CT Follow-Up of Postoperative Bronchopleural Fistula: Risk Factors for Progression to Chronic Complicated Infection (수술 후 기관지 흉막루의 전산화단층촬영 추적 검사: 만성 복합성 감염 진행의 위험인자 분석)

  • Purpose We evaluated the risk factors for progression to chronic complicated bronchopleural fistula (BPF) after pulmonary resection using follow-up CT. Materials and Methods We retrospectively reviewed 45 cases with BPF that had undergone pulmonary resection during 2010-2018. We compared the clinical and radiological characteristics of those with complicated BPF (n = 24) and those without complicated (sterilized) BPF (n = 21). The clinical and radiological risk factors for progression to chronic complicated BPF were examined by logistic regression analysis. Results The thickness of the pleural cavity wall (p = 0.022), the size of the pleural cavity (p = 0.029), and the size increase of BPF on follow-up (p = 0.012) were significantly different between the two groups. The risk factors for progression to chronic complicated BPF were age > 70 years (odds ratio, 6.43; 95% confidence interval, 1.2-33.7), the thickness of the cavity wall > 5 mm (odds ratio, 52.5; 95% confidence interval, 5.1-545.4), and an increase in the size of the pleural cavity on follow-up CT (odds ratio, 12.5; 95% confidence interval, 2.1-73.5), only in the univariate analysis. Conclusion The risk factors for progression to chronic complicated BPF can be evaluated using follow-up CT.

The Role of Angled Blade Plate in Treatment of Bone Tumor Occurred in Femur (대퇴골 골종양의 치료에 있어서의 Angled Blade Plate의 역할)

  • Purpose: Bone tumor occurred in femur frequently involve proximal intertrochanteric region or distal metaphyseal region. Sometimes, the pathologic fracture can happen according to the size of tumor due to the substantial biomechanical stresses. Therefore, the prognosis can be improved biomechanically by the angled blade plate considering the anatomic configuration after the excision of tumor. Materials and Methods: Between October 1991 and April 2005, there were a total of 16 patients(17 cases) who were treated by the excision of tumor and internal fixation with the angled blade plate for bone tumor occurred in femur. After the excision of tumor, we filled the cavity by bone graft in 11 cases and bone cement in 6 cases. The internal fixation was used by angled blade plate in all cases. Result: The average follow-up time was 55.5 months(6-144 months) in 16 patients(17 cases). No metal failure occurred after the operation. Reoperation was performed in 4 cases due to tumor recurrence, and the internal fixation was firm until that. Conclusion: The angled blade plate can prevent the fracture until grafted bone incorporated to host bone, and protect fragile connection between cement and host bone.

Gross-β Level in Dental Ceramic Materials (치과용 세라믹 재료의 전β 방사능 준위)

  • In all of dental ceramics, these materials contained the radio-isotopes with natural abundance ratio. After dental treatment, remained dental ceramics in the oral cavity is caused for human internal dose. In this study, the gross beta activity levels were measured in dental materials including 22 dental ceramics, 1 resin, and 2 cements for estimation of human internal dose. In dental ceramic samples, the obtained results showed that the gross beta activity level varied between 1.317 ~ 2.935 Bq/g and the gross beta activity level was 2.379 Bq/g. And the same level for dentine, opacious dentine, translucent and enamel were 2.479 Bq/g, 2.491 Bq/g, 2.470 Bq/g and 2.069 Bq/g, respectively. The gross beta activity level of temporary resin and cements were negligible, compare to the same level of dental ceramics. The high gross beta activity level observed in opacious dentine code OD-A4 is 2.935 Bq/g thus mainly ascribable to 40K. The reduction of the radiation level of natural radio-isotopes and the improvement of the dental ceramic materials should be required for internal dose reduction.

