  • Life cycle assessment (LCA) on two different sludge treatment systems, on-site treatment and pipe-collected intensive treatment was performed to estimate the environmental impact in the aspect of global warming effect. As a main parameter of the estimation, $CO_2$ was chosen and quantified through the whole life cycle of the treatment systems including construction, operation and dismantlement. In this study, the changes of $CO_2$ production unit (CPU) by up-scaling n currently used sludge treatment processes were also calculated. As the result, a larger amount of $CO_2$ was exhausted from the construction step of intensive treatment system than that of on-site treatment system, because an additional pipe-collection system was needed in intensive treatment system. However, the total amount of $CO_2$ exhausted from whole life cycle including not only construction and dismantlement but also 15 year-operation and maintenance was reduced by appling intensive treatment.

Effects of Korean Medical Intensive Treatment Program on Geriatric Depression Disorder (노인성우울증에 대한 한방 집중치료 프로그램의 효과)

  • Objectives: As the elderly population grows and the prevalence of depression increases, the incidence of geriatric depression disorder will continue to increase. The effect of an intensive Korean medicine treatment program on geriatric depression disorder was studied. Methods: Geriatric depression disorder patients who received treatment over a period of three years were reviewed, with the geriatric depression score (GDS) used as the primary outcome. The intensive Korean medical treatment program applied to the geriatric depression disorder patients consisted of herbal medicine, acupuncture, pharmacopuncture, moxibustion, meditation, qigong exercises, and reminiscence therapy. Results: The GDS scores of patients who received the intensive Korean medical treatment program decreased from an average of 19.57 (${\pm}5.52$) to 15.76 (${\pm}6.99$) and there was a significant difference. Conclusions: When the intensive Korean medical treatment program was applied to geriatric depression disorder patients, the symptoms of depression improved. This treatment program will be helpful for improving symptoms in patients and can be applied by trained clinicians.

Comparison of Experiences of Ethical Dilemma between Intensive Care Unit and General Unit Nurses regarding Treatment Decisions and Confidentiality (의료처치 및 환자 비밀유지에 대한 중환자실 간호사와 일반병동 간호사의 윤리적 갈등 경험 비교연구)

  • Purpose : This study compares experiences of ethical dilemma between nurses working in intensive care units and those in general units under specific situations of treatment decisions and confidentiality. Method : This cross-sectional descriptive study utilizes the self-report survey method. The survey questionnaires were completed by 50 and 52 nurses working in intensive care units and general units, respectively. The instrument, which consisted of 16 items of ethical dilemma situations about treatment decision and confidentiality, was used. The mean scores for each item were compared between the two groups. Results : The study found no differences in terms of age, gender, education level, clinical experience in years, and being educated on healthcare ethics. For 9 out of 16 items, the mean scores of nurses in intensive care units were significantly higher than those of nurses in general units. Conclusions : Nurses in intensive care units experienced ethical dilemmas regarding treatment decisions and confidentiality more often than those in general units. This study emphasizes the need to establish strategies for improving the ethical competence of critical care nurses.

Experience of Life-sustaining Treatment in Patient Care among Intensive Care Unit Nurses: Phenomenological Approach (중환자실 간호사의 연명치료환자 간호 경험: 현상학적 접근)

  • Purpose: The purpose of this study was to explore the subjective experience of life-sustaining treatment care among nurses in intensive care units. Method: A phenomenology was used for the study. Data were collected from October to December, 2015 using open-ended questions during in-depth interviews. Participants were nurses working in intensive care units and were contacted through purposive techniques. Eight nurses participated in this study. Results: Four categories emerged from the analysis using Colaizzi's method: (a) difficulties due to life-sustaining treatment care, (b) dilemma of extension or cessation of life-sustaining treatment, (c) repressed feelings and emotional exhaustion, and (d) forming values for life-sustaining treatment from nursing experience. Conclusion: Provision of clearer guidelines on life-sustaining treatment which reflect a family-oriented culture is important for nurses in ICU and will promote nurses involvement in the decision-making process of life-sustaining treatment of patients.

  • The fat embolism syndrome (FES) represents a condition, usually with traumatic etiology, which may pose challenges to diagnosis while its treatment usually requires supportive measures in the intensive care units (ICUs). The clinical criteria, including respiratory and cerebral dysfunction and a petechial rash, along with imaging studies help in diagnosis. Here we present three case reports of young male who developed FES and were admitted to our ICUs after long bones fractures emerging after vehicle crashes and we briefly review FES literature. All patients' treatment was directed towards: 1) the restoration of circulating volume with fresh blood and/or plasma; 2) the correction of acidosis; and 3) immobilization of the affected part. All patients recovered and were released to the orthopedic wards. The incidence of cases of patients with FES admitted in our ICUs records a significant decrease. This may be explained in terms effective infrastructure reforms in Greece which brought about significant improvement in early prevention and management.

