『의학입문(醫學入門)』의 인용서적으로 살펴본 요통(腰痛)의 분류와 기준 (The Classification and Criterion for Low Back Pain Examined from Reference Books of Yi Xue Ru Men(醫學入門))

  • Objectives : In order to find how reference books of Yi Xue Ru Men reflect the classification and criterion for low back pain(LBP). Methods : From reference books of Yi Xue Ru Men, select the texts on classification and criterion for LBP. Results : According to the causes of LBP, Chao Yuan Fang(巢元方) in Sui Dynasty assorted to 5 types of LBP at the very first. Chen Wu Ze(陳無擇) in Song Dynasty made 7 divisions by external, internal, and non-external, non-internal causes. According to the pulse of LBP, Yan Yong He(嚴用和) first categorized 4 groups, Zhu Zhen Heng(朱震亨) added another 4 groups. Aside from this standard, Zhu(朱震亨) adopted the cause standard. Depending on Yunqi(運氣), Lou Ying(樓英) classified 5 types. But his classification had been not adopted by any TCM books. According to symptom of 6 varieties(六變), Zhang Jie Bin(張介賓) assorted external(表), internal(裏), deficiency(虛), sufficiency(實), cold(寒) and heat(熱), add 2 groups besides them. But his categorization did not reflect Yi Xue Ru Men. Li Chan(李梴), the author of this book chose causes and pulse classification standards that Zhu Zhen Heng had adopt. Conclusions : In the side of classification and criterion for LBP, Li Chan first divided 2 group, external and internal injury. After it he subdivided both groups to 10 subgroup. His classification is similar to Chen(陳無擇)'s, but actually followed the classification for external and internal injury that was invented by Li Dong Yuan(李東垣).

금원대(金元代)의 소아과학(小兒科學)에 관한 연구 (The Medical Study about Pediatrics for Geum(金) and Won(元) Dynasty)

  • Pediatrics in Geumwon(金元) age had fanned peculiar children's etiology, Byeonggihak(病機學) and Byeonjeungnon(辦證論) curative system grounded on medical theory in Geumwonsadaega(金元四大家). Yuwanso(劉完素), who was front-running man in that age, used so many times 'Yanggyeoksan(凉隔散)' or 'Bangpungtongseongsan(防風通聖散)' which have Hanryang(寒凉) disposition for curing children's disease in view of pathology in Yeoldahanso(熱多寒少), which is his special academic thought, after then Judangye(朱丹溪) succeed Yuwanso(劉完素)'s this view of pathology, Yang-eumjahyeol(養陰滋血) developed the one and made it a standing rule to cure children. Also Idongwon(李東垣) grasped that the main cause of disease is starvation because of war. As a result of it, he insisted on theory, internal cause's outbreak of person's illness, Naesangbiwi(內傷脾胃), Baekbyeong-yusaeng(百病由牛). In this view of this principle, he assorted and used following medicine 'Seungma(升麻), Siho(柴胡), Hwanggi' as 'Seungbalyanggi(升發陽氣)' method to cure children, and concentrated on children's spleen and stomach cure with this way. Jangjongjeong(張從政) applied 'Purgation therapy[攻下法]', Hantoha(汗吐下) centered on the theory of exogenous factors[外因說] , which explains that children's disease is Sagichimseup(邪氣侵襲), too. Like this, in Geumwon(金元) age, they had studied very seriously about main cause of children' s disease in two part, an internal cause and an external cause. Also, in pathological part they had approached academically 'Hwayeolchibyeong(火熱致病)' and 'Gigiseunggangsiljo(氣機乘降失調)' from diverse angles.

  • This thesis is a study on the Significance of Pulse in Internal Classic(內經脈義) in the Chapters on Vitality of Pulse(脈神章) in the Complete Works of Zhang Jingyue(景岳全書) Vol. IV. The Complete Works of Zhang Jingyue(景岳全書) is composed of 64 volumes. Maishenzhang, the Chapters on Vitality of Pulse(脈神章) is composed of 3 chapters. The first chapter is the Significance of Pulse in Internal Classic(內經服義). The terminology, Maishen(脈神) means the Vitality of Pulse, which was introduced by Zhang Jingyue to emphasize that the keypoint of sphygmological diagnosis is the vitality of pulse. The Significance of Pulse in Internal Classic(內經脈義) is composed of 20 parts including Buwei(部位), Maidu(脈度), Sanbujiuhou(三部九候), Qizhen(七診), Liujingmaoti(六經脈體), ect. in Miraculous Pivot(靈樞). There are no commentaries in most of them, which is because there are explanatory notes in Leijing(類經). Therefore, I referred to explanatory notes in Leijing(類經) to study the Significance of Pulse in Internal Classic(內經脈義), and make a study as followings.

