ConsExpo 모델을 이용한 구강건강행위에 따른 불소노출평가 (Assessment of Fluoride Exposure by Oral Health Behaviors using the ConsExpo Model)

  • 치아를 칫솔질하거나 치약을 사용하는 것과 같은 구강 건강 행위는 구강 건강을 개선하며, 따라서 삶의 질을 향상시키는 중요한 부분이다. 그러나 화학 물질에 대한 연구도 필요한 실정이다. 따라서 본 연구는 구강 건강 행위로 인해 야기되어지는 불소 노출에 미치는 요인을 조사하여 정확한 구강 건강 지침을 제공하고자 한다. ConsExpo 5.0 모델에서 불소 화합물의 경구 노출을 적용한 결과, 일일 불소 인체노출량 추정은 성인남성의 모델 결과 oral external dose는 0.000196 mg/kg, oral acute(internal) dose는 0.000196 mg/kg, oral chronic(internal)dose는 0.000465 mg/kg/day로 추정되었다. 성인여성은 연구결과 oral external dose는 $4.1{\times}10^{-6}mg/kg$, oral acute(internal) dose $4.1{\times}10^{-6}mg/kg$, oral chronic(internal) dose $9.99{\times}10^{-6}mg/kg/day$로 추정되었다.

  • This paper describes the methodology of calculating the internal dose conversion coefficient in order to assess the radiological impact on non-human species. This paper also presents the internal dose conversion coefficients of 25 radionuclides ($^3H,\;^7Be,\;^{14}C,\;^{40}K,\;^{51}Cr,\;^{54}Mn,\;^{59}Fe,\;^{58}Co,\;^{60}Co,\;^{65}Zn,\;^{90}Sr,\;^{95}Nb,\;^{99}Tc,\;^{106}Ru,\;^{129}I,\;^{131}I,\;^{136}Cs,\;^{137}Cs,\;^{140}Ba,\;^{140}La,\;^{144}Ce,\;^{238}U,\;^{239}Pu,\;^{240}Pu$) for domestic seven reference animals (roe deer, rat, frog, snake, Chinese minnow, bee, and earthworm) and one reference plant (pine tree). The uniform isotropic model was applied in order to calculate the internal dose conversion coefficients. The calculated internal dose conversion coefficient (${\mu}Gyd^{-1}$ per $Bqkg^{-1}$) ranged from $10^{-6}$ to $10^{-2}$ according to the type of radionuclides and organisms studied. It turns out that the internal does conversion coefficient was higher for alpha radionuclides, such as $^{238}U,\;^{239}Pu$, and $^{240}Pu$, and for large organisms, such as roe deer and pine tree. The internal dose conversion coefficients of $^{239}U,\;^{240}Pu,\;^{238}U,\;^{14}C,\;^3H$, and $^{99}Tc$ were independent of the organism.

Verification of Harmonization of Dose Assessment Results According to Internal Exposure Scenarios

  • Background: The determination of the amount of radionuclides and internal dose for the worker who may have intake of radionuclides results in a variation due to uncertainty of measurement data and ingestion information. As a result of this, it is possible that for the same internal exposure scenario assessors could make considerably different estimation of internal dose. In order to reduce this difference, internal exposure scenarios for nuclear facilities were developed, and intercomparison were made to determine the harmonization of dose assessment results among the assessors. Materials and Methods: Seven cases on internal exposures incidents that have occurred or may occur were prepared by referring to the intercomparison excercise scenario that NRC and IAEA have carried out. Based on this, 16 nuclear facilities concerned with internal exposure in Korea were asked to evaluate the scenarios. Each result was statistically determined according to the harmonization discrimination criteria developed by IDEAS/IAEA. Results and Discussion: The results were evaluated as having no outliers in all 7 cases. However, the distribution of the results was spread by various causes. They can be divided into two wide categories. The first one is the distribution of the results according to the assumption of the intake factors and the evaluation factors. The second one is distribution due to misapplication of calculation method and factors related to internal exposure. Conclusion: In order to satisfy the harmonization criteria and accuracy of the internal exposure dose evaluation, it is necessary that exact guidelines should be set on low dose, and various intercomparison cases also be needed including high dose exposure as well as the specialized education. The aim of the blind test is to make harmonization evaluation, but it will also contribute to securing the expertise and high quality of dose evaluation data through the discussion among the participants.

