2003-2004 절기 서울지역의 인플루엔자 바이러스 분리 및 아형 분석 (Analysis of Influenza Virus Isolates in Seoul during 2003-2004 Season)

  • 인플루엔자바이러스는 거의 매년 겨울철에 유행을 일으키는 급성 호흡기 질환의 주요한 원인 바이러스중의 하나로 막대한 사회 경제적 손실을 가져온다. 본 실험은 2003-2004절기 동안 $38^{\circ}C$이상의 갑작스러운 발열과 더불어 기침 또는 인후통을 보이는 401명의 인플루엔자의사환자(ILI : Influenza-like illness) 검체로부터 124주의 인플루엔자바이러스를 분리하여 형 및 아형 분석과 그에 따른 유행양상을 분석하였다. 인플루엔자 의사환자 연령별 분포를 살펴보면 20-49세의 성인층의 환자수가 $23\%$로 가장 많았으며, 바이러스 분리율은 7-19세의 학령기에서 $50\%$로 가장 높았다. 분리된 인플루엔자바이러스 124주 중 A/H3N2 type는 83주, Type B는 41주였다. 지역별 인플루엔자 의사환자 발생율은 노원구, 서초구, 강남구의 발생율이 타 지역에 비해 비교적 높았으며 바이러스 분리율은 용산구 $66.7\%$, 강남구 $50.0\%$, 노원구 $39.9\%$, 강북구 $36.8\%$ 서초구 $27.8\%$, 동작구 $21.2\%$ 순이었다. 인플루엔자 의사환자의 예방접종현황조사결과 접종을 받았음에도 불구하고 인플루엔자 의사환자로 보고된 경우가 $40\%$였으며, 접종율은 20-49세 성인층이 가장 높았다. 이와 같은 인플루엔자 실험실 표본감시체계 결과의 분석을 통하여 새로운바이러스형 출현을 감시하며, 현행 예방백신의 효과 및 유행양상을 예측하여 국가 인플루엔자 관리대책을 수립하는데 활용하고자 한다.

감기바이러스(인플루엔자) 감염에 대한 마늘의 방어효과 (Effects of Garlic Extract for Protecting the Infection of Influenza Virus)

  • This study was designed to verify the efficacy of garlic extracts for protecting the infecton of influenza and Japanese B encephalitis virus. Influenza virus (AO/PR8 strain) and Japanese B encephalitis virus (JaGAr O1 strain) were used to attack mouse through nasal route and each vaccines were injected subcutaneously. 0.002 and 0.2 mL/day of garlic extracts were orally administered to mice. The blood and serum samples were taken from the mice to measure LD50, Defense Index (DI), virus-neutralizing antibody for comparing virus influence inhibiting activities. Defense indices of the male and female mice were not significantly different at every experiment. Vaccination effectively inhibited the influence of influenza virus and 0.002 mL/day garlic extract (0.55$\pm$0.05) resulted in significantly higher DI than the control (0$\pm$0.05) (p<0.05). Although 0.002 mL/day garlic extract (0.55$\pm$0.05) resulted in significantly lower DI than the vaccination (1.10$\pm$0.05), 0.2 mL/day garlic extract (2.05$\pm$0.05) resulted in 10 times higher DI than the vaccination (1.10$\pm$0.05). Garlic extract did not affect DI in Japanese B encephalitis virus influence of the vaccinated mouse, but significantly reduced DI of the non-vaccinated mouse (p<0.05). Garlic extracts did not affect the production of the neutralizing antibody against influenza by vaccination. However, neutralizing antibody production of Japanese B encephalitis was accelerated by vaccination. Consequently, the current study proved the efficacy of garlic on inhibition of influenza virus. Finally, it is very hard to show the higher preventing effect on flu through ingestion of garlic as a food than vaccination.

서울지역 급성호흡기 환자들로부터 분리한 인플루엔자 바이러스의 유행양상 (2010-2012) (Epidemiological Characterization of Influenza Viruses detected from Acute Respiratory Patients in Seoul during 2010-2012)

  • Objectives: For our survey of the incidence of influenza viruses among respiratory viral infections in Seoul, we evaluated their prevalence among infectious acute respiratory viral patients in Seoul from 2010 to 2012 through regular surveillance. Methods: For influenza virus detection, we conducted real-time PCR analyses on 2,544 throat specimens collected from patients with respiratory viral infections in Seoul between 2010 and 2012. They were collected and then tested for the presence of influenza viruses through reverse transcription (RT) - polymerase chain reaction (PCR). Results: 19.1% (486/2,544) of the throat specimens were determined to be positive for influenza viruses. The incidences of influenza viral infection in the case of respiratory viral infections through regular surveillance in Seoul were 23.0% (212/923) in 2010, 6.4% (47/738) in 2011, and 25.7% (227/883) in 2012, and 10.8% (275/2,544) of type A, and 8.3% (211/2,544) type B influenza viruses. In addition, the greatest prevalence was in the 20-49 age group (51.6% ), which shows that influenza viruses constituted a major causative agent of acute respiratory viral infections. Conclusions: The distributions of influenza viruses and the epidemiologic patterns of the viral pathogen in acute respiratory viral infectious patients may provide potentially effective data for epidemiological studies in Seoul, Korea.

