교통동맥분절의 내측벽에서 기원한 내경동맥류 치험 2례 - 증례보고 - (Internal Carotid Artery Aneurysms Arising from the Medial Wall of the Communicating Segment of the Internal Carotid Artery - Case Report -)

  • Aneurysms of the medial wall of the internal carotid artery(ICA) constitute a rare group of ICA aneurysms arising at locations other than at arterial division. Because these aneurysms usually have thin walls and wide necks associated with arteriosclerosis, neck clipping should be taken into special consideration. The authors report two cases of the aneurysm presented with subarachnoid hemorrhage, in which neck clippings ware performed with impunity.

총창으로 인한 우측 총경동맥의 가성 동맥류 치험 1 (False aneurysm of right common carotid artery due to gun shot wound: report of a case)

  • Vascular injuries from gun shot wound is rare in these days, in Korea. A Case of false aneurysm of the right common carotid artery due to penetrating injury to the neck by carbine. The confirmatory diagnosis was made by right carotid angiogram which revealed bean-sized aneurysmal sac at the mid-portion of the right common carotid artery. Despite of no symptoms, emergency false aneurysmectomy and reconstruction with on-lay vein patch graft using left greater saphenous vein for threat of rupture and embolization from mural thrombi. During repair of common carotid artery, cerebral circulation was maintained with internal shunt. The postoperative course was uneventful except limit of motion of right upper extremity due to initial injury.

Experimental of Cerebral Vasospasm and Measure the Mean Blood Flow Velocity in the Middle Cerebral Artery

  • To determine the appropriate concentration of papaverine hydrochloride(PPV) for therapeutic intraarterial infusion against cerebral vasospasm and to measure the mean blood flow velocity of the middle cerebral artery in rabbits. Vasospasm was induced in the experimental groups (3 days after infusion; group 1, n=3, 7 days after infusion; group 2, n=3) and a control group (n=l) by placing a blood clot in the subarachnoid space around the top of the internal carotid siphon. PPV (5 mg/kg) was infused into the internal carotid artery. The vascular diameters of the internal carotid artery (ICA) and middle cerebral artery (MCA) were measured on angiograms before and after infusion. The mean blood flow velocity in the MCA was measured on transcranial doppler sonograms before and 24 hours after infusion. After fixation, the MCA was dissected out, stained, and examined microscopically. After PPV infusion in both groups, vascular dilatation of about $20\%$ was seen. The mean increase in blood flow velocity in the group $1(30\%)$ was smaller than in the group $2(70\%)$. The mean blood flow velocity in the MCA decreased by about $30\%$ in both groups, but increased again after 24 hours nearly to the level before PPV infusion. PPV infusion may be more effective in early stages of vasospasm when vascular walls have fewer histologic changes.

Ideal Internal Carotid Artery Trapping Technique without Bypass in a Patient with Insufficient Collateral Flow

  • Internal carotid artery (ICA) trapping can be used for treating intracranial giant aneurysm, blood blister-like aneurysms and ICA rupture during the surgery. We present a novel ICA trapping technique which can be used with insufficient collaterals flow via anterior communicating artery (AcoA) and posterior communicating artery (PcoA). A patient was admitted with severe headache and the cerebral angiography demonstrated a typical blood blister-like aneurysm at the contralateral side of PcoA. For trapping the aneurysm, the first clip was placed at the ICA just proximal to the aneurysm whereas the distal clip was placed obliquely proximal to the origin of the PcoA to preserve blood flow from the PcoA to the distal ICA. The patient was completely recovered with good collaterals filling to the right ICA territories via AcoA and PcoA. This technique may be an effective treatment option for trapping the aneurysm, especially when the PcoA preservation is mandatory.

The efficacy and efficiency of percutaneous lidocaine injection for minimizing the carotid reflex in carotid artery stenting: A single-center retrospective study

  • Objective: To assess whether local anesthetic infiltration could minimize the carotid baroreceptor reflex (CBR) which has an incidence after carotid artery stenting (CAS) that varies from 29% to 51%. Methods: This retrospective single-center study included 51 patients (mean age, 70.47 years) who underwent CAS for carotid stenosis. The groups included patients who underwent CAS for asymptomatic ischemic stroke (n=41) or symptomatic disease (n=10). Preprocedural percutaneous lidocaine injections (PPLIs) were administered to 70.6% and 5.9% of patients who underwent elective CAS and emergency CAS, respectively. Results: Among patients who received PPLIs, the mean degree of stenosis was 80.5% (95% confidence interval [CI]: ±10.74, 51-98%). The mean distance from the common carotid artery bifurcation to the most stenotic lesion (CSD) was 8.3 mm (95% CI: ±0.97, 6.3-10.2 mm); the mean angle between the internal carotid artery and common carotid artery (CCA) trunk (IAG) was 65.6° (95% CI: ±2.39, 61-70°). Among patients who did not receive PPLIs, the mean degree of stenosis was 84.0% (95% CI: ±8.96, 70-99%). The mean CSD was 5.9 mm (95% CI: ±1.83, 1.9-9.9 mm); the mean IAG was 60.4° (95% CI: ±4.41, 51-70°). The procedure time was longer in the PPLI group than in the no PPLI group (28.19 [n=39] vs. 18.88 [n=12] days) (P=0.057); the length of intensive care unit stay was shorter in the PPLI group (20.01 [n=36] vs. 28.10 [n=5] days) (P=0.132). Conclusions: Targeted PPLI administration to the carotid bulb decreased aberrant heart rates and blood pressure changes induced by carotid stent deployment and balloon inflation. As CBR sensitivity increases with decreasing distance to the stenotic lesion from the CCA bifurcation, PPLIs may help stabilize patients during procedures for stenotic lesions closer to the CCA.

