Burnt-out Metastatic Prostate Cancer

  • A burnt-out prostate cancer tumor is a very rare clinical entity. The term 'burnt-out' refers to a primary tumor that has spontaneously and nearly completely regressed without treatment. Since metastasis of prostate cancer is usually encountered in the presence of advanced disease, distant metastasis with an undetectable primary tumor is very rare. We report herein a case of a burnt-out prostate cancer tumor that metastasized to the thoracic (T) spine and caused cord compression. A 66-year-old man visited the Emergency Department due to weakness of both legs for the past two days. His blood and urine tests were normal at the time. His spine magnetic resonance imaging (MRI) scans looked like bone metastasis that involved the T-7 vertebral body and a posterior element, and caused spinal cord compression. Other images, including from the brain MRI, neck/chest/abdomino-pelvic computed tomography (CT) scan and 18F-fluorodeoxyglucose (FDG)-positron emission tomography (PET) and endoscopy, revealed no lesions that suggested malignancy. After total corpectomy T-7 and screw fixation/fusion at T5 to T10, the pathology report revealed a metastatic carcinoma that was strongly positive for prostate-specific antigen (PSA). The serum PSA value was 1.5 ng/mL. The transrectal 12-core prostate biopsy and ultrasonography showed no definitive hypoechoic lesion, but one specimen had slight (only 1%) adenocarcinoma with a Gleason score of 6 (3+3). The final diagnosis was burned-out prostate cancer with an initial normal PSA value. Although metastatic disease with an unknown primary origin was confirmed, a more aggressive approach in seeking the primary origin could provide a more specific treatment strategy and greater clinical benefit to patients.

  • In Korea, chemical fertilizers and agricultural chemicals have had a significant impact on food production and these are today thought to be an indispensable part of modern agriculture. On the other hand, socioeconomical and IMF and WTO status in Korea are constant reminders of the vulnerability of our fossil fuel dependent agriculture. A new crop production system to improve economic, environmental and production efficiency must be exploited. Our particular concern has been given to an integrated cropping system involving recycling of crop residues, soil and N management, biological N fixation and multipurpose legumes. As a new integrated crop production system, a no-till direct-sown rice-vetch relaying cropping system has been proposed in this paper. The formulation of this system is based on the conception that N fertilizers being neither limitlessly available nor affordable, the current high-input crop production systems have produced troubled results severe labour shortage ill rural areas, balance of payment, environmental degradation and reduction of human health far exceed economic concerns. A natural and logical consequence is that long-term sustainability of agricultural systems must rely on the use and effective management of internal resources. Based on the information obtained throughout a series of experiments last years we have proved that the no-till direct-sown rice-vetch relaying cropping system dictates biological alternative which can augment, and in some cases replace, N fertilizers. Comprehensive discussions were made for the proposed system and it concluded that the system can offer an economically attractive and ecologically sound means of reducing external nitrogen input and improving the quality and quantity of internal resources, and consequently improving the farmers as well as the national returns.

  • Recently, we have used internal fixation with titanium miniplate which introduced by Champy in 1976 in facial bone fractures. In cases of mandibular fractures, particulary mandibular angle fractures, we usually underwent the open reduction extraorally. But extraoral approaches may leave scars and put the facial nerve at greater risk. So, we had undergone the intraoral approaches in 31 patients of mandibular angle fractures (male : 29, female : 2). These patients visited in Wonkwang university hospital from October 1991 to June 1993. We got easy operation time as average 20 minutes than extraoral approach. And reduced the postoperative nerve injury(14.2%), but postoperative infection is 6.1% nearly as same as extraoral approach. TMJ problems after intraoral open reduction was similar to extraoral approaches.

  • Fractures of the mandible occur with a greater frequency in the elderly. This study reports three cases of edentulous atrophic mandible fracture in elderly patients treated with open reduction technique. Three patients who presented with edentulous atrophic mandible fractures underwent surgical management using open reduction and internal fixation. After treatment, clinical evaluations and postoperative complications were examined with postoperative x-ray. Patients were followed with clinical and radiographic examinations. In the postoperative clinical evaluation, two male patients healed well, but one female patient complained of pain and swelling. In radiographic examinations, no union delay or lack of fusion was observed in the edentulous area. Open reduction technique is a viable treatment option for the edentulous atrophic mandible fractures in geriatric patients.

