficult to plan the angiography room unit due to domestic researches and data on this unit are not enough. Therefore, this study aims at bringing up basic issue for architectural planning of the angiography unit in general hospital.

경피적 최소침습 척추시술 중 디스크 내에서 방향제어가 가능한 Bio-SMA 캐뉼라 카테터의 개발 (Development of Controllable Cannular Catheter using Bio Shape Memory Alloy (SMA) during Percutaneous Minimally Invasive Spine Surgery)

  • As the Minimally Invasive Surgery (MIS) is developed, an interventional procedure becomes the major of the spine surgery in the world. Despite of the use of the expensive medical equipments, the success chance of the nucleoplasty is about 30%. The reason is that the shape of the cannular needle is similar to that of the conventional injector and looks like the straight. Because the tip of these straight needles is not able to reach in the vicinity of the disc bulging or the protrusion, which are the cause of the low back pain and because the far indirect plasma discharge results in the decompression, the nucleoplasty has the limit. Many incurable diseases has not been solved due to the unexistence of the advanced technique for the MIS human body cannula device. If 3-D direction controllable cannular catheter (whose direction is accurately controlled after inserting into the bodies to cure the lesion) is developed, it is expected that new devised cannular catheter can cure many incurable diseases simultaneously. Therefore, the aims of this research are to develop the new devised cannular catheter of SMA direction controller for the medical situation, which has been produced through many previous trial-error procedures, and to produce the commercial medical device.

Delayed Pneumocephalus Following Fluoroscopy Guided Cervical Interlaminar Epidural Steroid Injection : A Rare Complication and Anatomical Considerations

  • Cervical epidural steroid injection is indicated for radicular symptoms with or without axial neck pain. Complications are rare but can be serious. Here, we report the case of a 54-year-old man with cervical radicular pain who was treated with cervical epidural steroid injection. Injection was administered twice under fluoroscopic guidance with the loss-of-resistance technique using air to confirm the epidural space. After the second procedure, the patient complained of severe persistent headache and was diagnosed with pneumocephalus on brain computed tomography. The patient returned home without any neurological complication, after a few days of conservative treatment. Though, a fluoroscopic guidance cervical epidural injection is also known to diminish the risk of complications. Physicians should always keep in mind that it does not guarantee safety, particularly in the cervical region, related to its anatomical considerations.