『연라자내경도(煙蘿子內境圖)』와 『화타선생내조도(華佗先生內照圖)』의 비교 (Comparison of Yanluozi's and Huatuo's visceral figures)

  • Objectives : This research is aimed to compare the visceral figures published during the Song Dynasty, and to reveal the differences between anatomical and physiological knowledge in the presence of Taoist theoretical background and in its absence. It is also aimed to discuss the theoretical differences among existing medicines. Methods : First, we will find the commonalities and the differences in the drawings describing the internal organs of a human body in the two publications, and study what made such commonalities and differences and how they affected medical theories. Secondly, we will analyze and update the internal organ data specified in the two publications as it relates to the five viscera and six bowls. Finally, we will analyze the commonalities and the differences, and study the background and the significance of medical theories. Results & Conclusions : 1. The positions of the liver and the spleen in the drawing of the Yanluozi's and Huatuo's visceral figures are inverted in the drawing of the Huatuo's visceral figures. The positions of other internal organs are similar. 2. The section of the five viscera in the Yanluozi's and Huatuo's visceral figures describes the Taoist gods necessary to build up discipline by the means of meditation. 3. In the Zhutidian's visceral theory, a belly button is recognized as a very important organ for the activity of life. 4. In the Huatuo's visceral figures, the 'Zangzhen' generated from foods is described as the energy source for the vitality of the five viscera. 5. The Huatuo's visceral figures mentioned the functionality of a gall bladder and an Tripple Energizer, which was not mentioned in the Yanluozi's and Huatuo's visceral figures. 6. Both the "Yanluozi's and Huatuo's visceral figures" and the "Huatuo's visceral figures" specify that the moisture filtered through small intestines turns into urine.

명당장부도(明堂臟腑圖)에서 기원한 심장도(心臟圖) 29종의 비교 (Comparison of 29 Diagrams of Heart originated from MingTangZangFuTu(明堂臟腑圖))

  • Objectives : The transition of the 29 diagrams of Heart originated from MingTangZangFuTu will be used to understand the shape of heart and the changes of people's understanding of heart. Methods : The 29 diagrams of Heart originated from MingTangZangFuTu are divided according to their shape elements (Heart itself, internal curvatures, structures above heart, wuzangxi, and excursus). Then its transitions are analyzed, and each shape element is compared to modern anatomy and its textual basis is searched. Results : The lengthy cylinderical organ situated above the heart is composed of upper part consisting with joints and the lower part void of joints. The upper part is Pyewan (such as the lungs) and fall into trachea. The lower part is Xinxi or Feixi which are either relative vein (or aorta) or left bronchus that passes behind the heart. This depiction of the structures around the heart can be considered to have composed by actual observance of a physical heart, a method that is similar to anatomy. However, the shape of the heart itself is described as a lotus flower that has not been bloomed, a depiction which finds its origin from Zhongguangbuzhu huangdineijing suwen (762). The three short curvatures inside the heart is described as Pericardium, influenced by Shisijingfahui (1341) in its depiction, or as sammo, influenced by Nanjing. Structures that are connected directly from the heart to spleen, kidney, and uterus are not found in modern anatomy. The saying in Excursus "All cords of five internal organs belong to heart" is based on Huatuoxuanmenneizhaotu, and this is changed to the saying cords of four internal organs belong to heart in Leijingtuyi for the first time. Conclusions : The authors of medical scriptures at the time did not have a method of direct observance when they were copying heart diagrams. Therefore, they made changes to the source material's diagram and excursus while being influenced by Nanjing, Huatuoxuanmenneizhaotu, and Shisijingfahui. Then the doctors' understandings with regards to the shape and function of heart were reflected during that process.

소식(蘇軾)의 용호연홍설(龍虎鉛汞說)을 통한 단계호잠환(丹溪虎潛丸) 해석 (A Study on the Danxihuqianwan(丹溪虎潛丸) analyzed by the dragon-tiger and lead-mercury theory(龍虎鉛汞說) of Su Shi(蘇軾))

