Prognostic Factors and Survival in Acute Myeloid Leukemia Cases: a Report from the Northeast of Iran

  • Background: Acute myeloid leukemia (AML) is a clonal hematopoietic disorder resulting from genetic alterations in normal hematopoietic stem cells. The aim of this study was to evaluate prognostic factors and survival of AML patients in the Northeast of Iran. Materials and Methods: This retrospective study covered 96 patients with AML referred to Emam Reza Hospital, Mashhad city, Iran, from 2009 to 2015. Age, sex, blood group, type of AML, fever, consumption of amphotericin B, cytogenetic forms and survival were analyzed. Also, WBC, hemoglobin and platelet levels were checked. Mean follow-up was 30.5 months (60.4% mortality). Survival was plotted by GraphPad Prism 5 with Log-rank test. Results: The mean age for all AML patients at diagnosis was 40.4 years (range, 17-77 years). Some 42.7% patients were aged <35 years and 40.6% were male. In all patients, 76% had fever and 50% consumed amphotericin. T(15;17)(q22;q21) had the most prevalence (37.7%) compared to other forms. Out of 92 patients, O+(30.4%) was the most common blood group and AML-M5 (28.3%) the most common subtype. There was a significant difference in survival based on WBC and consumption of amphotericin B (P<0.05). Conclusions: WBC level, fever and consumption of amphotericin B proved to be factors for survival of AML patients. The mean age for patients in Iran is lower than other areas in the World and also survival in this study was higher than in other studies.

A Form Clustering Algorithm for Web-based Application Reengineering (웹 응용 재구성을 위한 폼 클러스터링 알고리즘)

  • A web-based information system, that is a dominant type of information systems, suffers from the "web crisis" in development and maintenance of the system. To cope with the problem, a technology of software clustering to web-based application, which is one of web engineering, is strongly needed. In this paper, we propose a Form Clustering Algorithm along with an application example, which are used for internal-system reengineering to web-based information system. A Form Clustering Algorithm focuses on Page-model which is the feature of the web among the various web-based information system's structural model. Specially, we applying distance matrix to navigation model of graph form for easily analyzing, and web log analysis for identifying core function object that have a highly loading. Also, we create web software structure that can be used to maximize reusability and assign hardware effectively through 2-phase clustering step. Form Clustering Algorithm might be used at web-based information system development and maintenance for reusable web component development and hardware assignment, respectively.

Anomaly Detection Analysis using Repository based on Inverted Index (역방향 인덱스 기반의 저장소를 이용한 이상 탐지 분석)

  • With the emergence of the new service industry due to the development of information and communication technology, cyber space risks such as personal information infringement and industrial confidentiality leakage have diversified, and the security problem has emerged as a critical issue. In this paper, we propose a behavior-based anomaly detection method that is suitable for real-time and large-volume data analysis technology. We show that the proposed detection method is superior to existing signature security countermeasures that are based on large-capacity user log data according to in-company personal information abuse and internal information leakage. As the proposed behavior-based anomaly detection method requires a technique for processing large amounts of data, a real-time search engine is used, called Elasticsearch, which is based on an inverted index. In addition, statistical based frequency analysis and preprocessing were performed for data analysis, and the DBSCAN algorithm, which is a density based clustering method, was applied to classify abnormal data with an example for easy analysis through visualization. Unlike the existing anomaly detection system, the proposed behavior-based anomaly detection technique is promising as it enables anomaly detection analysis without the need to set the threshold value separately, and was proposed from a statistical perspective.

Bioequivalence of Ramiprin® tablet to Tritace Protect® tablet (Ramipril 10 mg) (트리테이스프로텍트 ®정(라미프릴 10mg)에 대한 라미프린 ®정의 생물학적동등성)

