Purpose: Patients suffering from acute poisoning by different substances often visit the emergency department (ED) and receive various prognoses according to the toxic material and patients' condition. Hyperlactatemia, which is an increased blood lactate level that generally indicates tissue hypoperfusion, is commonly utilized as a prognostic marker in critically ill patients such as those with sepsis. This study was conducted to investigate the relationships between blood lactate and clinical prognosis in acute poisoned patients. Methods: This retrospective study was conducted from January 2013 to June 2014 at a single and regional-tertiary ED. We enrolled study patients who were examined for blood test with lactate among acute intoxicated patients. The toxic materials, patient demographics, laboratory data, and mortalities were also reviewed. Additionally, we analyzed variables including blood lactate to verify the correlation with patient mortality. Results: A total of 531 patients were enrolled, including 24 (4.5%) non-survivors. Patient age, Glasgow coma scale (GCS), serum creatinine (Cr), aspartate transaminase (AST), and serum lactate differed significantly between survivors and non-survivors in the binary logistic regression analysis. Among these variables, GCS, AST, and lactate differed significantly. The median serum lactate levels were 2.0 mmol/L among survivors and 6.9 mmol/L among non-survivors. The AUC with the ROC curve and odds ratio of the initial serum lactate were 0.881 and 3.06 (0.89-8.64), respectively. Conclusion: Serum lactate was correlated with fatalities of acute poisoning patients in the ED; therefore, it may be used as a clinical predictor to anticipate their prognoses.
Purpose: This preliminary study is aimed at developing standardized tools for psycho-social assessment of patients in needs for hospice/palliative care. To accomplish the purpose, investigators examined effects of perceptions of social workers on the importance of psycho-social domains of assessment in hospice/palliative care settings. Moreover, investigators paid attention to variances of perceptions of social workers' along with types of institution and credentials of those family settings. Methods: A form of questionnaire was first explored from an initial interview assessment of 10 government-certified hospice care providers and a literature review, second constructed with eight domains and 80 items, and sent by e-mail to 55 institutions and hospitals providing hospice/palliative cares in Korea. Lastly, a total of 31 agencies returned with a completed responses and consent form (56% response rate). SPSS program (version 18.0) was used for data analysis. Results: Study found that social workers perceived patients' family background (m=4.53, 5-point scale) as the most important assessment domain, whereas economic conditions (4.06 point) the least important. Social workers' perception varied by credentials (i.e., license types, training, full-time position, types of care facility). Conclusion: Based upon study findings, investigators can conclude strong needs for developing a assessment tool that measures multiple domains (i.e., psychological, social and ecological aspects) of patients. A standardized assessment tool should be structured with 2 axis (center/core and expanded/peripheral) and tailored for institution type. Second, professional trainings must be provided by strengthening legal institutionalization and fostering qualified social workers with full responsibilities of hospice and palliative care patients.
Batch experiments were carried out for adsorption equilibrium, kinetics and thermodynamic parameters of the brilliant brown R onto granular activated carbon. The operating variables studied were the initial dye concentration, contact time and temperature. Experimental equilibrium adsorption data were fitted to Langmuir and Freundlich adsorption isotherm by linear regression method. The equilibrium process was well described by Freundlich isotherm model and from the determined separation factor (1/n), granular activated carbon could be employed as an effective treatment for the removal of bismarck brown R. From kinetic experiments, the adsorption processes were found to confirm the pseudo second order model with a good correlation and the adsorption rate constant ($k_2$) increased with increasing adsorption temperature. Thermodynamic parameters like the activation energy, change of Gibbs free energy, enthalpy, and entropy were also calculated to predict the nature of adsorption in the temperature range of 298~318 K. The activation energy was determined as 8.73 kJ/mol for 100 mg/L. It was found that the adsorption of bismarck brown R on the granular activated carbon was physical process. The negative Gibbs free energy change (${\Delta}G$ = -2.59~-4.92 kJ/mol) and the positive enthalpy change (${\Delta}H$ = +26.34 kJ/mol) are indicative of the spontaneous and endothermic nature of the adsorption process.
