Aneurysmectomy and graft interposition for giant thrombosed proximal internal carotid artery aneurysm: Technical details

  • A giant thrombosed extracranial internal carotid artery aneurysm (ECCA) is extremely rare and its treatment is challenging. Despite the advance of endovascular technique, open surgery is still considered a first-line treatment in giant thrombosed ECCA. We describe a case of giant thrombosed ECCA which was successfully treated by aneurysmectomy and graft interposition with the technical details.

The Use of Arteriovenous Bundle Interposition Grafts in Microsurgical Reconstruction: A Systematic Review of the Literature

  • Microvascular reconstruction frequently requires anastomosis outside of the zone of injury for successful reconstruction. Multiple options exist for pedicle lengthening including vein grafts, arteriovenous loops, and arteriovenous bundle interposition grafts. The authors performed a systematic review of arteriovenous bundle interposition grafts to elucidate indications and outcomes of arteriovenous grafts in microvascular reconstruction. A systematic review of the literature was performed using targeted keywords. Data extraction was performed by two independent authors, and descriptive statistics were used to analyze pooled data. Forty-four patients underwent pedicle lengthening with an arteriovenous graft from the descending branch of the lateral circumflex femoral artery. Most common indications for flap reconstruction were malignancy (n = 12), trauma (n = 7), and diabetic ulceration (n = 4). The most commonly used free flap was the anterolateral thigh flap (n = 18). There were five complications, with one resulting in flap loss. Arteriovenous bundle interposition grafts are a viable option for pedicle lengthening when free flap distant anastomosis is required. The descending branch of the lateral circumflex femoral artery may be used for a variety of defects and can be used in conjunction with fasciocutaneous, osteocutaneous, muscle, and chimeric free flaps.

유리공장이식편을 이용한 합병된 부식성 식도협착의 재건술 (Jejunal Free Graft for Complicated Corrosive Esophageal Stricture)

  • 부식성 식도협착의 재건수술시 대장의 혈액순환이 나쁜 경우 또는 대장치환술 후 허혈성 부전이 나타난 경우 가장 적합한 재건장기로는 유리공장이식편을 들 수 있다. 복잡하게 합병된 부식성식도협착 환자에서 유리공장이식편을 이용하여 식도재건술을 시행한 4예를 보고하고자 한다. 일차식도재건술로서 4예 중 3예는 대장치환술을, 1예는 장천공에 의한 복막염으로 Whipple's 술식 및 공장루설치술을 받았다. 1예는 인두대장문합부의에 유리공장첩포 이식을, 2예는 대장이식편 부전 부위에 유리공장이식편 치환을 받았다. 1예는 대장의 일부와 유리공장이식편을 이용하여 인두공장대장공장문합술을 시행하였다. 수술 후 문합부위 누출로 인한 재수술이 1예 있었다. 장기추적 관찰상 모두 연하곤란이 없었다. 여러 가지 원인으로 인하여 합병된 식도재건술이 필요하여 유리공장이식편을 사용했던 경우 만족할 술 후 이환율과 연하기능을 보였다.

제1형 급성 대동맥 박리증의 상행 대동맥 치환술 후 하행 대동맥에 발생한 대동맥-기관지루 (Descending Aorto-bronchial Fistula after Ascending Aorta Graft Interposition for Type I Acute Aortic Dissection)

  • 6개월전 제1형 급성 대동맥 박리증으로 상행대동맥 치환술을 받은 59세 여자환자가 대량 객혈을 주소로 내원하였다. 하행 흉부대동맥과 좌하엽간의 누공이 의심되어 응급수술을 시행하였다. 체외순환하에 인공도관을 이용하여 하행대동맥 치환술을 시행하였으며 수술 후 별다른 합병증 없이 회복되었다. 대동맥 수술 후 원위부에 생기는 대동맥-기관지루는 매우 드문 합병증으로 조기진단 및 수술로 좋은 결과를 얻었기에 보고하는 바이다.

경부 식도협착 재건술에 있어서 유리공장 이식편의 이용 (Reconstruction of the Cervical Esophagus Using the Free Jejunal Graft)

  • The cervical esophageal stricture has various surgical modalities and difficulties in reconstruction. We had experienced a case of successful reconstruction of the cervical esophageal restenosis using the free jejunal graft, on 30 year old man had had esophageal stricture after ingestion of lye. He had undergone colon interposition[esophagocologastrostomy] with left colon feeding gastrostomy. But restenosis was occurred just above of the cervical esophagocolostomy site several times of balloon dilatation were failed. So, we decided to use of the free jejunal graft. The free jejunal graft was isolated about 15cm length with it`s vascular arcades. The graft was irrigated with the mixed solution as isotonic saline, heparin and papaverine chloride. The artery of graft was anastomosed to the branch of the external carotid artery in end to side with continuous sutures of the 8.0 Prolene. The vein of the graft was anastomosed to the branch of the anterior facial vein in end to end with continuous sutures of the 8.0 prolene. Postoperative course was uneventful and the patient was discharged after removal of the tracheostomy cannula and gastrostomy tube.

