Relationship between Organizational Culture Types and Organizational Effectiveness in Hospitals (병원의 조직문화유형과 조직유효성의 관계;간호사를 대상으로)

  • The purpose of this study is to investigate the relationship between organizational culture types and organizational effectiveness in the hospitals and to identify the cultural and organizational characteristics of the hospitals with high organizational effectiveness. Data were collected from May 12 to June 14. 1997 through questionnaire taken by 1.118 nurses working in 10 hospitals with more than 800 beds and from annual reports published by the hospitals. The instruments were used for collecting the data: Organizational Culture Questionnatire and Organizational Characteristics Questionnatire developed by the researcher. Mowday's Organizational Commitment Questionnaire. Taylor & Bovver's General Satisfaction Scale. The Results were as follows: 1. The meta culture of the hospital organizations was the conservative culture. 2. There were significant differences of the four organizational cultural types - affiliative culture. innovative culture. conservative culture. task culture among the hospitals(p=.00). 3. The hospital organizations were classified in to three cultural patterns. each of which had similar cultural composition. on the basis of the scores indicating the similarity and difference of the foul' organizational cultural types among the hospitals. The organization of each group represents conservative- dominant culture. innovative-dominant culture and competitive culture. 4. Nurses' organizational commitment and job satisfaction were significantly different among the cultural patterns(p=.00). In other words. the hospitals with innovative-dominant culture showed higher organizational commitment and job satisfaction than ones with conservative-dominant culture and competitive culture. And also. the growth rate of outpatients and inpatients were significantly different among the cultural patterns(p<.05). The hospitals with innovative -dominant culture showed higher growth rate of outpatients and inpatients than ones with conservative-dominant culture and competitive culture. 5. The hospitals with conservative-dominant culture and competitive culture showed higher level of centralization than ones with innovative -dominant culture(p=.00) And the hospitals with competitive and innovative-dominant culture showed higher level of communication than those with conservative-dominant culture(p=.00) Finally. the hospitals with innovative-dominant culture showed higher level of managerial strategy than those with conservative-dominant and competitive culture. among which the latter showed higher level of managerial strategy than the former(p=.00).

A Study of Stroke Patients' Leisure Activities and Quality of Life

  • This study was conducted in order to examine relationship between stroke patients' degree of satisfaction with leisure and quality of life according to their leisure activity types and whether they conducted leisure activities. A direct survey was performed from April 8 to May 3, 2013 on 92 inpatients and outpatients who received occupational therapy at hospitals located in Jeonju and Gunsan. A frequency analysis was carried out in order to look at the general characteristics of the subjects, their degree of participation in leisure activities, and their degree of satisfaction with leisure activities. A Pearson's coefficient was used to examine relationship between their degree of satisfaction and quality of life according to participation in leisure activities. Their degree of satisfaction according to participation in leisure activities was significantly higher when they took part in leisure activities such as handicraft activities, sports activities, or travel or tourism activities than when they did not perform any leisure activities. Their quality of life according to participation in leisure activities was higher when they conducted sports activities, outing activities, or tourism activities than when they did not. There was significant correlation between their degree of satisfaction with leisure and quality of life according to leisure activities.

Influence of the Nursing Practice Environment on Job Satisfaction and Turnover Intention

  • Objectives: To examine whether the nursing practice environment at the hospital-level affects the job satisfaction and turnover intention of hospital nurses. Methods: Among the 11 731 nurses who participated in the Korea Health and Medical Workers' Union's educational program, 5654 responded to our survey. Data from 3096 nurses working in 185 general inpatient wards at 60 hospitals were analyzed using multilevel logistic regression modeling. Results: Having a standardized nursing process (odds ratio [OR], 4.21; p<0.001), adequate nurse staffing (OR, 4.21; p<0.01), and good doctor-nurse relationship (OR, 4.15; p<0.01), which are hospital-level variables based on the Korean General Inpatients Unit Nursing Work Index (KGU-NWI), were significantly related to nurses' job satisfaction. However, no hospital-level variable from the KGU-NWI was significantly related to nurses' turnover intention. Conclusions: Favorable nursing practice environments are associated with job satisfaction among nurses. In particular, having a standardized nursing process, adequate nurse staffing, and good doctor-nurse relationship were found to positively influence nurses' job satisfaction. However, the nursing practice environment was not related to nurses' turnover intention.

