Proposal of East-west Integrative Medicine Manual for Rehabilitation after Lumbar Spine Surgery (요추 수술 후 재활 치료에서의 한양방협진 매뉴얼 제안)

  • Objectives This is one of the manuals of East-west integrative medicine which was created by the Committee on integrative medicine of Chung-Yeon Korean Medicine Hospital. The purpose of this manual is to support clinical decision making and communication during the rehabilitation of patients after lumbar spine surgery. Methods The drafting was done by two rehabilitation specialists in Korean Medicine. After a rehabilitation specialist in western medicine reviewed the draft and exchanged their ideas, a revised version that reflects the goal of consultation was made. Then the Committee agreed to adopt the manuals through the process of review and feedback in addition to face-to-face discussions. Results This manual describes clinical decision making for rehabilitation after lumbar microdiskectomy, interbody fusion, and kyphoplasty. Therefore it contains the schedule of rehabilitation treatment through the surgical technique, general goal of the rehabilitation by phase, scales for outcome measure and postoperative infection management. Conclusions The proposal of this manual has a significance for it provides information about decision making process and contents of treatment in one medical institution for East-west integrative rehabilitation treatment after lumbar spine surgery.

The Effects of Complex Exercise Program with Visual Feedback on Navicular Bone Height, Plantar Pressure and Low Extremity Alignment in Flat-Footed Patients (시각적 피드백을 병행한 복합운동프로그램이 편평발 환자의 발배뼈 높이, 족저압 및 다리 정렬에 미치는 영향)

  • Purpose : The most effective intervention for flat foot is strengthening exercises for the intrinsic and extrinsic of the foot. Additionally, visual feedback is necessary for movement accuracy. However, the effectiveness of the intervention when combined with visual feedback was not revealed. To confirm this, the research was to investigate the effect of visual feedback and a complex exercise program on navicular bone height, plantar pressure, and lower extremity alignment. Methods : The twenty eight adult men and women with flat foot were randomly assigned to group 1 (n=14) and group2 (n=14), group1 performed complex exercises with visual feedback, and group 2 performed only complex exercises. Both groups performed a 40 minute compound exercise program three times a week. Navicular drop test, plantar pressure test, and lower extremity alignment test were performed equally in both group. Results : As a result of comparing the change in navicular height within the group according to the intervention, both groups showed a significant difference before and after the exercise (p>.05). There was not significant difference comparing the difference between the groups in the navicular height (p>.05). Comparing the change in plantar pressure within groups, there was not significant difference in the change in plantar pressure in both groups (p>.05). Coparing the difference before and after exercise between groups, there was not significant plantar pressure (p>.05). Comparing the change in leg alignment within the group, there was a significant difference in the change in ankle before and after exercise in group 1 (p<.05), but there was not significant difference in group 2. There was not significant difference in pelvic tilt and knee tilt before and after exercise in both groups (p>.05). Comparing the before and after exercise difference between groups, there were not significant in all variables of leg alignment (p>.05). Conclusion : The results of this study showed that complex exercise applied to patients with flat foot were effective in increasing the height of the navicular bone and ankle angle, but there was no effect due to visual feedback.

Structural Mechanism of Governmental Reformulation (정부조직 개편의 메커니즘에 관한 시론)

  • This paper explores a structural mechanism underlying governmental reformulation. Studying cases of governmental reorganizations occurred during past 60 years, this paper constructs a causal map containing a principle of small government, an integrative organization of the government, and a committee organization. A positive feedback loop linking these factors produces an increase in the number of the committee organization in the government. Also, this paper points out that there might be a hidden self fullfilling prophecy that reinforces the principle of small government. Open experiment toward a big government is proposed to overcome the vicious circle of small government curse.

A Pilot Study on the Tourism Behavior of Family Groups with Children at the Asan Hot Springs (유아·아동 동반 가족의 아산온천지구 관광형태에 관한 예비 연구)

  • Purpose: The Asan Hot Springs are adjacent to the Seoul Metropolitan area and have excellent traffic access. However, a steady decline in tourist numbers followed the decline in the urban population. This study aims to provide participants with a hot springs visit as part of their independent tourism experience and then analyze their feedback to obtain meaningful suggestions to enhance local tourism. Methods: We recruited 12 families interested in taking a two-day, one-night tour, including a visit to the Asan Hot Springs. Participating families were grouped into two cohorts (Type C and Type A) depending on whether they included children aged one to ten. Each family answered a survey regarding their overall satisfaction with the Asan Hot Springs and provided targeted feedback regarding hot spring resorts, accommodations, restaurants, adjacent tourist spots, and tourism costs. Results: Most of the families that participated in the study were residents of Seoul, Incheon, or Kyunggi (74.47%); the majority were two-generation families (75%) with children (75%). The Type C group enjoyed the campsite (55.56%) accommodations at the Asan Hot Springs more than their Type A counterparts. Families visited an average of 3.6 places during their stay, and 61.11% of participating families visited entertainment and tourist attractions, including the Type C visit to the thermal bath. Compared with Type A, Type C families spent more on tourism and were more satisfied with the entertainment and tourist attractions experience. Overall, all study participants were satisfied with the thermal sources. Conclusion: Our results indicate that families with children have a higher added value than visitor groups consisting only of adults. Considering the significant impact on the local economy, the Asan Hot Springs and associated tourist attractions and services should be developed with these visitors in mind.

