족관절 골관절염에서의 관절 내 Hyaluronic Acid 주사의 유용성 (The Efficacy of Intra-Articular Hyaluronic Acid Injections in Ankle Osteoarthritis)

  • Purpose: This study sought to investigate the appropriate indications for intra-articular injection of hyaluronic acid in ankle osteoarthritis, its efficacy at each stage of osteoarthritis and to compare the efficacy related differences at each stage. Materials and Methods: A retrospective study was conducted from January 2016 to September 2019 on 43 patients (50 cases) diagnosed with ankle arthritis and given intra-articular injection of hyaluronic acid. Patients were classified according to the modified Takakura ankle osteoarthritis stages. Stage 2 comprised 24 cases; stage 3A, 15 cases; stage 3B, 8 cases; and stage 4, 3 cases. The clinical evaluation was carried out using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) score, and the Foot and Ankle Outcome Score (FAOS). The patients' satisfaction with the injection was evaluated at 1 year after injection. A radiographic evaluation was performed to gauge the progression of osteoarthritis before and after the injection. Results: The VAS, AOFAS score, and FAOS at 3 months and 1 year after injection showed statistically significant improvements compared to those before the injection. There was a significant difference in the VAS for each stage over time; however, this was not the case with the AOFAS score and the FAOS. A significant improvement of the VAS after injection was seen only in stage 3A when compared with the other stages. The overall satisfaction rate with the injection was 90%. There were no ankle osteoarthritic stage changes in the serial follow-up radiograph. Conclusion: Intra-articular injection of hyaluronic acid resulted in a significant clinical improvement up to 1 year after the injection. Therefore, the intra-articular injection of hyaluronic acid could be a treatment option for ankle osteoarthritis.

Effects of Gas Injection on the Heating Performance of a Two-Stage Heat Pump Using a Twin Rotary Compressor with Refrigerant Charge Amount

  • For heat pumps used in a cold region, it is very important to obtain appropriate heating capacity. Several studies using a variable speed compressor and an additional heater have been performed to enhance heating capacity at low ambient temperatures. However, for outdoor temperature conditions below $-15^{\circ}C$, it is still difficult to obtain enough heating capacity above the rated value. In recent studies, the application of gas injection technique into a two-stage heat pump yielded noticeable heating performance improvement at low temperature conditions. In this study, the heating performance of a two-stage gas injection heat pump with a rated capacity of 3.5 kW was measured and analyzed by varying refrigerant charge amount and EEV opening at the standard heating condition. The heating performance of the two-stage gas injection heat pump was compared with that of a two-stage non-injection heat pump. The heating capacity and COP of the two-stage gas injection heat pump were improved by 2-10% at the optimal charging condition over those of the two-stage non-injection heat pump.

  • Potential advantages of using vapor injection in a two stage rotary compressor for a $CO_2$ heat pump water heater system were addressed in this paper by numerical simulation. Vapor separated from a flash tank in the middle of the expansion process can be used for injection into the second stage suction plenum of the compressor to improve the system performance. Vapor injection increases the intermediate pressure between the two stages, thus increasing the first stage compressor work and reducing that of the second stage. As a whole, however, the compressor input power increases due to injected mass flow rate for the second stage. Computer simulation showed that increment of the cooling capacity by vapor injection exceeded that of the compressor work, thus improving the system performance. COP improvement by vapor injection was calculated to be about 5-14% for normal operating conditions. With vapor injection, a maximum COP was found when the displacement volume of the second stage becomes 90-95% of that of the first stage of the compressor.

2단 분사 방식을 적용한 디젤 DI-HCCI 연소특성에 관한 연구 (A Study on the Diesel DI-HCCI Combustion Characteristics using 2-stage Injection Method)

  • In this study, combustion characteristics and various performances of a Diesel fuel DI-HCCI engine using 2-stage injection method were investigated. From these researches, application ability of 2-stage injection strategy to a DI-HCCI engine was confirmed and improvement methods of performances were considered. As the results, Using 2-stage injection method, without change of engine specifications and loss of IMEP, exhaust of NOx and Smoke emissions could be reduced to about 1/3 (at 1400rpm, IMEP 6bar) compared to conventional Diesel combustion.