Evaluation of Patient-Centered Healthcare Provision in Hospitals and General Hospitals- Based on Patient Experience Assessment (병원과 종합병원의 환자중심 의료서비스 제공 수준 평가- 환자경험평가를 중심으로)

  • Objectives: The purpose of this study is to evaluate patient experience assessment of inpatients, and to prepare measures to improve the quality level of medical services and guarante patient rights. Methods: The study was conducted among 199 patients admitted to hospitals and general hospitals in the metropolitan area. The analysis method used was crossover analysis, including a comparison of means, and logistic regression analysis. Results: The overall average score of satisfaction with healthcare service was 3.39 for nurses, 3.35 for hospitals and 3.42 for general hospitals. Age at the time of hospitalization affected satisfaction. The overall average score of healthcare service satisfaction was 3.09 for doctors, 3.14 for hospitals, and 3.04 for general hospitals. The factors affecting hospital satisfaction were gender and subjective health status. The factors affecting satisfaction in general hospitals were education, medical department, and hospitalization route. Conclusions: Hospitals should also introduce a systematic management system of general hospitals and strengthen the guarantee of the rights of patients who can improve the quality of medical care through positive communication between medical personnel and patients.

A study on quality of handicapped inpatients' service satisfaction at special dental clinic and their transformed perceptions toward to the dental treatment services (일부 복지관 장애인치과 내원 환자들의 서비스 만족도 및 진료 후 변화 인식도 연구)

  • Objectives : The purpose of this study is about discovering the basic references to find the ways to vitalize handicapped dental clinics. Methods : The study was analyzed by the satisfactions of those patients who took the advantages of using the dental care at B-welfare center and also their transformed perceptions after the services are influenced. The analysis was the questionnaire consisting of 100 items and survey data. Results : 1. The satisfaction of the dental service for the disabled was respectively high which was the average of 4.49. 2. Their satisfactions of receiving kindness services by volunteers and employees were the highest in the entire research of successful handicapped dental services and it was 4.78 overall. 3. The satisfaction of using handicapped dental clinic has the higher range of female users than male's. In the mean time treatment details of the dental care and the satisfaction towards to the volunteers and faculty at the center show the statistical significance gap. 4. The oral care service after experiencing the dental clinic for handicapped relived their discomforts of using the regular dental clinic which shows their highest satisfaction as it is the point of 4.75. 5. the change perception after dental treatment for handicapped has the higher range of females than men's and solving the problems of mouth reference and discomfort of using regular clinics show the statistical significance gap. 6. In the change perception after having dental treatment for handicapped the thought of the possibility of periodical dental care shows the highest perception when the number of visiting is usually shorter and it shows the statistical significance. Conclusions : According to the satisfaction of those inpatients who use free dental care services that belong to dental clinics for handicapped in a part of Seoul welfare centers human services were appeared as the most important factor due to their advantages of taking services from volunteers and staff members. On the other hand to enhance the medical treatment information and environment which showed the weakest factors each inpatient should be specifically specialized for their needs and also further study on plans which enhance their perceptions toward to a better quality of oral-related life is required after using dental treatment service.

The Impact of Health Service Quality Factors on Patient's Satisfaction According to Duration of Hospitalization (입원기간에 따른 의료서비스 품질요인이 고객만족에 미치는 영향에 관한 연구)

  • The purpose of this study is to identify which factors of medical service quality were considered important by patients based on survey on eight hospitals located in Seoul. We analyzed the difference in impact of health service quality factors between the two groups with short and long inpatient days. In addition, we analyzed which service factors affected the customer satisfaction more differently across the inpatient groups. We found that patient satisfaction was not different between the two groups with short and long inpatient days. But the two groups showed different priority orders of medical service factors that affect patient satisfaction. In the group with long inpatient days, the order of medical service factors are cleanness, speciality, process. In the group with short stay, important factors are speciality, cleanness and convenience of facilities. These findings imply that hospitals should set up different marketing strategies across the different inpatients groups.

