  • Isolated primary tricuspid insufficiency except Ebstein`s anomaly is rare disease. Recently we experienced two cases of isolated primary tricuspid insufficiency and treated successfully with annuloplasty or valve replacement. We could not classify our case as one of the classification of isolated tricuspid insufficiency, because the morphology and microscopic finding did not suggest any similar variety.

  • Aortic valve insufficiency with ventricular septal defect is usually treatment by plication of prolased cusps. We have performed the aortic valvuloplasty by the Trusler`s method in 14 patients of ventricular septal defect associated with aortic insufficiency. The reoperations were performed in the 4 cases[29%] due to recurrent aortic insufficiency after aotic valvuloplasty. Two patients underwent aoritc valvular replacements for the tears of repaired aortic cusps after primary aortic valvuloplasty for aortic insufficiency. The other two patients had aortic valvuloplasties again. Death occured in one case of aortic valvular replacements. The cause of death was low cardiac output syndrome soon after aortic valvular replacement for recurrent aortic insufficiency .

  • To investigate whether vision therapy (vision training) are an effective way to influence the near point of convergence, fusion range and asthenopic symptoms in convergence insufficiency. For one chosen patient who had convergence insufficiency, vision therapy (vision training) was prescribed for 4 weeks. 4 weeks later vision therapy (vision training) was quite effective in convergence insufficiency. That is, vision therapy (vision training) was an effective means of reducing symptoms, improving fusion range and decompensating exophoria in patient with convergence insufficiency.

  • Background: Serum 25-hydroxyvitamin D insufficiency is very common in Pakistan and is often related to inferior prognosis in some cancers but limited data exist for hematopoietic malignancies. This study was conducted to determine the vitamin D insufficiency in B-chronic lymphoid leukemia (CLL) cases at the time of presentation and its possible correlation with clinical staging, hematological parameters and biochemical markers. Materials and Methods: This descriptive cross sectional study was carried at Liaquat National Hospital from January 2011 to June 2013. Sixty patients with B-chronic lymphoid leukemia were enrolled. Complete blood count, vitamin D levels, serum urea, creatinine, uric acid and LDH levels were assessed. Data were compiled and analyzed using SPSS version 21. Results: Out of 60 patients, 42 (70%) were male and 18 (30%) were female. Mean age was $59.0{\pm}9.2years$. The frequency of vitamin D insufficiency was found to be 56.7%. Overall insufficiency was more frequently seen in male gender (40%). Vitamin D insufficiency demonstrated a positive association with low lactate dehydrogenase levels (P=0.005). No links were established with age, clinical stage, hematological and other biochemical markers. Conclusions: Vitamin D insufficiency is high compared with Western studies. Whether normalization of vitamin D insufficiency in deficient B-CLL patients could improve the clinical outcome or delay disease progression will require further studies.

A Case Report of Primary Ovarian Insufficiency Treated with Korean Medicine Treatment (한의치료로 호전된 조기난소부전 환자 치험 1례 보고)

  • Objectives: The purpose of this study is to report the case of Korean medicine treatment on primary ovarian insufficiency. Methods: The patient in this case was 29-year-old female who was diagnosed with primary ovarian insufficiency. She had irregular menstruation and hypomenorrhea more than 6 months. She also suffered from hot flash and vaginal dryness. She was treated over 1 year with Korean medicine treatment, such as herbal medicine, acupuncture, and pharmacopuncture. We assessed the clinical symptoms, menstrual status and serum hormone level during the treatment. Results: After treatment, symptoms of primary ovarian insufficiency were relieved, level of serum FSH decreased and level of serum E2 increased. We maintained the treatment over 1 year and kept follow-up measurements of serum hormone level. Conclusions: This study shows that Korean medicine treatment can be effective in treating primary ovarian insufficiency. The report suggests the long treatment procedure for primary ovarian insufficiency.

  • Sacral insufficiency fractures are usually known to develop in elderly patients with osteoporosis without definite trauma history. It is difficult to diagnose the sacral insufficiency fracture at an early stage because lower lumbar diseases. concurrently or not, may also be presented with similar symptoms and signs. We report a rare case of sacral insufficiency fracture who was not diagnosed initially but, instead, showed progressively worsening of clinical symptoms and radiological findings after decompression surgery for upper level lumbar stenosis.

