Prioritization in the Policy of Integrated Medicine that Uses the Delphi Method and Analytic Hierarchy Process (델파이 기법과 AHP기법을 사용한 통합의료정책 우선순위 수립 연구)

  • The purpose of the study is to establish priorities in integrated medicine policy. Delphi method was used in conducting a survey on integrated medicine specialists regarding the policies of integrated medicine and actions. For specialists in integrated medicine, total 17 persons selected included scholars who performed minimum one study on integrated medicine, medical staff who performing integrated medical service at institutions that implemented integrated medical care or those who were involved in creating or implementing policies on integrated medicine. The first Delphi-method survey conducted from Apr. 14 through 18, while the second survey was carried out from Apr. 21 through 25. To prioritize the policies based on the results of the mini Delphi-method survey, Analytic Hierarchy Process(AHP) was performed for 15 persons answered to mini Delphi-method from May 2 through 16, 2014. From the Delphi-method survey, four policies and 16 actions were obtained. AHP showed the first priority placed on 'Building the Infrastructure of Industrialization in the Integrative medicine' of the 4 policies and 'Developing new diagnostic and treatment instruments' was given the highest priority of the 16 actions. For implementation of integrated medicine policy, the 4 policies and the 16 actions found in this research require government support and should be urgently implemented according to its priority.

  • We made a comparative study, A patient who had suffered dizziness and ataxia took Hyungbangsabaek-san(荊防瀉白散) at first and next took Hyungbangdojeock-san(荊防導赤散). He was diagnosed as a Soyangin suffering chest knotting syndrome(結胸證) by Sasang constitutional medicine(四象醫學) and took two kinds of herbs in a row. We observed his phase of dizziness and ataxia with the conditions of stool and urine excavation, pink eye, sweating, and tongue color and tongue coating. When he took Hyungbangdojeock-san these conditions improved more than when he took the other one and his dizziness and ataxia also improved. Hyungbangdojeock-san was more useful when the severity of the disease was shallower and he had more exterior syndromes than Hyungbangsabaek-san.

Anti-Platelet Aggregation Effect of Extract from Gamisopunghwalheol-tang in Vitro (가미소풍활혈탕의 혈소판 응집억제 작용에 대한 in vitro 연구)

  • The study was designed to test anti-platelet effect and find out anti-platelet mechanism of extract from Gamisopunghwalheol-tang in vitro. The extract was investigated for the inhibition against the aggregation of human platelet suspensions induced from collagen by aggregometer. And also the extract was investigated for the inhibition against the aggregation of human platelet suspensions who is taking aspirin or clopidogrel induced from collagen by aggregometer. In collagen-induced platelet aggregation test, the extract significantly inhibited collagen-induced platelet aggregation in a concentration-dependent manner(p<0.05). The extract significantly inhibited collagen-induced platelet aggregation of human platelet who is taking aspirin or clopidogrel ing 0 mg/ml concentration(p<0.05). And the extract inhibited more ingpatients who is taking aspirin. These results show that the extract from Gamisopunghwalheol-tang has anti-platelet aggregation effect.

Discriminant Model for Pattern Identifications in Stroke Patients Based on Pattern Diagnosis Processed by Oriental Physicians (전문가 변증과정을 반영한 중풍 변증 판별모형)

  • In spite of many studies on statistical model for pattern identifications (PIs), little attention has been paid to the complexity of pattern diagnosis processed by oriental physicians. The aim of this study is to develop a statistical diagnostic model which discriminates four PIs using multiple indicators in stroke. Clinical data were collected from 981 stroke patients and 516 data of which PIs were agreed by two independent physicians were included. Discriminant analysis was carried out using clinical indicators such as symptoms and signs which referred to pattern diagnosis, and applied to validation samples which contained all symptoms and signs manifested. Four Fischer's linear discriminant models were derived and their accuracy and prediction rates were 93.2% and 80.43%, respectively. It is important to consider the pattern diagnosis processed by oriental physicians in developing statistical model for PIs. The discriminant model developed in this study using multiple indicators is valid, and can be used in the clinical fields.

  • This study considers the stages of curriculum development for the integrated curriculum of Pusan National University Graduate School & Hospital of Korean Medicine, and specifically the KAS2021 (announced in 2019), improvement measures for the curriculum of the College of Traditional Korean Medicine, and the case of the College of Medicine. The introduction of integrated curriculum in the College of Traditional Korean Medicine starts from the members (doers)' agreement. In the process of development, the organization that represents the members, the organization that sets up a goal and designs the curriculum, and the organization that executes them should fulfill their own roles. The stage of development and operation should have the support system for manpower, institution, administration, and finance. The curriculum (draft) should be concrete enough to be operated in reality. For the smooth operation of integrated education, it is necessary to secure more full-time teachers than before, and it is also necessary to have an organization fully in charge of monitoring and improving the operation. For the introduction and operation of integrated curriculum in Traditional Korean Medicine education, the members' agreement, institutional change, support system, and the cultivation of manpower for the operation/evaluation/development of curriculum should be considered.

Weighting of Stroke Pattern Identification Using an AHP (AHP 기법을 이용한 중풍 변증지표의 가중치 설정)

  • In this study, we structuralized the diagnostic indices used for pattern identification (PI) of stroke, and suggested an AHP method to obtain the weights of PI indices. AHP of the subjects under consistency ratio 0.1 showed that the critical indices for stroke PI consists of 9 for Qi-deficiency, 13 for Phlegm/dampness, 7 for blood stagnation, 12 for Yin-deficiency and 16 for Fire/heat. Furthermore, AHP analysis rendered the weights of indices of each PI that will be useful for oriental medical experts to perform objective PI.