Nerve Cable Graft Interposition in Patients with Brachial Plexus Schwannoma: Case Reports

  • Schwannomas are rare benign tumors that develop in Schwann cells lining peripheral nerves. Schwannomas of the brachial plexus are especially rare, accounting for 5% of all cases. Although several treatments can be considered, the exact method of treatment is unclear owing to the scarcity and sporadic occurrence of schwannomas. Tumor resection is performed in most cases, and nerve damage is inevitable in cases of neuroinvasive schwannoma. In this case series, we present our successful use of transposition of cable-grafted nerves for the treatment of schwannomas. We performed cable-grafted nerve interposition in addition to tumor resection, leading to increased recovery of nerve damage. To relieve postoperative symptoms and minimize sequelae, precise surgical tumor resection followed by nerve interposition using a cable-grafted nerve may be recommended.

Scalp Free Flap Reconstruction Using Anterolateral Thigh Flap Pedicle for Interposition Artery and Vein Grafts

  • We experienced satisfactory outcomes by synchronously transplanting an artery and vein using an anterolateral thigh flap pedicle between the vascular pedicle and recipient vessel of a flap for scalp reconstruction. A 45-year-old man developed a subdural hemorrhage due to a fall injury. In this patient, the right temporal cranium was missing and the patient had $4{\times}3cm$ and $6{\times}5cm$ scalp defects. We planned a scalp reconstruction using a latissimus dorsi free flap. Intraoperatively, there was a severe injury to the right superficial temporal vessel because of previous neurosurgical operations. A 15 cm long pedicle defect was needed to reach the recipient facial vessels. For the vascular graft, the descending branch of the lateral circumflex femoral artery and two venae comitantes were harvested. The flap survived well and the skin graft was successful with no notable complications. When an interposition graft is needed in the reconstruction of the head and neck region for which mobility is mandatory to a greater extent, a sufficient length of graft from an anterolateral flap pedicle could easily be harvested. Thus, this could contribute to not only resolving the disadvantages of a venous graft but also to successfully performing a vascular anastomosis.

상장간막동맥류의 수술적 치료 -1례 보고- (Superior Mesentic Artery Aneurysm -A Case Report-)

  • Superior mesenteric artery aneurysm is the third most common lesion and comprises approximately 5.5 % of all visceral artery aneurysms. The first successful repair was performed by DeBakey and Cooley in 1949. Since then, more than 100 cases have been reported. Fifty to sixty percent of these aneurysms are mycotic in origin. Other less frequent causes include arteriosclerosis, trauma, and medial degeneration. The operations are bypass with autologous tissue or with artificial vascular graft and aneurysmorrhaphy. We have experienced a case of superior mesenteric artery aneurysm which had undergone aneurysmectomy and artificial graft interposition. This is the first domestic case which was successful surgical repaired.

대동맥류의 수술적 치료에 관한 임상적 연구 (Clinical Study of Surgical Treatment of Aortic Aneurysm)

  • 인제대학교 부속 부산백병 원 흉부외과학교실에서는 지난 87년 10월부터 96년 1월까지 총25례의 흉부 및 복부 대동맥류를 수술로 치험하였다. 환자의 연령은 최소 26세에서 최고 73세(평균연령 52. 5세)였으며 남자가 13명 여자가 12명이였다. 발생 부위에 따라흉부 대동맥류가 18례 복부 대동맥류가7례였다. 원인에 따른 분류에서 16례가 박리성이 였으며 9례가 비박리성 대동맥류였다. 동맥류의 위험 인자로는 고혈압이 가장 많았으며 기타 고지질증, Marfan증후군등이였다. 흉부 대동맥류는 침범 부위에 따라 동맥류 절제술과 인조혈관 대치술 및 관상동맥 이식술등을 증례에 따라적절히 시행하였다. 복부 대동맥류는 동맥류 절제후 침범 부위에 따라 일자 또는 역Y형 인조혈관 을 대 치시켰다. 총 5례의 술후 사망이 있어서 수술 사망률은 20%였으며 생존 환자는 특별한 합병증 없이 퇴원하였다.

경부식도 재건을 위한 유리 공장 이식술 (Free jejunal graft for cervical esophageal reconstruction)

  • Since Carrel in 1907 reported transfer of a free jejunal segment in dog, reconstruction of the cervical esophagus and hypopharynx has been accomplished successfully with free jejunal transplantation using microvascular technique. Free jejunal graft is useful in cases of failed colonic interpositions. Three patients with benign esophageal stricture had undergone reconstruction with right colon interposition. Because of necrosis and stricture of the interposed colon, in each case the defect was reconstructed with a free jejunal graft by using microvascular technique. The postoperative course in two patients was uncomplicated, and they were able to eat general diet. Graft necrosis occurred in one patient, but she is waiting for a reoperation.