Effects of Scutellaria Radix Extract on the Membranous Nephropathy Induced by Cationic Bovine Serum Albumin in Mouse Model (황금(黃芩)이 Cationic Bovine Serum ALbumin투여로 유발된 Membranous Nephropathy Mouse Model에 미치는 영향)

  • Background: Membranous nephropathy (MN) is the most common cause of adult nephrotic syndrome worldwide and has been defined as granular subepithelial deposition of immune complexes along the glomerular basement membrane (GBM). MN has few known treatments and gives rise to side effects under treatment with steroids and immunosuppressives. Objective: The purpose of this experimental study was to demonstrate the effects of Scutellariae Radix extract (SRE) treatment on MN mouse model induced by cBSA. Methods: We divided mice into 4 groups. The Normal group had no treatment. We induced MN mouse model to the other 3 groups by injecting cBSA into the abdominal cavity. The control group was treated with cBSA (10 mg/kg, i.p.) only. The second group, 'SRE-250', was treated with cBSA (10 mg/kg, i.p.) and SRE (250 mg/kg, p.o.). The third group, 'SRE-500', was treated with cBSA (10 mg/kg, i.p.) and SRE (500 mg/kg, p.o.). After cBSA and SRE treatment for 4 weeks, gain in body weight, 24hrs proteinuria, serum albumin, total cholesterol, triglyceride, BUN and creatinine of all groups were measured. TNF-$\alpha$, IL-6, IL-1$\beta$, IL-10, IFN-$\gamma$, IgA, IgM and IgG levels of all groups were gauged. H&E staining and electron microscopy of the kidney were observed. Results: SRE showed significant decrease in the 24hrs proteinuria, serum triglyceride, BUN, TNF-$\alpha$, IL-6, serum IgA, IgM and IgG levels compared with the control group. SRE showed increase in the serum IL-10 and IFN-$\gamma$ levels compared with control on RT-PCR. SRE considerably decreased in the thickening of the GBM on H&E staining and deposition of electron-density on electron microscopy of the kidney compared with the control. Conclusions: According to the above results, it is suggested that SRE decreases the symptoms of MN induced by cBSA in mouse model. Therefore, SRE seems to be applicable to MN in clinical practice.

A Study on the prescriptions of 『Sanbeon-bang(刪繁方)』 (『산번방(刪繁方)』의 의방(醫方)에 관한 연구)

  • This paper is mainly on the rsearch of the prescriptions of "Sanbeon-bang(刪 繁方)". For the research of prescription, investigated "Sanbeon-bang" from the side of symptoms of a disease, number of drugstuffs, table of contents, drugstuffs and acupuncture and moxibustion. With these investigation, made out a few tables, and with these tables made an attempt to understand the whole prescriptions of "Sanbeon-bang". "Sanbeon-bang" is of rich contents in internal medicine. Among internal medicine, related to Oro-Yukgeuk-Chilsang(五勞-六極-七傷) was most abundant. So we can guess "Sanbeon-bang" was a medical book specialized in exhaustion syndromes. It also deals with the Samcho-syndromes, as investigate from medical comments, has the perfect system of prescription in relative. From the investigation of the combination of drugstuffs, ascertained that, the number of simple prescription composed of one drugstuff was 38, combinational prescription composed of two drugstuffs was 9, minor prescription composed of three to five drugstuffs was 47, midum prescription composed of six to ten drugstuffs was 95, major prescription composed of eleven to twenty drugstuffs was 45, and mixed prescription composed over twenty drugstuffs was 1. Mentionable unique prescriptions in internal use were the forms of keeping in mouth. In external use they were fumigants, suppositories, powdered medicines, spraying the granular medication into the nasal cavity and eyedrops. And were abundant in soft extract(ointment) and plaster. In addition, there were the recordings of 18 types of cellulitis, types of incurable cellulitis and curing an illness by a charm. By the way, when comparing "Sanbeon-bang" with "Cheongeum-bang", all of which was quoted a lot in "Oedaebiyo-bang", "Cheongeum-bang" rather took medical comments of "Sanbeon-bang" than prescription. Although there were some prescriptons in "Sanbeon-bang" at the same category, "Cheongeum-bang" took another prescriptions which are more complexed than those of "Sanbeon-bang". In the same way, when comparing "Cheongeum-bang" with "Sonjinin-Cheongeum-bang(孫眞人千金方)", which didn't go through the correction of GyojeongUiseoguk of Bug-Song goverment, "Cheongeum-bang" often didn't take the prescriptions of "Sonjinin-Cheongeum-bang". Hence we can guess, "Cheongeumyo-bang" may have added a lot of prescriptions when undergoing the correction of of GyojeongUiseoguk. The total number of species of drugstuffs in "Sanbeon-bang" from the investigation was 284. The plant drugs were 208 species, the animal were 31 species, the minerals were 19 species and the other were 26 species. The prescriptions related to acupuncture and moxibustion in "Sanbeon-bang" were only moxibustional prescriptions. Which appeared one time per exhaustion and steaming of bone syndrome, Oro-Yukgeuk-Chilsang and Samcho syndrome. Appeared six times in muscle syndromes. But I cannot imagine the original form of acupuncture and moxibustion in "Sanbeon-bang" for deficiency of data.