In this study, Echo-sounder($E_s$) monitoring methodology was applied to test environmental impacts of artificial weirs(SCW and JSW) in the Yeongsan River on fish community and habitat structures at first time, and was compared with conventional fish sampling methodologies($C_s$). For the Es monitoring in the fields, parallel transect methods was employed in determining the survey distance interval with every 125 m within the upper-lower 2 km of the weirs. Four different fish sampling gears such as casting nets, kick nets, fyke nets, and gill nets were used for applications of $C_s$ monitoring methodology. According to the Echo-sounder monitoring, fish density, expressed as a number of individuals per square meter, along the longitudinal axis of the weir was significantly greater(JSW, t = 3.506, n = 30, p < 0.001) in the down-river reach of the weir than in the up-river reach. Also, fish density along the vertical water column was highest at mid-depths of Seungchon weir, which has simple habitat substrates, while fish density was highest at hypolimnetic depth of Juksan weir. According to fish sampling by the $C_s$ methodology, the fish compositions decreased as the river goes upward, and significant differences(JSW, t = 0.248, n = 30, p < 0.05) in the compositions of fish species occurred between up-reach and down-reach of the weirs. The dominant species near the two weirs were Opsarichthys uncirostris amurensis, Hemiculter eigenmanni and Coilia nasus. Overall, our fish and habitat data, based on $E_s$ and $C_s$ monitoring methodologies, suggest that the weirs disturbed the rivers due to initial habitat disturbances by the weir constructions as well as the barrier roles of weirs on fish passage and migrations. More long-term scientific and systematic fish impact monitoring and assessments($E_s$ and $C_s$) are required in the future to predict changes of ecological structures and functions on the constructions of the weirs.
Purpose : The initial Breast-Specific Gamma Imaging (BSGI) protocol included bilateral breast imaging with 2 views of each breast-craniocaudal (CC) and mediolateral oblique (MLO). Furthermore, Axillary lymph nodes view can be acquired easily. The most meaningful prognosis factor for prediction of breast cancer is whether or not the breast cancer has metastasized to the lymph nodes. However, axillary view doesn't conduct in clinical. This article collates a diverse data of BSGI and describes technical details to acquire optimal imaging. Materials and Methods : A retrospective review was performed on 343 patients who had undergone BSGI between May 2011 and March 2012. Patients who had undergone BSGI received intravenous injection of 740 MBq (20 mCi) $^{99m}Tc$-sestamibi. Results : The following contents are the technical details for optimal axillary imaging. $^{99m}Tc$-sestamibi should be administered using an indwelling venous catheter or scalp needle followed by 10 cc of saline to flush to reduce extravasation and vascular trapping. After administration, patients raise their arm over their head and exercise with stress ball for 1 full minute. A lead shield attached to the gamma camera is removed and patients axilla is placed as close as possible to the camera at a $90^{\circ}$ angle. A lead apron is placed across the shoulder to reduce background from other organs. Acquisition time is enough for 120 sec~180 sec. Conclusion : If patients undergo bilateral axillary imaging as a standard with CC, MLO views, it could improve cancer treatment. Result of this study could maximize efficiency axillary imaging of breast cancer patients.
Objectives : To evaluate the long-term survival effects of traditional Korean medicine (TKM) on refractory metastatic lung cancer and small cell lung cancer (SCLC), which have historically poor survival rates. Methods : A retrospective study was conducted using the medical records of two patients in Daejeon University hospital. The first patient, with SCLC, was treated from January 2000 to December 2009 and the other, with metastatic pulmonary cancer from primary hepatocellular carcinoma (HCC), was treated from September 2004 to February 2014. The patients were treated with herbal medicines at one-month intervals. During hospitalization, acupuncture and indirect moxibustion were performed concurrent with the administration of Western therapy. Treatment efficacy was assessed monthly using chest radiography, chest computed tomography, and laboratory examination data, and by measuring patient performance status. Results : Both patients exhibited a stable disease course for more than 9 years after the initial diagnosis of intractable lung cancer, suggesting that their disease status was controlled by TKM. Conclusions : We suggest that a combination of TKM with conventional Western therapy for refractory lung cancer patients is effective in controlling various symptoms related to lung cancer and improving quality of life, and may potentially prolong overall survival.
Background: To date, there are few data on the risk factors for severe cases and deaths associated with the 2009 pandemic H1N1 influenza A. Here, we describe the clinical and epidemiologic characteristics of patients hospitalized for pneumonia and identify those factors associated with the development of major complications (MC). Methods: We reviewed the medical records of 41 cases of pneumonia admitted to a university-affiliated tertiary hospital between Aug 26 and Dec 10, 2009, and who had confirmed H1N1 influenza A based on real-time reverse transcriptase-polymerase-chain-reaction assay. There were 7,962 patients that fit these criteria. We compared the clinical features and demographic characteristics of patients who developed MC to with those who did not develop MC. Results: During the study period, 10 patients developed MC (required admission to the intensive care unit, n=10; required ventilator therapy, n=6; death, n=4). Patients with MC were significantly older than those without MC and more frequently had underlying medical conditions (90.0% vs 41.9%, p-value <0.01). In the patients with developed MC, the median $PaO_2/FiO_2$ ratio of 230.0 (145.0~347.3) at admission and pneumonia severity index (PSI) score of 141.5 (88.3~158.5) were higher than patients without MC. However, no differences were observed in laboratory findings or in viral shedding between the 2 groups. Conclusion: In hospitalized pneumonia patients of 2009 H1N1 influenza, old age, a history of malignancy, initial hypoxemia, $PaO_2/FiO_2$ ratio, and PSI score appear to be risk factor significantly related to developing MC. These findings might be the basis to influence strategies for admitting patients to an intensive or intermediate care unit and for pre-emptive antiviral therapy.