  • To evaluate the bioequivalence of two ramipril formulations, a standard 2-way randomized cross-over study was conducted in twenty-six healthy male Korean volunteers. A single oral dose of 10 mg of test formulation $Ramiprin^{(R)}$ (tablet) or reference formulation Tritace $Protect^{(R)}$ (tablet) was administered with one-week washout period. Plasma concentrations of ramipril were assayed over a period of 12 hr with a well validated method using liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS). The values of area under the plasma concentration-time curve, from time zero to last sampling time $(AUC_t)$ and from time zero to time infinity $(AUC_{inf})$ were $77.45{\pm}44.78\;and\;78.96{\pm}45.64$ for test, and $70.30{\pm}42.27\;and\;71.99{\pm}43.55ng\;hr/mL$ for reference formulation, respectively. Similarly, maximum concentration $(C_{max})$ and elimination half-life $(t_{1/2})$ were $65.61{\pm}19.96ng/mL$ and $2.15{\pm}0.75hr$ for test, and $63.63{\pm}25.50ng/mL$ and $2.16{\pm}0.73hr$ for reference formulations, respectively. Time to reach maximum concentration $(T_{max})$ for the test and the reference, were $0.51{\pm}0.22hr\;and\;0.52{\pm}0.18hr$, respectively. The parametric 90% confidence intervals on the mean of the differences between the two formulations (test-reference) of the log-transformed values of $AUC_t\;and\;C_{max}$ were 1.03 to 1.19 and 0.98 to 1.17, respectively. The overall results indicate that the two formulations are bioequivalent and can be prescribed interchangeably.

Chemoradiotherapy in squamous cell carcinoma of the anal canal: a single institution experience

  • Purpose: We reviewed the treatment outcomes and prognostic factors for patients with anal canal carcinoma who were treated with curative intent chemoradiotherapy (CRT) at Severance Hospital from 2005 to 2011. Materials and Methods: Data for 38 eligible patients treated during this period were reviewed. All patients were treated with curative intent using radiotherapy (RT) with (n = 35) or without concomitant chemotherapy (n = 3). Among 35 patients who received CRT, most of the chemotherapeutic regimens were either 5-fluorouracil (5-FU) plus mitomycin C (23 patients) or 5-FU plus cisplatin (10 patients). Recurrence-free survival (RFS), colostomy-free survival (CFS), overall survival (OS), and locoregional control (LRC) rates were calculated using the Kaplan-Meier method and survival between subgroups were compared using the log-rank test. Cox's proportional hazard model was used for multivariate analysis. Results: Over a median follow-up period of 44 months (range, 11 to 96 months), 3-year RFS, CFS, OS, and LRC were 80%, 79%, 85%, and 92%, respectively. In multivariate analysis, tumor size >4 cm was an independent predicting factor for poorer RFS (hazard ratio [HR], 6.35; 95% confidence interval [CI], 1.42 to 28.5; p = 0.006) and CFS (HR, 6.25; 95% CI, 1.39-28.0; p = 0.017), while the presence of external iliac lymph node metastasis was an independent prognosticator for poorer OS (HR, 9.32; 95% CI, 1.24 to 70.3; p = 0.030). No treatment-related colostomies or deaths occurred during or after treatment. Conclusion: Curative intent CRT resulted in excellent outcomes that were comparable to outcomes in previous randomized trials. No severe treatment-related toxicities were observed.

  • The internal regions of nuclear small subunit rRNA from 6 plaeurotus species and 5 Pleurotus ostreatus strains were amplified by PCR and sequenced. The DNA sequences of 8 Pleurotus strains (P. ostreatus NFFA2, NFFA4501, NFFA4001, KFFA4001, KFCC11635, P florida, P. florida, P. sajor-cuju, P. pulmonarius, and P. spodoleucus) were idential, but P. cornucopiae differed from them in two bases out of 605 bases. However, p[hylogenetic analysis of the sequences by DNA-distance matrix and UPGMA methods showed that P. ostreatus NFFA2m1 and NFFA2m2, known as mutants of P. ostreatus NFFA2, belonged to anther group of Basidiomycotina, which is close to the genus Auricularia. The difference of the SSU rDNA sequences of P. cornucopiae from other Pleurotus species tested corresponds to the difference of mitochondrial plasmid type present in Pleurotus species as observed by Kim et al. (1993, Korean J. Microbiol. 31, 141-147).ishement of silencing at the HMR/hsp82 locus can occur in G1-arrested cells. Cell cycle arrest at G1 phase was achieved by treatment of early log a cell cultures with .alpha.-factor mating pheromone, which induces G1 arrest. The result suggests that passage through S phase (and therefore DNA replication) is nor required for re-establishing silencer-mediated repression at the HMNRa/HSP82 locus. Finally, to test whether de nono protein synthesis is required for re-establishment of silencer-mediated repression, cells were pretreated with cycloheximide (500 /.mu.g/ml) 120 min. It was apparent that inhibiting protein synthesis delays, but does not prevent, re-establishment of silencer-mediated repression. Altogether, these results indicate that re-establishment of silencer-mediated repression is not dependent on the DNA replication and has no requirement for protein synthesis.