RCCI 엔진의 디젤 분사 파라미터에 따른 연소 및 배출가스 특성에 대한 수치적 연구 (Numerical Study on the Effect of Diesel Injection Parameters on Combustion and Emission Characteristics in RCCI Engine)

  • 디젤엔진의 열효율을 높이면서 NOx와 PM을 효과적으로 저감시키기 위해 HCCI(Homogeneous Charge Compression Ignition), PCCI(Premixed Charge Compression Ignition), RCCI(Reactivity Controlled Compression Ignition) 등의 저온연소(LTC: Low Temperature Combustion)전략이 개발되어 왔다. 본 연구에서는 저반응성 연료로는 가솔린을 사용하고 고반응성 연료로는 디젤을 사용하는 RCCI 엔진에서 고반응성 연료인 디젤연료의 분사 시기와 이단 분사비율이 성능 및 배출가스에 미치는 영향을 수치해석을 통하여 파악하고자 하였다. 이단 분사 시 첫 번째 분사시기가 너무 진각되면 연소가 느려지면서 연소온도가 낮아져 연소성능이 저하되고 HC, CO가 증가한다. 대략 -60°ATDC 의 분사시기가 연소성능, 배출가스 및 최대압력상승률을 고려하였을 때 가장 최적의 분사시기라고 판단된다. 이단 분사 시 두 번째 분사시기를 변경하였을 때 연소성능 및 배출가스, 최대압력상승률 등을 고려하면 대략 -30°ATDC 부근에서 최적인 것으로 판단된다. 이단 분사 시 분사량 비율은 첫 번째 분사량을 60% 정도로 하였을 때 최적의 결과를 얻었다. 마지막으로 단일 분사보다는 이단 분사한 경우 연소성능 및 배출가스 부분에서 더 효과적인 것으로 판단된다.

단기통 디젤엔진에서 다단 파일럿 분할 분사 전략이 연소 및 배기가스 특성에 미치는 영향 (Effects of Multi-stage Pilot Split Injection Strategy on Combustion and Emission Characteristics in a Single-Cylinder Diesel Engine)

  • 본 논문은 커먼레일 단기통 디젤엔진에서 다단 파일럿 분할 분사 전략이 연소 및 배기가스 배출특성에 미치는 영향을 분석하는 데 초점을 두었다. 실린더 내부 최고 압력과 열발생률은 단일분사 조건에서 가장 높았으며, 파일럿 분사량이 균등하게 분할되어 분사 횟수가 증가할수록 감소하는 경향으로 나타났다. IMEP, 엔진 토크 및 연소 효율은 1단 파일럿 분사 조건에서 가장 낮게 나타나는 특성을 보였으나, 다단 파일럿 분할 분사 전략 적용 시 증가하는 경향으로 분석되었다. COVIMEP는 연소 효율이 가장 낮은 1단 파일럿 분사 조건에서 가장 높았으며, 이는 연소 안정성이 낮다는 것을 의미한다. 배기가스 중 산소농도는 단일분사 조건에서 가장 낮았고, 이산화탄소는 가장 높게 배출되는 특성을 보였다. 다단 분할 분사 전략 적용 시 저온 연소과정이 진행되기 때문에 일산화탄소의 산화율은 낮아지게 되고 배출수준은 증가하는 특성을 보였다. 탄화수소는 국부적으로 농후한 혼합기가 형성되는 단일분사 조건에서 가장 높은 결과를 보였다. 질소산화물은 다단 파일럿 분할 분사 전략 중 3단 파일럿 분사 조건에서 55.6%까지 감소하는 특성을 보였다.