Development of Complementary and Alternative Medicine Curriculum for Undergraduate Students at College of Oriental Medicine (한의과대학 학부생을 위한 보완대체의학 교육과정 개발 연구)

  • Objectives : Integrative medicine in Korea is the 21st century-style medical practice of two orthodox medical doctrines, traditional Korean medicine and western conventional medicine, as well as complementary and alternative medicine (CAM). CAM with scientific evidence should be incorporated in undergraduate curricula for the purpose of Korean integrative medicine. Methods : Items of detailed objectives, syllabi, textbooks, instructor's experiences, and effectiveness and reason for difficulty of the CAM curriculum for undergraduate students were analyzed and the preference of CAM therapies and others were also evaluated. Results and Discussion : The effectiveness of this CAM class curriculum was high (8.0$\pm$1.4) enough to be used in other Oriental medical colleges. Development of ability for self-study was rated as 7.0$\pm$1.7 and the helpfulness for clinical use was marked as 6.8$\pm$1.9. Students preferred placebo, Ayurveda, aromatherapy, yoga, functional food, bio-feedback and homeopathy. The difficulty degree was 7.2$\pm$1.6, and the amount of content was suggested as the major reason for it. We also found that this curriculum can be a model for self-oriented study and problem-based learning. Discussions were made for the improvement of the implemented CAM curriculum, which was shown to be very effective for the achievement of Korean integrative medicine. Conclusion : We have successfully installed a CAM curriculum for undergraduate students at the College of Oriental Medicine, and it can be used in others.

Methods: Drafting was based on literature review from the MEDLINE, EMBASE, OASIS, and CNKI databases using search terms such as 'mild cognitive impairment', 'mild neurocognitive disorder', 'acupuncture', 'herbal medicine' and 'traditional Chinese medicine'. An amendment reflecting the goal of consultation and detailed treatment contents was made by reviewing the draft and holding discussion with the rehabilitation specialist of western medicine. The committee then agreed to adopt the manual through the process of review and feedback in addition to face-to-face discussions. Results: This manual contains the diagnosis of mild cognitive impairment, the goal of consultation, and the cooperative treatment contents of the East-West medicine for mild cognitive impairment. Conclusions: This manual provides significant information about the decision making process and contents of treatment in one medical institution for East-West cooperative treatment of mild cognitive impairment.

Importance of End User's Feedback Seeking Behavior for Faithful Appropriation of Information Systems in Small and Medium Enterprises (중소기업 환경에서의 합목적적 정보시스템 활용을 위한 최종사용자 피드백 탐색행위의 중요성)

  • Small-and-medium sized enterprises(SMEs) represent quite a large proportion of the industry as a whole in terms of the number of enterprises or employees. However researches on information system so far have focused on large companies, probably because SMEs were not so active in introducing information systems as larger enterprises. SMEs are now increasingly bringing in information systems such as ERP(Enterprise Resource Planning Systems) and some of the companies already entered the stage of ongoing use. Accordingly, researches should deal with the use of information systems by SME s operating under different conditions from large companies. This study examined factors and mechanism inducing faithful appropriation of information systems, in particular integrative systems such as ERP, in view of individuals` active feedback-seeking behavior. There are three factors expected to affect end users` feedback-seeking behavior for faithful appropriation of information systems. They are management support, peer IT champ support, and IT staff support. The main focus of the study is on how these factors affect feedback-seeking behavior and whether the feedback-seeking behavior plays the role of mediator for realizing faithful appropriation of information systems by end users. To examine the research model and the hypotheses, this study employed an empirical method based on a field survey. The survey used measurements mostly employed and verified by previous researches, while some of the measurements had gone through minor modifications for the purpose of the study. The survey respondents are individual employees of SMEs that have been using ERP for one year or longer. To prevent common method bias, Task-Technology Fit items used as the control variable were made to be answered by different respondents. In total, 127 pairs of valid questionnaires were collected and used for the analysis. The PLS(Partial Least Squares) approach to structural equation modeling(PLS-Graph v.3.0) was used as our data analysis strategy because of its ability to model both formative and reflective latent constructs under small-and medium-size samples. The analysis shows Reliability, Construct Validity and Discriminant Validity are appropriate. The path analysis results are as follows; first, the more there is peer IT champ support, the more the end user is likely to show feedback-seeking behavior(path-coefficient=0.230, t=2.28, p<0.05). In other words, if colleagues proficient in information system use recognize the importance of their help, pass on what they have found to be an effective way of using the system or correct others' misuse, ordinary end users will be able to seek feedback on the faithfulness of their appropriation of information system without hesitation, because they know the convenience of getting help. Second, management support encourages ordinary end users to seek more feedback(path-coefficient=0.271, t=3.06, p<0.01) by affecting the end users' perceived value of feedback(path-coefficient=0.401, t=6.01, p<0.01). Management support is far more influential than other factors that when the management of an SME well understands the benefit of ERP, promotes its faithful appropriation and pays attention to employees' satisfaction with the system, employees will make deliberate efforts for faithful appropriation of the system. However, the third factor, IT staff support was found not to be conducive to feedback-seeking behavior from end users(path-coefficient=0.174, t=1.83). This is partly attributable to the fundamental reason that there is little support for end users from IT staff in SMEs. Even when IT staff provides support, end users may find it less important than that from coworkers more familiar with the end users' job. Meanwhile, the more end users seek feedback and attempt to find ways of faithful appropriation of information systems, the more likely the users will be able to deploy the system according to the purpose the system was originally meant for(path-coefficient=0.35, t=2.88, p<0.01). Finally, the mediation effect analysis confirmed the mediation effect of feedback-seeking behavior. By confirming the mediation effect of feedback-seeking behavior, this study draws attention to the importance of feedback-seeking behavior that has long been overlooked in research about information system use. This study also explores the factors that promote feedback-seeking behavior which in result could affect end user`s faithful appropriation of information systems. In addition, this study provides insight about which inducements or resources SMEs should offer to promote individual users' feedback-seeking behavior when formal and sufficient support from IT staff or an outside information system provider is hardly expected. As the study results show, under the business environment of SMEs, help from skilled colleagues and the management plays a critical role. Therefore, SMEs should seriously consider how to utilize skilled peer information system users, while the management should pay keen attention to end users and support them to make the most of information systems.