국내 석탄화력발전소 내 작업종사자의 입자 흡입에 따른 내부피폭 방사선량 평가 (Assessment of Internal Radiation Dose Due to Inhalation of Particles by Workers in Coal-Fired Power Plants in Korea)

  • Coal-fired power plants handle large quantities of coal, one of the most prominent NORM, and the coal ash produced after the coal is burned can be tens of times more radioactive than the coal. Workers in these industries may be exposed to internal exposure by inhalation of particles while handling NORM. This study evaluated the size, concentration, particle shape and density, and radioactivity concentrations of airborne suspended particles in the main processes of a coal-fired power plant. Finally, the internal radiation dose to workers from particle inhalation was evaluated. For this purpose, airborne particles were collected by size using a multi-stage particle collector to determine the size, shape, and concentration of particles. Samples of coal and coal ash were collected to measure the density and radioactivity of particles. The dose conversion factor and annual radionuclide inhalation amount were derived based on the characteristics of the particles. Finally, the internal radiation dose due to particle inhalation was evaluated. Overall, the internal radiation dose to workers in the main processes of coalfired power plants A and B ranged from 1.47×10-5~1.12×10-3 mSv y-1. Due to the effect of dust generated during loading operations, the internal radiation dose of fly ash loading processes in both coal-fired power plants A and B was higher than that of other processes. In the case of workers in the coal storage yard at power plants A and B, the characteristic values such as particle size, airborne concentration, and working time were the same, but due to the difference in radioactivity concentration and density depending on the origin of the coal, the internal radiation dose by origin was different, and the highest was found when inhaling coal imported from Australia among the five origins. In addition, the main nuclide contributing the most to the internal radiation dose from the main processes in the coal-fired power plants was thorium due to differences in dose conversion factors. However, considering the external radiation dose of workers in coal-fired power plants presented in overseas research cases, the annual effective dose of workers in the main processes of power plants A and B does not exceed 1mSv y-1, which is the dose limit for the general public notified by the Nuclear Safety Act. The results of this study can be utilized to identify the internal exposure levels of workers in domestic coal-fired power plants and will contribute to the establishment of a data base for a differential safety management system for NORM-handling industries in the future.

몬테칼로 시뮬레이션을 이용한 소아 핵의학검사 시 인체내부 장기선량 평가 (Evaluation Internal Radiation Dose of Pediatric Patients during Medicine Tests Using Monte Carlo Simulation)

  • In this study, a physical evaluation of internal radiation exposure in children was conducted using nuclear medicine test(Renal DTPA Dynamic Study) to simulate the distribution and effects of the radiation throughout the tracer kinetics over time. Monte Carlo simulations were performed to determine the internal medical radiation exposure during the tests and to provide basic data for medical radiation exposure management. Specifically, dose variability based on changes in the tracer kinetic was simulated over time. The internal exposure to the target organ (kidney) and other surrounding organs was then quantitatively evaluated and presented. When kidney function was normal, the dose to the target organ(kidney) was approximately 0.433 mGy/mCi, and the dose to the surrounding organs was approximately 0.138-0.266 mGy/mCi. When kidney function was abnormal, the dose to the surrounding organs was 0.228-0.419 mGy/mCi. This study achieved detailed radiation dose measurements in highly sensitive pediatric patients and enabled the prediction of radiation doses according to kidney function values. The proposed method can provide useful insights for medical radiation exposure management, which is particularly important and necessary for pediatric patients.

Surface Treatment of Eggshells with Low-Energy Electron Beam

  • Background: Salmonella enteritidis (SE) was the main cause of the pandemic of foodborne salmonellosis. The surface of eggs' shells can be contaminated with this bacterium; however, washing them with sodium hypochlorite solution not only reduces their flavor but also heavily impacts the environment. An alternative to this is surface sterilization using low-energy electron beam. It is known that irradiation with 1 kGy resulted in a significant 3.9 log reduction (reduction factor of 10,000) in detectable SE on the shell. FAO/IAEA/WHO indicates irradiation of any food commodity up to an overall average dose of 10 kGy presents no toxicological hazard. On the other hand, the Food and Drug Administration has deemed a dose of up to 3 kGy is allowable for eggs. However, the maximum dose permitted to be absorbed by an edible part (i.e., internal dose) is 0.1 Gy in Japan and 0.5 Gy in European Union. Materials and Methods: The electron beam (EB) depth dose distribution in the eggshell was calculated by the Monte Carlo method. The internal dose was also estimated by Monte Carlo simulation and experimentation. Results and Discussion: The EB depth dose distribution for the eggshells indicated that acceleration voltages between 80 and 200 kV were optimal for eggshell sterilization. It was also found that acceleration voltages between 80 and 150 kV were suitable for reducing the internal dose to ≤ 0.10 Gy. Conclusion: The optimum irradiative conditions for sterilizing only eggshells with an EB were between 80 and 150 kV.

Fibrinolysis with Lower Dose Urokinase in Patients with Complicated Parapneumonic Effusion

  • Background: Intrapleural urokinase is one of the most widely used fibrinolytic agents in the treatment of complicated parapneumonic effusion (CPPE). However, little research has been performed on the optimal urokinase dosage. The aim of this study was to evaluate the treatment efficacy of half dose urokinase compared with conventional dose urokinase. Methods: We retrospectively enrolled 92 patients with CPPE or empyema who underwent intrapleural urokinase treatment at two tertiary hospitals. Patients received antibiotics, chest tube drainage, and other treatments as part of routine care. The primary outcome was the treatment success rate in the half dose urokinase group (50,000 IU daily for maximal 6 days) and the conventional dose urokinase group (100,000 IU daily). Treatment success was defined as clinical and radiological improvements without surgical treatment or re-admission within one month. Results: Forty-four patients received half dose urokinase, whereas 48 patients were treated with conventional dose urokinase. Both groups were relatively well matched at baseline, excluding higher serum white blood cell count and higher empyema prevalence in the half dose urokinase group. The treatment success rate was not different between the two groups (p=0.048). There were no differences in the rate of in-hospital death and surgical treatment, hospitalization duration, and indwelling catheter duration. In the multivariate analysis, urokinase dose was not a predictor of treatment success. Conclusion: Half dose intrapleural urokinase is equally effective conventional dose urokinase in treating patients with CPPE or empyema.

