Identifying Latent Classes of Risk Factors for Coronary Artery Disease (잠재계층분석을 활용한 관상동맥질환 위험요인의 유형화)

  • Purpose: This study aimed to identify latent classes based on major modifiable risk factors for coronary artery disease. Methods: This was a secondary analysis using data from the electronic medical records of 2,022 patients, who were newly diagnosed with coronary artery disease at a university medical center, from January 2010 to December 2015. Data were analyzed using SPSS version 20.0 for descriptive analysis and Mplus version 7.4 for latent class analysis. Results: Four latent classes of risk factors for coronary artery disease were identified in the final model: 'smoking-drinking', 'high-risk for dyslipidemia', 'high-risk for metabolic syndrome', and 'high-risk for diabetes and malnutrition'. The likelihood of these latent classes varied significantly based on socio-demographic characteristics, including age, gender, educational level, and occupation. Conclusion: The results showed significant heterogeneity in the pattern of risk factors for coronary artery disease. These findings provide helpful data to develop intervention strategies for the effective prevention of coronary artery disease. Specific characteristics depending on the subpopulation should be considered during the development of interventions.

Factors Influencing Sleep Quality among Middle-aged Male Workers (중년남성 근로자의 수면의 질에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify quality of sleep and its influencing factor among middle-aged male workers. Methods: The subjects of this study were 411 middle-aged male workers living in Seoul and Gyeonggi-do. The data were based from self-reported using structured questionnaires asking about low urinary tract symptoms, circadian rhythm, quality of life and sleep quality including general characteristics. The data were collected from September 11 to October 31, 2013 and analyzed by t-test, one-way ANOVA, Pearson correlation coefficient and hierarchical regression. Results: Mean score of the sleep quality was $5.03{\pm}2.57$ (range: 0~21) and reported as bad sleep quality in 55.7%. Low urinary tract symptoms (${\beta}$=.30, p<.001), circadian rhythm (${\beta}$=-.17, p<.001), quality of life (${\beta}$=-.14, p<.001) were shown as significant association of sleep quality. Conclusion: Strategies to improve sleep quality in middle-aged male workers are necessary and nursing intervention should be developed to improve education program for prevention and management of low urinary tract symptoms. It is also necessary to legalize the institutional devices to upgrade work environment and to place regulations on overtime at work places in order for these workers to obtain sufficient sleep time for recovery of biological rhythms and improving sleep quality.

The Relation Between the Occupational Stress and Fatigue Level of Neonatal Nurses (신생아실 간호사의 직무 스트레스와 피로도와의 관계)

  • Purpose: The purpose of this study was to investigate occupational stress and fatigue level of neonatal nurses and to assess relationship between the two of them. Methods: Subjects were 109 neonatal nurses working at 2 university hospitals and 9 women's hospitals in D city, and the data were analyzed using descriptive statistics and binary logistic regression analysis by the SPSS/WIN 15.0. Results: The mean score of nurses' occupational stress level was 3.22, and the highest was 'night shift' among the 7 subcategories, and fatigue level was 62.0, and the highest was physical fatigue in configuration field. In logistic regression analyses, 'heavy workload' and 'inadequate physical environment' led to most occupational stress. Conclusion: In addition to significantly higher levels of occupational stress of neonatal nurses, the cumulative fatigue also resulted in decreased quality of nursing, job satisfaction and nursing productivity; therefore, appropriate number of nurses and improvement of work environment is necessary. Furthermore, to reduce the occupational stress and fatigue, we should try to develop adequate clinical guidelines and intervention strategies and apply them in neonatal care unit.

Predictors of Mammography Performance in Job Women (직장여성의 유방촬영술이행 예측요인)

  • Purpose: This study was to identify factors affecting mammography screening behavior in job women by attitude, social influence and self-efficacy model. Methods: The data were obtained from 171 job women in one residency area by structural questionnaire from March to June 2013 and analysed by using $x^2$-test, ANOVA, Spearman's correlation and logistic regression analysis. Results: The each performance rate was 45.1% in mammography, 44.9% in breast self examination and 48.5% in physical examination. The mammography performance rate in job women showed higher significance in the groups of 1) older age, urban residency, marital status or high economic state, 2) shorter office hours or higher job position, 3) childbirth experience or menopause and 4) preferring soy bean food, practicing regular exercise, suffering chronic disease or receiving radiation therapy. Attitude, social influences and self-efficacy made significant differences in mammography performance. Logistic regression analysis showed that 50 years or older, urban residency, social influences towards mammography and high self-efficacy were significant relationship. Conclusion: In order to increase the mammography performance rate, the intervention strategies are needed to increase positive social influences or self-efficacy and to offer public information to younger age.

A Grounded Approach to Bringing up experiences in Mothers who have the first baby (초산모의 양육경험에 대한 근거이론적 연구)

  • The purpose of this study was to explore bringing-up process of mothers with first baby in the context of Korean culture. The field survey was performed with four primiparae during four months from September, 2000 and grounded theory approach was used for data analysis. The results indicated that there were 27 primary categories and 12 secondary categories. Unclearness was extracted as an expression of the core category of bringing-up process. The subjects was faced with care of their baby after delivery and experienced formation and deepening of unclearness. After that, they also went through expression and disentanglement of complication caused by unclearness. It is found that the core concept of this study, the unclereness("Magmagham" in Korean) means a confusion, a burden, a helplessness and a hardness. New mothers make an effort to resolve this situation with strong motherhood in their mind. By supports from significant others, they are empowered and some mothers get depressive feelings. The findings of this study are contributed for nurses to understand new mothers in developmental crisis. We suggested that the nursing intervention strategies including learning baby care skills and preparing emotionally a motherhood must be given new mother during pregnancy period.

