The Change of Basic and Postgraduate Medical Education after Intern Training System Dismantling (인턴수련제도 폐지에 따른 기본의학교육-졸업 후 의학교육의 변화)

  • This study aimed to review the expected changes in the medical educational environment and to evaluate approaches to coping with the abolition of the postgraduate intern training system. It is expected that after the intern training system is dismantled, postgraduate medical students will be deprived of the opportunity to practice opportunity for clinical practice and to inquire into their medical specialization. Therefore, major improvements in the clinical education curriculum must be made so that students can do so through the clinical education program. Offering students the opportunity to perform clinical practice through the clinical education program might require a revision in the laws and regulations on clinical education as well as the standardization of the clinical education curriculum in line with international practices. Reform measures to provide students the opportunity to inquire their specializations might be the introduction of a medical curriculum containing diverse fields and the establishment of a matching program to assign medical students to their residency programs after medical school. Finally, the fact that the basic concern of postgraduate medical education is the cultivation of primary care physicians must not be forgotten even after the dismantling of the postgraduate intern training system.

Actual Situation of the Intern Physical Therapist at Hospitals in the Seoul Metropolitan Region (수도권 소재 일부 의료기관의 인턴물리치료사 관련 실태)

  • Purpose: The purpose of this study was to provide information and suggestions for improvement of the intern physical therapist. Methods: I used 'intern physical therapist' as a keyword, and searched using the Google Search Appliance. Hospitals in the Seoul metropolitan region were selected. Twenty six hospitals that operate the intern physical therapist system were selected. A survey was conducted using questionnaires from August 20, 2012 to August 30, 2012. Among the 26 submitted questionnaires, 24 responded, and the results were analyzed using IBM SPSS Statistics 19.0. Results: 1. Education: (1) Sexual harassment prevention training, infection control training was lower at the rehabilitation hospital and semi hospital than at the university hospital and general hospital. (2) The rehabilitation hospital provided more education for intern physical therapists than the university hospital. (3) University hospital practice location was more diverse than that of the rehabilitation hospital. (4) In the rehabilitation hospital, education of practical skill was somewhat higher. 2. The employment rate of people who have completed an internship was higher in the rehabilitation hospital than the university hospital. Conclusion: Trial employment is more reasonable than intern physical therapist at the semi general hospital and rehabilitation hospital. Council is needed for decisions regarding internship training curriculum.

Survey on the undergraduate curriculum in clinical pharmacology and interns' prescribing ability in South Korea

  • Appropriate prescription writing is one of the critical medical processes affecting the quality of public health care. However, this is a complex task for newly qualified intern doctors because of its complex characteristics requiring sufficient knowledge of medications and principles of clinical pharmacology, skills of diagnosis and communication, and critical judgment. This study aims to gather data on the current status of undergraduate prescribing education in South Korea. Two surveys were administered in this study: survey A to 26 medical schools in South Korea to gather information on the status of undergraduate education in clinical pharmacology; and survey B to 244 intern doctors in large hospitals to gather their opinions regarding prescribing education and ability. In survey A, half of the responding institutions provided prescribing education via various formats of classes over two curriculums including lecture, applied practice, group discussions, computer-utilized training, and workshops. In survey B, we found that intern doctors have the least confidence when prescribing drugs for special patient populations, especially pregnant women. These intern doctors believed that a case-based practical training or group discussion class would be an effective approach to supplement their prescribing education concurrently or after the clerkship in medical schools or right before starting intern training with a core drug list. The results of the present study may help instructors in charge of prescribing education when communicating and cooperating with each other to improve undergraduate prescribing education and the quality of national medical care.

Accelerating Change in Medical Education after the Dismantlement of the Intern Training System (인턴제도 폐지 논의 이후의 의학교육 변화)

  • In light of changes in today's medical environment, whether to dismantle the 50-year-old internship training system in the Republic of Korea is under debate. Although the question remains open, discussions on such issues have drawn attention to the quality of the clinical clerkship and student career advisory programs in medical colleges. The purpose of this study is to analyze the experiential clerkship and career exploration issues. Ensuring excellence of the clerkship and career advisory sessions is an essential responsibility of educational institutions regardless of whether the intern training system is dismantled. Important objectives of the experimental clerkship include reinforcing prerequisites established by law, introducing a student practice license, developing a standardized clinical assessment and student portfolio requirement, and publishing a guidebook for clinical directors. For career exploration, it is necessary to broaden participants' experiences of specialties and to manage the variety of student career guidance programs. It is imperative for the Korean Association of Medical Colleges, in collaboration with medical colleges, to play a leading role in focusing more attention and effort on such issues.