The Effect of Foreign Patients' Expectation and Experience on the Satisfaction of Medical service (외국인 환자의 기대수준과 경험수준이 의료서비스 만족도에 미치는 영향)

  • This research tried to understand the expectation and experience level of the medical service among the foreign patients who visit South Korea for a medical tourism, and to find out the effect of expectation and experience level on the satisfaction of medical service. Data collection was done through conducting a survey of inpatients in 4 hospitals located in Seoul, Korea. Total 110 cases were used for analysis. According to the study, expectation level had a significant difference between the group exposed to a word-of-mouth and the group not exposed to the word-of-mouth. The difference analysis of expectation and experience level of the foreign patients showed that all components of the service quality were satisfied. The component of perceived reliability was the only factor influencing the foreign patients' overall satisfaction on the medical service. The findings of this study will be provided a basic data for the marketing strategy which can apply to the satisfaction strategy differentiated from other countries.

Survey of Satisfaction with Oriental Medical Care for Traffic Accident Patients ; 35 Cases Report (교통사고 환자 35예에 대한 한방의료 서비스 만족도 조사)

  • Objectives : This study aimed to understand the satisfaction degree of patients due to traffic accident(TA) in order to provide basic data which would give help not only to improve the quality of services, but also to strengthen the competitiveness of oriental medical hospitals on sequela of TA patients. Methods : The data were collected from inpatients and outpatients who answered a self-report questionnaire. Independent t-test, analysis of variance(ANOVA), $X^2$-test, correlation analysis were used for data analysis. Results : According to the research on medical satisfaction degree, the overall satisfaction degree was 4.141. On each field, satisfaction degree related to the doctor service recorded the highest degree among others(4.474). Satisfaction degree related to the appropriateness of service was the second(4.091). On the other hand, satisfaction degree of using procedure was the lowest degree(3.857). On the difference of degrees of satisfaction by general characteristics, patients who were unemployed houseworker group showed lower doctor service satisfaction degree than that of other groups. However, on the difference of degrees of satisfaction related to other general characteristics, there were no significant differences(p<0.05). Conclusions : The service distribution of oriental medical hospital was almost equal without concentrating on specified social classes.

Patient satisfaction and recommendation intention at spine and joint hospital (척추·관절 전문병원의 환자 만족도와 추천 의향)

  • This study was to analyze the satisfaction and intention to recommend a hospital for spinal and joint patients. Using a structured questionnaire at a spinal and joint hospital in Seoul in 2019, and the relationship between demographic and social characteristics and patient satisfaction, factors influencing hospital satisfaction, and recommendation intention were analyzed. In outpatients, hospital satisfaction was higher in neatness and cleanliness of staff, kindness and facility environment comfort, and hospital satisfaction was low in questions related to waiting time. In inpatients, women's satisfaction was higher in the nursing and hospital environment areas. The correlation analysis between the satisfaction and the willing to recommend, there was a positive correlation. In the sub-analysis, hospital satisfaction was higher in the group that responded with respect and courtesy, listening, satisfaction of the nurse, hospital cleanliness and safety. This study is can be used to improve the quality of hospital care services in related hospitals.

Study for Improvement of the Doctor's Satisfaction and Completeness of the Medical Record in the EMR System (전자의무기록(EMR) 시스템하에서 의사의 만족도와 의무기록정보의 기재 충실도 향상 방안)

  • This study aims to present ways to enhance the stabilization of electronic medical records, ensure the commitment to filling in information of the medical record and improve the overall quality Electronic Medical Record(EMR) information. For that purpose, the present state of the incomplete record rate and the doctor's satisfaction in Electronic Medical Record(EMR) have been surveyed by comparing and analyzing Paper-based Medical Record(PMR) and Electronic Medical Record(EMR). The survey was conducted on 31 doctors in charge of EMR system and each PMR and EMR inpatients were collected for a period of 5 months and analyzed. The results showed that the doctor's satisfaction level was higher for EMR, and the rate of incomplete record appeared to be lower in EMR in departments of both internal and external medicine. In this context, it can be said that the higher efficiency of EMR helped accomplish the increase in commitment to completing medical record information and improve the quality of the data.