Slow Development of Diabetic Cataract in Streptozotocin-induced Diabetic Rats via Inhibition of Aldose Reductase Activity and Sorbitol Accumulation by Use of Aster koraiensis Extract (알도즈 환원효소 활성억제와 소비톨 축적 억제로 인한 벌개미취 추출물의 당뇨병성 백내장 발병 지연 효능)

  • Diabetic cataract is a major complication of diabetes mellitus. Excess accumulation of sorbitol plays an important role in the pathogenesis of diabetic complications such as cataract formation. In this study, we investigated the inhibitory effect of the extract of the aerial parts of Aster koraiensis (AK) on diabetic cataractogenesis. To examine this further, we evaluated sorbitol accumulation during cataract development using streptozotocin-induced diabetic rat, an animal model of type 1 diabetes. Diabetic rats were treated orally with AK (100 mg/kg and 200 mg/kg body weight) once a day orally for 9 weeks. In vehicle-treated diabetic rats, lens opacity was increased, and lens fiber swelling and membrane rupture were observed. In addition, sorbitol accumulation in diabetic lens was markedly enhanced. However, AK treatment delayed the progression of diabetic cataract through the inhibition of sorbitol accumulation, and prevented lens fiber degeneration in a dose-dependent manner. These observations suggest that AK treatment can delay the progression of lens opacification in the diabetic rats during the early diabetic cataractogenesis.

Development of an Integrated Education Model for Acupuncture and Physiology in Traditional Korean Medicine with Needling Practice and Pain Sensitivity (한의대 학부학생을 대상으로 한 침시술과 감각생리의 통합실습모델 개발)

  • Objective : To develop an integrated curriculum for acupuncture treatment and pain sensitivity practice, and discuss its usefulness. Method : We established an integrated practice program incorporating acupuncture and pain on physiology perspectives, and measured pain sensitivity at Yin (PC6${\sim}$PC4) and Yang (TE5${\sim}$TE9) meridians before and after the acupuncture treatment. Results : The Yang (39.4${\pm}$23.5) and Yin (46.0${\pm}$25.5) meridians have significantly different degrees of pain sensitivity (n=118, p=0.018). Pain sensitivity was significantly decreased after acupuncture treatment at Yang (54.63${\pm}$28.02/ 40.81${\pm}$29.39) and Yin (56.13${\pm}$27.88/ 38.72${\pm}$25.17) meridians (n=16, p<0.001). Discussion and Conclusion : The difference between the Yin and Yang meridians can be materialized in this integrated practice curriculum model. The effectiveness of this integrated class and suggestions for improvement of integrated curricula were discussed.

Comparison of Diagnostic Accuracy and Prediction Rate for between two Syndrome Differentiation Diagnosis Models (중풍 변증 모델에 의한 진단 정확률과 예측률 비교)

  • In spite of abundant clinical resources of stroke patients, the objective and logical data analyses or diagnostic systems were not established in oriental medicine. In the present study we tried to develop the statistical diagnostic tool discriminating the subtypes of oriental medicine diagnostic system, syndrome differentiation (SD). Discriminant analysis was carried out using clinical data collected from 1,478 stroke patients with the same subtypes diagnosed identically by two clinical experts with more than 3 year experiences. Numerical discriminant models were constructed using important 61 symptom and syndrome indices. Diagnostic accuracy and prediction rate of 5 SD subtypes: The overall diagnostic accuracy of 5 SD subtypes using 61 indices was 74.22%. According to subtypes, the diagnostic accuracy of "phlegm-dampness" was highest (82.84%), and followed by "qi-deficiency", "fire/heat", "static blood", and "yin-deficiency". On the other hand, the overall prediction rate was 67.12% and that of qi-deficiency was highest (73.75%). Diagnostic accuracy and prediction rate of 4 SD subtypes: The overall diagnostic accuracy and prediction rate of 4 SD subtypes except "static blood" were 75.06% and 71.63%, respectively. According to subtypes, the diagnostic accuracy and prediction rate was highest in the "phlegm-dampness" (82.84%) and qi-deficiency (81.69%), respectively. The statistical discriminant model of constructed using 4 SD subtypes, and 61 indices can be used in the field of oriental medicine contributing to the objectification of SD.

An integrated approach with homeopathic medicine and electro-acupuncture in anaesthesiology during breast cancer surgery: Case reports

  • This study investigates the effect of a combination of homeopathic medicine and electro- acupuncture in two patients with breast cancer and severe liver disease who could not receive standard anaesthesia therapy due to liver problems. Specifically, measurable and quantifiable parameters were used to evaluate whether an integrated approach-consisting of electro- acupuncture and a homeopathic medicine diluted above Avogadro's limit (that is, above a potency of 12CH) during the pre-surgical, surgical and post-surgical phases -can improve general well-being of a patient undergoing breast cancer surgery. In breast cancer surgery, we employed an integrated approach consisting of induction with hypnotics and muscle relaxants, followed by maintenance with anaesthetic gas, combined with a homeopathic treatment (Arnica montana 15CH and Apis mellifica 15CH) before and after surgery and an electro- acupuncture treatment performed in the pre- and post-surgical phases without any analgesic/pain relieving medications. Both of the patients treated with the integrated approach improved their overall condition without need for other common pain relieving medicines. Additionally, thanks to their rapid awakening, the patients were not relocated to a protected area and the hospitalization was shorter. A multidisciplinary approach incorporating homeopathic medicine and electro-acupuncture can be a solution for patients who need or ask about a different and/or safer alternative to the standard treatment. This approach can offer a safe, much less expensive, non-invasive and viable alternative for such cases. Moreover it can be useful for an opioids free anesthesia.