Percutaneous Catheter Drainage of Lung Abscess (폐농양의 경피적 카테타 배농법)

  • Background: Recently, lung abscess tends to be increased in patients with underlying disease, most of whom are unsuitable for surgery when medical treatment fails. The patients with giant lung abscesses do not frequently respond to antibiotics and often have life-threatening complications. Therefore, more intensive cares are required in these patients. We studied the results and effects of percutaneous catheter drainage in these patients. Method: We performed fluoroscopy-guided percutaneous pigtail catheter (8.3 F) drainage by Seldinger technique in 9 cases of lung abscess (in 7 cases, intractable to medical treatment for an average of 8.4 days and in 2 cases, catheter drainage immediately performed due to a large cavity that was initially 10 cm in diameter). We compared 10 cases of lung abscess as control group which had receieved conventional medical treatment alone. Results: Seven of the 9 patients in study group of percutaneous drainage and 7 of the 10 patients in control group of medical treatment alone clinically improved in the average of 1.8 and 8.7 days, respectively. The mean duration of drainage was 13.2 days. There were 3 cases of death from massive hemoptysis, asphyxia of pus, and sepsis in control group, as compared with 2 cases of death from hepatic encephalopathy and sepsis in study group. The malfunctions of catheter occurred in these 2 cases, obstruction and dislodgement. But there were no significant pleuropulmonary complications of percutaneous drainage. Conclusion: Percutaneous drainage is effective and relatively safe in the management of lung abscesses refractory to medical therapy or giant lung abscesses.

  • This study is a case study of the research study and education with high school students of natural sciences. Recent development of the Internet of Things (IoT) based on the subject of various studies exist. This study is one of the most portable communication devices of these infancy, yet students were easy to define the molding process of contacting a pager that can go along way in making research topics. We called a pager. Today, it is given a large and complex smart mobile communication devices that can be used for big data. Prior pagers are taught the meaning of the first mobile communication means in our lives were given device. The internal structure is relatively simple and is thought to function relatively simple, just suitable as a teaching practices of high school students with an interest in science and engineering universities. This study can see all of the mechanical, electronic information contents of the radio pager in a relatively simple analysis of the injection molding ONE CAVITY analysis. Furthermore, the students also rated good educational practices that give meaning to remind the convergence training on mass production and process automation.

Clinical Cases Analysis of Forearm Free Fasciocutaneous Flaps on Oral Cavity Defect Area (구강 결손부에 적용된 요골 유리전완 피부피판 적용례 분석)

  • The radial forearm free flap (RFFF) has become a workhorse flap as a means of reconstructing surgical defects in the head and neck region. We have transferred 12 RFFFs with fasciocutaneous type on oral cavity defects in 12 patients after cancer resection and submucous fibrotic lesion ablation from 2005 to 2007 at Department of oral and maxillofacial surgery, Pusan National University Hospital. We reviewed retrospectively patients' charts and followed up the patients. Clinical analysis on the cases with RFFFs focusing on flap morbidity, indications and available vessels was done. The results of study are follows: 1. RFFF could be applied for all kind of defects after resection of tongue, floor of mouth, buccal mucosa, denuded bone of palate, maxilla, and mandible. 2. All free flaps could be used for primary reconstruction. The survival rate of 12 RFFFs was 92%. Partial marginal loss of the flaps was shown as 3 cases among 12 cases. Large size-vessels like superior thyroid artery, facial artery, internal jugular vein were favorable for microvascular anastomosis. 3. Parenteral nutrition instead of nasal L-tube also can be favorable for postoperative a week for better healing of the flap if the patients couldn't be tolerable with nasal tubing. 4. Donor sites with thigh skin graft were repaired with wrist band for 2 weeks. The complications included scarring, abnormal sensation on hand, and reduced grip strength in few patients, but those didn't induce major side effects. 5. Most RFFFs were well healed even if mortality rate of cancer patients was shown as 50% (5/10 persons). The mortality of patients was not correlated with morbidity of the flaps. We could identify the usefulness of RFFF for restoration of oral function, esthetics if the flap design, tissue transfer indications, and well controlled operation are proceeded.

