Although the general concept of suffering care includes palliative care for the terminally ill that is designed to alleviate pain, it is much more holistic and encompasses emotional, spiritual and other life dimensions. Human suffering is multi-dimensional including spiritual and religious aspects, which diverge from the concept of pain understood in the context of materialistic medical approach. In this caring perspective, the body, mind and spirit are integrated so that objectivity and subjectivity can merge. The extended awareness with inner source or energy, and positive thinking about the personally-relevant God can be meaningful the dying person, family members and the caring team. Despite the importance of an inclusive understanding of human suffering, actual nursing practice still does not fully embrace the full understanding of human suffering. A more fundamental meaning of human suffering from the nursing perspective may fruitfully adopt a more inclusive view of human suffering.
Purpose: The purpose of this study was to develop an evidence-based dysphagia nursing care protocol for nursing home (NH) residents in Korea. Methods: The protocol was developed based on international guidelines and literatures. After testing content validity by experts, the protocol was applied to the intervention group (n=35) for 4 weeks at one NH in December, 2011, whereas the control group (n=34) received routine care. Results: The protocol was composed of one-page algorithm and detailed guidelines. Algorithm pathway was organized in 3 parts, including screening dysphagia risk, grouping by dysphagia risk level, and assigning nursing care into each group. The nursing care included positioning, oral care, meal time care, diet modification, providing exercise and maneuver, and checking dysphagia signs and symptoms. The experts verified the content validity. Protocol was revised to fit NH practice after the participant observation. Clinical validity was established upon evaluating usefulness, appropriateness, and convenience of the protocol by NH nurses. Dysphagia risk of the intervention group was significantly decreased. Conclusion: Developed protocol will improve the quality of dysphagia care in nursing homes as it can serve as a consistent and integrated standard for nursing care of residents with dysphagia.
Purpose: This study was to explore the postnatal care experiences among first time Chinese immigrant mothers living in Korea. Methods: Data were collected by focus group interviews with 12 first time Chinese immigrant mothers utilizing 3 focus groups. After obtaining written informed consent from all participants, each session of the focus group was audio-recorded and transcribed verbatim. Data were analyzed by content analysis to identify major themes. Results: Four major themes for postnatal care experiences were extracted from the data as follows. 1) Range over traditional, modern, Korean or Chinese postnatal care, 2) Struggle with newborn care, 3) Another struggle with breastfeeding, and 4) Difficulties in becoming a mother as an immigrant. Conclusion: Based on their postpartum care experiences, it is concluded that participants requested more professional practical and individualized education/training for successful breastfeeding and newborn care. Health care providers need to develop and implement integrated support and mentoring programs that include information and peer support system for the first time Chinese immigrant mothers living in Korea. This would decrease the difficulties of becoming a mother.
Purpose - With the increasing medical demands of the public, the development of future Internet medical care has come to represent a major problem. Therefore, the purpose of this study is to discuss future development strategies for Interne medical care while taking China's Internet hospitals as an example case. Research design, data, and methodology - This study conducted a case study of China's Internet hospitals to summarize the fundamental problems faced by Internet hospitals and propose future development strategies to overcome these problems for Internet medical care. Result - Although Internet hospitals have been regarded as the ultimate product of Internet medical care, from the perspective of the government, medical institutions, platforms builders and maintainers, and patients, they still face some basic issues. Conclusion - This study concludes that the government and medical institutions play an important role in the future development of Internet medical care and suggests that the government should make overall plans for the policies and standards and should play the main role in enhancing the public trust in Internet medical care, while medical institutions should take steps such as seizing policy opportunities, driving online and offline collaborations, and constructing suitable evaluation systems to promote the development of Internet medical care.