  • Objective : This study will analyze the constitution of Danxihuqianwan(丹溪虎潛丸) with the dragon-tiger and lead-mercury theory of Su Shi(蘇軾). It would help understand more the usage of Danxihuqianwan over the scope of Sa-nam and bo-buk(瀉南補北) theory that has been well known until now. Methods : This study tried to analyze the explanation about Danxihuqianwan in Yixuerumen(醫學入門) that had used the dragon-tiger and lead-mercury concept with the theory of this kind that appears in Sushenliangfang(蘇沈良方). Results & Conclusion: Based on the internal alchemy theory, Su Shi proclaimed that dragon[龍] corresponds to mercury[汞] Jeong(精) Hyul(血), and tiger[虎] corresponds to lead[鉛] Qi(氣). It is said to be in good position that dragon appears from fire and tiger is hidden in water. Su Shi also expalined the relation between dragon and tiger as the relation of water[水] and fire[火] between heart and kidney. As Li Yan(李梴) did, the dragon-tiger theory could be applied to the analysis about Danxihuqianwan, and it explains well the effect of this prescription that makes tiger hidden in water and thus help cure the diseases such as atrophy syndrome.

폐암(肺癌)의 한방치료 임상연구 방법론 (Clinical research methodology for Traditional Korean Medicine treatment of lung cancer : Evidence-based approach)

  • Background : I investigated clinical research methodology of Traditional Korean Medicine(TKM) for treatment of lung cancer with Evidence Based Medicine(EBM) approach. Methods : I conducted the internet search of National Central Library, the National Assembly Library, Korea Education & Research Information Service, Korea Institute of Science and Technology Information for Korean theses and papers, and MEDLINE for the outside articles. Conclusion : In addition to outcomes of clinical trial, abundant clinician's experience and patients' preference are also considered. Pragmatic trials and quasi-experimental trials are better than clinical trials to plan clinical design of TKM for lung cancer and appropriate endpoints includes overall survival, disease-free survival, patient reporting outcome.

『관아외편(串雅外編)』의 외치료법(外治療法)에 관한 연구 - 내과질환을 중심으로 - (The Observation on the Five Vicera's A Study of External Treatment on the 『Chuan-Ya-Wai-Bian(串雅外編)』 -part of internal medicine-)

  • 1) Objective External treatments have various curative effects. So it had been used to cure various patients. But, it has a limited sphere of application in the present South Korea. And treatments of wander doctors had been disregarded by general doctors and the public. But Zhao-Xue-Min(趙學敏) had thought that there are useful treatments in it. So, the "Chuan-Ya(串雅)" had been written by Zhao-Xue-min in 1759. The book had included treatments of wander doctors. Especially, it had included various external treatments. Therefore we would like to bring out it's external treatments theory 2) Conclusions We have researched external treatments of "Chuan-Ya", and then we have arrived at following conclusions. "Chuan-Ya" has 48 sentences that have related to external treatments in internal medicine. And it had recorded fire conducting method(引火法) to explain process of external teratment principle.

내상잡병(內傷雜病) 개념 형성에 대한 고찰(考察) (A Study on the Concept Formation of Neishangzabing)

  • Objectives : In this article, the concept of Neishang and Zabing and the establishment of the terms, are investigated. Methods : The literatures related to the establishment of Neishang and Zabing, for example, Huangdineijing, Shanhanlun, Jinguiyaolue etc are selected and then the analysis and investigation of the significant documents has followed. Results : The concept of Neishang originally means the group of disease caused by internal pathogens, for example, emotions in Huangdineijing; however, it has become rarely mentioned in Shanhanlun, Jinguiyaolue. The other concept, Zabing that is used once as a name of chapter in Huangdineijing means the group of diseases without exogenous diseases after Shanhanzabinglun was published. Conclusions : The concept of Neishang has divided into qiqingneishang that is caused by emotion disorders in Sanyinfang in the Song dynasty, and yinshineishang argued by Lidongyuan, a doctor in the Jinyuan dynasty: the latter has been the main meaning of Neishang. The concept of Zabing had been used as a narrow sense, the rest diseases until it was extended to a broad sense, the name of disease group except for internal and body-based diseases.

  • 1) Objective External treatments have various curative effects. So it had been used to cure various patients. But, it has a limited sphere of application in the present South Korea. And the "Li-Yao-Pian-Won" had been written by Wu-Shang-Xian(吳尙先) in 1864. The book is the first technical book of external treatments. So, he has been held in respected as the father of external treatments by many people. Therefore we would like to bring out it's external treatments theory 2) Conclusions He had insisted that external treatments principles are identical with internal treatments. So, prescriptions of external treatment are selected and made by principles of internal treatment. But external treatments have following characteristics 1. Each of external treatment prescriptions have various effects. 2. Exchange of treatments is very easy. 3. We can make free use of various external treatments for care at a time. 4. External treatments are very economical. 5. External treatments are very easy to study. 6. External treatments are simple, speedy, precision.
