Pulmonary Thromboembolism after Post-menopause Hormonal Replacement Therapy (폐경기 호르몬 대체요법 후 발생한 폐 색전증)

  • Hormonal replacement therapy (HRT) has been proven for treatment of postmenopausal symptoms such as hot flushes, night sweats and urologic symptoms. HRT became very popular in the 1990's, when there were several reports showing that it also helped with other menopausal complications such as osteoporosis and cardiovascular disease. Recent studies report that the incidence of breast cancer, endometrial cancer, cerebral infarction, coronary artery diseases, deep vein thrombosis and pulmonary thrombembolism could rise after HRT. Among these side effects of HRT, the risk of pulmonary thromboembolism increases 2 to 4 fold after HRT, but can vary with the use of different doses and preparations. Here, we summarize the risk factors and clinical courses for 5 patients who developed pulmonary thromboembolism after postmenopausal HRT.

The Experimental study of Persicae Semen and Caesalpiniae Lignum on the endotoxin induced thrombosis (도인(桃仁), 소목(蘇木)이 Endotoxin으로 유발(誘發)된 어혈병태(瘀血病態)모델에 미치는 영향(影響))

  • Objective : This experimental study was carried out to investigate the effects of Persicae Semen(PS) and Caesalpiniae Lignum(CL) on the intravascular coagulation which is considered as thrombosis. Methods : Rats were pretreated orally with PS and CL extracts(PS: 8.45mg/100g, CL: 4.4mg/100g), and 1 hour later, to induce thrombosis, were given an endotoxin(0.01mg/100g) injection into the caudal vein. After 4hours we collected blood by cardiac puncture and measured the platelet count, the prothrombin time, the level of fibrinogen and the FDP(fibrinogen degradation product). Results : PS and CL supressed the decrease of the Platelet count and the prolongation of prothrombin time. In these factors, both groups showed significant effect. Both groups supressed the decrease of the fibrinogen level and the increase of the FDP level. But, only the PS group showed a significant effect on the fibrinogen level, and only the Cle CL group showed a significant effect on FDP level. Conclusions : PS had significant effects on the platelet count, the prothrombin time, and the FDP level. CL had significant effects on the platelet count, the prothrombin time, and the fibrinogen level. Therefore, PerSicae Semen and Caesalpiniae Lignum seem to be applicable to treating the diseases related to thrombosis.

Korean medicine treatment including oral administration of Gyejibongnyeong-hwan and acupuncture therapy for calf edema and pain due to deep vein thrombosis of lower leg: A Case Report (하지 심부정맥혈전증으로 인한 부종 및 통증에 계지복령환 투약과 침 요법을 포함한 한의 치료를 시행한 증례 1례 보고)

  • Objectives: This study was aimed to report changes in clinical symptoms and signs after treatment with Korean medicine in patients who suffered from calf edema due to deep vein thrombosis (DVT). Methods: A 46-year-old male patient, who discharged home after receiving standardized treatment for acute DVT in the right leg, suffered pain and heat sense due to edema of the affected calf. Oral administration of herbal medicine (Gyejibongnyeong-hwan 4 g, twice daily) and acupuncture therapy were given to him for 6 weeks. The change in chief complaints, Villalta score, the right and left side difference of the circumference and the skin surface temperature of both calves, and blood level of D-dimer were observed before and after treatment. Results: The patient's chief complaints began to significantly improve from 2 weeks after treatment, and disappeared completely within 4 weeks. At the end of the treatment, a substantial decrease in the difference between the right and left calves in the circumference and skin surface temperature was observed. This effect was maintained even at the time of follow-up 3 months after the end of treatment, and the patient completely recovered indoor and outdoor life to the level before onset. Conclusions: This case suggests that Korean medicine treatment, including Gyejibongnyeong-hwan administration and acupuncture therapy, can be a viable option to improve edema and related clinical problems in the affected limbs due to DVT.

  • Radionuclide venographies were performed in 138 limbs of 58 patients and X-ray contrast venographies were performed in 23 paitents of them. Positive radionuclide venography findings were area of decreased sponding to the region of thrombosis, abnormal collateral flows below the lesion. The success rate of radionulide venography was 89% and the overall concordance between radionuclide venography and X-ray contrast venography was 91%. Radionuclide venography is simple and easy to perform and less invasive than X-ray contrast venography. These results indicate that radionuclide venography is the most ideal screening test for the detection of venous thrombosis in patient with signs & symptoms of deep vein thrombosis.