  • Background: The results of treatment in functional velopharyngeal insufficiency (VPI) was not good compared to physician's common practice. Objectives: Authors conducted this study to evaluate the efficacy of fat injection on posterior pharyngeal wall in the functional velopharyngeal insufficiency as the supportive treatment. Materials and Methods: The preoperative assessment includes history of patients, the perceptual analysis of patient's voice, nasopharyngoscopic finding of velopharyngeal movements, nasometer, movement findings of soft palate during phonation and swalling. Fat which was taken from umbilical area was injected in 5 patients with conducted functional velopharyngeal insufficiency. Results: All 5 patients had good results in voice quality after fat injection. Conclusions: Fat injection is a good treatment method in functional velopharyngeal insufficiency as a supportive method.

  • We report a case of a healthy 38-year-old man presenting with insufficiency fracture of proximal tibia associated with unrecognized osteoporosis. Radiologic bone DEXA (Dual Energy X-ray Absorptiometry) assessment revealed osteoporosis, but the patient displayed no apparent risk factors for insufficiency fractures and osteoporosis except for a history of smoking. Following right proximal tibial open reduction and internal fixation, the patient commenced smoking cessation and began taking alendronate. The patient was treated for gait rehabilitation, which included quadriceps strengthening exercises and his condition improved with independent walking. This report highlights an unusual site of proximal tibial insufficiency fractures and the potential effect of smoking on low bone mineral density in a young male patient.

Adrenal insufficiency development during chemotherapy plus anti-programmed death receptor-1 monoclonal antibody (tislelizumab) therapy in patients with advanced gastric cancer: two case reports

  • Immune checkpoint inhibitor (ICI)-associated adrenal insufficiency is rare but may become a serious adverse event in patients treated with ICIs. The present case report documents two cases of adrenal insufficiency developed during chemotherapy plus tislelizumab (百泽安, Baize'an; BeiGene Ltd.) therapy in patients with advanced gastric cancer. Adrenal insufficiency developed after 6 and 13 cycles of treatment and was well controlled with hydrocortisone. The patients also developed hypothyroidism, which was managed with levothyroxine. Two patients showed a partial response, and one patient out of two achieved a near-complete response, sustaining over 11 months. Increased awareness of ICI-related adrenal insufficiency is crucial for early detection and prompt management of patients treated with ICIs.

Risk factor for pituitary dysfunction in children and adolescents with Rathke's cleft cysts

  • Purpose: This study evaluated the clinical manifestations of and risk factors for pituitary insufficiency in children and adolescents with Rathke's cleft cysts. Methods: Forty-four patients with Rathke's cleft cysts younger than 19 years who visited Seoul National University Children's Hospital between January 1995 and September 2009 were enrolled. Rathke's cleft cysts were confirmed histologically through an operation in 15 patients and by brain magnetic resonance imaging (MRI) in 29 patients. The clinical, hormonal, and imaging features were reviewed retrospectively. Results: The clinical presentation of symptomatic patients was as follows: headache (65%), endocrinopathy (61%), and visual disturbance (19%). Endocrinopathy included central precocious puberty (18%), diabetes insipidus (14%), general weakness (11%), and decreased growth velocity (7%). After surgery, hyperprolactinemia resolved in all patients, but growth hormone insufficiency, hypothyroidism, and diabetes insipidus did not improve. Pituitary insufficiency except gonadotropin abnormality correlated significantly with severe headache, visual disturbance, general weakness, and cystic size. Suprasellar extension of cysts and high signals in the T2-weighted image on brain MRI were related to hypothyroidism, hypocortisolism, and diabetes insipidus. Multivariable linear regression analysis showed that only general weakness was a risk factor for pituitary insufficiency ($R^2$=0.549). Conclusion: General weakness is a risk factor for pituitary insufficiency in patients with Rathke's cleft cysts. When a patient with a Rathke's cleft cyst complains of general weakness, the clinician should evaluate pituitary function and consider surgical treatment.
