Long-Term Observations of Korean Medicine Treatment in Patients with Mild Cognitive Impairment: Two Case Reports (경도인지장애 환자의 장기간 한의약 치료 경과관찰 증례보고 2례)

  • Objectives: To examine the effect of long-term Korean medicine treatment on mild cognitive impairment (MCI). Methods: This study was conducted on two patients who were diagnosed with mild cognitive impairment and received long-term Korean medicine treatment. In order to evaluate the degree of cognitive decline, SNSB and K-MMSE were performed 2~3 times at the first visit and during the treatment process. Results: Both cases 1 and 2 were diagnosed with MCI with a high possibility of developing dementia. In case 1, MCI progressed to early dementia at one year after the diagnosis of MCI. However, in case 2, that MCI status was maintained without progressing to dementia for 4 years after the diagnosis of MCI. The cause of such difference in the course of MCI might be attributed to the fact that Case 1 had lower K-IADL level but higher SGDS level than Case 2 at the time of initial diagnosis, with weak family support and irregular herbal medicine intake. Conclusions: Korean medicine treatment could improve and manage symptoms of cognitive decline due to MCI.

A Case Report of Advanced Gastric Cancer Patient Treated with Bojungikgi-tang Gamibang (절제불가능한 위암의 위장관 출혈에 대한 보중익기탕 가미방 투여 1례)

  • Approximately 10 percent of advanced stomach cancer patients have internal bleeding in their stomach. It is crucial to treat internal bleeding since it severely deteriorates patient's condition, and disturbs process of chemotherapy. There are hemostatic agents and dressing, radiotherapy, endoscopic ligation and coagulation, surgical methods to treat bleeding. However, these methods cannot be executed in some cases in which patient is in terminal stage or not in desirable condition to take these treatments. We are going to introduce a case of advanced gastric cancer patient having stomach bleeding who made a choice to take oriental medical treatments. The patient was on third stage of stomach cancer, had 3200cc of blood transfusion for five times. After diagnosed as not being able to have resection, he started to oriental medical treatments to stop bleeding. From March 28, 2009 to April 16, 2009, we administer 120cc Bojungikgi-tang gamibang to the patient in 90minutes after each meal. During 19days of having Bojungikgi-tang gamibang, we observed that the patient needed significantly lower amount blood transfusion, to 640cc. The patient improves in performance, and was able to eat more. Despite of short period of observation, this case indicates that Bojungikgi-tang gamibang treatment to the patient could work as hemostat, further improved quality of patient's life.

A Study on Applying a Korean Medical Integrative Program for Sleep Disorder (수면장애에 대한 한방통합프로그램의 적용 연구)

  • Objectives : Korea's average life expectancy is getting longer and longer and the pace of change in society has increased every year as people experience stress more easily. Under this situation, chronic sleep disorder is increasing, but there is no specific program for the treatment of sleep disorder in Korean medicine. Therefore, we studied a Korean medical treatment program for sleep disorder to apply to outpatient clinic patients. Methods : For this study, we evaluated the Insomnia Severity Index (ISI), BDI, STAI, FSS on 29 insomnia patients who visited Kyunghee Korean medical hospital from July 2010 to May 2012. Herbal medicine, acupuncture, herbal acupuncture, moxibustion, sleep hygiene, and medical qigong were applied. We compared ISI scores before treatment and after 4 weeks of treatment. Results : ISI score was improved after Korean medical treatment for sleep disorder. Before the treatment ISI score was 18.1, which means clinical insomnia. After the Korean medical treatment, the score was 7.5, which means no clinically significant insomnia. Conclusions : The study showed a significant effect on the Korean medical treatment program for sleep disorder to apply to outpatient clinic patients. ISI score was improved and the patients were satisfied with the treatment, so it would be more helpful to apply in sleep clinics.

  • Objectives: This study is aimed to recognize the role of TMJ in Korean medicine basic theory and clinic, which is the foundation of human life sustaining. Methods: It was identified and confirmed the organic connection between TMJ and Korean medicine specially Meridian theory. Results: In Korean medicine be based policies to promote of Oriental Medicine Promotion Act, the therapy of TMJ balancing is one of the parts of Korean medicine treatment of technique. TMJ balancing performs a primary role to treat diseases in the brain and the cervical vertebrae. Conclusions: TMJ balancing medicine is the part of integrative medicine for systemic diseases treatment to be based on Korean medicine basic theory.

Proposal of Evidence-based East-West Integrative Medicine Manual for Vascular Dementia (혈관성 치매에 대한 근거기반 의한 협진 매뉴얼 제안)

  • Objectives: This study was made by Chung Yeon Korean Medicine Hospital in order to perform appropriate East-West integrative medicine. The purpose of this manual is to support decision-making and communication in the implementation of the East-West cooperative treatment of vascular dementia. Methods: In order to carry out this study, it is based on search terms such as 'vascular dementia', 'acupuncture', 'herbal medicine', 'integrative medicine', 'chinese traditional medicine', and 'cognitive function' in databases such as MEDLINE, EMBASE, OASIS and CNKI We collected references. The drafting proceeded with the collaboration of two specialists of the Korean medicine, and the disagreement on the basis of the quotation was determined through a two person agreement. After, The draft was reviewed by a western medical doctor(rehabilitation specialist). Then, The opinions of the entire medical staff of the committee were reflected in the draft and finalized the agreement. Results: Through this study, manuals for diagnosis, treatment, and other considerations in the process of applying East-West integrative medicine to vascular dementia were derived. Conclusions: This study has significance in that it provides manual information about the decision structure, treatment contents, role distribution, etc. of East-West integrative medicine within the medical institution that conducts the vascular dementia consultation. In order for this study to function as a generalized medical guideline, it is necessary to improve the research methodology and carry out professional consensus procedures.