  • Anterior cerebral arteries are paired and supply the major portion of the medial surface of the brain. They are branches of the intracranial part of the internal carotid artery and form the anterior portion of the circle of Willis (CW) which is situated in the interpeduncular fossa. During routine dissection in the department of anatomy, All India Institute of Medical Sciences, Bibinagar, Hyderabad, a rare variation had been observed in the CW, azygos anterior cerebral artery associated with hypoplastic A1 fragment of the right anterior cerebral artery in a 63-year-old female cadaver. It is important to identify and study this kind of rare variation for surgeons, anatomists, and radiologists during dissection, surgical, radiological, and diagnostic interventions.

우총경동맥류의 수술적 치료 (Surgical Treatment of a Right Common Carotid Artery Aneurysm)

  • 38세 남자가 우측 경부의 약 $5{\times}6cm$ 크기의 박동성 종괴를 주소로 내원하였다. 종괴는 최근 2개월간 빠르게 커졌으며 간헐적인 경부 통증과 애성 등의 증상이 동반되었다. 방사선학적 검사를 통하여 우측 총경동맥 분지부 직하방에 위치한 동맥류를 확인하였으며 그것은 내경동맥과 외경동맥을 압박하고 있었다. 내부 단락을 사용하여 뇌혈류를 유지한 상태에서 우내경동맥 일부 내막절제술을 시행하였으며 동맥류를 절제하고 도관대치를 시행하였다. 수술 중 주위 신경조직의 손상은 없었으며 별다른 합병증 없이 회복하여 환자는 수술 후 7일에 퇴원하였다.

Left Carotid-to-Subclavian Artery Bypass Grafting for Recurrent Angina Caused by Coronary-Subclavian Steal Syndrome

  • A 60-year-old man visited the outpatient clinic due to one month of recurrent exertional chest pain. Eleven years earlier he had undergone off-pump coronary artery bypass grafting using bilateral internal thoracic artery (ITA) Y-composite grafts based on the left ITA. Preoperative coronary angiography showed patent distal graft anastomoses and visualized the left ITA retrogradely. The arch aortography revealed near-total occlusion of the left subclavian artery at the level of the ostium. The patient underwent left carotid-to-subclavian artery bypass grafting using a 6 mm vascular conduit. Postoperative computed tomographic angiography revealed a patent bypass conduit between the left common carotid artery and left subclavian artery. The patient was discharged on postoperative day 4 with no symptoms or signs of myocardial ischemia.

The Effect of Silver Spike Point Therapy(SSP) that applied to Acupoints on the Blood Flow Rate of Internal Carotid Artery and Pain Relief of Headache Patients

  • The aim of this is to examine the effects of SSP therapy on the internal carotid arteries blood flow of 24 tension type headache patients(study group) and non tension type headache patients(control group), and on the reduction of their headaches, when applied to the acupuncture points. It stimulated the acupuncture point of headache 6 place with the SSP. It measured VAS and the blood flow of the internal carotid arteries with TCD. When the internal carotid arteries blood flow of study group and control group were compared by period, a significant difference was found in the 4th period(p<.05). When the Visual Analog Scale of study group and control group were compared by period, a significant difference was found in the 4th period(p<.05). The comparison of each measurement result of the internal carotid arteries blood flow of the study group found significantly increased. The comparison of each measurement result of the VAS of study group found significantly decreased. With regard to the control group, the VAS significantly decreased. The silver spike point low frequency electrical stimulation treatment, when applied to the acupuncture point, can significantly influence the internal carotid arteries blood flow of headache patients and can significantly reduce their headaches.

  • Aneurysms arising at locations other than arterial division are rare and the incidence of intraoperative bleeding is far higher in such aneurysms than in usual aneurysms. The authors report a case of intraoperative rupture and laceration on internal carotid artery(ICA) wall during dissecting a dorsal wall aneurysm on supraclinoid ICA and successful repair of the laceration on the parent ICA with microsuture and a Sundt clip-graft.