Awareness of good death, perception of life-sustaining treatment decision, and changes in nursing activities after decision to discontinue life-sustaining treatment among nurses in intensive care units at tertiary general hospitals (상급종합병원 중환자실 간호사의 좋은 죽음인식, 연명의료결정 인식 및 연명의료중단 결정 후 간호활동의 중요도 변화)

  • Purpose : This study aims to explore nursing activities after the decision to discontinue life-sustaining treatment, awareness of a good death, and perception of life-sustaining treatment decisions among nurses in intensive care units (ICUs) at tertiary general hospitals. Methods : Participants were 173 nurses working in two tertiary general hospitals. The data were collected using structured questionnaires and analyzed using an independent t-test, paired t-test, one-way ANOVA, Scheffé's test, and Pearson's correlation coefficient. Results : Participants were 173 nurses working in two tertiary hospitals. The nursing activity increase was the greatest in the spiritual domain, and the physical domain was where the activities decreased the most. There were significant associations between Awareness of good death (Clinical) and Perception of life-sustaining treatment decision(r=.26, p <.001), Awareness of good death (Closure) and Perception of life-sustaining treatment decision(r=.36, p <.001), and Awareness of good death (Personal control) and Perception of life-sustaining treatment decision(r=.49, p <.001). Conclusion : Based on the results, systematic education programs and job training are required to improve the awareness regarding good death and perception of life-sustaining treatment decision for nurses in ICUs where discontinuing life-sustaining treatment decisions are made.

  • Objective : This single-subject study explored the effect of short-term intensive sensory integration therapy program which was provided in a 2006 sensory integration treatment course on adaptive response of a four-year-girl with sensory modulation disorder(mixed pattern). Method : Based on results of the assessment to the child, her therapist offers an organized sensory integration program during four days. Results : Improvement of child's movement quality and variety in physical environment in therapy rooms and interaction with her therapist were observed. Conclusions : This report of the case will be helpful for occupational therapists who plan to intervent with a short-term intensive sensory integration therapy program. In addition, the need for consideration of consultation with the child's parents is discussed.

Preventive Effects of Aspirin on Cardiovascular Complications in Prostate Cancer Cases after Endocrinotherapy

  • Objective: To explore the preventive effect of aspirin on the cardiovascular complications in prostate cancer after endocrinotherapy. Materials and Methods: A total of 92 patients with prostate cancer were divided into observation group (n=44) and control group (n=48). The control group was treated with medical castration plus anti-androgenic drugs. Based on the above treatment, the observation group was added aspirin. The follow-up duration was 2 years. The changes of partial prothrombin time (PT), activated partial thromboplastin time (APTT), platelet aggregation rate (PAG), prostate-specific antigen (PSA) and serum testosterone (T) before and after treatment as well as incidence of cardiovascular disease were observed. Results: The 2-year survival rates of patients without cardiovascular disease in observation group and control group were 95.45% (42/44) and 72.92% (35/48), respectively, and significant difference was presented between two groups by comparison to the survival rates ($x^2=8.5453$, p=0.0035). There was no statistical significance between two groups as well as before and after treatment regarding PT (p>0.05). After treatment, APTT went down and PAG was gradually on the rise in control group, while PAG down and APTT on the rise increasingly in observation group. Significant differences were presented between two groups as well as before and after treatment (p<0.01). Both PSA and T levels were decreased significantly in two groups after treatment (p<0.01), but there was no statistical significant between two groups (p>0.05). Conclusions: Application of endocrinotherapy in prostate cancer can easily lead to occurrence of cardiovascular disease, but cardiovascular complications can be prevented by aspirin, without affecting the effect of endocrinotherapy.

  • Purpose: Intensive Care Unit (ICU) is an important field of inpatient treatment, in which critically ill patients have been treated intensively with advanced medical technology. The level of treatment in the intensive care unit and the modernization of related facilities are important indicators of the quality of medical care. At present, when regional public hospitals are expanding frequently, reasonable planning of ICU has become an important part of medical institutions that treat ICU. The purpose of this study is to provide basic data on the net area of each unit, which can be used in ICU building planning. Methods: The investigation and analysis of the ICU were conducted in 12 medical institutions, based on theoretical analysis based on relevant guidelines and literature, and analysis of actual space composition and net area through architectural drawings. Results: This research provides basic data, such as the location relationship with other important departments, regional division, spatial composition, the relationship between main activities and regions, the composition of facilities in the region, and the area and proportions of each region. Implications: It is expected that the results of this article will provide effective reference materials for the reasonable spatial organization and effective operation of the intensive care units of ordinary public hospitals of different sizes in the future.

Effects of Korean Medical Combination Treatment for Lumbar Spinal Herniated Intervertebral Disc : accompanying or not High-intensive zone(HIZ) (요추 추간판 탈출증에 대한 한방 치료의 효과 연구 : High-intensive zone(HIZ) 유무에 따라)

  • Objectives : The purpose of this study was to analyze the effects of lumbar spinal herniated intervertebral Disc on MRI accompanying or not High-intensive zone(HIZ) by Korean Medical Combination Treatment Methods : 324 patients who received inpatient treatment from January 2017 to September 2017 in the Daejeon-Jaseng of Korean Medicine Hospital were divided into 2 groups by High-intensive zone(HIZ) confirmed with a Lumbar spine magnetic resonance imaging. All patients received a combination of treatment including acupunture, chuna manual therapy, pharmacopunture, herbal medication. They were compared and analyzed on the basis of improvement between measuring Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5 Dimension Index (EQ5D Index), physical examination as they were hospitalized and as they were discharged. The statistically significance was evaluated by SPSS 18.0 for windows. Results : After treatment, Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5 Dimension Index (EQ5D Index), physical examination was improved respectively. But there was no statistically significance between 2 group's improvement after treatment. Conclusions : After inpatient treatment by Korean Medical Combination Treatment, Most patient's pain, disability and Health Related Quality of Life was improved significantly. But there was no statistically difference between accompanying or not High-intensive zone(HIZ). So We think that future research of higher quality and correct statistics shall be necessary.