A Case Study Evaluating the Effectiveness of an Interdisciplinary Team-Based Integrated Medical Service Model in Improving the Quality of Life of a Fibromyalgia Patient and Caregiver through Psychological and Various Counseling Interventions (섬유근통 환자와 보호자의 삶의 질 증진을 위한 의·한 협진 기반의 통합의료서비스모델 적용평가 사례 연구: 심리 및 매체 상담개입을 중심으로)

  • Objectives: To evaluate the effectiveness of an integrated medical service model for a fibromyalgia patient and their caregiver, focusing on reducing pain and improving quality of life. Methods: A single-case study design was employed, involving a fibromyalgia patient and their primary caregiver treated at W University Hospital. The integrated medical service program, based on the Ministry of Health and Welfare's model, included medical consultations and complementary therapies such as psychological counseling, art therapy, music therapy, horticultural therapy, yoga, and meditation. The program was conducted weekly for 8 weeks, with each session lasting up to 100 minutes. Data collection involved both quantitative and qualitative assessments. Quantitative data included demographic surveys, psychological tests, health-related quality of life measures, pain indices, and sleep quality indices. Qualitative data were gathered through feedback evaluations and emotional assessments. Results: The patient showed improvements in mobility, self-care, daily activities, and anxiety/depression, with EQ-VAS scores increasing from 20 to 40 and pain perception decreasing from 67.41 to 42.58. The caregiver reported reduced anxiety/depression and an increase in EQ-VAS scores from 95 to 98. Both patient and caregiver exhibited emotional changes, with decreased depression and increased happiness. However, the patient showed an increase in fear and anger. Conclusions: The integrated medical service model positively impacted the emotional and psychological well-being of the fibromyalgia patient and their caregiver. Despite the limitations of a small sample size and a single-case study design, the findings suggest that an integrated approach can be beneficial. Larger-scale studies are needed to confirm and generalize these results.

The Effects of Craniocervical Flexion Exercise on Deep Cervical Flexor Muscle Thickness and Gait for Children with Hemiplegic Cerebral Palsy (머리-목 굽힘 운동이 편측 뇌성마비 환자의 깊은 목 굽힘근의 두께와 보행에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the effect of craniocervical flexion exercise on deep cervical flexor thickness and gait in children with hemiplegic cerebral palsy. Methods : Twelve children with hemiplegic cerebral palsy were recruited for this study. All subjects performed active craniocervical flexion exercise 3times a week over the course of 6weeks. using a pressure bio feedback unit. Ultrasonography was used to assess deep cervical flexor thickness, and a 10m walking test was used to assess gait function. For the statistical analysis, a paired t-test was used to compare the differences pre- and post-value. SPSS Statistics version 20.0 was used for statistical analysis, and statistical significance was defined as a p-value less than 0.05. Result : The results of this study indicate that children with hemiplegic cerebral palsy experienced statistically significant positive changes in both deep cervical flexor thickness (p<.05). and gait(p<.05) following the intervention. Conclusion : In conclusion, craniocervical flexion exercise can positively affect deep cervical flexor thickness in children with hemiplegic cerebral palsy, which in turn positively affects gait.

  • The purpose of the study is to realize and develop the healing process that applies the Forest Healing Program to complementary and alternative therapies within the integrated medical health care place call ' Healing Forest'. Various healing programs need to develope and distribute to educate tentatively name ' Forest Healing Instructor' whom will be the key roll player on the program. Hereupon, collected the feedback of the program from 20 of professors, health care professionals and complementary alternative related professionals who are deeply related on the dendrology using Delphi examine.