Burden and Quality of Life in Caregivers of Patients with Rare and Incurable Disease (재가 희귀.난치성질환 돌봄 제공자의 부담감과 삶의 질)

  • Purpose: The purpose of the study was to investigate burden and the qualify of life in caregivers who are taking care of home-based rare and incurable disease patients. Methods: The subjects of this study were 300 caregivers of rare and incurable disease patients registered at five health centers in Seoul. A survey was conducted by mail and visit in person during the period from the 25th of March to the 12th of May 2005. Collected data were analyzed through t-test, ANOVA, Pearson's correlation coefficient. Result: The mean burden of caregivers was 3.42, and the mean qualify of life of caregivers was 2.71. Burden and QoL showed significant differences according to caregivers' characteristics such as sender, age, relation to the patient, academic qualification, religion, occupation, monthly household income and perceived health condition. Caregivers' burden was in an inverse correlation with their quality of life. Conclusions: According to the results of this study, rare and incurable disease caregivers' burden and their quality of life were in a significant correlation with each other. In order to improve caregivers' quality of life by reducing their burden, we need to reestablish comprehensive policies for rare and incurable disease management including nursing intervention strategies for caregivers.

Factors Related to Regular Mammography Screening for Outpatients (일 지역 외래내원여성의 규칙적 유방촬영술 이행에 영향을 미치는 요인)

  • Purpose: This study was to identify factors affecting regular mammography screening behavior of outpatients. The target subjects were 150 women who had visited the breast clinic at the university hospital, and the study period was around 10 months from March to December 2006. Data were analyzed by using $\chi^2$-test, t-test and logistic regression analysis. Results: Of the subjects, 50% experienced mammography and 31.6% took mammography regularly. In the relational analysis between various factors and regular mammography, there were significant differences among the $41{\sim}50$ year old group, the employed group, the high income group, the regular exercise group, the non-drinking group, and the previously x-rayed group. And, in the Logistic Regression analysis, those who preferred bean food were 3.20 times more likely to take mammogram regularly, and those who married were 3.49 times more likely to do than the unmarried. Also, those who had low health belief and who were under age 51 were less likely to take regular mammogram. Conclusion: In order to increase the rate of conducting regular mammography, there must be different intervention strategies according to food habit, marital status and age, and a support system must be devised to increase health belief about breast cancer.

Compliance with a Low-Salt Diet, Sodium Intake, and Preferred Salty Taste in the Hypertensive Elderly (노인 고혈압 환자의 저염식이 이행, 나트륨 섭취 및 짠 맛에 대한 기호도)

  • Purpose: This study aimed to examine the compliance to a low-salt diet, sodium intake, and preferred salty taste in elderly patients with hypertension. Methods: A cross-sectional descriptive design was used. Participants were 105 elderly patients with hypertension living in a rural area. The compliance with a low-salt diet, sodium intake, and preferred salty taste, blood pressure, body mass index, and waist circumference were measured, and compared according to the general characteristics and the levels of blood pressure. Descriptive statistics, $X^2$-test, t-test, and ANOVA were used for data analysis. Results: compliance with a low-salt diet was marginally elevated. Sodium intake was relatively high and the main sources were seasonings and vegetables. The participants tended to prefer high levels of salt. Sodium intake was significantly higher the hypertensive individuals (stage I and II) compared to prehypertensive subjects on a normal maintenance diet. Sodium intake from vegetables was also significantly different between the two groups. Conclusion: Nursing intervention for hypertensive elderly patients should include strategies to decrease sodium intake.

  • Snoring and obstructive sleep apnea are the representative sleep disordered breathings, caused by the temporary and repetitive constriction or obstruction of upper airway during sleep. They present with excessively vibratory noise and repetitive cease of respiration. These disorders commonly result in sleep disturbance and the subsequent daytime sleepiness, chronic fatigue. Furthermore, they can cause the serious and extensive complications including increased risk of hypertension, cardiac arrhythmia, cardiovascular disease, cerebrovascular accident, neurocognitive disturbance, traffic and occupational accidents, type II diabetes, childhood growth interruption, awakening headache and finally, relatively increased mortality rate. Because appropriate therapeutic intervention is best way for patients to relieve their symptoms and prevent their possible complications, it is very important for dentists to recognize their own role and responsibility in diagnosis and treatment of these disorders. For this, the present article provides the understanding of the clinical features, possible complications, various treatment modalities, and suitable treatment strategies for snoring and obstructive sleep apnea.

A Study on Traits and Relation Type of Old and New with Design strategy of Conversion Design (신.구요소의 관계유형 분석에 의한 컨버전디자인의 전략과 그 특성)

  • Regeneration of old buildings is one of the most imperative responses to the 21th century issue of sustainable urban architectural environment. In particular, creative circulation of generation which conversion means highlights the conversion as a new design method. In this study, by examining buildings built over the last decade, the mutual relationship between old elements of existing buildings and new elements of newly required functions was analyzed and types of relationships such as 'alien insertion', 'mediate intervention', 'endogenous expansional enveloping', 'contrastive confliction' and 'over-layering' were defined. In addition, design strategies including the paradoxical curio, fore-grounding and unification were derived from each relationship. Finally, according to the relation type and design strategy, this study suggests several features of conversion design; (1) maximization of symbolism using unfamiliarities - emphasizing futuristic or contextual symbol with intentional contrasts by preserving the work or using contemporary addition; (2) time confusion using neutral space - meeting the need by harmonizing and cooperating existing elements and new elements; (3) mutual emphasizing by individualization - revealing differences between old and new for an aesthetical tension; (4) synchronism by constructional combination - making the one unity of different layers; (5) expansion by enveloping - experience of visual reverse and creation of extra space by roofing above building groups. The reconversion should be continuously researched in the future since conversion is a sustainable design method for the future of architecture.