Forest tree diversity inventory and its periodical monitoring are important to understand changes in tree population structure and to provide information useful for biodiversity conservation and reserve management. In a long-term forest dynamics program in Indian tropical dry evergreen forest, this communication deals with tree diversity changes at decadal interval. The initial inventory of tree diversity was carried out in 2003, in four tropical dry evergreen forest sites - (much disturbed sites Shanmuganathapuram - SP and Araiyapatti - AP and moderately disturbed sites - Karisakkadu - KR and Maramadakki - MM) on the Coromandel Coast of peninsular India, by establishing four 1ha permanent plots, one in each site. In 2013, the four plots were re-inventoried for tree diversity (${\geq}10cm\;gbh$) changes which yielded 56 species from 46 genera and 26 families. The studied forest sites are threatened by disturbance due to multiple reasons; cutting of trees inside of the forest, grazing by goats, construction of temple approach road, and some aspects cultural attachment of local people like constructing new, additional strctures of temple by denuding a portion of forest etc.. Tree species richness over a decade increased by four species in site SP, two species in site AP, and one species in site KR, but decreased by one species in site MM. Tree density decreased drastically by 480 (28.92%) and 102 (12.63%) stems $ha^{-1}$ respectively in sites SP and AP, but moderately increased by 82 (12.09%) stems $ha^{-1}$ in site KR and 26 (3.46%) stems $ha^{-1}$ in site MM. Tree basal area declined in site KR from $21.6m^2$ to $20.26m^2ha^{-1}$ and in site SP from 21.1 to $20.38m^2ha^{-1}$, but increased from $19.1m^2$ to $19.43m^2$ and from 15.5 to $18.63m^2ha^{-1}$ in sites AP and MM respectively. Three tree species (Allophylus serratus, Maytenus emarginata and Ehretia pubescens) were lost out of the 57 species recorded in 2003, and two species (Jatropha gossypiifolia and Streblus asper) were new additions in ten years. The long-term forest monitoring data will be valuable to understand forest dynamics and for conservation and management of this and similar tropical forests.
Composition of the major and trace elements, Rb-Sr isochron age Sr-Nd isotope composition were determined for foliated in the Jeonju area, in the middle part of the Ogcheon Fold Bet, Korea. The geochemical characteristics of the Jeonju foliated granite indicate that the granite had been crystallized from a calc-alkaline series, and formed in a volcanic are environment. The isotopic compositions of the Jeonju foliated granite give Rb-Sr whole rock errorchron age of 168.2${\pm}$8 Ma(2${\sigma}$), corresponding to the middle Jurassic period, with the Sr initial ratio of 0.71354${\pm}$0.00031. $^{143}$Nd/$^{144}$Nd ratios, ${\varepsilon}$Nd and ${\varepsilon}$Sr values range from 0.511477 to 0.511744, -15.4${\sim}$-21.2, and +108.8${\sim}$+l42.6, respectively. Model ages were caculated to be 1.82${\sim}$2.89Ga. The isotopic data of Jeonju foliated granite indicate that the source material may have been derived from partial melting of continental crust materials.
The purpose of this study is to develop and utilize new assessment of liquefaction potential based on DSC(disturbed state concept) and dissipated energy concept. The term liquefaction has suddenly loses its shear strength and behaves like a fluid. Liquefaction has been a source of a major damage during severe earthquake. In this study, the cyclic undrained behavior of Joomoonjin strand is investigated by using an automates triaxial testing device(C. K. Chan type). In order to assess liquefaction potential of saturated strand, DSC method and energy method are applied for the experimental data. The use of DSC method and energy method to define the liquefaction potential is verified through laboratory testis of cyclic triaxial test on saturated sand specimens. Based on the analytical results of DSC method, the relationship between the factor affecting liquefaction characteristics(Dr) and physical properties of the saturated santa(fs and D.) is found. Based on the analytical results of energy method, it is found that the initial liquefaction of rand is related to the significant change in the dissipated energy. Finally, it is shown that the DSC method and energy method can capture the liquefaction mechanism.