  • 목적: 부모의 소아 자녀에 대한 인플루엔자 및 인플루엔자 백신 관련한 인식도 차이 및 변화를 홍보물 교육 유무에 따라 비교 분석하였다. 방법: 2015년 서울, 경기 북부 5개 지역 거주자 중 생후 6개월부터 59개월까지의 자녀가 있는 보호자 1,034명을 대상으로, 설문지 조사를 시행하였다. 설문내용은 인플루엔자 및 인플루엔자 백신에 대한 인식 및 변화 및 인플루엔자 예방접종 의향 및 이유 등을 포함하였다. 부모는 3군으로 나누어 교육 유무(A군: 교육받지 않음, B군: 교육받음)에 따른 인식도 차이와 교육 전후(C군)의 인식도 변화를 분석하였다. 결과: A군과 B군 간 교육에 따른 인식도 차이 중 의미 있는 항목은 '자녀가 고위험군임을 인식함', '독감 예방접종은 효과적', '독감 예방접종은 비쌈' 등이었다. C군에서 교육 전후에 따른 인식도 변화 중 의미 있는 항목은 '자녀가 고위험군임을 인식함', '독감 예방접종은 안전함', '독감 예방접종은 비쌈', '독감 예방접종을 부정적으로 인식함' 등이었다. 결론: 인플루엔자와 인플루엔자 예방접종에 대한 부모의 인식은 홍보물 교육 이후 긍정적인 효과를 보였다.

Bioviolence Agents에 따른 건물내 미생물오염원의 효율적인 제거방안에 관한 연구 (A Study on the Effective Removal Method of Microbial Contaminants in Building According to Bioviolence Agents)

  • As Influenza A virus(H1N1) has been spreading more rapidly around globe, the study on the airborne disease which is transimitted through the respiratory system is on the rise. In this study, the multizone simulation of the public building against bioviolence is performed in the case of unexpected spread of microbial contaminants, such as bioviolence agent, Influenza A, Smallpox, B. anthrax and transport and control characteristics of above three kinds of bioviolence agents are evaluted. Results suggest that Influenza A and Smallpox which has small mean diameter can be more removable than B. anthrax by using high UVGI grade condition and B. anthrax which has large mean diameter can be more removable than Influenza A and Smallpox by using high filter grade condition. Results also suggest that installing a combined air treatment system is more effective to reduce the damage and engineers will use immune building technology for removing the bioviolence agents effectively.

신종플루 바이러스를 통한 인플루엔자 바이러스의 해석 및 전망 (Interpretation and Prospection of Influenza Virus through Swine-origin Influenza Virus)

  • Swine influenza virus (SIV) or swine-origin influenza virus (S-OIV) is endemic in swine, and classified into influenza A and influenza C but not influenza B. Swine influenza A includes H1N1, H1N2, H3N1, H3N2 and H2N3 subtypes. Infection of SIV occurs in only swine and that of S-OIV is rare in human. What human can be infected with S-OIV is called as zoonotic swine flu. Pandemic 2009 swine influenza H1N1 virus (2009 H1N1) was emerged in Mexico, America and Canada and spread worldwide. The triple-reassortant H1N1 resulting from antigenic drift was contained with HA, NA and PB1 of human or swine influenza virus, PB2 and PA polymerase of avian influenza virus, and M, NP and NS of swine influenza virus, The 2009 H1N1 enables to transmit to human and swine. The symptoms and signs in human infected with 2009 H1N1 virus are fever, cough and sore throat, pneumonia as well as diarrhea and vomiting. Co-infection with other viruses and bacteria such as Streptococcus pneumoniae can occur high mortality in high-risk population. 2009 H1N1 virus was easily differentiated from seasonal flu by real time RT-PCR which contributed rapid and confirmed diagnosis. The 2009 H1N1 virus was treated with NA inhibitors such as oseltamivir (Tamiflu) and zanamivir (Relenza) but not with adamantanes such as amantadine and rimantadine. Evolution of influenza virus has continued in various hosts. Development of a more effective vaccine against influenza prototypes is needed to protect new influenza infection such as H5 and H7 subtypes to infect to multi-organ and cause high pathogenicity.

