만성 발목 불안정증의 치료에서 관절경 수술 (Arthroscopic Procedure in the Treatment of Chronic Lateral Ankle Instability)

  • The open Broström procedure is considered the surgery of choice for treating chronic lateral ankle instability. The role of arthroscopy has gained popularity in the surgical treatment of chronic lateral ankle instability, partly for the ability to manage the intra-articular pathology combined with ankle instability. Arthroscopic techniques can be divided broadly into the arthroscopic-assisted Broström technique and arthroscopic all-inside ligament repair. The clinical results of these arthroscopic techniques are similar to open procedures. The arthroscopic technique may be an excellent alternative to the open Broström procedure in treating chronic lateral ankle instability when applying the appropriate indications.

  • We report on the case of a patient with chronic instability of the ball-and-socket ankle joint. The patient, a 21-year-old male, was diagnosed 10 years previously with chronic instability of the ball-and-socket ankle joint. He underwent Broström procedure and augmentation using a periosteal turn down from the fibula for this chronic instability despite having received conservative treatment since the diagnosis. After the procedure, the clinical symptoms of ankle instability were improved and the patient is being periodically followed-up. We report on this case of using a Broström procedure as a treatment option for patients with instability of the ball-and-socket ankle joint with normal range of foot and normal ankle joint alignment without damage in the joint and cartilage.

  • PURPOSE: This study examined the comparative effects of an ankle sensorimotor training program combined with hip strengthening exercise (ASTPCHSE) and ankle sensorimotor training program (ASTP) alone on muscle strength, static balance, and dynamic balance in individuals with functional ankle instability. METHODS: Sixteen research participants with functional ankle instability were enrolled in this study. The participants were divided randomly into the ankle sensory motor training program group and the ankle sensory motor training program combined with the hip strengthening exercise group. Each group performed a series of exercise programs two times per week for four weeks. The Cumberland ankle instability tool (CAIT) was used to measure the participants' functional ankle instability. A Balance trainer 4 was applied to assess the static and dynamic balance, and a Primus RS multimodal dynamometer was used to evaluate the muscle strength. RESULTS: No significant differences in static balance, dynamic balance, and muscle strength were found between the ASTP and ASTPCHSE groups (p > .05). On the other hand, the dynamic balance and muscle strength improved in the ASTP and ASTPCHSE groups after the intervention (p < .05). The static balance was not enhanced in both groups after the intervention (p > .05). CONCLUSION: Ankle exercise and an ankle sensorimotor training program combined with hip strengthening exercise are effective in improving muscle strength and dynamic balance in individuals with ankle instability. On the other hand, there are no meaningful differences between ankle exercise and ankle and hip combined exercise.

  • Severe ankle sprain secondarily leads to chronic lateral ankle instability in 20-30%. Many surgical procedures have been presented for reconstruction of the lateral ankle instability, but controversy still remains for the ideal surgical option. Br$\ddot{o}$strom procedure or its modifications have been widely used but they have some limitations for the instabilities for the over-weight, physically high demanding patients and especially for significantly deficient or attenuated ligaments. Moreover the tenodesis procedures using peroneus brevis tendon are non-anatomical and sacrifice important lateral stabilizing tendon. Therefore recently, many reconstructive procedures for lateral ankle instability utilizing free allograft or autograft tendon have been introduced to anatomically stabilize the lateral ligaments to various degrees.

  • Acute ankle sprain is the most common injury in the lower extremities, and approximately 10% to 40% of acute lateral ankle ligament injury causes chronic pain or instability. For chronic symptoms lasting after an acute sprain, the possibility of joint damage, such as bony structures, ligaments, cartilage, and nerves around the ankle joint, should be considered. Patients with chronic lateral ankle instability usually complain of repeated sprains or giving way sensations. There has been steady progress in the treatment options until recently, however new treatments are still being attempted. This paper describes the causes, diagnosis, and recent trends in the conservative and operative treatment of chronic lateral ankle instability.

  • Chronic lateral ankle instability occurs in 10% to 20% of individuals after acute ankle sprain. The management of chronic lateral ankle instability is traditionally conservative treatment in the acute phase. On the other hand, surgical intervention is considered if conservative treatment fails and the symptoms are ongoing. This review focuses on the surgical approaches to treatment of chronic lateral ankle instability, including $Brostr{\ddot{o}}m$ surgical techniques, with a review of the traditional procedure and newer techniques.