This paper describes the architecture and development of real-time monitoring system for ILRT (Integrated Leakage Rate Test). Object Oriented approach is used during the analysis and design. The architecture can be spited into two modules. First one is the Interface module, which, takes care of the communication between the server and the clients, and the second module is the Graphical User Interface (GUI) module, which takes care of the GUI in the web browser of the client. Bioinformatics is a rapidly emerging field of biomedical research. A flood of large-scale genomic and postgenomic data means that many of the challenges in biomedical research are now challenges in computational sciences. Clinical informatics has long developed methodologies to improve biomedical research and clinical care by integrating experimental and clinical information systems. The informatics revolutions both in bioinformatics and clinical informatics will eventually change the current practice of medicine, including diagnostics, therapeutics, and prognostics. Postgenome informatics, powered by high throughput technologies and genomic-scale databases, is likely to transform our biomedical understanding forever much the same way that biochemistry did a generation ago. The paper describes how these technologies will impact biomedical research and clinical care, emphasizing recent advances in biochip-based functional genomics and proteomics. Basic data preprocessing with normalization, primary pattern analysis, and machine learning algorithms will be presented. Use of integrated biochip informatics technologies, text mining of factual and literature databases, and integrated management of biomolecular databases will be discussed. Each step will be given with real examples in the context of clinical relevance. Issues of linking molecular genotype and clinical phenotype information will be discussed.
우리 사회의 급속한 변화 현상중의 하나를 선택한다면 아마도 급속한 고령화 인구의 증가라 할 수 있을 것이다. 고령화 인구의 증가는 다양한 사회적 문제들을 야기시키고 있다. 이와 관련하여 점점 중요성이 높아지면서, 해결해야 되는 문제가 의료정보 시스템일 것이다. 의료융합 산업이 관광산업의 한 축이 될 수 있는 환경에서는 의료정보 시스템의 중요성은 아무리 강조해도 지나침이 없다. 가까운 태국은 의료산업을 기반으로 한 관광산업의 대표적인 나라라 할 수 있다. 이러한 추세를 반영하여 관련 연구들이 활발히 이루어지고 있으며, 관련 응용 상품들도 개발되고 있다. 이에 본 논문에서는 IT와 의료산업의 융복합 형태인 통합 의료정보 시스템에 대한 현황 및 문제점, 향후 발전 방향등에 대해 고찰하였다.
Wound healing is a complex and dynamic process, making the accurate and timely assessment of skin wounds a crucial aspect of effective wound care management, especially for chronic wounds. Unlike conventional wound dressings that simply cover the wound area once some form of medicine is administered onto the wound, recent studies have introduced versatile approaches to smart wound dressings capable of interacting with wound fluids to monitor physicochemical and pathological parameters to determine the wound healing status. Such electrochemical wound dressings can be integrated with on-demand, closed-loop drug delivery or stimulation systems and ultimately expanded into an ideal technological platform for the prevention, treatment, and management of skin wounds or illnesses. This article briefly reviews the wound healing mechanism and recent strategies for effective wound care management. Specifically, this review discusses the following aspects of smart wound dressings: sensor-integrated smart bandages to detect wound biomarkers, smart bandages developed to accelerate wound healing, and wireless, closed-loop automatic (on-demand) wound healing systems. This review concludes by providing future perspectives on effective wound care management.
우리나라에서 통합적인 공공 복지전달체계 구축을 위한 시도는 통합사례관리까지 지속적으로 이루어지고 있고, 특히 최근 '커뮤니티 케어'와 관련해 중요성을 더해가고 있다. 하지만 대부분 부분적인 조직개편에 그치거나 민간자원의 동원에 그치고 있는 것이 현실이다. 이에 본 연구에서는 사회서비스 정책 발전에 있어서 통합적 개혁의 경험을 가지고 있는 영국의 보건의료와 사회서비스의 통합적 접근 사례, 우리나라 통합사례관리와 유사한 문제경험가족 프로그램 사례를 각각 런던 자치구와 지방의회를 방문하여 조사하였다. 그 결과 통합적 전달체계를 실현하기 위해서는 인력, 조직, 재정 등 제도적 장벽의 종합적인 해소가 시급하고, 관련된 전문 집단과 기관간의 분명한 목표 공유와 파트너십 구축이 필요하며, 사례에 대한 정보 공유 체계와 기술적 지원이 뒷받침되어야 함을 발견할 수 있었다.