Flow Stress Properties of Electric Resistance Welded Small-Sized Subsea Pipeline Subjected to Temperature and Strain Rate Variations (심해저용 전기 저항 용접 소구경 송유관 소재의 온도 및 변형률 속도 에 따른 유동 응력 특성)

  • A subsea pipeline for oil/gas transportation or gas injection is subjected to extreme variations in internal pressure and temperature, which can involve a strain rate effect on the pipeline material. This paper describes the flow stress characteristics of a pipeline material called API 5L X52N PSL2, using and experimental approach. High-speed tensile tests were carried out for two metal samples taken from the base and weld parts. The target temperature was 100℃, but two other temperature levels of –20℃and 0℃ were taken into account. Three strain rates were also considered for each temperature level: quasi static, 1/s, and 10/s. Flow stress data were proposed for each temperature level according to these strain rates. The dynamic hardening behaviors of the base and weld metals appeared to be nonlinear on the log-scale strain rate axis. A very high material constant value was required for the Cowper-Symonds constitutive equation to support the experimental results.

Binary Search on Tree Levels for IP Address Lookup (IP 주소 검색을 위한 트리 레벨을 사용한 이진 검색 구조)

  • Address lookup is an essential function in the Internet routers, and it determines overall router performance. In this paper, we have thoroughly investigated the binary-search-based address lookup algorithms and proposed a new algorithm based on binary search on prefix lengths. Most of the existing binary search schemes perform binary search on prefix values, and hence the lookup speed is proportional to the length of prefixes or the log function of the number of prefixes. The previous algorithm based on binary search on prefix lengths has superior lookup performance than others. However, the algorithm requires very complicated pre-computation of markers and best matching prefixes in internal nodes since naive binary search is not possible in their scheme. This complicated pre-computation makes the composition of the routing table and incremental update very difficult. By using leaf-pushing, the proposed algorithm in this paper removes the complicated pre-computation of the Previous work in performing the binary search on prefix lengths. The performance evaluation results show that the proposed scheme has very good performance in lookup speed compared with previous works.

A Study of Acquisition and Analysis on the Bios Firmware Image File in the Digital Forensics (디지털 포렌식 관점에서 BIOS 펌웨어 이미지 파일 수집 및 분석에 관한 연구)

  • Recently leakages of confidential information and internal date have been steadily increasing by using booting technique on portable OS such as Windows PE stored in portable storage devices (USB or CD/DVD etc). This method allows to bypass security software such as USB security or media control solution installed in the target PC, to extract data or insert malicious code by mounting the PC's storage devices after booting up the portable OS. Also this booting method doesn't record a log file such as traces of removable storage devices. Thus it is difficult to identify whether the data are leaked and use trace-back technique. In this paper is to propose method to help facilitate the process of digital forensic investigation or audit of a company by collecting and analyzing BIOS firmware images that record data relating to BIOS settings in flash memory and finding traces of portable storage devices that can be regarded as abnormal events.

Estimating the Survival of Patients With Lung Cancer: What Is the Best Statistical Model?

  • Objectives: Investigating the survival of patients with cancer is vitally necessary for controlling the disease and for assessing treatment methods. This study aimed to compare various statistical models of survival and to determine the survival rate and its related factors among patients suffering from lung cancer. Methods: In this retrospective cohort, the cumulative survival rate, median survival time, and factors associated with the survival of lung cancer patients were estimated using Cox, Weibull, exponential, and Gompertz regression models. Kaplan-Meier tables and the log-rank test were also used to analyze the survival of patients in different subgroups. Results: Of 102 patients with lung cancer, 74.5% were male. During the follow-up period, 80.4% died. The incidence rate of death among patients was estimated as 3.9 (95% confidence [CI], 3.1 to 4.8) per 100 person-months. The 5-year survival rate for all patients, males, females, patients with non-small cell lung carcinoma (NSCLC), and patients with small cell lung carcinoma (SCLC) was 17%, 13%, 29%, 21%, and 0%, respectively. The median survival time for all patients, males, females, those with NSCLC, and those with SCLC was 12.7 months, 12.0 months, 16.0 months, 16.0 months, and 6.0 months, respectively. Multivariate analyses indicated that the hazard ratios (95% CIs) for male sex, age, and SCLC were 0.56 (0.33 to 0.93), 1.03 (1.01 to 1.05), and 2.91 (1.71 to 4.95), respectively. Conclusions: Our results showed that the exponential model was the most precise. This model identified age, sex, and type of cancer as factors that predicted survival in patients with lung cancer.