심상 기법을 이용한 단축발 운동이 만성 발목 불안정성에 미치는 효과 (Effect of Short Foot Exercise Using an Imagery Technique on Chronic Ankle Instability)

  • Purpose : This study was aimed at investgating the effect of short foot exercise(SFE) using an imagery technique on chronic ankle instability(CAI). Methods : Twenty subjects with chronic ankle instability were selected by using the Cumberland ankle instability tool. They were randomly assigned to one of two groups(10 in each group) : namely the short foot exercise group and imagery technique group. The short foot exercise group performed short foot exercise for 6 weeks(3 times a week, 15 min per session). The imagery technique group performed short foot exercise(3 times a week, 15 min per session) and the imagery technique(3 times a week, 5 min per session) for 6 weeks. We measured chronic ankle instability, balance and threshold of vibration sensation before and after exercise by using the Cumberland ankle instability tool, Biodex Balance System and VSA-II, respectively. Results : We found statistically significant differences in cumberland ankle instability tool, balance and threshold of vibration sensation between the groups(p<.05). Conclusion : We confirmed the effect of short foot exercise using an imagery technique. Thus, we thought these results could be used as basic data and reference for musculoskeletal therapy or intervention using an imagery technique.

Effect of Ankle Taping Type and Jump Height on Balance during Jump Landing in Chronic Ankle Instability

  • Background: Chronic ankle instability is a common injury that decreases balance and negatively affects functional movements, such as jumping and landing. Objectives: To analyze the effect of taping types and jump heights on balance with eyes open and closed during jump landings in chronic ankle instability. Design: Within-subject design. Methods: The study involved 22 patients with chronic ankle instability. They performed both double-leg and single-leg drop jump landings using three conditions (elastic taping, non-elastic taping, and barefoot) on three different jump platforms (30, 38, and 46 cm). Balance was measured using the Romberg's test with eyes open and closed. Results: Interaction effect was not statistically significant. Balance with eyes open and closed was significantly improved in both the elastic taping and non-elastic taping conditions compared to the barefoot condition. There was no significant difference according to the jump height. Conclusion: Individuals with chronic ankle instability demonstrated increased balance ability with eyes open and closed when jump landing. Elastic taping and non-elastic taping on the ankle joint can positively affect balance during landing in individuals with chronic ankle instability.

  • Purpose: One of the main contributors to proximal fifth metatarsal fracture is ankle inversion and the incidence of recurrence may increase in patients with ankle instability. So, the authors confirmed the patients of proximal fifth metatarsal fracture with ankle instability by checking the history and magnetic resonance imaging (MRI) and assessed the value of MRI as therapeutic prognosis and clinical indicators for prevention of recurrence. Materials and Methods: Patients with proximal fifth metatarsal fractures visited our hospital during recent five years were reviewed. 35 patients with suspected damage by ankle inversion had been identified a history of ankle instability and checked the hindfoot malalignment through hindfoot alignment view and MRI was performed prospectively. The patients was devided to three groups on the location of fracture site and the groups were compared each other. Results: The mean time from injury to checking MRI was 10.7 days. There was no structural abnormality and was no significant difference according to the location of fracture. The patients with history of ankle inversion were 31(88.6%) and the patients with history of chronic or recurrent injury were 22 patients (62.9%). The lesion of MRI related to lateral ankle instability were identified in all patients. Conclusion: This study noted a high incidence of lateral ankle instability that was identified by MRI in the patients of proximal fifth metatarsal fracture. Aggressive treatment for lateral ankle instability should be needed for complications as proximal fifth metatarsal fracture to reduce the recurrence and occurrence.

The Effects of Neuromuscular Training of Ballet Dancers with Chronic Ankle Instability on Ankle Stability and Posture Control Ability

  • Objective: The purpose of this study was to investigate the effects of neuromuscular training of ballet dancers with chronic ankle instability on ankle stability and posture controlability. Design: A randomized controlled trial. Methods: A total of Thirty-twoyoung female ballet dancers with chronic ankle instability have voluntarily participated in the study. The participants were randomized to the neuromuscular training group(n1=11), elastic resistance squat group(n2=11), and control group(n3=10). The intervention was applied to a total of 18 exercises per six weeks and conducted in each group for one hour a day, three times a week. All subjects were evaluated for Cumberland ankle instability tool(CAIT), static balance at before-after intervention. Results: In the comparison of the effects within the groups, the effect of the intervention on CAIT and posture control was significantly increased in the neuromuscular training group and elastic resistance squat group (p <0.05). In the comparison of the effects between the groups, the neuromuscular training group, and elastic resistance squat group were found to significantly increase more than the control group (p <0.05). The effect of ankle stability is similar in neuromuscular training and elastic resistance squat training, neuromuscular training is more effective in improving posture control rather than elastic resistance squat training. Conclusions: When planning a rehabilitation training program for a ballet dancer with chronic ankle instability, neuromuscular training can be applied as an intervention method to improve ankle stability and posture control ability.