  • 족부 주상골의 골절 및 거주상 관절의 탈구는 그 빈도가 매우 드물다. 이에 대하여는 관혈적 혹은 비관혈적의 정복술이 요구된다. 저자들의 단독 주상골 골절 및 거주상 관절 탈구에 관하여 관혈적 정복술 및 K-강선을 이용한 내고정을 이용한 치료를 하여 만족할만한 결과를 얻었으며, 문헌 고찰과 함께 보고하는 바이다.

  • This is a report of 2 cases on old maxilla fractures accompanied with sagittal palatal fracture and severe malocclusion. We treated them by using of classic Le Fort I osteotomy and modified Le Fort I osteotomy along the old fracture lines satisfactorily. The results obtained from treatment are as follows : 1. Careful examination and correct care on sagittal palatal fracture should be need during initial diagnosis and emergency care of maxilla fracture showed malocclusion. 2. Although early definite treatment of maxilla injuries is difficult due to major organ injuries associated with accident, the positive effort to induce normal occlusion is always necessary as soon as possible. 3. In the cases of malocclusion due to transverse discrepancy of maxillary dentition associated with injury as like as our cases, classic and modified Le Fort I osteotomy and rigid internal fixation were useful to correct occlusion, to ease operation and return normal functions early.

  • A 30-year-old, male truck driver had a traffic accident and visited a hospital due to left hip pain. The patient's femoral head was fractured, and he was treated conservatively. For 14 years afterwards, he walked without a limp, had no pain, and drove his truck. He was involved in another traffic accident and experienced a comminuted fracture of the left distal femur 14 years after the initial injury. Although he was symptom-free, while being treated by open reduction and internal fixation for the distal femur fracture, he was concerned about the status of his left femoral head. Pelvis radiographs and reconstructed CT images were done, and they showed a spur change around the femoral head which had a dense sclerotic band within and revealed a slight depression of subchondral bone of the medial portion of the femoral head. The diagnosis was a Pipkin type-II fracture of the femoral head.

  • Pulmonary artery aneurysm is Behcet`s disease is rare and can be fatal due to rupture. We experienced a case of pulmonary artery aneurysm in Behcet`s disease. The patient was 21 year old woman who was adimitted with three month history of dyspnea, fever and cough. On examination, she had aphthous ulcer in the mouth and erythema nodosum on the left popletial fossa and forearm, but didn`t have any lesion at eyes and genitalia. The latex fixation test for rheumatoid factor, VDRL test for syphillis, antinuclear antibody and LE cell test were all negatives. The third and fourth components of complement in the serum, serum immunoglobulin concentrations[IgG, IgM, IgA] were within normal range. The chest radiography revealed a 5x6cm sized radiopaque mass density in the left hilar region. Two months later, the mass was enlarged to 6x7cm. The IV-DSA showed a single aneurysm at the proximal part of left lower lobe artery with lingular segment artery and no distal perfusion by thombotic obstruction. The steroid therapy was done for a month, but symptoms not improved. We performed resection of lingular segment and lower lobe including the aneurysm. The microscopic findings of the operative specimen were intimal hyperplasia and fragmentation of the internal elastic fibers. She was improved without remarkable event, except infection of the operative wound.

안정된 내고정이 시행된 하악각 골절에서 골절선상의 치아 발거에 따른 감염률 비교평가 (Comparative Evaluation of Infection Rate according to Extraction of Teeth in the Line of Mandibular Angle Fractures Treated with Stable Internal Fixation)

  • Purpose: Mandibular angle fractures constitute approximately 30% of mandibular fractures, and the mandibular third molar is usually in line with the fracture. This study evaluated the relationship between the extraction of a tooth in line with a mandibular angle fracture and the infection rate. Methods: One hundred and forty seven patients with mandibular angle fractures containing a tooth in line with the fracture from 2005 to 2008 were enrolled in this study. The patients were divided into two groups based on an extraction, and the infection rates were evaluated in both groups. Results: An extraction was performed in 70 patients (47.6%) and postoperative infections occurred in 14 patients (9.5%). Six (8.6%) of the 70 patients in the extraction group and 8 (10.4%) of the 77 patients in the non-extraction group had an infection. There was no significant difference between the two groups (P=0.708). Conclusion: These results show that there is no increased risk of postoperative complications when a tooth is present