보중익기합대칠기탕(補中益氣合大七氣湯) 추출물의 ICR마우스에서 경구 단회투여독성 평가 (Single Dose Toxicity Test of Bojungikkeehapdaechilkitang water extract in Male and Female ICR Mice)

  • An herbal water extract of Bojungikkeehapdaechilkitang(BDT) was prepared to test it for single-dose and repeated-dose toxicity, genotoxicity and reproductive toxicity, and to obtain a 50% lethal dose$(LD_{50})$, approximated lethal dose(ALD), and approximated target organs for BDT. The extract was tested on female and male ICR mice according to KFDA Guideline 1999-61 at doasge level of 2000, 1000, 500, 250 and 125mg/kg/10mL In this study, clinical signs, mortalities and gross findings of principal organs were observed for 14 days of single dosing, and afterwards in some cases. The ALD and $LD_{50}$ of BDT extract obtained in this study was>2000mg/kg for both male and female ICR mice. Also, any possible digestive toxicity of BDT extract was found to be above 1000mg/kg in both male and female ICR mice. The results of this study strongly suggest that BDT extract has no toxic effect at dosage level below 500mg/kg.

Effect of a Dose-Escalation Regimen for Improving Adherence to Roflumilast in Patients with Chronic Obstructive Pulmonary Disease

  • Background: The adverse effects of the phosphodiesterase-4 inhibitor roflumilast, appear to be more frequent in clinical practice than what was observed in chronic obstructive pulmonary disease (COPD) clinical trials. Thus, we designed this study to determine whether adverse effects could be reduced by starting roflumilast at half the dose, and then increasing a few weeks later to $500{\mu}g$ daily. Methods: We retrospectively investigated 85 patients with COPD who had taken either $500{\mu}g$ roflumilast, or a starting dose of $250{\mu}g$ and then increased to $500{\mu}g$. We analyzed all adverse events and assessed differences between patients who continued taking the drug after dose escalation and those who had stopped. Results: Adverse events were reported by 22 of the 85 patients (25.9%). The most common adverse event was diarrhea (10.6%). Of the 52 patients who had increased from a starting dose of $250{\mu}g$ roflumilast to $500{\mu}g$, 43 (82.7%) successfully maintained the $500{\mu}g$ roflumilast dose. No difference in factors likely to affect the risk of adverse effects, was detected between the dose-escalated and the discontinued groups. Of the 26 patients who started with the $500{\mu}g$ roflumilast regimen, seven (26.9%) discontinued because of adverse effects. There was no statistically significant difference in discontinuation rate between the dose-escalated and the control groups (p=0.22). Conclusion: Escalating the roflumilast dose may reduce treatment-related adverse effects and improve tolerance to the full dose. This study suggests that the dose-escalated regimen reduced the rate of discontinuation. However, longer-term and larger-scale studies are needed to support the full benefit of a dose escalation strategy.

Feasibility of normal tissue dose reduction in radiotherapy using low strength magnetic field

  • Purpose: Toxicity of mucosa is one of the major concerns of radiotherapy (RT), when a target tumor is located near a mucosal lined organ. Energy of photon RT is transferred primarily by secondary electrons. If these secondary electrons could be removed in an internal cavity of mucosal lined organ, the mucosa will be spared without compromising the target tumor dose. The purpose of this study was to present a RT dose reduction in near target inner-surface (NTIS) of internal cavity, using Lorentz force of magnetic field. Materials and Methods: Tissue equivalent phantoms, composed with a cylinder shaped internal cavity, and adjacent a target tumor part, were developed. The phantoms were irradiated using 6 MV photon beam, with or without 0.3 T of perpendicular magnetic field. Two experimental models were developed: single beam model (SBM) to analyze central axis dose distributions and multiple beam model (MBM) to simulate a clinical case of prostate cancer with rectum. RT dose of NTIS of internal cavity and target tumor area (TTA) were measured. Results: With magnetic field applied, bending effect of dose distribution was visualized. The depth dose distribution of SBM showed 28.1% dose reduction of NTIS and little difference in dose of TTA with magnetic field. In MBM, cross-sectional dose of NTIS was reduced by 33.1% with magnetic field, while TTA dose were the same, irrespective of magnetic field. Conclusion: RT dose of mucosal lined organ, located near treatment target, could be modulated by perpendicular magnetic field.