A Study on the Job Environment for Interns and Residents in University Hospital (일부 대학병원 인턴, 레지던트의 직무환경 분석)

  • This study was conducted to evaluate the satisfaction level of interns and residents on training and job environment of two university hospitals in Pusan. The concept of subject's satisfaction was evaluated in general characteristics, socioeconomic status, working condition, human relationship, and job status and scored from 1 to 5. The newly developed questionnaire for this study(32 items) was revised and modified by the preliminary survey. The internal consistency of questionnaire was 0.73(Cronbach's alpha). The self-administered questionnaire was provided to 218 subjects and collected the answering from March 2 to March 25th, 1997, and statistical significances were tested by $x^2-test$, t-test and ANOVA. The satisfaction level between the residents group was significantly different, but satisfaction level of working condition, human relationship and job status showed no difference. The service division part showed highest(2.73) and the surgical division part showed lowest(2.57). The job satisfaction level between the subjects showed higher satisfaction level in working condition, socioeconomic status, and total satisfaction dimension, and intern group showed higher satisfaction level than resident group. Satisfaction level in human relationship dimension, resident group showed higher satisfaction level than intern group, but showed no statistically difference. To improve the working condition for residents, public welfare facilities was firstly suggested and the next was salary increase, overloaded work, respectively. For intern groups, overloaded work and waste work were firstly suggested and the next was public welfare facilities and salary increase, sequentially. The career gets longer, the satisfaction level on general characteristics of working condition, socioeconomic status, and total satisfaction level were increased. The subjects who have religion and higher socioeconomic status showed higher satisfaction level than the other group. The general satisfaction level of subjects in all dimensions showed lower aver age(3.0) and the overall satisfaction level showed below than average. Thus, in order to improvement of subject's working condition, higher quality of life is necessary rather than economic state. In future, these results might be considered to improve the working condition for interns and residents.

  • The subinternship (student internship), a subtype of bedside and clinical training was first developed in the United States. Currently, some medical schools conduct a student internship and many other universities are considering the implementation of a student internship in Korea. If the intern system is abrogated beginning in 2016 as in the United States, then the importance of the student internship will be greatly emphasized for clinical training. To produce good and competent medical doctors, members of medical schools and affiliated hospitals must acknowledge the role of the educational hospital and support student internships. In addition, the effort of the medical community to develop and apply a standard curriculum to the student internship is also required. Above all, the attention of society and the nation is essential to make legal policy changes regarding the conducting of student internships and enhance understanding about the authorized practices in university hospitals. The medical community's effort to draw attention to this issue is greatly required to implement a student internship at this time.

  • 다수의 수련병원이 단일 병원군을 형성하려 전공의를 공동으로 모집선발하고, 또한 공동 책임에 수련시키는 전공의 통합수련프로그램인 "병원군별 전공의 총정원제"에서 인턴 순환근무 수련일정 계획은 순환교육과 임상경험을 위란 인턴의 일년간 수련일정을 수립하는 의사결정문제이다. 전공의의 순환배치와 관련하여 병원간의 갈등 또는 인턴의 불만 등이 발생할 수 있기에 병원군별 전공의 총정원제는 질적으로 우수하면 객관적인 인턴수련일정을 필요로 한다. 본 논문은 인턴일정계획의 수리모형을 제시하고, 면실의 데이터를 사용해 산출된 순환일정을 다각도로 분석한 결과를 제시한다. 작성된 수련일정은 9개병원이 참여한 CMC병원군에서 2003년 현재 263명 인턴의 일년수련 순환배지에 사용되고 있다.

Application of XAI Models to Determine Employment Factors in the Software Field : with focus on University and Vocational College Graduates (소프트웨어 분야 취업 결정 요인에 대한 XAI 모델 적용 연구 : 일반대학교와 전문대학 졸업자를 중심으로)

  • The purpose of this study is to explain employment factors in the software field. For it, the Graduates Occupational Mobility Survey by the Korea employment information service is used. This paper proposes employment models in the software field using machine learning. Then, it explains employment factors of the models using explainable artificial intelligence. The models focus on both university graduates and vocational college graduates. Our works explain and interpret both black box model and glass box model. The SHAP and EBM explanation are used to interpret black box model and glass box model, respectively. The results describes that positive employment impact factors are major, vocational education and training, employment preparation setting semester, and intern experience in the employment models. This study provides a job preparation guide to universitiy and vocational college students that want to work in software field.

A Study on the Historical Status of Giovanni Marinoni in the Tradition of Cadastre and Cartography in 18th Century Italy (18세기 이탈리아의 지도제작의 전통과 조반니 마리노니의 역사적 위상에 관한 연구)

  • This research focuses on the role of Giovanni Marinioni during the formation of the modern cartography and cadastre during the 18th century. Initial study began with Giambattista Nolli's Roman map noticing not much information was available to acknowledge his activities during his Milan period before the departure to Rome. It became evident that Marinoni was a key person to understand the complex circumstances in which the professional training and formation of Giambattista Nolli took place as later worked as an anonymous intern during the elaboration of Theresian Cadastre of Milan. The other important figures are Leandro Anguissola and Giovanni Filippini. Anguissola's position and precedent work facilitated Marinoni's multidisciplinary activities that he had performed in Vienna and Milano in the field of making urban maps of those two cities. On the other hand, Filippini not only collaborated with Marinoni but also introduced Nolli in the field of cartography. These activities show transitional and dual aspects that characterized the period in which important irreversible changes that occur during the reign of Habsburg empire and in the rest of the Europe toward the formation of modern society and state. Marinoni's theories and praxis greatly influenced Nolli's later commitment under the Savoia and later on the elaboration of the 'Pianta Grande di Roma' in 1748.