Estimating Influenza-associated Mortality in Korea: The 2009-2016 Seasons

  • Objectives: Estimating influenza-associated mortality is important since seasonal influenza affects persons of all ages, causing severe illness or death. This study aimed to estimate influenza-associated mortality, considering both periodic changes and age-specific mortality by influenza subtypes. Methods: Using the Microdata Integrated Service from Statistics Korea, we collected weekly mortality data including cause of death. Laboratory surveillance data of respiratory viruses from 2009 to 2016 were obtained from the Korea Centers for Disease Control and Prevention. After adjusting for the annual age-specific population size, we used a negative binomial regression model by age group and influenza subtype. Results: Overall, 1 859 890 deaths were observed and the average rate of influenza virus positivity was 14.7% (standard deviation [SD], 5.8), with the following subtype distribution: A(H1N1), 5.0% (SD, 5.8); A(H3N2), 4.4% (SD, 3.4); and B, 5.3% (SD, 3.7). As a result, among individuals under 65 years old, 6774 (0.51%) all-cause deaths, 2521 (3.05%) respiratory or circulatory deaths, and 1048 (18.23%) influenza or pneumonia deaths were estimated. Among those 65 years of age or older, 30 414 (2.27%) all-cause deaths, 16 411 (3.42%) respiratory or circulatory deaths, and 4906 (6.87%) influenza or pneumonia deaths were estimated. Influenza A(H3N2) virus was the major contributor to influenza-associated all-cause and respiratory or circulatory deaths in both age groups. However, influenza A(H1N1) virus-associated influenza or pneumonia deaths were more common in those under 65 years old. Conclusions: Influenza-associated mortality was substantial during this period, especially in the elderly. By subtype, influenza A(H3N2) virus made the largest contribution to influenza-associated mortality.

인플루엔자 바이러스에 대한 Zanthoxylum속의 항바이러스 효과 검정 (Antiviral Activity of Zanthoxylum Species against Influenza Virus)

  • We aimed to investigate the antiviral activity of Zanthoxylum species against influenza virus A/WS/33, A/PR/8 and B/Lee/40 used by sulforhodamine B (SRB) assay and the action of leaves extracts of Zanthoxylum piperitum on life cycle of influenza virus A/WS/33. Among the twelve extracts, only the leaf extract of Z. piperitum exhibited strong antiviral activity at low concentration of less than 10${\mu}g/m{\ell}$ with no citotoxicity (50${\mu}g/m{\ell}$) against all of three viruses. In addition, only oseltamivir showed antiviral activity with $IC_{50}$ of 65.3${\mu}g/m{\ell}$ against influenza A/WS/33 among the viruses. Furthermore, the leaf extract of Z. piperitum suppressed infection of influenza virus A/WS/33, when added just prior (-1 hr) or after virus inoculation (0 hr). Leaf extract of Z. piperitum directly affect the infectivity of influenza virus A/WS/33 particles. Therefore, Leaf extract of Z. piperitum exhibited higher antiviral activity against three influenza viruses than that of the oseltamivir, which directly interacts with influenza A/WS/33 particles, affecting the initial stages of infection such as receptor binding and virus entry.

A "Prime and Deploy" Strategy for Universal Influenza Vaccine Targeting Nucleoprotein Induces Lung-Resident Memory CD8 T cells

  • Lung-resident memory T cells (TRM) play an essential role in protecting against pulmonary virus infection. Parenteral administration of DNA vaccine is generally not sufficient to induce lung CD8 TRM cells. This study investigates whether intramuscularly administered DNA vaccine expressing the nucleoprotein (NP) induces lung TRM cells and protects against the influenza B virus. The results show that DNA vaccination poorly generates lung TRM cells and massive secondary effector CD8 T cells entering the lungs after challenge infection do not offer sufficient protection. Nonetheless, intranasal administration of non-replicating adenovirus vector expressing no Ag following priming DNA vaccination deploys NP-specific CD8 TRM cells in the lungs, which subsequently offers complete protection. This novel 'prime and deploy' strategy could be a promising regimen for a universal influenza vaccine targeting the conserved NP Ag.

Evaluation of immunogenicity of the 2008-2009 seasonal influenza vaccines by microneutralization test

  • Purpose: For evaluating the immunogenicity of an influenza vaccine, the microneutralization (MN) test has a higher sensitivity and specificity as compared to the hemagglutination inhibition (HI) test. However, the MN test is more time consuming and is difficult to standardize. We performed the MN test to determine its usefulness as an alternative or complementary test to the HI test for evaluating the immunogenicity of influenza vaccines. Methods: We compared the MN test with the HI test using 50 paired samples taken from a previous clinical study (2008-2009) in Korean children under 18 years of age. Results: The linear correlation coefficients of the 2 tests for H3N2, H1N1, and influenza B were 0.69, 0.70, and 0.66, respectively. We identified a high index of coincidence between the 2 tests. For an influenza vaccine, the postvaccination seroprotection rates and seroconversion rates determined by the MN test were 78.0% and 96.0%, 90% and 42.0%, and 42.0% and 48.0% for H3N2, H1N1, and influenza B, respectively. Geometric mean titer fold increases of H3N2, H1N1, and influenza B were 2.89, 5.04, and 4.29, respectively, and were 2.5-fold higher. We obtained good results in the evaluation of the immunogenicity of the 2008-2009 seasonal influenza vaccines. Conclusion: We found that the MN test was as effective as the HI test. Therefore, we suggest that the MN test can be used as an alternative or complementary test to the HI test for evaluating the immunogenicity of influenza vaccines.