In vivo verification of regional hyperthermia in the liver

  • Purpose: We performed invasive thermometry to verify the elevation of local temperature in the liver during hyperthermia. Materials and Methods: Three 40-kg pigs were used for the experiments. Under general anesthesia with ultrasonography guidance, two glass fiber-optic sensors were placed in the liver, and one was placed in the peritoneal cavity in front of the liver. Another sensor was placed on the skin surface to assess superficial cooling. Six sessions of hyperthermia were delivered using the Celsius TCS electro-hyperthermia system. The energy delivered was increased from 240 kJ to 507 kJ during the 60-minute sessions. The inter-session cooling periods were at least 30 minutes. The temperature was recorded every 5 minutes by the four sensors during hyperthermia, and the increased temperatures recorded during the consecutive sessions were analyzed. Results: As the animals were anesthetized, the baseline temperature at the start of each session decreased by $1.3^{\circ}C$ to $2.8^{\circ}C$ (median, $2.1^{\circ}C$). The mean increases in temperature measured by the intrahepatic sensors were $2.42^{\circ}C$ (95% confidence interval [CI], 1.70-3.13) and $2.67^{\circ}C$ (95% CI, 2.05-3.28) during the fifth and sixth sessions, respectively. The corresponding values for the intraperitoneal sensor were $2.10^{\circ}C$ (95% CI, 0.71-3.49) and $2.87^{\circ}C$ (1.13-4.43), respectively. Conversely, the skin temperature was not increased but rather decreased according to application of the cooling system. Conclusion: We observed mean $2.67^{\circ}C$ and $2.87^{\circ}C$ increases in temperature at the liver and peritoneal cavity, respectively, during hyperthermia. In vivo real-time thermometry is useful for directly measuring internal temperature during hyperthermia.

The Role of the Upper Gastrointestinal Study in Evaluation of Patients with Head and Neck Cancers (두경부종양환자에서 시행한 상부위장관검사의 유용성)

  • Background and Objectives: Multiple primary tumors of the upper aerodigestive tract are not unusual. We examined head and neck cancer patients to discover the presence of second primary cancer in their upper gastrointestinal tract, using esophagogastroscopy. Materials and Methods: Endoscopic examination of the upper gastrointestinal tract was performed on 51 patients whose head and neck cancers were treated at department of therapeutic radiology from August 1996 to April 1999. Two of all patients had been studied by barium swallowing study. In 51 patients, twenty-four had a primary tumor in the larynx, 8 in the oropharynx, 6 in the nasopharynx, 6 in the oral cavity, 6 in the hypopharynx, and 1 in the nasal cavity. Endoscopically pathologic lesions were biopsied. In control group, endoscopy was performed on 1097 patients who didn't complain any symptoms. Results: Endoscopy showed early malignant lesions in 4 cases(7.7%). Histology of esophageal cancers showed squamous cell carcinoma. Malignant lesions of stomach in 2 cases were histologically identified as adenocarcinoma. Two esophageal cancers occurred in patients whose primary lesions had oropharynx and hypopharynx. Two cases of gastric cancer were also accompanied by oropharynx and hypopharynx. The incidence of second primary cancer was 2 in oropharynx and 2 in hypopharynx. In all cases, second primary cancers were found simultaneously. In control group, 9(0.8%) of 1097 patients were confirmed as early esophageal and gastric cancers. Conclusion: The majority of esophageal and gastric cancer detected by endoscopy were early stage in both head and neck cancer and control group. The incidence of esophageal and gastric cancer of head and neck cancer patients was 10 times as high as that of control group. Although followup period was short, all second primary cancers were detected simultaneously. We would recommend that endoscopic evaluation be included in the workup and followup of all patients with newly diagnosed head and neck cancer.