  • Lemierre syndrome is caused by acute oropharyngeal infections with secondary septic thrombophlebitis of the internal jugular vein and is characterized by frequent metastatic infections. A 56-year-old man presented with severe reddish inflammatory swelling of the right cervical soft tissue. Thrombophlebitis in the right internal jugular vein and multiple pulmonary embolisms were identified on neck and chest computed tomography (CT). He was treated with antibiotics and heparin for 4 weeks and then discharged without other complications.

  • Thoracic endovascular aortic repair has become a widespread alternative treatment option for thoracic aortic aneurysm. The debranching of arch vessels may be required to provide an acceptable landing zone for an endovascular stent graft. We report a case where the bypass graft used in the thoracic endovascular aortic repair procedure compressed the left internal jugular vein, causing acute thrombotic occlusion.

  • May-Thurner syndrome is a deep vein thrombosis of the ilio-femoral vein due to compression of the left common iliac vein by the overlying right common iliac artery. Although, catheter directed thrombectomy (CDT) and thrombolysis with stent insertion has become the standard treatment method for acute or subacute May-Thurner syndrome, because of technical feasibility and lower recurrence rate, however, sometimes this methods make fatal complications. Furthermore, there are few reports on optimal treatment strategies for patients in a chronic state of May-Thurner syndrome. We now present two cases of chronic (> 1 month since onset of symptoms) May-Thurner syndrome treated by surgical thrombectomy and femoral arteriovenous shunt with simultaneous stent insertion after failed endovascular treatment. This technique may provide a significant benefit for patients who are not suitable for conventional endovascular treatment.

  • The incidence of pulmonary thromboembolism increases with age. The risk factors of pulmonary thromboembolism include surgery, malignancy, obesity, lupus anticoagulants, and vascular conditions such as deep vein thrombosis. Thromboembolism in younger patients or in unusual locations, the possibility of primary thrombophilic conditions should be considered. Primary thrombophilic states include myeloproliferative disorders (MPD). JAK2 V617F mutation is found commonly in patients diagnosed with MPD, in 90~95% of polycythemia vera (PV) and in 50~60% of essential thrombocytosis (ET) patients. Sometimes the JAK2 V617F mutation is found in cases without MPD. The relationship between JAK2 V617F mutation and thrombosis has not been defined. Recently, clinical evidence suggests that this mutation may be variably associated with thrombosis. We present one case of pulmonary thromboembolism in a young patient, who was positive for the JAK2 V617F mutation and did not have MPD.

  • Pulmonary embolism (PE), which can originate as a consequence of deep vein thrombosis (DVT), is the most frequent and potentially fatal venous thromboembolic event. Despite the fact that the incidence of venous thromboembolism (VTE) in Asians is lower than that in the Western populations, a recent epidemiologic study demonstrates an increasing incidence of VTE in the Korean population. Anticoagulants, including low molecular weight heparin (LMWH) and vitamin K antagonist (VKAs), have been the main treatments for PE, however, recently new oral anticoagulants (NOACs) were introduced. We will review how well patients with PE can be managed with the existing anticoagulants and NOACs along with the time span of treatment, which still pose some challenges for clinicians.

  • A thromboembolic event in patients later given a diagnosis of cancer is the result rather than the cause of the cancer. The risk of hidden cancer is significantly higher for patients with recurrent idiopathic thromboembolism compared to those with secondary deep vein thrombosis. Microemboli from hepatic or adrenal metastases and large-sized emboli from the great veins invaded by the tumor are the sources of tumor embolization The intraarterial tumor emboli less likely invade the arterial wall. Thrombus formation and organization may be capable of destroying tumor cells within pulmorlary blood vessels. Therefore, all tumor emboli are not true metastases. The treatment of deep vein thrombosis and pulmonary embolism in patients with cancer consists of anticoagulation with heparin and warfarin, venacaval filters, appropriate anti-neoplastic agents, and surgical methods(embolectomy, thromboendarterectomy). However, considerable literatures suggest that oral anticoagulant such as warfarin is ineffective in the treatment of those. We report a case of primary unknown squamous cell carcinoma incidentally found in the thrombus after pulmonary embolectomy.