순환자원을 활용한 강관다단공법용 주입재의 초기강도 발현에 대한 기초연구 (A Fundamental Study on The Expression of Initial Strength of Injection Materials for Steel Pipe Multi-Stage Method using Circulating Resources)

  • 본 연구는 순환자원을 대량 활용한 지반안정재를 터널공사 시 보조공법으로 이용하는 강관다단공법의 주입재로 활용을 검토하기 위해 다양한 주입재 배합비에 따른 겔타임, 호모겔 강도를 비교하였다. 강관다단공법은 터널의 보조공법으로써 시공 후 초기강도의 발현이 중요한 요인으로, 초기에 강도발현이 우수할수록 강관다단공법에 적합한 주입재로 활용이 가능하다. 실내시험 결과, 순환자원을 활용하는 주입재의 호모겔타임은 시멘트를 주입재로 활용하는 배합비에 비해 많은 시간이 필요한 것으로 나타났으며, 2.0MPa를 만족하는 초기강도 양생시간은 24시간 이상이 필요한 것으로 확인되었다. 따라서 순환자원 활용 주입재는 동일 배합비의 시멘트에 비해 긴 초기경화 시간이 필요한 것으로 나타났다.

AMESim기반 CRDi용 인젝터의 2단분사 동적거동 특성해석 (Analysis of Dynamic Characteristics in Two-stage Injection for CRDi Injectors Based on AMESim Environment)

  • For reduction of CO, NOx and soot emission emitted by diesel diffusion combustion, the authors focused on injection actuator to improve fuel availability inside combustion chamber. In this study, it was investigated the internal dynamic characteristics of two-stage injection with diesel injectors with different driving type for the common rail direct injection by using the AMESim simulation code. The analysis parameter defined such as fuel pressure, injection hole's diameter and driven voltage. As the results, it was shown that the piezo-driven injector had a faster response and had better control capability than the solenoid-driven injector. It was found the piezo-driven injector can be utilized effectively as multiple injector than solenoid-driven injector.

사각판 사출성형품의 최종무게 예측에 관한 연구 (A Study on the Prediction of the Final Weight for the Injection Molded Rectangular Plates)

  • In the present paper the effect of various process conditions on the final weight of injection molded rectangular plates has been investigated in detail. The main parameters involved in the simulations were melt temperature, mold temperature, injection speed and packing pressure. The dimensions of the plate used were 100mm long, 2mm of width and polystyrene was used as a molding material. The shear viscosity of the polymeric material was treated as a function of shear rate, temperature and pressure through the whole processes including packing and cooling stages. By increasing a packing pressure the final weight of sample increased linearly. Furthermore, as the melt temperature, the mold temperature and the injection speed increased, the final weight of the injection molded plate decreased within the molding window.

초기 족관절 골관절염 환자에서의 히알루론산 관절강 내 주사 요법 (Efficacy of Intra-articular Hyaluronic Acid Injection in Early Stage Ankle Osteoarthritis)

  • Purpose: This study evaluated the efficacy of an intra-articular injection of sodium hyaluronate in the treatment of early stage osteoarthritis of the ankle. Materials and Methods: Early stage ankle osteoarthritis (Takakura stage I and II) patients who received hyaluronic acid injection therapy were retrospectively reviewed. Patients underwent intra-articular injections of 2 mL of sodium hyaluronate for 3 consecutive weeks. Clinical evaluation and Visual Analogue Scale (VAS) scoring were performed at every three months after treatment. Results: Total 27 patients were involved and mean age was 55 year old (range 33 to 77 years). There were 13 male and 14 female patients. Mean follow up duration was 13 month and. Pre-intra-articular VAS score was $8.9{\pm}0.7$ and three month follow up score was $3.8{\pm}2.8$. VAS score of last follow up was $3.2{\pm}3.4$. The effect of hyaluronic acid continued about one year when analyzed the VAS score change of the patients followed for more than one year. Patients' satisfaction was "very satisfied" in nine, "satisfied" in 12, "fair" in one, and "not satisfied" in five patients. Overall satisfaction rate was 82%. There were no ankle osteoarthritis stage changes in serial follow up radiograph. Conclusion: Symptomatic relief of signs and symptoms of osteoarthritis of the ankle was achieved by injection of an intra-articular hyaluronate injection. Efficacy of Hyaluronate acid injection persisted more than 1 year in our study. Intra-articular hyaluronate injection to ankle osteoarthritis is safe and effective as knee joint and should be considered as a valid conservative treatment for ankle osteoarthritis.