A Systematic Review of the Effectiveness of Eastern-Western Integrative Medicine for Musculoskeletal Disorders (근골격계 질환의 한·양방협진치료 효과 연구에 대한 체계적 문헌 고찰)

  • Objectives: This study aims to systematically evaluate the effectiveness of Eastern-Western integrative medicine for musculoskeletal disorders compared with Eastern or Western Medicine. Methods: Systematic sea-mes of 9 major Korean databases were conducted for articles published up to July 2011. Searching keyword was "Integrative Medicine". Prospective studies dealt with musculoskeletal disorders by Eastern-Western integrative medicine were included. Quality assessment of included studies was evaluated by Cochrane risk of bias tool. Results: 470 studies in total were seamed, Amalg them 7 studies met our inclusion criteria(3 randomized controlled clinical trials, 2 non-randomized controlled trials and 2 uncontrolled clinical trials). Included studies dealt with lumbar pan(3), shoulder pain(2), pain after traffic injury(1), spinal compression fracture(1). Eastern-Western integrative medicine showed better or equal effect for the treatment of various muscloskeletal disorders compared with Eastern or Western medicine alone. Conclusions: There is very weak evidence for the effectiveness of Eastern-Western integrative medicine for musculoskeletal disorders compared with Eastern or Western medicine alone. Well designed randomized controlled trials with rigorous methodology are needed to evaluate the effectiveness of Eastern-Western integrative medicine objectively in the future.

  • 1. Objectives The incidence of Gallbladder(GB) cancer is increasing through out the world. And the death rate of GB cancer is highest compared with other kinds of cancers. We treated Stage IV GB canter with lung metastasised patient with Sasang constitutional medicine. 2. Methods We fellowed up the changes of Symptoms and QOL. 3. Results The symptoms and quality of lift(QOL) of Stage IV GB cancer with both lung metastasis patient are improved markedly by Sasang constitutional medicine. 4. Conclusions The symptoms of GB cancer with both Lung mets were effectively managed through the constitutional treatment So we report the healing process and result of this patient in this study.

Tumor Regression Effect of Allergen-removed Rhus Verniciflua Stokes based Traditional Korean Medicine on a Patient with Metastatic Papillary Thyroid Carcinoma : Single Case Report (전이성 유두상 갑상선암 환자를 대상으로 투여한 알러젠 제거 옻나무 추출물의 종양 소퇴 1례)

  • Papillary thyroid carcinoma is the most common type of thyroid malignancy. It has better prognosis and rarer incidence of distant metastasis than other types of thyroid malignancy. However, once distant metastasis happens, its result will be bad. They mostly spread to the lungs and bones. and rarely to the brain. We report a patient with a papillary thyroid carcinoma metastasis to the lymph nodes. the ribs. the brain, and the lungs. The patient underwent total thyroidectomy, radioiodine ablation, rib partial resection, and lung wedge resection. However, upon progress of metastasis. the patient insisted on being treated with traditional Korean medical treatment using allergen-removed Rhus Verniciflua Stokes (A-RVS) instead of the conventional treatment including chemotherapy. The size of the tumor partly decreased, and the patient showed no side effects. This case report brings forth the importance of a thorough study in papillary thyroid carcinomas and their metastasis from the traditional Korean medical point of view, along with the long-term effect of allergen-removed Rhus Verniciflua Stokes (A-RVS).

A Study on the Development of the Standard Manual for ETE (Emotion To Emotion) Therapy (오지상승위치료법의 표준매뉴얼 개발을 위한 타당화 연구)

  • Objectives: The purpose of this study was to develop a standardization manual for Emotion To Emotion therapy. In this study, the processes and categories derived through literature search related to the Emotion To Emotion treatments, were revised and supplemented by the expert FGI (Focus Group Interview). Afterwards, the expert Delphi was conducted, to develop a standard manual for the disease types, purpose, and method of Emotion To Emotion therapy. Methods: In this study, literature analysis and expert Delphi, as a quantitative research method, were conducted, and the expert Focus Group Interview (FGI) was conducted as a qualitative study. The manual was completed by leading the consensus, on the standardization manual for Emotion To Emotion therapy. After that, a clinical expert Delphi was conducted to test the reliability as well as validity of the manual, through quantitative consensus on the manual of the Emotion To Emotion therapy. Results: First, as a result of literature studies, to date, studies related to Emotion To Emotion therapy have been qualitatively and quantitatively limited, as comparative literature related to clinical cases. Second, through expert FGI, the manual was structured with eight sub-factors for the indication diagnosis, six sub-factors for the implementation method, and 13 detailed factors. Third, through an expert Delphi, the consensus did the factor of indication, implementation methods, and implementation process, and developed a standardization manual for Emotion To Emotion therapy ver 1.0. Conclusions: Through literature analysis, expert FGI, and expert Delphi, the Emotion To Emotion therapy standardization manual ver 1.0 was completed, and will proceed with the revision and improvement report.

Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital (동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.
